In one Oxfordshire village, an idea is gathering traction: that it's time for a new agricultural revolutionOxford city council has decided that we need more houses and jobs – not least in my own village of Wolvercote, to the north-west of the city. Under the coalition's neighbourhood development order (part of the localism bill) we, the yokels, the ordinary Joes, have some say in what should be done.So now we plot and ponder in the village hall – and we are witnessing what I hope will prove to be a seismic shift in public mood, in the economy, and in the balance of power. For more and more people are beginning to feel that "development" shouldn't mean more of the same – more city-bound jobs and city-sprawl. Instead what we need is an agrarian renaissance: small-scale farming, includi...

MedWorm Sponsor Message: Find the best January Sales in the UK.

The 2010 Australian government tax review suggested Australia move to a uniform excise tax rate for all alcoholic beverages. Here, a model is presented and calibrated that shows the optimal per litre of pure alcohol (LAL) tax rates for beer, wine, spirits, and ready-to-drink spirits are substantially different to both current alcohol tax rates and the uniform tax rate recommended by the tax review. Specifically, given an individual consumer utility model, the best estimate values of the welfare maximising LAL tax rates are: $37 for beer, $11 for wine, $50 for spirits, and $77 for ready-to-drink spirits. The variation in the optimal tax rate across beverage types flows from differences in the externality costs associated with the consumption of each beverage type, and differences in the pro...

SUMMARYThere are theoretical reasons to expect that high risk of mortality or morbidity during young adulthood decreases investment in human capital. However, investigation of this hypothesis is complicated by a variety of empirical challenges, including difficulties in inferring causation due to omitted variables and reverse causation. For example, to compare two groups with substantially different mortality rates, one typically has to use samples from different countries or periods, making it difficult to control for other relevant variables. Reverse causation is important because human capital investment can affect mortality and morbidity. To counter these problems, we collected data on human capital investments, fertility decisions, and other economic choices of people at risk for Hunt...

ABSTRACTWe examine the effect of a health plan's pay for performance incentives on the percentage of outpatient drug prescriptions that are filled with generic rather than brand‐name drugs in physicians' practices in an established physician network – the generic prescription rate (GPR). The financial reward was based on the performance of the entire network, but the network implemented rewards at the practice level. Practice‐level rewards were awarded on an all‐or‐nothing basis if the GPR met or exceeded specialty‐specific targets that increased each year. Although that design gave the practices a strong incentive to meet the target, practices performing far below the target might ‘give up’, costing the network its reward. Using a partial adjustment model, we estimate that...

Conclusion Children aged 6–7 years can feasibly complete utility instruments when interviewer-administered. The reliability of the instruments
is of concern and requires further study. With respect to content validity and other psychometric properties, the CHU-9D is
favoured to the EQ-5D-Y. Until the EuroQol group produces tariff values for the EQ-5D-Y, we recommend that the EQ-5D-Y is
not used for utility elicitation in this age group.

Conclusions Although pain is an important outcome of trials examining treatment for bone metastases, the current review suggests that
there is little consistency in PRO measurement across studies. Furthermore, presentation of measures often lacked clear description,
information on measurement properties, citations, clarity regarding method of administration, and consistent instrument names.
Recommendations are provided for instrument validation within the target population, assessment of content validity, use of
PRO instruments recently developed for patients with bone metastases, clear description of instruments, and implementation

Authors: Sugden R
    Abstract
The discovery of the altruistic punishment mechanism as a replicable experimental result is a genuine achievement of behavioural economics. The hypothesis that cooperation in hunter-gatherer societies is sustained by altruistic punishment is a scientifically legitimate conjecture, but it must be tested against real-world observations. Guala's doubts about the evidential support for this hypothesis are well founded.

