Authors: Goodyear HM, Jones M
In the last few years, the NHS has undergone a frantic pace of change and reform. These include the Modernising Medical Careers (MMC) reorganization of medical training (Department of Health, 2004), introduction of the European Working Time Directive, Darzi centres and a community focus to health-care provision (Department of Health, 2009). These affect all career pathways although the exact impact of moving care into the community upon careers is yet to be seen. In addition, there is now much more overlap between traditional 'medical' and 'surgical' specialties, e.g. cardiology and radiology are becoming increasingly interventional. There is also increased regulation of doctors throughout their careers with revalidation and Postgraduate Medical Education ...

MedWorm Message: Get the very latest Swine Flu news via the MedWorm Swine Flu RSS news feed - updated hourly from thousands of authoritative health and news sources.

Background: Modernising medical careers has been fully implemented nationally in the UK.1 The foundation-year programme is the keystone of this plan, and as a consequence a large number of foundation trainees are exposed to specialties (e.g. orthopaedics) previously reserved for aspirant surgeons.2 When coupled with the undergraduate curriculum changes instigated by the UK registry and accrediting body for medicine, the General Medical Council (GMC),3,4 it appears self-evident that the type of training now being provided to these juniors is very different from that formerly available. We aimed to assess whether foundation-year doctors are being trained adequately.Method: Junior doctors were set an original test at the start of their orthopaedic training to assess their knowledge (this was ...

Conclusions: The survey reveals that the modern surgical trainee is gaining a low and inconsistent level of exposure to day case surgery despite being aware of the importance of this modality of training. An urgent review is required to ensure trainees become actively involved in day case surgery and are not missing on this vital training opportunity.
Specialty choice in UK junior doctors: Is psychiatry the least popular specialty for UK and international medical graduates?

Conclusion:
Based on the number of applicants per place, there was some consistency in the most popular specialties for both UK and international medical graduates, but there were differences in the popularity of psychiatry. With anticipated decreases in the number of new international medical graduates training in the UK, university departments and professional associations may need to review strategies to attract more UK medical graduates into certain specialties, particularly psychiatry and paediatrics.
Aim: The impact of modernising medical careers (MMCs) on the basic surgical training (BST) of oral and maxillofacial higher surgical trainees. Method: A list of surgical trainees was obtained from online forums and websites. Data was gathered from questionnaires regarding current position, year of training, duration and specialties worked at BST level, stage of completion of MRCS and CCBST, completion of basic surgical skills, ATLS and CRiSP courses and satisfaction with training. Comparisons were made between SpR (pre-MMC) and ST (post-MMC) trainees.

Conclusions
In spite of many pressures on training in the clinical setting, the number of cases and senior supervision in specialist modules for trainee anaesthetists in our teaching hospital has been maintained. Continuous monitoring of in-theatre supervision is one way of confirming that training is not compromised as changes occur in hospital workload. (Source: British Journal of Anaesthesia)

opportunities under Modernising Medical Careers (MMC) and compares them with
the existing specialist registrar (SpR) grade; the UK psychiatric educational
literature is reviewed, and trainee concerns reported. Anecdotal reports of
higher training losses under MMC suggest disparity between specialty training
year 4 (ST4) and the first SpR year. The roles, controversies and losses of
protected clinical special interest sessions and the research day are
reviewed. UK psychiatric literature notes under-utilisation and poor
supervision of these highly valued protected training sessions, with
suggestions for improvement. The sacrifice of protected training to service

The objective of the awards is to develop high-quality capacity and secure long-term sustainability and enhanced outcomes in research and teaching at senior lecturer level. Awards will be made in areas of clinical medicine and dentistry.

HEFCE and the local NHS trusts are offering funding for up to 200 posts through five annual rounds of awards. Each post runs for five years and will normally be taken up at any time between 1 August and 31 July of the academic year following the award.

