by Michael Agus, MD - director of Children&#8217;s Medicine Critical Care Program
A few weeks ago, multiple waves of teams from Children&#8217;s Hospital Boston left under the auspices of Project Hope for the USNS Comfort, which remains anchored in the harbor of Port-au-Prince. As Comfort&#8217;s mission began winding down, Robert Truog, MD was able to transfer and join a land-based effort already underway through Partners In Health (PIH). I was scheduled for the Comfort as well and due to Bob&#8217;s efforts was able to re-deploy to the same site right after he left.

Conclusion&nbsp;&nbsp; This study showed that alveolar procalcitonin performed by BAL does not help the clinician to identify VAP. It confirmed that
serum procalcitonin is not an accurate marker of VAP. In contrast, microbiological resources available at the time of VAP
suspicion (BAL gram staining, last available endotracheal aspirate) combined or not with CPIS are helpful in distinguishing
VAP diagnosed by BAL from patients with a negative BAL.

Conclusion: This study strongly highlights the need for patient education or counselling on use of antidiabetic and concomitant drugs, monitoring of blood glucose and glycosylated haemoglobin (HbA1c) levels, diet control, and correction of diabetic complications. Metabolic control was poor and HbA1c monitoring was underutilized. Clinical monitoring of patients' adherence to prescribed treatments is recommended and measures should be taken to improve it. The provision of parenteral nutrition (PN) to 'stressed' patients often results in hyperglycaemia, which may be detrimental. In animal models limited amounts of enteral nutrition (EN) improve intestinal integrity and stimulate intestinal incretin production, which may lead to improved glucose control. We set out to assess if combining EN with PN results in improved glucose homeostasis rather than PN given alone. We conducted a randomised trial in a university teaching hospital of patients undergoing a 'curative' oesophagectomy for adenocarcinoma. Differences between the two intervention groups were assessed for continuous glucose measurement, insulin sensitivity using insulin tolerance...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Conclusions&nbsp; The influence of size and location of the tumors on their propensity to cause seizures varies with the grade of the tumor. In high-grade gliomas, rapidly growing tumors, particularly those situated in deeper structures, present with non&ndash;seizure-related symptoms. In low-grade gliomas, lesions in the temporal lobe or the insula grow large without other symptoms and eventually cause seizures. Quantitative image analysis allows for the mapping of regions in each group that are more or less susceptible to seizures. (Source: Archives of Neurology)

Conclusions:
Sputum smear reversion develops in 10% of patients with smear-positive pulmonary TB, with 30% due to viable M. tuberculosis bacilli. Isolation and regimen modification may not be necessary for all drug-susceptible patients who already have radiographic improvement and develop smear reversion after two months of treatment or after sputum culture conversion.
Conclusions
In patients with cirrhosis, refractory ascites and type 1 HRS not responding to vasoconstrictor treatment, MARS is ineffective in improving systemic haemodynamics and renal function despite reduction in NO levels, suggesting that vasodilatation in advanced cirrhosis is not due to excess systemic vasodilators alone. Transient reduction in serum creatinine indicates direct removal by MARS, and may not represent improved renal function. (Source: Gut)

Summary: Objectives: To evaluate the application of the Sequential Organ Failure Assessment (SOFA) in describing the severity of organ dysfunctions and the associated mortality rates in critically ill patients at a teaching hospital.Research methodology: Prospective longitudinal study performed in 1164 adult, critically ill patients who were admitted consecutively into intensive care units between January 2004 and December 2005. We analysed static evaluation of SOFA and dynamic changes in the SOFA scores. The discriminative power of SOFA was evaluated using ROC curves. Results: There was an increase in the mortality rate when the SOFA scores increased (, p (Source: Intensive and Critical Care Nursing)

Conclusions:
The incidence of Gram-negative bacteremia was significantly higher in bacteremic ICU patients with septic shock than in those with sepsis or severe sepsis. Furthermore, CRP and IL-6 levels were significantly higher in Gram-negative bacteremia than in Gram-positive bacteremia. These findings suggest that differences in host responses and virulence mechanisms of different pathogenic microorganisms should be considered in treatment of bacteremic patients, and that new countermeasures beyond conventional antimicrobial medications are urgently needed.
(NaturalNews) In the first Paxil birth defect trial against GlaxoSmithKline, much of evidence focused on the doctors on Glaxo's payroll involved in the corruption of the medical literature and seminars given to promote the off label use of Paxil with pregnant and nursing mothers. On October 13, 2009, the trial of Kilker v Glaxo ended with a Philadelphia jury awarding $2.5 million in compensatory damages to the family of Lyam Kilker, after finding that Glaxo "negligently failed to warn" the doctor treating Lyam's mother about the risks of Paxil and the drug was a "factual cause" of the child's heart defects.Glaxo's lead attorney at trial was King and Spalding partner Chilton Varner, and the family's lead attorney was Sean Tracey from Houston.During his opening statement on September 15, 2009...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

