It is important that pharmacists be willing to dispense oral emergency contraception (OEC) and provide evidence-based counseling. The purpose of this study is to determine whether changes in knowledge, behavior, confidence and professional responsibility (KBCPR) regarding OEC following a novel educational intervention in first-year pharmacy students are maintained after 8 weeks.
This article summarises their initial findings. Self-management programmes aim at structural behaviour change to sustain treatment effects after programmes have been completed. The programmes should include techniques aimed at behavioural change, be tailored individually, take the patient's perspective into account, and may vary with the course of the patient's disease and co-morbidities. Assessment should include process variables. This report is a step towards greater conformity in the field of self-management. To enhance clarity regarding effectiveness, future studies should clearly describe their intervention, be properly designed and powered, and include outcomes that focus more on the acquisition and practice of new skills. In this way more evidence and a better comprehension on self...

Abstract Imaging is key in the accurate monitoring of response to cancer therapies targeting tumour vascularity to inhibit its growth
and dissemination. Dynamic contrast enhanced ultrasound (DCE ultrasound) is a quantitative method with the advantage of being
non-invasive, widely available, portable, cost effective, highly sensitive and reproducible using agents that are truly intravascular.
Under the auspices of the initiative of the Experimental Cancer Medicine Centre Imaging Network, bringing together experts
from the UK, Europe and North America for a 2-day workshop in May 2010, this consensus paper aims to provide guidance on the
use of DCE ultrasound in the measurement of tumour vascular support in clinical trials.

The final criteria and working methods that the Research Excellence Framework (REF) panels will use when assessing research are published today.
Publication of this information, by the REF team on behalf of the four UK higher education funding bodies, is a key stage in preparations for the REF. Through the REF, the quality of research in all UK Universities and higher education colleges will be assessed, and the results will be used by the UK funding bodies to allocate research funding.
Universities and higher education colleges will be invited to submit their highest quality research in 36 subject groupings called units of assessment (UOAs). The submissions will be assessed by an expert sub-panel for each UOA, working under the leadership and guidance of four main panels. Today's guidanc...

Analgesic trials pose unique scientific, ethical, and practical challenges in pediatrics. Participants in a scientific workshop sponsored by the US Food and Drug Administration developed consensus on aspects of pediatric analgesic clinical trial design. The standard parallel-placebo analgesic trial design commonly used for adults has ethical and practical difficulties in pediatrics, due to the likelihood of subjects experiencing pain for extended periods of time. Immediate-rescue designs using opioid-sparing, rather than pain scores, as a primary outcome measure have been successfully used in pediatric analgesic efficacy trials. These designs maintain some of the scientific benefits of blinding, with some ethical and practical advantages over traditional designs. Preferred outcome measures...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Role of Environmental Chemicals in Diabetes and Obesity: A National Toxicology Program Workshop Report

Conclusions: Overall, the review of the existing literature identified linkages between several of the environmental exposures and type 2 diabetes. There was also support for the “developmental obesogen” hypothesis, which suggests that chemical exposures may increase the risk of obesity by altering the differentiation of adipocytes or the development of neural circuits that regulate feeding behavior. The effects may be most apparent when the developmental exposure is combined with consumption of a high-calorie, high-carbohydrate, or high-fat diet later in life. Research on environmental chemical exposures and type 1 diabetes was very limited. This lack of research was considered a critical data gap. This workshop report outlines the major themes that emerged from the workshop and discu...

CONCLUSION: Our faculty development program appears to have a significant positive effect on medical teachers' competencies, and we suggest that our educational intervention is effective in achieving its aims. Further research should investigate whether this faculty development program actually results in improved teaching performance.

This article describes the development of an interprofessional care plan for an older adult woman with breast cancer. Two collaborative expert workshops were used; 15 clinical experts reviewed an online patient case and were asked to prepare a uniprofessional care plan. In workshop 1, participants worked from a draft interprofessional care plan, synthesized from the uniprofessional care plans by research associates, to arrive at consensus on an ideal interprofessional care plan. Using qualitative inductive content analysis of workshop transcripts, specific changes and overall key principles were identified and used to revise the draft plan. Based on these findings, a generalized interprofessional care plan/oncology model was developed. Revisions and proposed model were validated through co...

