Conclusions&nbsp; These findings suggest that GABA, the major inhibitory neurotransmitter in the brain, may be implicated in adolescent MDD and, more specifically, in those with anhedonia. In addition, use of a continuous rather than categorical scale of anhedonia, as in the present study, may permit greater specificity in evaluating this important clinical feature.
Conclusions&nbsp;&nbsp;Based on our results, taking into consideration the heavy workload in our hospital, it would seem that more experienced endoscopists
are required to help provide training in ERCP, and that the use of lead acrylic goggles is required to decrease radiation
exposure to the eyes.

Conclusions:
Among patients with SCI, there was a nonlinear relation between the TTO and life expectancy that violated the fundamental assumption of risk neutrality toward life years. Accordingly, TTO utilities elicited for different life expectancies should not be compared in quality-of-life assessments or used in cost-utility analyses. In contrast, SG utilities remained stable at different lengths of life expectancy in SCI patients. Moreover, certain patient characteristics such as age, time lapse since the injury, neurologic severity, risk attitudes, and depressive status can account for some TTO and SG variations among patients with SCI. The outcome is dependent on the available human and material resources. The University of Calabar Teaching Hospital is a 410-bed hospital. It has a 3-bed general ICU consisting of 2 adult and 1 pediatric beds. A retrospective analysis of all ICU admissions as well as the mortality rate during a 12-month period that spans April 2009 and March 2010 was done. The data were collected from the ICU admissions and nurses' report books. The data extracted were the patients ages, stratified to pediatric (0-18 years) and adult (>18 years); the source of admission, primary diagnosis, the duration of admission, and the patients who were ventilated were also noted.
This study evaluated the effectiveness of two commercial e-prescribing systems in reducing prescribing error rates and their propensities for introducing new types of error. Methods and Results We conducted a before and after study involving medication chart audit of 3,291 admissions (1,923 at baseline and 1,368 post e-prescribing system) at two Australian teaching hospitals. In Hospital A, the Cerner Millennium e-prescribing system was implemented on one ward, and three wards, which did not receive the e-prescribing system, acted as controls. In Hospital B, the iSoft MedChart system was implemented on two wards and we compared before and after error rates. Procedural (e.g., unclear and incomplete prescribing orders) and clinical (e.g., wrong dose, wrong drug) errors were identified. Presc...

Conclusions. These divergent origins of influence on guideline adherence point to different approaches for improvement. As habits strongly influence staff members, methods that focus on changing habits (e.g., automated decision support systems) are possible interventions. As residents&rsquo; intention seems to be guided mainly by external influences and experienced control, this may make feedback, convenient guideline formats, and guideline familiarization more suitable.
Conclusion&nbsp;&nbsp;Necrotizing enterocolitis has affected neurodevelopmental outcome but not growth in VLBW infants at 18–24&nbsp;months of corrected
age. Both those treated medically or requiring surgery need close neurodevelopmental follow-up for the first years of life.

According to a medical report issued from the Royal Victoria Teaching hospital (RVTH), Yaya was noted to be restless, with marked deformity of the anterior cervical and thoracic region. The report further stated that sub-mental region was attached to the extensive thoracic hypertrophic scars, as a result he was unable to move his neck reasonably without moving his trunk.
Abstract&nbsp;&nbsp;Patients in England and Wales with rheumatoid arthritis (RA) receive treatment from the National Health Service (NHS) with
therapies approved by the European Medicines Agency (EMA), under guidance from the National Institute for Health and Clinical
Excellence (NICE). This document overviews the current NICE guidelines for the treatment of RA and identifies scenarios when
such guidance may not represent the optimum management strategy for individual patients. Specifically, we consider the use
of tocilizumab or abatacept as the most appropriate treatments for some patients. In such scenarios, it may be possible for
the clinician to secure access to the required therapy through an application procedure known as an ‘individual funding request’,

