Conclusions. Frequent attendance puts severe pressure on OHS resources. Much of the frequent attendance can be explained by the increased incidence of chronic diseases and psychiatric co-morbidity. (Source: Family Practice)

Effect of nutritional counselling and nutritional plus exercise counselling in overweight adults: a randomized trial in multidisciplinary primary care practice

Discussion. Nutritional counselling by a dietician resulted in modest reductions in weight and waist circumference in overweight adults, which were sustained up to 12 months. Adding exercise counselling by a physiotherapist did not significantly enhance the effect on body weight. Exercise counselling may, however, further improve waist circumference.
Conclusions. Overall, antidepressants were no more effective on headache intensity or frequency and analgesic use than placebo. Propranolol seemed to have negative effects on depression in TTH patients when compared with placebo or biofeedback. No evidence was found for the use of muscle relaxants alone or 5-HT receptor agonist.
Conclusion. In family practice, most children with acute non-traumatic hip pathology present with pain as the initial symptom. FPs need to be more aware that fever is the main distinguishing factor between a harmless condition and a life-threatening condition. Transient synovitis is the diagnosis with the highest incidence rate. (Source: Family Practice)

Conclusions. As the participants had to make multiple behavioural changes over time, advice needed to be delivered, reiterated and reinforced to achieve understanding and uptake. The more specific and personalized the information and advice, the more likely it was for the participant to give a positive and engaged response. Seizing every opportunity to deliver good quality personalized and/or responsive advice is essential in order to facilitate effective behavioural change. (Source: Family Practice)

Conclusions. Great variations exist in the quality of primary care for asthma and considerable scope for improvement. Asthma care improved over time. The preliminary findings that quality of asthma care varied with deprivation support the idea that primary care may be targeting care to those in most need. However, variations were small and only significant for two QIs. (Source: Family Practice)

Conclusions. Success of the national immunization programme will depend on overcoming the challenges of providing explanations to ensure that individuals understand the potential benefits of HPV vaccination. Primary care can play an important role, but this study suggests there are fundamental issues that need further clarification.
Conclusions:
The success of population-based screening for CRC will largely be determined by GP attitudes and support, particularly with regard to FOBt. Previous research has implied that South Asian GPs are more likely to have negative attitudes towards FOBt screening, however, our research suggests that this is not a group requiring targeted interventions to increase their support for the NHSBCSP. Of the available CRC screening tests, GPs perceived FOBt to be the most appropriate for population-based screening. (Source: BMC Family Practice)

Conclusions:
Documentation of preventive screening interventions by general practitioners was relatively low compared to the number of patients eligible for preventive screening. Some physicians opt to screen for PSA and DRE which is not recommended by the Canadian Task Force on Preventive HealthCare.
Background:
Though screening and intervention for alcohol and tobacco misuse are effective, primary care screening and intervention rates remain low. Previous studies that have increased intervention rates using vital signs screening for tobacco misuse and clinician prompts for screen-positive patients for both alcohol and tobacco misuse. This pilot study's aims were: (1) To determine the feasibility of combined vital signs screening for tobacco and alcohol misuse, (2) To assess the impact of vital signs screening on alcohol and tobacco screening and intervention rates, and (3) To assess the additional impact of tobacco assessment prompts on intervention rates.
Methods:
In five outpatient practices, nurses measuring vital signs were trained to routinely ask a single tobacco question, a pre...