The Obama Administration is working to develop a national strategy to respond to the impacts of climate change on our nation’s natural resources. The draft National Fish, Wildlife, and Plants Climate Adaptation Strategy has been developed with input by federal, state, and tribal organizations and seeks to provide “a unified approach—reflecting shared principles and science-based practices—for reducing the negative impacts of climate change on fish, wildlife, plants, and the natural systems upon which they depend.”

The strategy is unique in that it aims to be national in scope, not just a federal framework for cooperative climate response.

A series of online public workshops will be held in January and February to educate the public about the draft strategy a...

MedWorm Sponsor Message: Find the best January Sales in the UK.

    Abstract
Training for health services staff often focuses on improving individual practitioner's knowledge and skills, with less emphasis given to the broader organizational context, in particular those elements that support successful implementation of changes post-training. This paper compares the effectiveness of a standard training model for suicide prevention to an enhanced training model. The training involved the public mental health workforce throughout the State of Queensland, Australia and was developed in collaboration with the State health department and as such took place within a policy and practice context. The standard training involved participation in a one-day training workshop, which provided information on evidence-based suici...

AbstractOlder adults are among the highest at risk for completing suicide, and they are more likely to seek mental health services from providers outside of traditional mental health care, but providers across the spectrum of care have limited training in suicide risk assessment and management and particularly lack training in suicide prevention for older adults. An educational program was developed to increase awareness and improve suicide risk assessment and management training for a range of healthcare providers who may see older adults in their care settings. One hundred thirty‐two participants from two Veterans Affairs Medical Centers participated in a 6.5‐hour‐long workshop in the assessment and management of suicide risk in older adults.
Summary.  Clinical research should form a core component of the role of haemophilia nurse specialists. The UK Haemophilia Nurses Association sought to determine the barriers that prevent nurse specialists from engaging in research and to seek ways to promote clinical research by haemophilia nurses in the UK. Web‐based survey with subsequent workshop discussion was conducted. Responses were received from 32 nurses (a 50% response rate), all of whom agreed that haemophilia nurses should be actively involved in nursing research although only 21 had actually participated in research specifically related to haemophilia practice. Of these, most research had been related to educational programmes or (less commonly) was limited to data collection as part of multidisciplinary studies. Involveme...

Conclusion: Participants in a short course focusing on EBM and clinical research methodology had significant improvements in scores on tests of knowledge gained. Widespread implementation of similar courses may bridge knowledge gaps for surgeons, surgical trainees and health professionals. Whether shorter knowledge gain sare retained in the longer term remains unknown.

We must find a new way of understanding human beingsThe unlucky fact that our current form of mechanistic materialism rests on muddled, outdated notions of matter isn't often mentioned today. It's a mess that can be ignored for everyday scientific purposes, but for our wider thinking it is getting very destructive. We can't approach important mind-body topics such as consciousness or the origins of life while we still treat matter in 17th-century style as if it were dead, inert stuff, incapable of producing life. And we certainly can't go on pretending to believe that our own experience – the source of all our thought – is just an illusion, which it would have to be if that dead, alien stuff were indeed the only reality. We need a new mind-body paradigm, a map that acknowledges the...

This article describes the development of Promoting Access to Health Services (PATHS), an intervention to promote regular use of clinical preventive services by women with physical disabilities. The intervention was developed using intervention mapping (IM), a theory-based logical process that incorporates the six steps of assessment of need, preparation of matrices, selection of theoretical methods and strategies, program design, program implementation, and evaluation. The development process used methods and strategies aligned with the social cognitive theory and the health belief model. PATHS was adapted from the workbook Making Preventive Health Care Work for You, developed by a disability advocate, and was informed by participant input at five points: at inception through consultation...