Abstract&nbsp;&nbsp; The aim of this study was to establish if there are any symptoms which can predict increased patient satisfaction following
Endoscopic Sinus Surgery (ESS) and whether these symptoms correlate with Lund-Mackay score on Computerised Tomography (CT).
A prospective observational study was performed. Ninety-three consecutive patients who were offered ESS were recruited from
an otolaryngology department in a UK Teaching Hospital. All patients had failed medical therapies for chronic rhinosinusitis
(CRS), recurrent acute sinusitis and/or nasal polyposis. Patients were asked to complete a questionnaire pre-operatively and
12&nbsp;months after surgery. Symptoms were assessed using a visual analogue scale. Endoscopic examination of the nose was performed

Considerations in developing and delivering a non-pharmacological intervention for symptom management in lung cancer: the views of health care professionals

Discussion&nbsp;&nbsp; The participants agreed that the novel symptom management NPI should be individually personalised to the needs of each patient
and be available for patients when they become receptive to it. Moreover, they agreed that the intervention would be most
effective if delivered to patients individually rather than in groups, outside acute medical settings where possible and closer
to patient’s homes, should be delivered by an HCP rather than a trained volunteer or lay person and should involve informal
carers wherever practicable.

Conclusions: Elderly account for a high proportion of hospitalizations, mainly determined by critical health conditions, advanced age and functional dependence. Poor health conditions (high comorbidity and presence of chronic multi-organ diseases), functional dependence but not critical social factors were the main determinants of multiple hospital admissions.
Context A complaint of acute onset headache mandates that subarachnoid haemorrhage (SAH) be excluded as the cause. Because of the less than perfect sensitivity of unenhanced head CT, standard practice is to follow a negative CT with lumbar puncture (LP) to exclude subarachnoid haemorrhage. Improvements in CT technology have raised the question of whether modern third-generation CT scanners are sensitive enough to adequately exclude subarachnoid haemorrhage. Methods This was a prospective multicentre cohort study conducted at 11 university affiliated tertiary care teaching hospitals in Canada between November 2000 and December 2009. Consecutive patients aged 15 or older who presented with acute non-traumatic headache or with syncope associated with a headache were eligible for the study. CT...

In this study, a significant number of patients were both under- and over-treated. Moreover, the class of antibiotics most commonly prescribed – quinolones – is known to have limited activity against enterococci. Both the authors and the invited commentators conclude that, due to the rarity of infectious complications associated with Enterococcus, ABU ought not to be treated. The results from this study support current efforts to curtail inappropriate antimicrobial use through stewardship programs, focused antimicrobial use campaigns and mindful practice...
Conclusion&nbsp;&nbsp;Electrocardiographic features consistent with atrio-ventricular and ventricular repolarization abnormalities are associated
with CAN in sickle cell anaemia. Further studies are required to evaluate the prognostic implications of these findings in
sickle cell patients with cardiovascular autonomic dysfunction.

Does smoking kill? A study of death certification and smoking

Conclusion
This study demonstrates that smoking is rarely cited on death certificates, even in cases where the causal link with smoking is very strong. There are many reasons why smoking is not cited on death certificates. One frequently cited reason is the reluctance of doctors to stigmatise the deceased. Interestingly, such reluctance did not extend to citing alcohol as a COD. By not recording smoking on death certificates doctors are failing to gather important epidemiological and pathological data.
Abstract:An 8‐year‐old male neutered Pomeranian dog was presented to the Veterinary Teaching Hospital at Oregon State University for surgical treatment of hydronephrosis of the left kidney and a left cranial abdominal mass. A primary ureteral mass was found during exploratory surgery, and the mass was resected and ureteral anastomosis was performed. Cytologic evaluation of the mass revealed 3 distinct cell populations, including a large number of multinucleated giant cells, a moderate number of thin spindle‐shaped cells, and cohesive clusters of transitional epithelial cells. The cytologic diagnosis was giant cell sarcoma. The diagnosis was confirmed by histologic examination, and immunohistochemical staining was performed. The spindle‐shaped cells and multinucleated giant cells we...

