Scientists at The Virginia Commonwealth University Medical Center found that an emphasis on compliance with non-pathogen specific infection control practices such as hand hygiene, efforts to reduce device-related infections and chlorhexidine bathing (a daily bath with the same antibacterial agent used by surgeons to "scrub in" before an operation), is successful in reducing rates of healthcare-associated methicillin-resistant Staphylococcus aureus. The findings were presented at the Fifth Decennial International Conference on Healthcare-Associated Infections... (Source: Health News from Medical News Today)

Antiretroviral therapy (ART) for treating HIV infection in ART-eligible pregnant women.

CONCLUSIONS: In ART-eligible pregnant women with HIV infection, ART is a safe and effective means of providing maternal virologic suppression, decreasing infant mortality, and reducing MTCT. Specifically, AZT/3TC/NVP, AZT/3TC/LPV-r, and AZT/3TC/ABC have been shown to decrease MTCT. More research is needed regarding the use of specific regimens and their maternal and infant side-effect profiles.

CONCLUSIONS: No RCTs or CCTs evaluating MIS in the treatment of solid intra-thoracic or intra-abdominal neoplasms in children could be identified, therefore no definitive conclusions could be made about the effects of MIS in these patients. Based on the currently available evidence we are not able to give recommendations for the use of MIS in the treatment of solid intra-thoracic or intra-abdominal neoplasms in children. More high quality studies (RCTs and/or CCTs) are needed. To accomplish this, centres specialising in MIS in children should collaborate.

CONCLUSIONS: There is evidence of moderate quality that initiating ART at CD4 levels higher than 200 or 250 cells/microL reduces mortality rates in asymptomatic, ART-naive, HIV-infected people. Practitioners and policy-makers may consider initiating ART at levels </= 350 cells/microL for patients who present to health services and are diagnosed with HIV early in the infection.

CONCLUSIONS: We were unable to identify any randomised controlled trials or quasi-randomised controlled trials on the efficacy of neuraminidase inhibitors for the treatment of influenza infection in people with cystic fibrosis. The absence of high level evidence for the effectiveness of these interventions emphasises the need for well-designed, adequately powered, randomised controlled clinical trials.

CONCLUSIONS: The limited number of patients available for analysis, methodological flaws and a significant over-representation of patients with FIGO stage IIIB prohibit drawing definite conclusions regarding the impact of adding hyperthermia to standard radiotherapy. However, available data do suggest that the addition of hyperthermia improves local tumour control and overall survival in patients with locally advanced cervix carcinoma without affecting treatment related grade 3 to 4 acute or late toxicity.

CONCLUSIONS: There is insufficient evidence to determine whether organisational travel plans are effective for improving health or changing travel mode. Organisational travel plans should be considered as complex health promotion interventions, with considerable potential to influence community health outcomes depending on the environmental context in which they are introduced. Given the current lack of evidence, organisational travel plans should be implemented in the context of robustly-designed research studies, such as well-designed cluster randomised trials.

CONCLUSIONS: We are not able to favour either epirubicin or doxorubicin when given with the same dose. Based on the currently available evidence on heart failure, we conclude that in adults with a solid tumour liposomal-encapsulated doxorubicin should be favoured over doxorubicin. For both epirubicin versus doxorubicin and liposomal-encapsulated doxorubicin versus conventional doxorubicin no conclusions can be made about the effects of treatment in children treated with anthracyclines and also not in patients diagnosed with leukaemia. More research is needed. For other combinations of anthracycline derivates not enough evidence was available to make definitive conclusions about the occurrence of cardiotoxicity in patients treated with anthracyclines.

CONCLUSIONS: There is evidence that laparoscopic fundoplication surgery is more effective than medical management for the treatment of GORD at least in the short to medium term. Surgery does carry some risk and whether the benefits of surgery are sustained in the long term remains uncertain. Treatment decisions for GORD should be based on patient and surgeon preference.

CONCLUSIONS: There is evidence to show that therapy with dornase alfa over a one-month period is associated with an improvement in lung function in CF; results from a trial lasting six months also showed the same effect. Therapy over a two-year period (based on one trial) significantly improved FEV(1) in children and there was a non-significant reduction in the risk of infective exacerbations. Voice alteration and rash appear to be the only adverse events reported with increased frequency in randomised controlled trials.

CONCLUSIONS: The number of outcome events is too small to allow reliable conclusions to be drawn and more trial data are required to establish whether any differences do exist. Nevertheless, there is some evidence that other synthetic (e.g. PTFE) patches may be superior to collagen impregnated Dacron grafts in terms of perioperative stroke rates and restenosis. Pseudoaneurysm formation may be more common after use of a vein patch compared with a synthetic patch.