Round four outcomes

In round four, the selection panel recommended that we fund 37 out of the 94 applications we received. The awards were made across 15 institutions and encompassed 20 different medical specialisms. A more detailed breakdown by institution and subject specialism is available.

CONCLUSIONS Accuracy in the diagnosis of the wide range of skin conditions presenting to an out-patient clinic was shown toincrease over a 1-year period. We feel that this improvement resulted from regular clinical exposure supported by a structuredlearning programme. The shortening of the specialist training period may affect the acquisition of diagnostic skills by traineesand impact on the confidence with which they commence consultant practice.

This is a time of rapid change in postgraduate training, the implications
of which are wide ranging. This paper reviews the current situation regarding
dual training in psychiatry. Included is an analysis of utilisation of dual
training in psychiatric specialities gleaned via the College membership data
and an examination of the current employment prospects in relation to dual
training. The impact on dual training of Modernising Medical Careers (MMC),
competency-based training and the possibility of a single Certificate of
Completion of Training (CCT) in psychiatry is considered. Suggestions are then

Postgraduate medical education has been through many changes and reforms in recent years. Under pressure from the UK Department of Health, graduate medical education has developed into a more didactic and formalized model than it had been. These developments, known sometimes as the “Calman” reforms, were designed to bring the UK in line with other European countries. This replaced a loosely organized yet lengthy period of experiential higher training with a shorter, more formalized training, the completion of which enables the trainees to acquire a certificate of completion of training (CCT). On top of all this, more recently the government imposed Modernising Medical Careers (MMC), ostensibly to bridge the gap between medical school and structured specialist training. The way in which...

MedWorm Message: Get the very latest Swine Flu news via the MedWorm Swine Flu RSS news feed - updated hourly from thousands of authoritative health and news sources.

Abstract Traditionally, the UK and Ireland have followed the same postgraduate surgical training of orthopaedic surgeons. Modernising
medical careers (MMC) and European Working Time Directive (EWTD) have radically changed the way surgical training is delivered
in the UK. In Ireland, however, the traditional structure of surgical training system continues with an emphasis to modernise
the training with more objective assessment tools. The aim of this review is to highlight the current differences in the higher
surgical training in Orthopaedics in the UK and Ireland.

AIMS AND METHODS

Following the 2007 Modernising Medical Careers process, an online survey of
applicants for psychiatry training posts in Scotland was undertaken. Views on
short-listing, interview, communication and future recruitment were sought,
qualitative and quantitative analyses being employed.

RESULTS

Of 200 doctors, 115 responded, representing all regions and grades. Over
75% (n=84) expressed lack of confidence in short-listing. There was
no significant difference between responses of successful and unsuccessful
candidates (2=3.66, d.f.=2, P>0.05). A total of
45% perceived interviews as invalid (n=55). About two-thirds (66%,
n=69) felt communication was poor. Constructive suggestions for 2008
were made.

CLINICAL IMPLICATIONS

Domestic medical graduate numbers will almost double between 2005 and 2012, necessitating substantial increases in supervision at prevocational and vocational levels. New approaches to resourcing and governance of training are needed to expand the capacity of the health system to deliver quality training; new settings will also be required to expand training capacity, while ensuring that trainees are exposed to a broad range of clinical experiences. With increasing demand for training placements, entry to specialty training is likely to become highly competitive; new vocational training positions must be created to ensure that bottlenecks in training do not occur and that training is not unnecessarily prolonged. Substantial increases in government funding wil...

Modernising Medical Careers (MMC) asks trainees to commit to their specialty sooner than ever before. There has been a great deal of good-quality research on the career intentions of doctors in training. It is time to review this data with MMC's changes in mind and reflect on how earlier enforced choice may affect trainees. There are distinct patterns to career choice: those who commit strongly and early; those who wait and commit later; and those who move on from previous career choices. The specialty is imperative for some that commit early. Those in this group who fail to be selected will need focused career support. Doctors who commit late are more likely to be affected by quality of life and flexibility. The present 'one size fits all' system does not reflect the...