    Abstract
White light bronchoscopy [WLB] has been used for identification and localization of intra-epithelial pre-neoplastic and neoplastic lesions within the bronchus. Aim of the study was to evaluate the uses of WLB to detect and localize the precancerous and cancerous lesions, and in addition to analyze morphologic presentation, and association to histological type and the variation between genders.A total of 4983 patients were examined by WLB from 2004 to 2009 in a local tertiary teaching hospital. The following parameters were collected: morphological presentation, biopsy sites, histology. The patients' records of age, sex, smoking status, blood-gas, X-RAY/CT, CB...

Abstract&nbsp;&nbsp;A male 2.5-year-old American short hair cat was admitted to the animal hospital with the primary complaints of abdominal distension
and depression for several days. Ultrasound revealed a large hypoechoic cavity with a well demarcated thin wall on a liver
lobe. On exploratory laparotomy, a hepatic haematoma was observed. Subsequently, hepatic lobectomy was performed. Histologically,
an eosinophilic substance stained with Congo red was deposited in the cyst wall which showed an optical property of apple
green birefringence under polarized light. Based on immunohistochemical staining, amyloid A was diagnosed. To our knowledge,
this is the first report of a spontaneous hepatic haematoma with an amyloid A deposition in a cat.

Abstract&nbsp;&nbsp; The Q248H mutation in the gene SLC40A1 which encodes for the cellular iron exporter ferroportin is relatively common in Africa.
This mutation has been associated with resistance to hepcidin and therefore we hypothesized that iron-related parameters and
the prevalence of opportunistic infections in HIV might be influenced by the Q248H mutation. We conducted a cross-sectional
study among 200 HIV-positive women in the Butare University Teaching Hospital in Rwanda. Polymerase chain reaction (PCR) and
restriction enzyme digestion were used to identify the Q248H mutation. Physical examination was carried out and WHO HIV disease

Conclusion:
Global health research should provide benefits that address both the micro and macro level issues of justice in order to forestall exploitation. Embracing the two is however challenging in terms of how the various competing interests/needs should be balanced ethically, especially in the absence of structures to guide the process. This challenge should point to the need for greater dialogue to facilitate value clarification among stakeholders. (Source: Philosophy, Ethics, and Humanities in Medicine)

    Abstract
Background and purpose Treatment of hip fracture patients is controversial. We implemented a new operative and supervision algorithm (the Hvidovre algorithm) for surgical treatment of all hip fractures, primarily based on own previously published results. Methods 2,000 consecutive patients over 50 years of age who were admitted and operated on because of a hip fracture were prospectively included. 1,000 of these patients were included after implementation of the algorithm. Demographic parameters, hospital treatment, and reoperations within the first postoperative year were assessed from patient records. Results 931 of 1,000 operative procedures were performed according to the a...

Conclusion&nbsp; The computerized synoptic operative report is superior to the dictated report in the documentation of important preoperative and intraoperative data. Although checklists and templates are not new in medicine, the use of a synoptic operative report as a surgical educational tool is a novel concept. Each resident who participated in our study had to develop a better understanding of the operative procedure in order to complete a more accurate synoptic report.
Conclusions: Secular trends prior to the 2003 work hour reform may have caused a marked decrease in operative trauma experience among general surgery residents. Radiologic advancements in intervention and imaging may have had a role in this decline. Rare cases, such as cardiac, pancreatic, and GU trauma, remain rare. Post work hour reform, operative trauma case volumes have stabilized. (Source: Journal of Surgical Research)

Conclusions: The implementation of electronic discharge summaries for patients discharged from surgical services improved the timeliness of summary completion without sacrificing quality. Excessive copy and paste can reduce the readability of discharge summaries and strategies to discourage this practice should be employed. (Source: Journal of Surgical Research)

Conclusions&nbsp;&nbsp;Identifying AKI associated with DEG is difficult. Detailed drug history, increasing metabolic acidosis, and multiorgan deterioration
despite peritoneal dialysis should arouse suspicion. Simple diagnostic tests need to be developed and facilities for hemodialysis
of infants and financial support provided. Recurrences can be prevented by creating awareness, improving manufacturing practices,
field-testing of drugs, and international monitoring of pharmaceuticals imported for manufacture.