(Society for Healthcare Epidemiology of America) Scientists at the Virginia Commonwealth University Medical Center found that an emphasis on compliance with non-pathogen specific infection control practices such as hand hygiene, efforts to reduce device-related infections and chlorhexidine bathing (a daily bath with the same antibacterial agent used by surgeons to "scrub in" before an operation), is successful in reducing rates of health-care-associated methicillin-resistant Staphylococcus aureus. The findings were presented today at the Fifth Decennial International Conference on Healthcare-Associated Infections. (Source: EurekAlert! - Medicine and Health)

The UK is soon to have a general election. The expected schism in government is encouraging a last-minute waterfall of new policies and programmes. Nowhere is this more apparent than in international health. The Department for International Development (DFID—currently rebranding itself as UKaid) has been in a frenzy of activity these past few weeks. It has published an education strategy, launched its undernutrition strategy, and hosted an international Millennium Development Goal conference billed as “the turning point of poverty”. Its undernutrition strategy won much praise from delegates (ranging from African Health Ministers to the more sceptical end of non-governmental organisations) at the launch event last week. DFID has clearly established itself as a respected leader in glob...

A US National Institutes of Health conference entitled Vaginal Birth After Cesarean: New Insights, held in Bethesda, MD, USA on March 8–10, examined why the rate of vaginal birth after caesarean (VBAC) delivery has fallen. This decline has contributed to a striking increase in the overall rate of caesarean delivery in the USA, from 21% in 1996 to 33% in 2007. Evidence presented in the conference statement includes a survey done by the American College of Obstetricians and Gynecologists showing that 26% of its fellows stopped offering trials of labour (TOL), a prerequisite of VBAC between 2003–06.
Conclusions: We have begun the process of developing a questionnaire that can identify the clinical activities, perceived self-competence and professional behaviour of RNSs. This method of characterizing advanced nursing practice offers potential as a model for nurse specialists in other disciplines. Further work is required to validate the questionnaire on a large cohort of RNSs. Copyright © 2010 John Wiley & Sons, Ltd. (Source: Musculoskeletal Care)

The Model for End-Stage Liver Disease (MELD) score is widely used to prioritize patients for liver transplantation. One of the pitfalls of the MELD score is the interlaboratory variability in all three components of the score (INR, bilirubin, creatinine). The interlaboratory variability in the INR has the largest impact on the MELD score, with a mean difference of around 5 MELD points in most studies. During the 3rd conference on Coagulopathy and Liver disease, a multidisciplinary group of scientists and physicians discussed possible solutions for the INR problem in the MELD score with the intention to provide a constructive contribution to the international debate on this issue. Here we will discuss possible solutions and highlight advantages and disadvantages. (Source: American Journal o...

The special issue arose from the 2008 Qualitative Research in Management and Organization Conference, held at the University of New Mexico, to celebrate the twentieth anniversary of John Van Maanen’s (1988) book Tales of the Field. The theme of the conference, Telling Tales, reflected two of the core issues in the book—the need to think about the ways in which we carry out (our methods) and write up ethnographic work. This introductory paper sets the scene for the four papers to follow by giving an overview of ethnography: what ethnographic work entails, a brief history, issues to consider when doing ethnography, and offers a range of references for potential ethnographers. In addition, it introduces the four authors and situates their ‘‘tales’’ within t...

This essay tries to be true to a podium talk I presented at a conference in March, 2008. But, of necessity, certain consolidation liberties are taken. Beginning with a brief and broad treatment of ethnography as a paired written representation of and lengthy personal experience in a particular social world, I move to consider why the former, the text, has been so infrequently examined in lieu of the latter, the so-called method. I then move to ethnographic texts themselves and look at what I take to be some broad changes the seem apparent — particularly within the organizational ethnography domain — over the past 20 or so years. Alongside these changes comes the emergence of several distinct genres treated only lightly (or not at all) in Tales of the Field. I end by considering...

MedWorm Message: Get the very latest Swine Flu news via the MedWorm Swine Flu RSS news feed - updated hourly from thousands of authoritative health and news sources.

The recently published Pathway report by the Office of the Independent Adjudicator (OIA) has considered for the first time publishing the results of its adjudications. How can HEIs adjust to being exposed to this new consumer-led environment?

HEIs can not afford to become complacent. What will Lord Browne's funding and fees review mean for HEIs? What will future students expect from their investment in education when they perceive fees to be linked to high quality education?

This conference will bring together experts from HEI's, Student Unions and Student Services to answer these questions. They will discuss how, in a relationship with redefined parameters, students' expectations as consumers can be met, their complaints handled effectively, and a HEIs reputation be preserved. Approximately 2,000 to 3,000 people are diagnosed with mesothelioma cancer in the United States each year.

Only few studies employed randomised controlled settings to test the value of treatment to a specific target. However, they provided unanimous evidence for benefits of targeted approaches. Nevertheless, more data on radiographic and functional outcomes and on patients with established RA are needed. (Source: Annals of the Rheumatic Diseases)
