 which is an important correlate of lymphoid neoplasia. The most frequently used procedures for this purpose were developed
 by the BIOMED-2 consortium. One of the criteria for establishing monoclonality using PCR of the antigen receptor genes is
 the finding of an abundant amplicon within a size range determined by the positions of the PCR primers and the known variability

            <description>Abstract&amp;nbsp;&amp;nbsp;Recent successful trials of antibodies to vascular endothelial growth factor (VEGF) in diabetic retinopathy implicate this
 cytokine as a major cause of diabetic retinopathy (DR) and diabetic macular oedema (DME). The mechanisms which cause VEGF
 to be over-expressed to cause the vasculopathy are not entirely clear. This review explores the earliest changes to the retina
 in DR and the factors that predispose or prevent DR, including sleep apnoea, receptor degenerations laser treatment and VEGF
 polymorphism. The review also presents the evidence that retinal hypoxia, existing in the earliest stages, causes DR. This
 hypoxia is much increased by dark adaptation, indicating a new and possibly superior therapy.

            <title>Foot lymphoedema—is there a role for surgery?</title>

            <description>We describe a case of symptomatic foot lymphoedema that was successfully treated
 with radical debulking, skin grafting and compression garments. Our aim is to highlight the lack of literature on the surgical
 management of foot lymphoedema and, more importantly, to stimulate discussion on the role of debulking surgery in the management
 of symptomatic refractory foot lymphoedema. We conclude that, although amputation always remains a potential risk, in selected
 cases, debulking of a lymphoedematous foot should be attempted before amputation, as it significantly improves quality of
 life. We highly recommend debulking and reconstruction as a limb-saving approach with a significant improvement in quality
 of life.

            <description>Abstract&amp;nbsp;&amp;nbsp;Prolactin (PRL) is well recognised for its role(s) in mammary gland development and function. Moreover, its role in skin biology,
 including the potent regulation of human hair growth, is becoming clearer. Less widely appreciated, however, is the potential
 role of PRL in the pathobiology of psoriasis. While the relationship between PRL and psoriasis remains enigmatic, several
 recent publications on the PRL–psoriasis connection have demonstrated a reawakening of interest in this conundrum. We take
 the occasion of these reports to underscore the importance of dissecting the role(s) of PRL in the aetiopathology of psoriasis,
 not least since this may help to identify novel hormonal treatment strategies in its management.

Sequencing of the sodA gene is of little additional value in discriminating between species, but M. massiliense can be rapidly identified by amplification of the truncated erm(41) gene without the need for amplicon sequencing. We have applied the method to 81 isolates from 40 patients from two hospitals,

            <description>Abstract&amp;nbsp;&amp;nbsp;Vascular injury following hip resurfacing arthroplasty is rare: we present a case of profunda femoris pseudoaneurysm complicating
 hip resurfacing arthroplasty performed via an anterolateral approach. Over recent years, it has been recognized that an adverse
 reaction to metal debris from metal-on-metal arthroplasties can also result in the development of cystic masses around the
 hip. This case highlights a potential rare differential diagnosis that needs to be considered when imaging cystic mass lesions
 around the postoperative hip. Imaging can provide a definitive diagnosis if this entity is considered and its appearances
 recognized, resulting in appropriate management of this potentially limb- and life-threatening complication.

            <description>Abstract&amp;nbsp;&amp;nbsp;The risks of cancers other than breast and ovarian amongst BRCA1 and BRCA2 mutation carriers are based on relatively few family based studies with the risk of specific cancers tested in population
 based samples of cancers from founder populations. We assessed risks of “other cancers” in 268 BRCA1 families and 222 BRCA2 families using a person years at risk analysis from 1975 to 2005. Cancer confirmations were overall higher than in previous
 family based studies at 64%. There was no overall increase in risk for BRCA1 carriers although oesophagus had a significant increased RR of 2.9 (95% CI 1.1–6.0) and stomach at 2.4 (95% CI 1.2–4.3),
 these were based mainly on unconfirmed cases. For BRCA2 increased risks for cancers of the pancreas (RR 4.1, 95% CI 1.9–7.8)...</description>

            <description>Abstract&amp;nbsp;&amp;nbsp;On routine neonatal examination, a newborn term male was noted to have unilateral enlargement of the right lower limb, loose
 thickened red skin over the palm and widening of all the fingers on the right hand. His body was pinker and warmer on the
 right side compared with the left and he had a right undescended testicle and hypoplastic scrotum. Radiological examination
 of the lower limbs demonstrated the enlargement of the soft tissue of the right lower limb compared to the left (Fig. 1). Therefore, the diagnosis was unclear from this constellation of findings and an ophthalmic assessment was requested.

            <description>The Daily Telegraph has today revealed the findings of the latest Dr Foster Hospital Guide. The guide, published annually, closely scrutinises a range of healthcare data to measure hospital performance and detect trends that could save lives.
As well as listing the hospital trusts in England that score above and below average on a range of different mortality measures, this year’s guide also found that:

  The rate of patient deaths in England is 20% lower than it was 10 years ago, in part because of improved hospital care. 
  For certain conditions, patients admitted to hospital at the weekend are less likely to get treated quickly and have a higher chance of dying.

Rates of CDI increased sharply in the UK from 2002 reaching a peak in 2007. National guidance on managing CDI was first published in 1994 with further updates in 2000 and 2008. Since 2006 health care providers in the United Kingdom have been set targets to reduce CDI rates. Despite rigorous infection control practices, the rates of CDI at County Durham and Darlington Foundation Trust (CDDFT) failed to fall in line with national rates and as a result the Trust was placed in escalation by the Health Care Commission with 6 months to resolve the situation.             <description>Abstract&amp;nbsp;&amp;nbsp;Anatomic placement of the femoral tunnel in anterior cruciate ligament (ACL) reconstruction confers biomechanical advantages
 over the traditional tunnel position. The anteromedial portal technique for anatomic ACL reconstruction has many well-described
 technical challenges. This paper describes the TransLateral technique. The technique produces anatomic femoral tunnel placement
 using direct measurement of the medial wall of the lateral femoral condyle and out to in drilling. All work is carried out
 through the lateral portal with all viewing via the medial portal. There is no need for an accessory medial portal or hyperflexion
 of the knee.

 cancer risk, particularly breast cancer, but many of these ‘first generation’ studies had study design and analysis flaws,
 and were inconclusive. A small number of ‘second generation’ studies are now emerging in which the applied pharmaco-epidemiological