SummaryThe integration of the veterinary, medical and environmental sciences necessary to predict, prevent or respond to emerging zoonotic diseases requires effective collaboration and exchange of knowledge across these disciplines. There has been no research into how to connect and integrate these professions in the pursuit of a common task. We conducted a literature search looking at the experiences and wisdom resulting from collaborations built in health partnerships, health research knowledge transfer and exchange, business knowledge management and systems design engineering to identify key attributes of successful interdisciplinary (ID) collaboration. This was followed by a workshop with 16 experts experienced in ID collaboration including physicians, veterinarians and biologists from...

Authors: Petersen PE, Baez RJ, Lennon MA
    Abstract
Dental caries is the most prevalent chronic disease affecting human populations around the world. It is recognized that fluoride plays a significant role in dental caries reduction. Meanwhile, several low- and middle-income countries of Asia have not yet implemented systematic fluoride programs; contributing factors relate to misconceptions about the mechanisms of fluoride, low priority given to oral health in national health policy and strategic plans, and lack of interest among public health administrators. A workshop on the effective use of fluoride in Asia took place in Phang-Nga, Thailand, in 2011. A series of country presentations addressed some of the topics mentioned above; in addition, speakers from countries of the region ...

SummaryWhole‐body ultraviolet (UV)A1 (340–400 nm) phototherapy was first introduced 30 years ago, but is currently available in the UK in only three dermatology departments. A workshop to discuss UVA1 was held by the British Photodermatology Group in May 2009, the aim of which was to provide an overview of UVA1 phototherapy and its role in practice, and to identify areas in which further studies are required. The conclusions were that UVA1 phototherapy is an effective treatment in several inflammatory skin diseases, including localized scleroderma and atopic eczema (AE); however, deficiencies and limitations exist in the published evidence base. For most diseases, such as AE, other treatments also exist, which are generally more effective than UVA1. However, for some diseases, part...

    Abstract
The Partnerships for Families project is a randomized clinical trial designed to evaluate the implementation of Alternatives for Families: A Cognitive-Behavioral Therapy (AF-CBT), an evidence-based treatment for family conflict, coercion, and aggression, including child physical abuse. To evaluate the effectiveness of a training program in this model, 182 community practitioners from 10 agencies were randomized to receive AF-CBT training (n = 90) using a learning community model (workshops, consultation visits) or Training as Usual (TAU; n = 92) which provided trainings per agency routine. Practitioners completed self-report measures at four time points (0, 6, 12, and 18 months following baselin...

Conclusions:
Our approach revealed substantial caveats in current RTI interventions in Peru, and fundamental negative effects on several components of the sectors and systems involved. It also highlighted programmatic issues that should be applied to guarantee an effective implementation and evaluation of these policies. The findings from this study were discussed with key stakeholders for consideration in further designing and planning RTI control interventions in Peru.
Student doctors need to gain experience in postgraduate assessment methods, ie, workplace-based. Unlike the more formal assessments such as the OSCE, Placement-based Assessments are not always well received and can be perceived as unfair by students, and too time consuming by the clinicians asked to be assessors.
This workshop aims to identify and discuss the problems and their solutions in the introduction of Undergraduate Placement Based Assessment.

    Abstract
My mother, Leonore, was diagnosed with Huntington's disease (HD) in 1968 at age 53. I was 23, my sister Alice 26, and our father, Milton Wexler, 60 years old. The same year, our father created the Hereditary Disease Foundation (HDF), dedicated to finding treatments and cures for HD. HD is an autosomal dominant, neurodegenerative disorder. Alice and I each have a 50% chance of inheriting and dying from the disorder. Over the past 43 years, we have been proud to change the face of science. Through Milton Wexler Interdisciplinary Workshops, judicious funding, and focusing on innovation and creativity, the HDF is an integral partner in key discoveries. The HDF recruited and supported >100 scientists worldwide who worked together as the Huntington's Diseas...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Idiopathic pulmonary fibrosis (IPF) is a chronic progressive disease of unknown aetiology. It has a very poor prognosis and no effective treatment. There are two major barriers to the development of novel treatments in IPF: an incomplete understanding of its pathogenesis and the fact that current models of the disease are poorly predictive of therapeutic response. Recent studies suggest an important role for the alveolar epithelium in the pathogenesis of IPF. However, practical limitations associated with isolation and culture of primary alveolar epithelial cells have hampered progress towards further elucidating their role in the pathogenesis of the disease or developing disease models that accurately reflect the epithelial contribution. The practical limitations of primary alveolar epith...