We estimate changes in sexual behavior for HIV-positive individuals enrolled in an AIDS treatment program using longitudinal household survey data collected in western Kenya. We find that sexual activity is lowest at the time that treatment is initiated and increases significantly in the subsequent six months, consistent with the health improvements that result from ART treatment. More importantly, we find large and significant increases of 10 to 30 percentage points in the reported use of condoms during last sexual intercourse. The increases in condom use appear to be driven primarily by a program effect, applying to all HIV clinic patients regardless of treatment status.
Abstract An important health policy issue is the low rate of patient enrollment into clinical trials, which may slow down the process
of clinical trials and discourage their supply, leading to delays in innovative life-saving drug treatments reaching the general
population. In the US, patients’ cost of participating in a clinical trial is considered to be a major barrier to patient
enrollment. In order to reduce this barrier, some states in the US have implemented policies requiring health insurers to
cover routine care costs for patients enrolled in clinical trials. This paper evaluates empirically how effective these policies
were in increasing the supply of clinical trials and speeding up their completion, using data on cancer clinical trials initiated
in the US betwe...

MedWorm Sponsor Message: Find the best January Sales in the UK.

Discussion In contrast to our hypothesis, ADMA was lower and the L-arginine/ADMA ratio was higher in PKU patients. Therefore, in PKU
patients, the regulating function of ADMA on NO synthesis is altered and may thus contribute to oxidative stress.

There is increasing evidence that hemodynamic optimization by fluid loading, particularly when performed in the early phase of surgery, is beneficial in high-risk surgery patients: it leads to a reduction in postoperative complications and even to improved long-term outcome. However, it is also true that goal- directed strategies of fluid optimization focusing on cardiac output optimization have not been applied in the clinical routine of many institutions. Reasons are manifold: disbelief in the level of evidence and on the accuracy and practicability of the required monitoring systems, and economics. The FOCCUS trial examined perioperative fluid optimization with a very basic approach: a standardized volume load with 25 ml/kg crystalloids over 6 hours immediately prior to scheduled surger...

This article describes the development of a methodology for calculating the cost of an EMS system to its community. This includes a tool for calculating the cost of EMS (the “cost workbook”) and detailed directions for determining cost (the “cost guide”). The 12‐step process that was developed is consistent with current theories of health economics, applicable to prehospital care, flexible enough to be used in varying sizes and types of EMS systems, and comprehensive enough to provide meaningful conclusions. It was developed by an expert panel (the EMS Cost Analysis Project [EMSCAP] investigator team) in an iterative process that included pilot testing the process in three diverse communities. The iterative process allowed ongoing modification of the toolkit during the developmen...

This article examines various issues regarding inhaled oxygen and cluster headache, including efficacy, proposed mechanism
of action, utilization, and the economics of oxygen usage for cluster headache patients. Much of the data analyzed comes from
the recently published United States Cluster Headache Survey. This is the largest study of cluster headache patients ever
published and is the first study to focus on inhaled oxygen and cluster headache in a large, non-clinic-based population.

Abstract Most existing studies on parallel trade conclude that it reduces pharmaceutical firms’ profits. One special feature of the
pharmaceutical industry is the presence of price regulation in most countries. Taking into account the impact of parallel
trade on the regulated pharmaceutical prices [Pecorino, P.: J. Health Econ. 21, 699–708 (2002)] shows that a pharmaceutical firm’s profit is greater in the presence of parallel trade. The present paper
relaxes the assumption on identical demands among countries, and takes into account transaction costs. The results of our
model show that a firm’s profits may increase or decrease in the presence of parallel trade, depending on its bargaining power
in the price negotiation and market size of the drug. Changes in social...

ABSTRACTWe use 4 years of data from the retiree health benefits program of the University of Michigan to estimate the effect of price on the health plan choices of Medicare beneficiaries. During the period of our analysis, changes in the University's premium contribution rules led to substantial price changes. A key feature of this ‘natural experiment’ is that individuals who had retired before a certain date were exempted from having to pay any premium contributions. This ‘grandfathering’ creates quasi‐experimental variation that is ideal for estimating the effect of price. Using regression discontinuity methods, we compare the plan choices of individuals who retired just after the grandfathering cutoff date and were therefore exposed to significant price changes to the choices ...