KEYSTONE, COLO. — Small-particle–size inhaled corticosteroids may provide increased clinical efficacy in tough to control asthma with marked distal airways inflammation. “A lot of our asthma patients are really easy to treat—it doesn't matter what medication you use, so long as there's an inhaled steroid in there,” Dr. Richard J. Martin said at a meeting on allergy and respiratory disease. “But we should really ask in some of our more difficult to control patients if we are missing the inflammatory response in the distal lung.” (Source: Family Practice News)

In California, reducing maternal mortality has become a major public health goal. That's in part because the state has 550,000 births per year, or about 1 in 8 of the births in the United States. The other factor driving action is that state mortality data seem to indicate a rise in maternal mortality in recent years. (Source: Family Practice News)

I consider myself a computer literate physician. In high school, I taught myself to program in BASIC on my Atari 800, which had a whopping 48K RAM. In college, I transitioned to the Apple Macintosh. At medicine morning rounds, I saw the Palm Pilot Pro and had to have one. I've been desktop and mobile computing ever since. (Source: Family Practice News)

I have just read your piece about the two HPV vaccines (“Gardasil and Cervarix Are Not Interchangeable,” Family Practice News, Feb. 15, 2010, p. 8). I really understand the issues about these vaccines. I am also hounded by drug reps that believe they have the best vaccine. I read the data and it looks as though the answer will not be known for years. I don't think that most of our patients' families are going to understand these issues to be able to choose between the vaccines. I am the chair for our entire vaccine task force. I am struggling with this decision on the HPV vaccines. Is there more direction that you can offer me or point me to some other resource on which vaccine to recommend?
Misdiagnosis of melanoma is a major cause of litigation against both physicians and dermatopathologists. Of all claims between 1985 and 2001, 14% involved the misdiagnosis of melanoma, Dr. Ashfaq A. Marghoob reported at the annual Hawaii Dermatology Seminar sponsored by Skin Disease Education Foundation.
The use of nonsteroidal anti-inflammatory drugs does not reduce the risk of cutaneous squamous cell carcinoma, according to the findings of a retrospective study. In a case-control study involving more than 800 subjects, the dose, duration, and type of NSAID exposure exerted “no clear effect” on the risk of developing squamous cell cancer (SCC), said Dr. Maryam M. Asgari of Kaiser Permanente of Northern California, Oakland, and associates. (Source: Family Practice News)

NEW YORK — A wealth of new data is emerging that will help clinicians anticipate and manage endocrine complications of psychotropic drugs, according to a leading researcher. “We can't take a gene chip and come up with firm recommendations,” said Dr. Harold E. Carlson, who has published widely in the field as professor of medicine and endocrinology at Stony Brook (New York) University Health Sciences Center. But based on new data, he said, “We can think about individualized pharmacotherapy tailored to your patients' needs.'
Pregnant women with rheumatoid arthritis face an increased risk of adverse obstetric outcomes, and they deserve heightened prenatal attention, according to a recent report. Specifically, mothers with rheumatoid arthritis (RA) were 1.47 times more likely than unaffected mothers to have a low-birth-weight baby and 1.20 times more likely to have a baby deemed small for gestational age. Women with RA also had a higher risk for developing preeclampsia (adjusted odds ratio 2.22) or having to undergo a cesarean section (adjusted OR 1.19), according to investigators (Ann. Rheum. Dis. 2010 Feb. [doi:10.1136/ard.2008.105262]). (Source: Family Practice News)

One in every 640 young adults in the United States survived cancer as a child, and approximately two-thirds of them have at least one chronic health problem. Better cancer treatments in recent decades increased pediatric cancer survivor rates from below 50% to today's average of 80%, and as a result there is more focus on the long-term medical needs of survivors. Depending on their disease and the treatment they received, these patients are at higher risk for secondary cancers, cardiovascular and lung diseases, learning disabilities and memory difficulties, vision and hearing problems, or infertility.
The Food and Drug Administration has launched an initiative to reduce unnecessary radiation exposure from three types of medical imaging procedures: computed tomography, nuclear medicine studies, and fluoroscopy. The FDA said it will issue targeted requirements for device manufacturers to develop safer technologies and to provide training to support safe use. In addition, the agency said it will help develop a patient medical imaging history card for patients to track their own medical imaging history and share it with their physicians. The FDA also recommended that professional societies continue to develop diagnostic radiation reference levels for medical imaging procedures and increase their efforts to develop one or more national registries for radiation doses. “The goal of FDA's ini...