This report describes an audit carried out in the emergency department of this hospital, specifically looking at the issue of re-attendance and if there is anything that can be done to prevent it. Traditionally, emergency departments in the United Kingdom have been staffed at junior doctor level by senior house officers (SHO); however, with the advent of modernising medical careers in the United Kingdom, departments are being staffed at this level by foundation year 2 doctors (FY2). Whereas SHO range in experience from a year post-qualification to a possible 5–6 years post-qualification, all FY2 doctors have only one year post-qualification clinical experience. In this audit, FY2 doctors, despite seeing fewer patients per head than the middle-grade tier, had a higher level of re-atte...

Today's (Thursday 8 May, 2008) Health Select Committee report on reforms to UK medical training is a damning indictment of the government's failure to listen to doctors, the BMA says. The report criticises the government for pressing ahead with the disastrous implementation of Modernising Medical Careers and ignoring the BMA's repeated calls for a delay from as far back as June 2006. The Douglas Review said changes to medical training and appointment should be introduced only after careful piloting." This has been reinforced by the recommendations in the Tooke report around evidence-based policy making and the need to involve stakeholders in trialling changes, where possible, and also to subject changes to rigorous monitoring and evaluation. The Department of Health is in the process of establishing a number of pilots in order to rigorously test a variety of new methods for recruiting and selecting trainees for specialist training.
The implementation of modernising medical careers (MMC) has resulted in some specialties being allocated very inexperienced trainees such as ophthalmology. We aim to describe the process of implementation of MMC and how it may affect the service provision in smaller specialities such as ophthalmology. A methodical approach in a district hospital setting was used to provide early core training to such trainees involving managerial support. The quality of service provided by newer trainees can be enhanced by providing early structured training during induction to create an atmosphere of enthusiasm and continued learning. This example can be used in other units and specialties. (Source: Postgraduate Medical Journal)

BMA Warns Against Inferior Grade Of Doctor, UK

The BMA today expresses strong opposition to the idea of a new grade of NHS doctor, who would have a specialist qualification, but not the same expertise as a consultant. The warning is part of the BMA's response to Sir John Tooke's interim report on the UK-wide Modernising Medical Careers reforms. The BMA describes the report as "excellent" but argues that a new grade below the level of consultant is not necessary, would not improve the quality of care, nor save the NHS money. Mr Ram Moorthy, chairman of the BMA Junior Doctors Committee, says: "The CMO is supposed to be the link between the government and the medical profession. The BMA surveyed its members on the Modernising Medical Careers (MMC) reforms, which aim to streamline doctors' postgraduate training and, in some cases, shorten the amount of time it takes to qualify as a specialist.
In 1993, the UK General Medical Council published Tomorrow's Doctors leading to a nationwide restructuring of undergraduate medical courses. Traditional courses with distinct pre-clinical and clinical phases gave way to a more integrated approach to undergraduate medical education, with an emphasis on the quality and variety of teaching provided. More than a decade after Tomorrow's Doctors, postgraduate medical training is being transformed. Modernising Medical Careers is leading to the introduction of a two-year Foundation Programme, with subsequent streamlined specialist training. In the context of these changes, we consider how the creation of posts for medically-qualified anatomy demonstrators would present an opportunity to fulfil needs in both undergraduate education and postgraduate...

The new UK Foundation Programme is competency based, with a trainee’s progression dependent on achieving and demonstrating a range of competencies. However, we do not know whether all rotations, comprising different combinations of specialties, will enable the foundation programme trainees to acquire these competencies. Questionnaires were sent to 100 consultants from a range of specialties asking them to predict if a foundation year 1 or foundation year 2 doctor could acquire each competency when working in the consultant’s specialty. Views on the competency framework as a whole were also sought. Results suggest that the competencies trainees may acquire depend on the specialties they experience in their rotation. Moreover, a range of competencies may prove difficult to acquir...