    Abstract
Medicine has changed dramatically in the past ten decades thanks to the introduction of innovative diagnostic and therapeutic procedures. However, besides the unmistakable advances achieved in medicine, the costs of all health care systems have risen dramatically. In contrast to the escalation in expenditures, only moderate gains in proceeds have been accomplished. This situation requires that future financial resources be judiciously expended. The field of health economics has set as its goal the analysis of medical measures in terms of costs and benefits to be able to provide information on these parameters to those involved in the public health sector. The emerging problems are diverse and extend from assessment of e...

I first realised this during an episode of BBC Question Time, a politically-themed game show in which morons compete with a dimly-sentient studio audience to see who can make the most stupid remark; the prize being fleeting infamy on Twitter. It was November, 2009. Hackers had stolen some boring e-mails about climate change from a university, and dullards on the internet were busy concocting elaborate conspiracy fantasies about them.
This study contributes to the literature on the relationship between socioeconomic status and child health by testing the hypothesis that the increasing effect of family income on children's health with age relates to the children's use of health care services. It also investigates the role of specific health conditions, injuries or maternal health in explaining the steepening gradient. Drawing on a nationally representative survey from Canada, the National Longitudinal Survey of Children and Youth from the period 1994/95-2008/09, this study provides further evidence of a steepening socioeconomic gradient in child health with age. It finds that accounting for health care use does not explain the steepening gradient and that the protective effect of income appears to be greater for those wh...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

This article reports two experiments that aim to test for preference reversals over outcomes defined by health status, using both ‘open’ and ‘assisted’ valuation procedures. Although systematic preference reversals in the direction generally reported in the literature were not observed, the rates of non‐systematic reversal were substantial, measuring 35–40%. By analysing the respondents' explanations for their answers, it is clear that many of them used heuristics to answer the questions, and they may have good reasons to do so, which undermines the notion that preferences are always fixed and stable. These results, and others like it, pose a challenge to those who unquestioningly assume procedural invariance and maintain that population preferences should be used to inform hea...

Conclusions Over the past 15 years, disappointingly the number of printed child hydrocephalus papers appeared to have only plateaued.
Strikingly, only a very small number of these papers were directed toward randomized control studies, the sine qua non of
high-grade clinical evidence. Moreover, very few papers make reference to cost analysis or economics in the treatment of hydrocephalus
— an issue coming increasingly before the nation at this point.

Love, addiction, attachment, commitment, co-dependence, community, parisitism, leaning on a crutch, all have in common the basic dynamics. You lose an inclination or ability to do something for yourself thereby becoming a part of a large whole.read more (Source: Psychology Today Relationships Center)

Abstract Unlike other industrialised countries, the UK deferred the routine introduction of disease-modifying therapies (DMTs) for
multiple sclerosis (MS) in favour of an experiment. Between 2002 and 2005, MS sufferers were identified, were offered DMTs
only if deemed suitable by their physicians, and were monitored thereafter to assess long-term outcomes. It has been demonstrated
for other therapies that judgements about suitability to receive treatment are conditioned by the patient’s deprivation status.
We hypothesised that this would have been the case for DMTs also. Using individual patient data for samples in Nottingham
and in Glasgow, we matched patients’ postcodes of residence with deprivation scores and confirmed that patients from more
deprived areas were les...

(University of Chicago Press Journals) US states that enacted unilateral divorce laws saw substantial increases in violent crime in the years following the reform, according to research in the Journal of Labor Economics. But the ill-effects of the new laws appear to be largely temporary. (Source: EurekAlert! - Social and Behavioral Science)

Context Asthma is common and flare-ups associated with exposure to a variety of triggers may result in emergency department (ED) visits, school and work absenteeism, hospitalisations and significant healthcare costs. The majority of adult patients presenting with acute asthma are discharged home following management in the ED. Many national and international guidelines have been developed on this topic.1 In the outpatient setting, care is focused on the use of systemic (SC) and inhaled corticosteroid (ICS) agents to regain asthma control. While prednisone is the most common SC used, there has been an interest in reducing the duration of treatment using agents with longer half life such as dexamethasone. Due to compliance and palatability concerns, this question has been most frequently add...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