This study aims to evaluate medical student and intern awareness of ionising radiation exposure from common diagnostic imaging procedures and to suggest how education could be improved. Fourth to sixth year medical students enrolled at a Western Australian university and interns from three teaching hospitals in Perth were recruited. Participants were asked to complete a questionnaire consisting of 26 questions on their background, knowledge of ionising radiation doses and learning preferences for future teaching on this subject. A total of 331 completed questionnaires were received (95.9%). Of the 17 questions assessing knowledge of ionising radiation, a mean score of 6.0 was obtained by respondents (95% CI 5.8[ndash]6.2). Up to 54.8% of respondents underestimated the radiation dose from c...

Flexible working and the contribution of nurses in mid-life to the workforce: A qualitative study

Conclusions: The findings suggest that there are limits to the implementation of flexible working for nurses. In some areas there is evidence that the implementation of flexible working may be producing an inflexible workforce as older nurses are required to compensate for the flexible working patterns of their colleagues. Ward managers have a key role in the implementation of family friendly policies and require support to fulfil this role. There is a need for creative solutions to address implementation of flexible working for all nurses to ensure that workforce policy addresses the need to retain nurses in the workforce in a fair and equitable way. of 42 dogs with CRF and 34 age- and sex-matched clinically healthy control dogs. The level of AT III in the CRF group was
significantly lower (P &lt; 0.05), but the fibrinogen concentration was significantly higher (P &lt; 0.05) than in the control group. Additionally, the cholesterol level in the CRF group was significantly higher than in the
control group (P &lt; 0.05) and was positively correlated to creatinine (R = 0.5, P &lt; 0...

The objective of this study was
to determine the clinical, laboratory, and radiographic features of patients with JIA, as well as to highlight their management.
A total of 23 cases (F-16:M-7) were seen over an 8.5-year period. Polyarticular presentation was the commonest while systemic
onset was the least. The mean age at presentation was 12.7&nbsp;years. Most of the patients presented late with mean of 3.7&nbsp;years
before presentation. The knees and ankles were most commonly involved. ESR was elevated in most of the cases. Rheumatoid factor
and Antinuclear antibodies were invariably negative. Treatments were with standard drugs. Although JIA is uncommonly seen
in our population, the clinical and laboratory features are similar as elsewhere, though there are some differences.

This case series describes five female dogs with congenital ectopic ureter presenting with delayed-onset urinary incontinence out of 22 female dogs diagnosed with ectopic ureter over a 102-month period at a veterinary teaching hospital. All five dogs improved following surgical treatment of ectopic ureter, but only three remained continent long-term without further intervention. Delayed-onset urinary incontinence in female dogs with ectopic ureters may be more common than the current literature suggests. Congenital ectopic ureter should be considered and investigated as a possible contributing factor in female dogs presenting with delayed-onset urinary incontinence.
Physician Order Entry Or Nurse Order Entry? Comparison of Two Implementation Strategies for a Computerized Order Entry System Aimed at Reducing Dosing Medication Errors

Conclusions: NOE can increase physicians' compliance with warnings and recommended dose and frequency and reduce nonintercepted medication dosing errors in the neonatal ward as effectively as POE or even better. In settings where there is major physician resistance to implementation of CPOE, and nurses are willing to participate in the order entry and are capable of doing so, NOE may be considered a beneficial alternative order entry method.
Summary: A cross-sectional study was conducted during the period of August 2007–April 2008 at Al-Wahda Teaching Hospital in Yemen to investigate prevalence and risk factors for placental malaria and anaemia and their effects on birthweight. Sociodemographic characteristics were gathered, maternal haemoglobin was measured and blood films were examined for malaria. Newborn birthweight was recorded. Out of 900 parturient women, malaria blood films were positive in 32 (3.6%) cases: in six sets of peripheral, placental and cord samples; in 15 placental and cord samples; and in 11 placental samples only. Malaria was not associated with age and parity, but it was significantly associated with history of fever [odds ratio (OR) 8.5, 95% CI 3.7–19, P (Source: Transactions of the Royal Society of...