The U.S. National Research Council (NRC) report on "Toxicity Testing in the 21st century" calls for a fundamental shift in the way that chemicals are tested for human health effects and evaluated in risk assessments. The new approach would move toward in vitro methods, typically using human cells in a high-throughput context. The in vitro methods would be designed to detect significant perturbations to "toxicity pathways," i.e., key biological pathways that, when sufficiently perturbed, lead to adverse health outcomes. To explore progress on the report’s implementation, the Human Toxicology Project Consortium hosted a workshop on 9–10 November 2010 in Washington, DC. The Consortium is a coalition of several corporations, a research institute, and a non-governmental organization...

AbstractXenopus is an essential vertebrate model system for biomedical research that has contributed to important discoveries in many disciplines, including cell biology, molecular biology, physiology, developmental biology and neurobiology. However, unlike other model systems no central repository/stock center for Xenopus had been established until recently. Similar to mouse, zebrafish and fly communities, which have established stock centers, Xenopus researchers need to maintain and distribute rapidly growing numbers of inbred, mutant and transgenic frog strains, along with DNA and protein resources, and individual laboratories struggle to accomplish this efficiently. In the last five years two resource centers were founded to address this need: the European Xenopus Resource Center (EXRC...

Professionalism is an important issue across all of the health professions, including but not limited to nursing, midwifery, medicine, dentistry, pharmacy, health professions including occupational health, social work, etc. The regulatory bodies expect higher education to recognise, teach and assess professionalism, which manifests itself in a range of behaviours.
The HPC is currently consulting on 'fitness to practice and registration' (http://www.hpc-uk.org/aboutus/consultations/index.asp?id=131), and the other regulatory bodies have focussed extensively on evidencing professionalism in the past.
This highly intereactive one day workshop features renowned international expert Professor Maxine Papadakis and UK colleagues facilitating discussion of what professionalism means in ...

MedWorm Sponsor Message: Find the best January Sales in the UK.

This article will emphasize the critical role played by exposure during the juvenile stage of development. This article will also highlight several key issues that distinguish DIT testing of pharmaceuticals. Representatives from the pharmaceutical, biotechnology, academic, and regulatory sectors (both FDA and EMA) were brought together during a two-day workshop in May 2010 to consider the current state of the science of DIT as it pertains to the testing of pharmaceuticals. It is important to emphasize at the onset that there are currently no regulatory guidelines for either drugs or nondrug chemicals specifically focused on assessment of DIT, although some general guidelines are included in both developmental and reproductive toxicity and general immunotoxicology guidance documents.

In conclusion, the outcome of this workshop improved the understanding of the participants of some important factors that may impact the design of an ITS in function of its purpose (e.g. screening, or early decision making versus regulatory), the context in which they need to be applied (e.g. ICH guidelines, REACH) and the status and quality of the available tools. A set of recommendations of best practices was established and the importance of the applicability of the individual tests as well as the testing strategy itself was highlighted.

"How do you make the biology we teach as exciting as the biology that we do?" was the challenging question posed by V. Celeste Carter to participants at the National Academy of Sciences convocation, "Thinking Evolutionarily: Evolution Education across the Life Sciences," held in October. Carter, program director at the National Science Foundation, and others at the convocation discussed the converging efforts to improve biology education, to better motivate students, and to integrate evolution across learning experiences.

Simply regurgitating the biological knowledge generated by the scientific community or conducting "cookbook" laboratory experiments does not result in genuine understanding or excitement on the part of students, Carter and other speakers stressed. Instead, the nature and...

MedWorm Sponsor Message: Find the best January Sales in the UK.