This above all: to thine own self be true,
And it must follow, as the night the day,

Inflammatory rheumatic diseases are common. It is estimated that ~2.1% of the population has an inflammatory rheumatic disease (Andrianakos A, Trontzas P, Christoyannis F et al. Prevalence of rheumatic diseases in Greece: a cross-sectional population based epidemiological study. The ESORDIG Study. J Rheumatol 2003;30:1589–601). For diseases such as RA, PsA and AS, onset is most frequent between the ages of 30 and 50 years. The impact of inflammatory rheumatic diseases on physical functioning can be significant. Patients can suffer from swollen joints that cause pain and disability. This can reduce sufferers’ ability to lead fully productive lives. This has major financial consequences for sufferers and their families and there is an economic impact on society. The National Inst...

Scientists have created a genetically engineered microbe that turns the algae into low-carbon biofuel, but must make the technique commercially viableThe ancient art of seaweed farming could provide a solution to a 21st-century energy dilemma, with the creation of a genetically engineered microbe that turns the algae into low-carbon biofuel, scientists said on Thursday.Biofuels have been touted as low-carbon replacements for petrol and diesel, but those made from crops like corn and sugar have been blamed for increasing global food prices and delivering only modest benefits.Earlier studies have indicated that large-scale use of seaweed as an energy source could in theory supply the world's needs several times over and the UK government envisages between 560 and 4700 km sq of seaweed farms ...

Follow-up is over 2 years. The design of the study indicates recruitment from primary and secondary care of 204 participants who have a history of 3 or more episodes of Major Depression but who are currently well. Measures assessing depressive relapse/recurrence, time to first clinical intervention, treatment expectancy and a range of secondary outcomes and process variables are included. A health economics evaluation will be undertaken to assess the incremental cost of MBCT.DiscussionThe results of this trial, including an examination ...

ABSTRACTThe derivation of population norms using simple generic health‐related quality of life measures to inform policy has been recommended in the literature. This letter illustrates the derivation of population norms for the SF‐6D in the United Kingdom. It uses a sample of 22,166 respondents from the 2010 wave of the study Understanding Society. Understanding Society is a national representative sample of British citizens. The survey of this study contains the SF‐12. It is possible to derive health state utilities from the SF‐12 (and from the SF‐36) using a relatively new instrument, the SF‐6D. The SF‐12 and the SF‐36 belong to the most widely used generic health‐related quality of life measures.Mean SF‐6D utility scores for males and females are 0.81 and 0.79, respe...

Conclusions This study builds upon existing research having demonstrated the potential usefulness of mapping disease-specific instruments
onto utility measures. The specific issue of longitudinal validity is addressed, as mapping models derived from baseline patients
need to be validated on post-therapy samples. The underestimation of post-treatment utilities in the present study, at least
in more severe patients, warrants further research before it is prudent to conduct cost-utility analyses in the context of
RA by means of the MHAQ alone.

Abstract Cost-effectiveness analysis as a means to evaluate medical innovations has become well accepted in the UK and several other
Western countries. An important assumption underlying this method is that costs and effects are constant over time. In reality,
however, and especially in the short run, variations in costs and effects are likely to occur. These variations can lead to
considerable deviations from the outcome of a conventional economic evaluation, which in turn may lead to serious implementation
problems at a local level. Taking time into account explicitly in economic evaluations in health care may enhance their utility
for both societal and local decision making, and may ultimately smooth the adoption of new and basically cost-effective health
care technolog...

Conclusions: The cost-effectiveness of a screening intervention was largely dependent upon two key factors. As would be expected, the cost (per patient) of the intervention drives the model substantially, with higher costs leading to higher cost-effectiveness ratios. Likewise, the baseline prevalence (risk) of hearing impairment also affected the results. In scenarios where the baseline risk was low, the intervention was less likely to be cost-effective compared to when the baseline risk was high.