Background:
Loss of basal forebrain cholinergic neurons is attributable to the proapoptotic signaling induced by nerve growth factor receptor (NGFR) and may link to Alzheimer's disease (AD) risk. Only one study has investigated the association between NGFR polymorphisms and the risk of AD in an Italian population. Type 2 diabetes mellitus (DM) may modify this association based on previous animal and epidemiologic studies.
Methods:
This was a case-control study in a Chinese population. A total of 264 AD patients were recruited from three teaching hospitals between 2007 to 2010; 389 controls were recruited from elderly health checkup and volunteers of the hospital during the same period of time. Five common (frequency>=5%) haplotype-tagging single nucleotide polymorphisms (htSNPs) were selec...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Find the best <a href="http://www.januarysales.org/" target="_blank">January Sales</a> in the UK.</p></div>

Conclusion
This research suggests that rates of hypoparathyroidism complications after thyroidectomy surgery are lowest when surgeons are aged 35–50 years old compared with younger and older colleagues. However, the surgeons’ age was not related to the complication of laryngeal nerve palsy.
This study raises the question of whether complication rates after thyroid surgery may be related to surgeon experience or age. However, significant limitations mean that these research findings may not be applicable to all surgeons or surgical procedures.
There are many issues to consider when interpreting the results. The study looked at just one surgical procedure – thyroidectomy. This study does not provide information on the many thousands of other surgical procedures carried out or those ...

Conclusions: This theory advances the nursing perspective of uncertainty in clinical practice. The theory is relevant to nurses who are faced with uncertainty and complex clinical decisions, to managers who support nurses in their clinical decision‐making, and to researchers who investigate ways to improve decision‐making and care delivery.
Abstract&nbsp;&nbsp;Hemifacial spasm (HFS) is a common neurological disorder characterized by involuntary tonic and clonic contractions of the
muscles innervated by the facial nerve. We aimed to describe clinical features, common antecedents, triggers and relieving
factors in patients with hemifacial spasm to study the correlation of hypertension and HFS, and to compare clinical features
of primary and secondary cases of HFS. The data for the study were collected prospectively on a predesigned and pre-tested
format at the first attendance in all consecutive HFS patients attending the movement disorders clinic of a tertiary teaching
hospital in India. The demographic profile, HFS symptoms, antecedent illnesses and neurological examination were recorded
and analyzed. Muscle power in in...

in a tertiary care and teaching hospital of Armed Forces Medical Services of India over a period of three and half years.
Hearing loss was the most frequent indication for HRMDCT and congenital bilateral profound sensorineural hearing loss for
evaluation prior to possible cochlear implant formed the single largest group (62, 42%) among all indications for HRMDCT of
temporal bones. Major vestibule-cochlear and semicircular canal anomalies were noted in 11 such cases. Seven patients of microtia
were studied and all had positive CT scan findings. All patients of vertigo had normal HRCT study. Extent of temporal bone
injuries and inflammatory conditions were clearly delineated in all cases. Thinner collimations allowi...

Burian, who not only laid the foundations of this branch in Czechoslovakia, but also influenced many contemporary plastic
surgeons. Burian spent 6&nbsp;years as a military surgeon in the Balkan wars and in World War I. Up until World War I, only separate
plastic operations by excellent surgeons were mentioned. The experience gained from the First World War led to an undreamed-of
development in plastic surgery and to the need for specialisation. It was during the First World War that European plastic
surgery was formed as a special branch. Burian probably opened the first department of plastic surgery in the world at the