Conclusions: Encouragingly, Surgery appears to be a top career choice among medical students in West Africa. Direct mentoring from local surgeons, as well as formal mentoring experiences like the AAS workshop, may be vital to sparking and nurturing this interest in Surgery. Additionally, improvement in work-life balance during residency, perhaps through duty hour regulations, may further this interest, increasing the number of surgical trainees and ultimately helping to bridge the existing gap in the surgical workforce. Abstract
The study of malaria parasites on the Indian subcontinent should help us understand unexpected disease outbreaks and unpredictable disease presentations from Plasmodium falciparum and Plasmodium vivax infections. The Malaria Evolution in South Asia (MESA) research program is one of ten International Centers of Excellence for Malaria Research (ICEMR) sponsored by the US National Institutes of Health. In this second of two reviews, we describe why population structures of Plasmodia in India will be characterized and how we will determine their consequences...

Summertime brings new residents to South Florida. One of my contributions to orienting the new University of Miami and Cleveland Clinic residents to the wonderful world of plastic surgery is to conduct a digital imaging workshop. My motivation is part eleemosynary and part enlightened self-interest. Annoyed at having had to sit through countless conferences where poorly shot photos detracted from the discussion, I realized that nobody had taught the residents how to shoot acceptable photos. Not wanting to be subjected to further presentations that were the visual equivalent of hearing fingernails scratched across a chalkboard, I applied that frequently quoted surgical adage – if you want something done correctly, do it yourself. Thus the workshops were born. (Source: Journal of Plastic, ...

SummaryThis critical review refers to the new report on terminology and concepts for the organization of epilepsies by the Commission of the International League Against Epilepsy (ILAE). It is unfortunate that most of the proposals in the Commission’s report are modified interpretations and nomenclature of previous ILAE classifications; new terms are not better than the old ones, and recent advances have not been incorporated. Hence, the new ILAE report met with considerable dissatisfaction from several expert epileptologists. The Commission abandoned (1) the disease‐syndrome distinction, although “disease” is generally differentiated from “syndrome” in most medical texts as well as in the ILAE classification itself; (2) the distinction of “generalized” and “focal” for ...

MedWorm Sponsor Message: Find the best January Sales in the UK.

These days, we hear a lot about the disorder of autism, but researchers at the University of Utah have created a program that helps kids with autism focus on building their skills and utilizing an aptitude for visual-spatial thinking, computers and other electronic media. One of the program participants is 12-year-old Christopher Charles, who was diagnosed with what's now known as high-functioning autism when he was 18 months old.
The aim of this qualitative study was to describe and investigate the effects of a programme of stand up comedy and improvisation workshops on people with early stage dementia. Interviews from participants (n = 6), their carers (n = 6), and the comedian facilitator were analysed using constant comparative analysis. The findings indicated that dementia did not prevent participants from laughing appropriately or successfully creating and performing comedy. The data suggest that the programme may have therapeutic benefits as improvements in memory, learning, sociability, communication and self esteem were demonstrated. The study also develops a set of hypotheses for further research which includes: that active participation by people with dementia (PWD) in performing to create laughter is mor...

In this issue of the Journal of Pediatric and Adolescent Gynecology we focus on the Juvenile Justice System as addressed in our Mini-Review, authored by Drs. Risk and Alderman. Specifically, girls charged with a juvenile offense are a special group of individuals that require clinicians to understand the unique problems associated with this population. Depression, severe neglect, sexual and physical abuse, and this occurs oftentimes while in the care of a government agency, is the background to the problem. New laws have been enacted that appear to “allow children to be housed in county jails outside the oversight, supervision or even inspection(s) by anyone experienced with the care of children”; the Florida Department of Justice has been front and center on this issue.
The outcome prediction in cancer is usually achieved by evaluating tissue samples obtained during surgical removal of the primary tumor focusing on their histopathological characteristics. Tumor staging (AJCC/UICC-TNM classification) summarizes data on tumor burden (T), presence of cancer cells in draining and regional lymph nodes (N), and evidence for metastases (M). However, this classification provides limited prognostic information in estimating the outcome in cancer and does not predict response to therapy. It is recognized that cancer outcomes can vary significantly among patients within the same stage. Recently, many reports suggest that cancer development is controlled by the host's immune system underlying the importance of including immunological biomarkers for the prediction of ...

