In this article, we focus on the education, training and practice of non‐medical prescribers in the UK. There are around 20,000 nurse independent prescribers, 2,400 pharmacist supplementary/independent prescribers, several hundred allied health professional supplementary prescribers and almost 100 optometrist supplementary/independent prescribers. In recent years, a broad consensus regarding the necessary competencies has been achieved. Each of these is a complex mix of knowledge, judgement and skills. Surveys of those on the threshold of their medical careers have revealed widespread lack of confidence in writing prescriptions. All hip fracture patients with a cardiac murmur have an echocardiogram as a part of their preoperative work-up in our unit. We performed a retrospective audit to assess the impact obtaining a pre-operative echocardiogram on the management of hip fracture patients.
Methods:
All hip fracture patients (N=349) between 01/06/08 and 01/06/09 were included in the study. 29 patients had pre-operative echocardiogram (echo group). A computer generated randomised sample of 40 patients was generated from N, 'non-echo' group. Data was obtained from medical records and the Hospital Information Support System (HISS). The groups were compared using Student's t test. Approval was obtained locally from the clinical governance department for this project.

    The National Service Framework for Older People resulted in the widespread introduction of intermediate care (IC) services. However, although these services have shared common aims, there has been considerable diversity in their staffing, organisation and delivery. Concerns have been raised regarding the clinical governance of IC with a paucity of data to evaluate the effectiveness, quality and safety of these services. This paper presents the results of a national pilot audit of IC services focusing particularly on clinical governance issues. The results confirm these concerns and provide support for a larger scale national audit of IC services to monitor and improve care quality.

This has resulted in staff being increasingly satisfied with the turnaround time for approval of improvement activities they are undertaking as well as for the methodological support provided. There are currently no advocated tools for recording progress and personal audit. CuSum (Cumulative Sum) graphs sequentially monitor an outcome. Their uses have included assessing mortality (Bridgewater et al.) and new surgical techniques (Leandro et al.). Cases are plotted sequentially along the x-axis and cumulative achievement of a Gold-standard along the y-axis. A proportional linear relationship exists when all cases are successful.  &amp;nbsp; For example, arrangements for the provision of PCT community services are under review and many are in the process of being re-shaped with potential merger with other organisations.

            <description>This article outlines the activities undertaken during asthma camps and highlights the benefits for children and young people. These include: improved understanding of asthma, increased concordance with medication, greater participation in physical activity and enhanced self-confidence. In the absence of a nationally agreed offsite activity policy, the author demonstrates the use of a clinical governance framework to develop a local offsite activity policy for asthma camps to ensure the safety of children and young people as well as staff.