In this study, 70 patients on MHD in the teaching hospitals in Urmia were divided into two groups and were requested to fill in the validated SF-36 QoL questionnaire. One group of 35 patients received dietary counseling while the other did not and acted as controls. The SF-36, a short-form QoL scoring system consists of 36 questions that are compressed into eight multi-item scales covering all aspects of QoL. The two groups studied were similar in age, level of education, gender and duration of dialysis treatment; 46.8% of the patients were female and 52% were male. The total SF-36 score was slightly higher in males compared with females, but this difference was not statistically significant (P = 0.05). The scores were higher in the group counseled about diet.
The authors report their initial experience with propranolol, a nonselective β-blocker, used to treat subglottic hemangiomas (SGHs) in 10 patients referred to 1 of 2 pediatric teaching hospitals in Sydney, Australia, and Auckland, New Zealand. Subglottic hemangiomas remain uncommon but may cause airway compromise, with tracheostomy required in 39% in addition to traditional treatment with corticosteroids and/or surgical excision.
, a provision of the Affordable Care Act, aims to bring transparency to the practice of pharmaceutical and device companies providing meals, gifts, and payments to physicians. It requires that these companies, as well as biologic manufacturers and medical suppliers, report any “payment or other transfer of value” of $10 or greater to physicians and teaching hospitals. The manufacturer must report alltpayments to a single physician if they reach $100 in aggregate value for a year. The Centers for Medicare and Medicaid Services released a proposed ruleonn December 2011 outlining how the disclosures would work. Drug and device manufacturers can hold off collecting information until the CMS issues a final rule later this year.
Abstract&nbsp;&nbsp;Interleukins (ILs) are key mediators of the immune response and inflammatory process. Plasma levels of IL-10, IL-1Ra, and
IL-6 are associated with metabolic conditions, show large inter-individual variations, and are under strong genetic control.
Therefore, elucidation of the genetic variants that influence levels of these ILs provides useful insights into mechanisms
of immune response and pathogenesis of diseases. We conducted a genome-wide association study (GWAS) of IL-10, IL-1Ra, and
IL-6 levels in 707 non-diabetic African Americans using 5,396,780 imputed and directly genotyped single nucleotide polymorphisms
(SNPs) with adjustment for gender, age, and body mass index. IL-10 levels showed genome-wide significant associations (p &lt; 5 × 10−8) with e...

Conclusion&nbsp;&nbsp; The longevity of treated teeth based on tooth survival was considerably greater than that of periapical healing. Both outcome
measures were affected by a number of socio-demographic, pre-, intra-, and post-operative factors.

Clinical relevance&nbsp;&nbsp;Root canal-treated teeth may continue to function for a considerable period of time even though there may be radiographic
periapical lesion present. Decision for extraction may be due to reasons other than a failure of the periapical tissues to
heal.

Abstract:
Retained surgical items were the most frequently reported sentinel event in 2010, according to The Joint Commission. Perioperative nurse leaders at Children's Hospital Boston, a pediatric teaching hospital, conducted a quality improvement initiative to reduce or eliminate incorrect counts and count discrepancies, which increase the risk of an item being unintentionally retained after surgery. Work included educating the perioperative staff members, standardizing count practices, formally reviewing every reported count discrepancy with the nursing team, and reviewing and revising the count policy for prevention of retained surgical items. The initiative reduced the number of incorrect counts and count discrepancies by 50% between 2009 to 2010. These initiatives continue to be exp...

    Abstract
The pandemic H1N1/09 emerged rapidly in Korea. Here, we describe the clinical characteristics of outpatients in Seoul, Korea who were infected in the 2009 H1N1 pandemic. We reviewed the cases of outpatients with pandemic H1N1/09 who visited a tertiary care teaching hospital between September 1 and December 31, 2009. Infection with pandemic H1N1/09 was confirmed by molecular tests. Of a total of 7,182 tests, 3,020 (42.0%) were positive. Compared with 473 cases of influenza- like illness (ILI), the 586 confirmed cases of pandemic H1N1/09 differed in age [odds ratio (OR) 0.975] and fulfilling at least one of the following factors: age &lt;5 or ≥65 years, history of contact with other pandemic H1N1/09-infected individ...