CONCLUSION: This study indicates that SWI is not being used by the majority of midwives participating in the study, although there is a strong desire by midwives to learn about and explore its use. Greater access to information and workshops on SWI is highlighted. In response to the findings of this survey the authors are currently developing an online resource and training to support units to introduce SWI.

The Bildirici Center for Diabetes Care and Research opened its doors to its first patients in August, 2007, as the first center in Israel to offer comprehensive integrated multidisciplinary care for people with diabetes. In addition to direct patient care the Center has organized courses, lectures and workshops for people with diabetes, as well as for professionals caring for people with diabetes, and for the general community. The ability of individuals within different departments to develop and implement such a center may have been facilitated by the smallness of our institution and the familiarity they share as a result. We have shown how a community hospital, without ties to a major medical ce...

The objectives of this study are to (a) assess baseline experience, knowledge, and comfort of seniors about "real-life" experiences, and (b) assess the efficacy of a 4-hr educational intervention on perceptions of understanding financial planning, real estate choices, intern preparedness, and physician wellness. Acute Care College seniors (classes of 2009 and 2010) attended the intervention after match day and completed a survey to gather demographic data and assess preexisting knowledge and a postintervention survey (1-7 Likert scale). Forty-nine students (45% male; M age = 25.5 years) participated. Prior experiences: 43% no break in education, 51% no full-time job, 38% never signed a rental lease and 94% had not purchased real estate, 90% did not have (or were not aware of having) disabi...

Authors: Kiser PF, Johnson TJ, Clark JT
    Abstract
There is renewed interest in the development of long-term, controlled-release dosage forms for the intravaginal delivery of antiretrovirals for HIV prophylaxis. This interest has catalyzed a renaissance in vaginal drug delivery, increasing the fundamental understanding of determinants of controlled drug delivery in the vagina as well as development of new materials, delivery platforms, and animal models. Our goal in writing this review from the perspective of engineers and pharmaceutical scientists interested in prevention of sexually transmitted infections is to highlight the current state of the art, progress in preclinical programs, new drug-delivery device designs, and to discuss some of the important unknowns in this area of HIV...

CONSORT-EHEALTH: Improving and Standardizing Evaluation Reports of Web-based and Mobile Health Interventions

Conclusions: CONSORT-EHEALTH has the potential to improve reporting and provides a basis for evaluating the validity and applicability of eHealth trials. Subitems describing how the intervention should be reported can also be used for non-RCT evaluation reports. As part of the development process, an evaluation component is essential; therefore, feedback from authors will be solicited, and a before-after study will evaluate whether reporting has been improved.
The gold standard treatment for primary ureteropelvic junction obstruction (UPJO) is a pyeloplasty; the approach can be open, laparoscopic, or robotic and could be a matter of surgeon preference. The authors need to be congratulated for describing a conventional and novel approach for performing endopyeloplasty with a 3.5-mm pediatric laparoscopic needle-holder.
We genuinely appreciate the comments of Desai et al regarding our feasibility study describing a more “conventional” way to perform percutaneous endopyeloplasty. This technique was performed without any patient selection. Thus, we think that in dependent position ureteropelvic junction (UPJ), the 2 endopyelotomy incision apexes are wide apart, thus endopyeloplasty is best suited for short UPJ obstruction. However, in high inserted UPJ, a longer endopyelotomy can be performed until reaching the more dependent renal pelvic part. Mostly in this case, the 2 tissue edges are adjacent, so endopyeloplasty is easier. (Source: Urology)

NEW DELHI — Midwifery practices in India got some much-needed attention at a recent regional workshop here organized by the UN Population Fund, UNFPA, and the International Confederation of Midwives. The workshop, held 28 November to 3 December, attracted about 60 participants from 7 Asian countries, who were oriented on the latest midwifery skills and standards of education and regulation introduced by the confederation earlier this year.