LAS VEGAS — The federal government is stepping up its audit activities in Medicare, and that could mean greater scrutiny of billing practices. One development that physicians should keep a close eye on is the recent nationwide rollout of Medicare's Recovery Audit Contractor program, said Edward R. Gaines III, vice president and chief compliance officer at CBIZ Medical Management Professionals Inc. The program, known as the RAC, began as a demonstration project in New York, California, and Florida.
A 6-month genetics educational initiative significantly improved the confidence and competence of family physicians, according to the findings of a small study. “Primary care providers are going to be increasingly involved in the delivery [of genetics services]. There's a huge literature out there that we're deficient in. We have to be able to assess risk. We have to know when to refer. We have to provide counseling and follow-up after test results. And we are probably increasingly going to be asked to tailor our preventive care, medication choice, and treatments to people's individual genetic profiles,” said Dr. June Carroll, the Sydney G. Frankfort Chair in Family Medicine at Mount Sinai Hospital and the University of Toronto. (Source: Family Practice News)

The White House is trying to get health care workers ready to help physicians in computerizing their medical records. Nearly $1 billion in American Reinvestment and Recovery Act awards will be made to help health care providers become “meaningful users” of health information technology and to train nurses and other allied health professionals for careers in health information technology. Jobs created will include nurses, pharmacy technicians, and IT technicians and trainers, according to Health and Human Services Secretary Kathleen Sebelius and Labor Secretary Hilda Solis. It's unknown which-if any-historical factors are most useful for identifying rotator cuff tears, because no studies evaluating their accuracy with rotator cuff tears have been done. As for physical findings, no single physical examination finding is sensitive or specific enough to detect partial-thickness rotator cuff tears. The combination of the painful arc sign, drop-arm sign, and infraspinatus muscle strength test are helpful in detecting a full-thickness rotator cuff tear. A negative supraspinatus muscle strength test alone is sensitive enough to decrease the likelihood of a significant rotator cuff tear.

Authors: Wiltz SA, Nelson RM
The best approach for screening patients hinges on the number, degree, and age of relatives diagnosed with colorectal cancer (CRC) or adenomatous polyps (AP). Screening should begin at 40 years of age for patients with a family history of CRC or AP in at least 1 first-degree relative or CRC in at least 2 second-degree relatives. Patients at highest risk-who have 1 first-degree relative diagnosed with CRC or AP before 60 years of age or multiple first-degree relatives diagnosed at any age-should begin screening with colonoscopy at 40 years of age or 10 years younger than the earliest affected relative and undergo a repeat colonoscopy every 5 years. Patients who have a first-degree relative diagnosed with CRC or AP after 60 years of age or 2 or more second-de...

First, consider possible causes of the inadequate response to an antidepressant, then weigh treatment options in light of the characteristics of the individual patient and therapy. When managing a patient with nonpsychotic depression and inadequate response to the maximum dose of a single antidepressant, the physician should first identify factors that may contribute to the poor response, such as suboptimal dosage resulting from nonadherence, inadequate duration of therapy, and comorbid medical and psychiatric conditions. The literature supports several treatment alternatives, including augmentation with cognitive therapy, switch therapy, and combination-augmentation therapy; not enough studies exist to recommend the best treatment. All options reviewed produced...

A 10-year-old African-American girl was brought to our clinic for right anterior thigh pain that she'd had for 3 weeks. She had been playing normally prior to the onset of pain and said she had not experienced any trauma or injury. She was unable to run due to the pain, but continued to walk with a limp. She denied any other joint pain, fevers, rash, or other constitutional symptoms. The patient, who was overweight and not in any distress, held her right leg in a slight external rotation. There was no asymmetry, deformity, or swelling in her right hip. She was slightly tender to palpation in the area of the right proximal anterior femur. The remainder of the right lower extremity was nontender. The patient's active flexion was limited to approximately 90 degrees,...