    Abstract
The resistance phenotypes and genomic diversity of 185 Acinetobacter baumannii isolates obtained from the intensive care unit (ICU) of a local teaching hospital in Kuala Lumpur from 2006 to 2009 were determined using antimicrobial susceptibility testing and pulsed-field gel electrophoresis (PFGE). Antibiogram analyses showed that the isolates were fully resistant to β-lactam antimicrobials and had high resistance rates to the other antimicrobial agents tested. However, the isolates were susceptible to polymyxin B. Resistance to cefoperazone/sulbactam was only detected in strains isolated from 2007 to 2009. Some environmental isolates and an isolate from the hands of a healthcare worker (HCW) had identical resistance profile...

Background:
Worldwide 15.5% of neonates are born with low birth weight, 95.6% of them in the developing countries. Prematurity accounts for 10% of neonatal mortality globally. The purpose was to evaluate the effects of birth weight and gender on neonatal outcome.FindingsThe data of 278 neonates managed in the Special Care Baby Unit (SCBU) of Jos University Teaching Hospital (JUTH) over a 2 year period from July 2006 to June 2008 were analyzed. One hundred and fifty nine (57.2%) were males and 119(42.8%) females. There were 87(31.3%) preterm and 191 (68.7%) term babies. Twelve of the babies died. Seven (2.52%) and 5 (1.80%) being males and females respectively. The neonatal mortality rate by gender was not significant (p > 0.05). The neonatal mortality was 25.2 deaths per 1000 live births fo...

CONCLUSIONS: This study suggests a high rate of noncompliance with scheduled out-patient endoscopy, particularly for colonoscopy. Because this may be a contributing factor to colorectal cancer disparities, increased community outreach on colorectal cancer education is needed and may help to reduce noncompliance.

This study uses hospital administrative data to ascertain the differences in the patient characteristics, process and outcomes of care between the Emergency Department (ED) triage categories of patients admitted from an ED presentation into a large metropolitan teaching hospital with a Stroke Care Unit. Bayesian Networks (BNs) derived from the administrative data were used to provide the descriptive models. Nearly half the patients in each stroke subtype were triaged as &lsquo;Urgent&rsquo; (to be seen within 30 minutes). With a decrease in the urgency of triage categories, the proportion admitted within 8 hours decreased dramatically and the proportion of formal discharge increased. Notably, 45% of transient ischaemic attacks (TIAs) were categorized as &lsquo;Semi-urgent&rsquo; (to be att...

Posterior myocardial infarction: are we failing to diagnose this?

Conclusions
A significant proportion of doctors and paramedics were unable to diagnose PMI. Hence, the majority of PMIs may be being missed. Routine use of posterior-leads in the standard assessment of patients with chest pain may identify up to an additional 7% of STEMIs, allowing prompt reperfusion therapy, which would reduce morbidity and mortality.
Predictive Factors of Splenic Injury in Colorectal Surgery: Data from the Nationwide Inpatient Sample, 2006-2008 [Original Article]

Conclusions&nbsp; Type of resection (transverse, total, or left colectomy), type of pathology (malignancy or diverticulitis), open operation, and teaching hospital are potent independent predictors of splenic injury. Male sex, peripheral vascular disease, and emergent admission are less effective predictors. Surgeons should be aware of these risk factors and inform patients accordingly. In higher-risk circumstances, it may be appropriate to consider prophylactic vaccination. (Source: Archives of Surgery)

Hepatic Resection vs Minimally Invasive Radiofrequency Ablation for the Treatment of Colorectal Liver Metastases: A Markov Analysis [Poster Session]

Conclusions&nbsp; Classical Markov analysis demonstrates that based on current literature, resection is superior to RFA in the treatment of colorectal liver metastases. When input is limited to laparoscopic RFA in patients with resectable lesions, projected 5-year survival is superior to that of hepatic resection. (Source: Archives of Surgery)

Conclusion&nbsp; Carbon dioxide digital subtraction angiography is a versatile technique that can be safely used for diagnostic and therapeutic endovascular procedures. Morbidity and mortality are acceptable with preservation of renal function. Thus, CO2-DSA is a safe alternative to iodinated contrast. (Source: Archives of Surgery)