Abstract&nbsp;&nbsp; The operative deliveries can expose the fetus to acute and systemic hypoxia along with an increase in perinatal morbidity.
The aim of this study was to reveal any relationship between delivery type and Chitotriosidase and Troponin T levels in cord
blood. Ninety babies born in Ankara Etlik Maternity and Women’s Health Teaching Hospital were involved in the study. The babies
were divided into three groups; Group 1: Normal vaginal; Group 2: Caesarean section; Group 3: Forceps application. Cord blood
samples were drawn from umbilical arteries of the babies soon after the birth. Chitotriosidase enzyme activities in group
3 (141 nmol/ml/h (0–246)) were found higher than groups 1 (100 nmol/ml/h (0–208)) and 2 (91 nmol/ml/h (0–202)) (p&lt;0.01 and

Black women are at significantly increased risk for developing a potentially deadly weakening of the heart muscle around the time of childbirth, researchers report. A study examining the incidence of peripartum cardiomyopathy in women who gave birth at a Medical College of Georgia's teaching hospital between July 2003 and July 2008, showed that while 55 percent of the women were white, 93 percent of those who developed cardiomyopathy were black, said Dr. Mindy B. Gentry, an MCG cardiologist... (Source: Health News from Medical News Today)

Abstract&nbsp;&nbsp; One out of every five children suffering from ureteropelvic junction obstruction (UPJO) requires pyeloplasty. This prevalence
indicates an urgent necessity to identify high-grade UPJO as early as possible to avoid renal damage. A novel non-invasive
proteomic urine test has recently been introduced that is able to detect these patients at an early stage. In the study reported
here, we tested this approach to assess its use in our centre and to expand its application to older children. Twenty-seven
children (median age 0.4&nbsp;years, range 0.1–8.8&nbsp;years) with hydronephrosis who had been scheduled a nuclear diuretic renal scan
(DR) to identify urodynamically relevant UPJO were included in our prospective study. Patients with prior surgery of the urinary

Abstract&nbsp;&nbsp;Depression and coronary heart disease (CHD) are significant contributors to the burden of disease in both developed and developing
countries. Although depression seems to be a marker of increased risk after the diagnosis of CHD, it is currently unclear
whether depression can be considered as an independent risk factor and whether its treatment lowers the risk. We review the
data from prominent trials and recent analyses in regard to the association of depression with CHD. We also review some of
the mechanisms that might contribute to this association.

Conclusions There is a need for effective training programmes to assist nurses in working together within a nursing team and an interdisciplinary ward team. The SCN and the PA models of care have been found by nurses to support most aspects of interdisciplinary and intradisciplinary communication. The applicability of both models of care to wards with a varying skill mix of nurses is suggested. Further studies of larger samples with varying compositions of skill mix and varying models of care are required.Implications for nursing management Nurse managers can use varying models of care to support interdisciplinary communication and enhance patient safety.
Conclusions
Prescribing and medication administration errors are not uncommon in paediatrics, partly as a result of the extra challenges in prescribing and administering medication to this patient group. The causes and extent of these errors need to be explored locally and improvement strategies pursued. (Source: Archives of Disease in Childhood)

An audit of local practice was undertaken as regards requests for thyroid function tests (TFT) in children aged between birth and 3 months at two local hospitals in South West England between 2005 and 2008. A total of 406 tests were performed (2.6 tests per week) over a 2-year period at a teaching hospital (70 tests per 1000 live births yearly), with 233 tests (1.5 tests per week) performed over a 3-year period at a local district hospital (39 tests per 1000 live births yearly). The highest proportion of all the tests was performed as routine investigation of prolonged neonatal jaundice (64% and 55%, respectively). Other common indications were maternal thyroid disorders (5.4% vs 4.7%) and suspected neonatal thyroid disorders (3% vs 9%). There was no confirmed diagnosis of infantile thyroi...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Conclusions:
Utilization of advanced brain imaging and invasive diagnostic testing was greater at the university hospital, but was not associated with improved clinical outcomes. This could not be explained on the basis of stroke severity or patient characteristics. This variation of practice suggests substantial opportunities exist to reduce costs and improve efficiency of diagnostic resource use as well as reduce patient exposure to risk from diagnostic procedures.
To the Editor: Inpatient dermatology consultation remains an essential component of residency training; with the decline in direct admissions to dermatology services, dermatology inpatient care is provided more often in the consultative setting and can directly impact accurate diagnosis and management of patients. In 1994, Falanga et al reviewed 591 dermatology consults to a teaching hospital, concluding that common diseases may not be recognized or diagnosed correctly by non-dermatologists; misdiagnosis occurred in 78%, and two thirds resulted in treatment change. Nahass et al offered skin exams to medical inpatients; 35.9% of 231 had cutaneous findings, with one third of these directly related to systemic disease and/or reason for hospitalization. Recently Helms, Helms, and Brodell discu...