The Advisory Committee on Immunization Practices (ACIP) made a number of major new recommendations last year. These new recommendations address: expanded use of hepatitis A virus (HAV) vaccine; preferences for combination vaccines; timing of poliovirus vaccine doses; resumption of the normal Haemophilus influenzae Type b (Hib) schedule, as shortages have resolved; the use of a new bivalent human papilloma virus (HPV2) vaccine in women and quadrivalent (HPV4) vaccine in men; a reduced-dose schedule for rabies postexposure prophylaxis; proof of immunity against mumps, measles, and rubella for health care workers; and recommendations for meningococcal vaccine boosters.

Prescribe selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), or selected anticonvulsants as first-line treatment for generalized anxiety disorder (GAD). Avoid the use of benzodiazepines for long-term treatment of GAD. Do not recommend valerian, kava extract, or St. John's wort for the treatment of GAD; tell patients there is insufficient evidence of their efficacy.

CONCLUSION: This 5-weekend program, developed and tested in Ontario, stimulates community practitioners to learn how to answer research questions emerging from their practices. A 1-day seminar is adequate to train facilitators to successfully run these programs. Evaluations by both facilitators and program participants were very positive, with many participants stating that their clinical practices were improved as a result of the program. The program has been adapted for residency training, and it has already been used internationally.

CONCLUSION: For family physicians, successful implementation of the 2006 Canadian Clinical Practice Guidelines on the Management and Prevention of Obesity requires access to adequate training, compensation, and evidence-based interventions related to nutrition. This study highlights current nutrition counseling practices in family medicine and identifies several obstacles to integrating the current guidelines in primary care settings.

Conclusions:
The creation of a primary care team is a very challenging process that goes beyond the normative policy definitions of who is on the team or what the team has to do. To fulfil expectations of quality improvement through team-based care, health care professionals who are required to work together need shared time/space contexts to communicate; to overcome interprofessional and interpersonal conflicts; and to make sense of and define who they collectively are and what they do as a clinical team.
Background:
Abnormal results of diagnostic laboratory tests can be difficult to interpret when disease probability is very low. Although most physicians generally do not use Bayesian calculations to interpret abnormal results, their estimates of pretest disease probability and reasons for ordering diagnostic tests may - in a more implicit manner - influence test interpretation and further management. A better understanding of this influence may help to improve test interpretation and management. Therefore, the objective of this study was to examine the influence of physicians' pretest disease probability estimates, and their reasons for ordering diagnostic tests, on test result interpretation, posttest probability estimates and further management.
Methods:

Although the barriers to children and adults receiving vaccines differ, experts agree that new strategies are desperately needed to address the lack of compliance with national immunization guidelines. Simply recommending that a group get immunized is clearly not getting the job done.
SNOWMASS, COLO. — Serious questions exist about the safety and efficacy of the popular practice of high-dose vitamin D supplementation across a broad swath of the population. One of these concerns is that not all of the extra calcium absorption promoted by boosting vitamin D is going into bone to prevent fractures. Some of it may actually be taken up by atherosclerotic plaque, increasing the risk of cardiovascular events, Dr. Lenore M. Buckley cautioned at a symposium sponsored by the American College of Rheumatology. (Source: Family Practice News)

The Obama administration wants to combat waste, fraud, and abuse in the Medicare and Medicaid programs and plans to spend more than $500 million to do it. As part of the administration's budget proposal for fiscal year 2011, the Health and Human Services department is proposing to invest $561 million in discretionary funding to fight health care fraud, a $250 million increase over FY 2010. Specifically, the department plans to expand the Health Care Fraud Prevention and Enforcement Action Team (HEAT), which brings together high-level officials at HHS and the Department of Justice to spot trends and develop new fraud prevention tools.