This article discusses specific issues related to the market access of biosimilars. Biopharmaceuticals are complex molecules
produced by living cells. Copies of these medicines, called biosimilars, are not identical to their reference medicine and
therefore specific regulatory requirements apply. When considering the use of biosimilars, the question of the degree of comparability
between a biosimilar and the reference biopharmaceutical needs to be considered for registration, pricing and reimbursement
purposes in addition to the cost issue. To date, many key concepts (like clinically meaningful differences) remain undefined
and the question of the degree of comparability is not yet resolved.

Large and Olav make a number of points in relation to my paper reviewing the area of the prediction of violence. They appear to make three substantive points. These relate to (1) whether risk is appropriately defined (2) whether in addressing the area of prediction sufficient consideration is given to developments in non-linear mathematics and (3) whether there is any evidence that the prediction of risk has had any positive effect in terms of reducing harm. Taking these in order, a reading of my paper will make clear that it was specifically addressed to the area of prediction of violence. While the broader areas of uncertainty, hazard, risk, risk assessment, risk management and the utility of risk assessment or management are certainly important topics in their own right, they were large...

Conclusions In higher transmission settings, EPI strategies will be most efficient, but vaccination additional to the EPI in targeted low transmission settings, even at modest coverage, might be more efficient than national-level vaccination of infants. The feasibility and economics of mass vaccination, and the circumstances under which vaccination will avert epidemics, remain unclear. The approach of using an ensemble of models provides more secure conclusions than a single-model approach, and suggests greater confidence in predictions of health effects for lower transmission settings than for higher ones.

The classical Metropolis sampling method is a cornerstone of many statistical modeling applications that range from physics, chemistry, and biology to economics. This method is particularly suitable for sampling the thermal distributions of classical systems. The challenge of extending this method to the simulation of arbitrary quantum systems is that, in general, eigenstates of quantum Hamiltonians cannot be obtained efficiently with a classical computer. However, this challenge can be overcome by quantum computers. Here, we present a quantum algorithm which fully generalizes the classical Metropolis algorithm to the quantum domain. The meaning of quantum generalization is twofold: The proposed algorithm is not only applicable to both classical and quantum systems, but also offers a quant...

Credit rating agencies keep getting it wrong but still make billions. Why don't politicians stand up to them? One point of view has counted more than any other in Europe over the past few days. After deciding on Friday night that France should be stripped of its top-of-the-range AAA credit rating, the agency Standard & Poor's has faced a lambastin g from EU commissioners. It has heard wheedling from France's prime minister: "We will do everything to get [the triple A] back." Back it or attack it, no politician, economist or commentator has denied that S&P's verdict matters. So much for the death of the critic.Sure, the carpers will bring up all the times the credit rating agencies have got it wrong in the past. Sophisticates might point out that America lost its triple-A last year and yet w...

CONCLUSION: While higher than expenditures previously reported for a cancer-only population in an urban Ontario setting, the costs were still within the parameters of the US Medicare Hospice Benefits, on a par with the per diem funding assigned for long-term care homes and lower than both average alternate level of care and hospital costs within the Province of Ontario. The study results may assist service planners in the appropriate allocation of resources and service packaging to meet the complex needs of palliative care populations.

ABSTRACTAlmost all health systems seek to offer some form of publicly financed healthcare insurance, and governments must therefore choose the size of the benefit package and the types of treatments to cover. Conventionally, the usual approach of economists has been to recommend choices on the basis of cost effectiveness of treatments, using metrics such as the ‘cost per quality adjusted life year’. However, this approach is based on the assumption of health maximization subject to a budget constraint and ignores the potential impact of any additional concern with protecting individuals from the financial consequences of a health shock. Furthermore, it does not take account of the possible availability of complementary privately funded health care. This paper develops a model in which ...