Abstract&nbsp;&nbsp;Nephrotic syndrome is known to cause venous thromboembolism (VTE) due to urine loss of antithrombin III and activation of
the coagulation system. We hypothesized that the degree of proteinuria may predict the development of VTE. This was a retrospective
case-controlled study of in-patients urban academic teaching hospital from April, 2007 to March, 2009 and who had undergone
an imaging study for VTE. All radiology reports (N&nbsp;=&nbsp;1,647) for CT angiography of chest and Doppler sonogram of extremities
were reviewed. The following data were collected: race/ethnicity, degree of proteinuria on urinalysis, serum protein and albumin
levels, risk factors for VTE and renal function. The study population consisted of 284 patients with VTE and 280 age/sex matched

Conclusion&nbsp;&nbsp;Current recommendations for treatment of SPN of the pancreas rely mainly on case series as single institutional experiences
and retrospective reviews. Although the level of evidence is relatively low, the way of management discussed above is likely
to provide an excellent prognosis in typically young patients with SPN.

whom basal cell carcinoma (BCC) is most frequent. We evaluated all patients with a histologic diagnosis of skin malignancy
of the head and neck presenting to the University of Calabar Teaching Hospital, Calabar from January 2005 through December
2008. This was compared with total skin malignancy during the same period. Seventy patients presented with skin malignancy
during the study period. The 15 patients with lesions located on the head and neck accounted for 21% of skin malignancy seen
during the same period. SCC 7 (46%) ranked first while the second was Kaposi sarcoma (KS), 4 (27%), all human immunodeficiency

This study aims to investigate the nature and resolutions of patient complaints and further to explore the use of complaints to drive quality improvement in a selected hospital in Taiwan. A teaching hospital (i.e. the Case Hospital) in Taiwan was purposefully chosen for a case study. The author conducted the critical incident technique (CIT) using questionnaires to obtain information about the complaints and the process of their resolutions. To enhance the reliability of the study, the author also conducted non-participant observations as an outsider at the Case Hospital. In this study, 59 complainants registered 87 complaints. The CIT found that care/treatment, humaneness and communication were the most common causes of complaints. The response time of patient complaints averaged 1.76 day...<div id="medworm"><p><b><i>MedWorm Message:</i></b> Get the very latest Swine Flu news via the MedWorm <b><a href="http://www.medworm.com/rss/search.php?qu=%2Bswine+%2B%28influenza+flu%29&t=Swine+Flu&f=infectiousdiseases&r=Any&o=d" target ="_self">Swine Flu RSS news feed</a></b> - updated hourly from thousands of authoritative health and news sources.</p></div>

Abstract&nbsp;&nbsp;Intramedullary interlocking nailing for diaphyseal fractures is a standard treatment option in affluent societies. These procedures
are often performed under image intensifier guidance. The cost of these gadgets precludes their common use in resource poor
regions. External jig-aided intramedullary interlocking nailing is relatively cheap and offers the chance for performing these
procedures in resource poor regions. The aim of this study was to document the advantages, challenges and outcome of this
form of treatment in a resource poor setting. The Surgical Implant Generation Network (SIGN) implants and instrumentation
were used for this study. Thirty-seven limbs in 35 patients were included. There were 30 males and five females giving a ratio
of 6:1. The mean age...

Gaps in care and outcomes between older and younger ischemic stroke patients are diminishing as hospitals improve adherence to treatments outlined in the American Heart Association/American Stroke Association's Get With the Guidelines&ndash;Stroke initiative, according to a new UCLA study.

The study was published online Feb. 8 in Circulation, the Journal of the American Heart Association, in advance of the journal's Feb. 23 print edition.

This study showed that important differences existed between teaching hospitals in the use of instruments and that obstetrical practices are far from being uniform. However, in 2007, the routine use of more than one instrument in each centre was most usual, as opposed to the situation some decades ago. The use of obstetrical vacuum extractors has been largely disseminated. Each of the three types of instruments has specific indications and mastering at least two instruments seems more secure in the management of the numerous obstetrical situations. It is up to teaching hospitals to make sure that the use of at least two instruments is taught for operative deliveries. The recent publication of Clinical Practice Guidelines would probably help in standardizing operative deliveries in years to...

Conclusions
S. pneumoniae resistance to levofloxacin was uncommon and we managed to identify only one case of probable in vivo resistance development in the 2.5 years of the study. Strain replacement accounted for the majority of incidences where there was an apparent shift towards colonization with isolates of reduced levofloxacin susceptibility.
Conclusions&nbsp; Cardiovascular events and microemboli on TCD have markedly declined with more intensive medical therapy. Less than 5% of patients with ACS now stand to benefit from revascularization; patients with ACS should receive intensive medical therapy and should only be considered for revascularization if they have microemboli on TCD.

There is a wide spectrum of disease burden in hepatocellular carcinoma accompanied by several options for surgical management. However, the associated mortality of such procedures is not well defined. Accurate predictions of patients' perioperative risk would be helpful to guide decision making. The Nationwide Inpatient Sample was queried for data from 1998 to 2005. A cohort of patients who were discharged for hepatic procedures with a diagnosis of primary liver neoplasm was assembled. Procedures were categorized as hepatic lobectomy, wedge resection, or enucleation/ablation. Logistic regression and bootstrap methods were used to create an integer risk score for estimating the risk of in-hospital mortality using procedure type, patient demographics, comorbidities, and hospital type. The aims of this study was to obtain the frequency and therapy of retinopathy of prematurity (ROP) in Markusovszky Teaching Hospital, Szombathely, Hungary. A population based study on ophthalmological status of preterm infants was performed, between 01.01.1989 and 01.01.2009. During the study period, ophthalmological status was detected in 543 premature infants. Among them, 34 children (6.3%) suffered from retinopathy of prematurity, and all of them were bilateral. ROP occurred in the highest rate (23/78 cases, 29.5%) at the birth weight of lower than 1000 grams, but frequency was only l.2% (2/169 cases) between weight 1250-1500 grams. Spontaneous recovery was observed in 19 eyes of 10 cases. The frequency of spontaneous resolution was 40% and 50% in t...

[Perspectives] Profile: Tony Mok: leading light in Hong Kong oncology

It's not always easy to pinpoint the moment your life or career changed direction. But Tony Mok had no problem finding an answer to this question. An established oncologist with a booming practice in Toronto, Canada, Mok dropped by the Prince of Wales Hospital, the Chinese University of Hong Kong's major teaching hospital, for a casual “look around” when he was visiting his parents in Hong Kong. He was introduced to the head of the Oncology Department and by the end of the encounter was presented with a job offer. It was a surprise, says Mok. But he took the offer seriously and decided it was a chance he should not pass up. “How did I make that decision? I knew that if I stayed in Toronto I would be the same guy for the rest of my life.” (Source: LANCET)

To (1) identify and describe the type and frequency of postoperative complications after pylorectomy and gastroduodenostomy in dogs and (2) identify preoperative and intraoperative risk factors, including the presence of neoplasia, prognostic for patient mortality after surgery. Case series. Dogs (n=24) treated by pylorectomy and gastroduodenostomy. Medical records (2000[ndash]2007) for 2 teaching hospitals of dogs treated that had pylorectomy and gastroduodenostomy were reviewed. Pre-, intra-, and postoperative data were obtained from the medical record. Of the 24 dogs, 75% survived 14 days, but 10 (41%) died by 3 months. Overall median survival time (MST) was 578 days. On log-rank univariate analysis, preoperative weight loss (P=.001) and malignant neoplasia (P=.01) were associated with ...

Conclusion
Treatment of patients with chronic lateral epicondylitis with PRP reduces pain and significantly increases function, exceeding the effect of corticosteroid injection. Future decisions for application of the PRP for lateral epicondylitis should be confirmed by further follow-up from this trial and should take into account possible costs and harms as well as benefits.
Conclusion: Despite the effectiveness of professional support interventions, particularly those delivered in the antenatal and immediate post-natal period, access to these sources of support was very low. For breastfeeding outcomes to be improved, effective professional support strategies need to be much more widely available.
CONCLUSION. Pulmonary valve replacement for severe pulmonary regurgitation after tetralogy of Fallot repair is a safe procedure. However, the indications for such an operation in asymptomatic patients remain controversial. Further studies are required to better delineate the timing of pulmonary valve replacement in this patient group.

The Liaison Nurse is a key feature in providing outreach services from Australian ICU, with their role reported to reduce discharge delay, prevent deterioration requiring re-admission to ICU or prolonged hospital stay and support ward staff, patients and relatives transition from ICU. This project evaluated the clinical efficacy of a Critical Care outreach service at each of the three adult teaching hospitals in Perth (Royal Perth Hospital; Sir Charles Gairdner Hospital; and Fremantle Hospital). The service centred on a nurse role which involves follow-up visits by specialist intensive care nurses who reviewed and assessed patients who were to be, or had been, discharged to the general wards from the ICU. In addition it provided an education and clinical support role to the ward staff. A b...

Surgical and oncological treatment of liver metastases of CRC has made a significant progress in the last twenty years. Although chemotherapy has developed enormously, only successfully resected (R0) patients may hope a long-term survival. The almost 60% recurrence rate after resection is a significant challenge. Authors review patients data operated at the Surgical Department of Uzsoki Teaching Hospital between 1995 and 2008 with hepatic metastases of colorectal origin. Oncological and surgical principles are summarized in the sight of the recent literature review and authors own experience with repeat hepatic resection. Recent literature data demonstrate - which is confirmed by our experience - that R0 resection of recurrent hepatic metastases provides sim...

Oludiran O O, Umebese PFAIndian Journal of Plastic Surgery 2009 42(2):189-193Children are a vulnerable to burns, an injury, which is often preventable. A study of the profile of cases of children admitted for burns will provide background information to suggest locally doable preventive strategies as well as supply basic information for future reference. We studied the records of 62 children aged 0-16 years, admitted for burns, at the University of Benin Teaching Hospital, Benin City, between January 2002 and December 2006. There were 34 male and 28 female children. Children under three years constituted 56.5&#x0025;. Whereas the leading cause of burns in all the children was flame burns from kerosene explosions (52&#x0025;), scalds were responsible for 68.6&#x0025; of cases in those under...

Conclusions:&#x0026;lt;/b&#x0026;gt; Although complications occurred, the beneficial effects of lip adhesion in combination with a guidance plate outweighed the risks for anatomical reconstruction of a platform for definitive lip and nose repair.
Conclusions&nbsp;&nbsp;Operative notes do not always fully comply with the standards set forth in the guidelines published in the Netherlands. This
could influence adjuvant treatment and future patient treatment, and it may make operative notes less suitable background
for other purposes. Therefore operative note writing should be taught as part of surgical training, definitions should be
provided, and procedure-specific guidelines should be established to improve the quality of the operative notes and their
use to improve patient safety.

A population-based study on the determinants of hospice utilization in the last year of life for Taiwanese cancer decedents, 2001-2006

Conclusions: Despite the 1.33 times increase in hospice utilization among cancer patients who died from 2001 to 2006, only one in six Taiwanese cancer decedents used hospice care in their last year of life. Our findings regarding determinants of hospice utilization should be used by healthcare professionals and policy makers to guide the development of policies and interventions that facilitate prognosis disclosure and EOL care discussions between physicians and patients, especially younger patients, to help the transition from curative to palliative care.
Abraham Flexner's seminal 1910 report, Medical Education in the United States and Canada, is widely credited with instigating important changes in U.S. medical education that have shaped today's system of training physicians. Although Flexner's report publicly articulated recommendations for widespread change, the stage had been set for reform for quite some time. In this commentary, the author examines the landscape of change in medical education at the turn of the 20th century, highlighting the roles of several important contributors, especially the American Medical Association's Committee on Medical Education. Now, 100 years later, academic medicine is poised for further reforms to enable medical schools and teaching hospitals to meet the needs of 21st-century patients and physicians. T...

Conclusions &nbsp;&nbsp;Polypectomy using DBE may obviate the need for repeated urgent operations and small-bowel resections leading to short-bowel
syndrome. Patients are indicated for prophylactic procedure and polypectomy of the entire small bowel. Both DBE and IOE facilitate
exploration and treatment of the small intestine. DBE is less invasive and more convenient for the patient. Both procedures
are generally safe and useful.

Conclusions:
This study contributes to previous research in describing a repertoire of pedagogical strategies used in clinical education. The findings showed that three superordinate qualitatively different ways of teaching could be identified that fit Ramsden's model. Each of these pedagogical strategies encompass different focus in teaching; either a focus on the teacher's knowledge and behaviour or the student's behaviour and understanding. We suggest that an increased awareness of the strategies in use will increase clinical teachers' teaching skills and the consequences they will have on the students' ability to learn. The pedagogical strategies need to be considered and scrutinized in further research in order to verify their impact on students' learning. (Source: BMC Medical Educati...
