Recommendations in pediatrics call for general developmental screening of young children; however, research suggests social-emotional development, in particular, is important as an initial indicator of general well-being versus risk, and may warrant inclusion in screening protocols.
Via a social-emotional screening program, significant percentages of children can be identified as being at risk for social-emotional problems, and colocation of an early childhood psychologist promotes the ability to effectively address young children&rsquo;s social-emotional development within their medical home.
School-based health centers (SBHCs) have become an important method of health care delivery for the youth of our nation. Although they only represent 1 aspect of a coordinated school health program approach, SBHCs have provided access to health care services for youth confronted with age, financial, cultural, and geographic barriers. A fundamental principle of SBHCs is to create an environment of service coordination and collaboration that addresses the health needs and well-being of youth with health disparities or poor access to health care services. Some pediatricians have concerns that these centers are in conflict with the primary care provider&rsquo;s medical home. This policy provides an overview of SBHCs and some of their documented benefits, addresses the issue of potential confli...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Please support the <a href="http://www.doctorsinchains.org/" target="_blank">Doctors In Chains</a> campaign for the <a href="http://www.doctorsinchains.org/">medics</a> tortured and sentenced for up to 15 years in <a href="http://www.doctorsinchains.org/">Bahrain</a>. <a href="https://twitter.com/#!/search/%23FreeDoctors">#FreeDoctors</a></p></div>

Although most health care services can and should be provided by their medical home, children will be referred or require visits to the emergency department (ED) for emergent clinical conditions or injuries. Continuation of medical care after discharge from an ED is dependent on parents or caregivers&rsquo; understanding of and compliance with follow-up instructions and on adherence to medication recommendations. Approximately one-third of patients fail to obtain priority medications from a pharmacy after discharge from an ED. The option of judiciously dispensing ED discharge medications from the ED&rsquo;s outpatient pharmacy withi...

With its own privately coordinated medical home project in full swing, Central Ohio is not part of a growing state initiative to train primary care doctors as wellness quarterbacks.

The patient-centered medical home (PCMH) delivery model is expected to coordinate and improve the quality of care, but little research has investigated the effects of the PCMH on provider and support staff morale, according to a report published Jan. 9 in the Archives of Internal Medicine. To learn how they feel, researchers surveyed employees of healthcare organizations with PCMH-like characteristics and received positive overall responses.
CONCLUSIONS&nbsp;&nbsp;Having an increasing number of first-access components in a primary care office may improve preventive services receipt, and
more components may be required for those services requiring greater provider contact (e.g., prostate exam) versus those that
require less (e.g., mammography). In primary care redesign, the largest gains in preventive services receipt likely will come
with redesign of multiple components simultaneously. While our study is a necessary step towards broadly understanding the
relationship between first-contact access and preventive service receipt, other important questions remain. Certain components
may drive greater improvements in the receipt of different services, and the effect of some of these components may depend
on individual patient...

ABSTRACT&nbsp;&nbsp;Improving health literacy is one key to buoying our nation’s troubled health care system. As system-level health literacy
improvement strategies take the stage among national priorities for health care, the patient-centered medical home (PCMH)
model of care emerges as a compelling avenue for their widespread implementation. With a shared focus on effective communication
and team-based care organized around patient needs, health literacy principles and the PCMH are well aligned. However, their
synergy has received little attention, even as PCMH demonstration projects and health literacy interventions spring up nationwide.
While many health literacy interventions are limited by their focus on a single point along the continuum of care, creating
a “room” for he...

Patient-centered medical homes (PCMH), healthcare organizations that employ a coordinated delivery model designed to provide cost-effective care, are emerging as efforts to cure the fragmented nature of the healthcare system have escalated. However, there are few standards to guide evaluations of PCMHs' effectiveness, which has prompted the Agency for Healthcare Research and Quality (AHRQ) to make PCMH evaluation recommendations. The program offers three levels of accreditation based on the readiness of the ACO. As with the NCQA's recognition program for patient-centered medical homes, achieving accreditation at each level is based on accruing a certain number of points by meeting standards and fulfilling some “must-pass” criteria.
CONCLUSIONS: While previous studies have described expected barriers to hypothetical DA distribution, our analysis reveals barriers reported by physicians, staff, and patients based on actual experience in practice. Decision aids can improve patient participation in health decisions. However, physicians and patients have to recognize a need for these tools before they will be adopted in practice.

Elizabeth Galik It is an exciting time to be involved in the care of older adults. The burgeoning aging population with declining numbers of clinicians with geriatric expertise, health care reform, and the advancement of new models of care, such as medical homes, will require advanced practice nurses who care for older adults to serve as clinical experts, work collaboratively with members of the interprofessional team, and inspire and educate the next generation of clinicians. During the past year, the Gerontological Advanced Practice Nurses Association (GAPNA) completed a strategic planning process to set goals and priorities so that we may continue to grow and be successful in an ever-changing health care environment. GAPNA’s mission is to promote excellence in advanced practice nursi...

Advances in a wide range of biological, behavioral, and social sciences are expanding our understanding of how early environmental influences (the ecology) and genetic predispositions (the biologic program) affect learning capacities, adaptive behaviors, lifelong physical and mental health, and adult productivity. A supporting technical report from the American Academy of Pediatrics (AAP) presents an integrated ecobiodevelopmental framework to assist in translating these dramatic advances in developmental science into improved health across the life span. Pediatricians are now armed with new information about the adverse effects of toxic stress on brain development, as well as a deeper understanding of the early life origins of many adult diseases. As trusted authorities in child health an...

Additional strategies are needed to protect children from vaccine-preventable diseases. In particular, very young infants, as well as children who are immunocompromised, are at especially high risk for developing the serious consequences of vaccine-preventable diseases and cannot be immunized completely. There is some evidence that children who become infected with these diseases are exposed to pathogens through household contacts, particularly from parents or other close family contacts. Such infections likely are attributable to adults who are not fully protected from these diseases, either because their immunity to vaccine-preventable diseases has waned over time or because they have not received a vaccine. There are many challenges that have added to low adult immunization rates in the...

CONCLUSIONS: Transforming practices to more patient-centred models of care will be a priority for primary care providers. Identifying opportunities and challenges associated with implementing change is critical for successful improvement programmes. Successful strategies for enhancing the adoption and uptake of PCMH elements should leverage areas of concordance between practice members' perceived needs and planned improvement efforts.

Care management saved N.C. Medicaid nearly $1 billion over four years, according to a new analysis by a national health care consulting firm.

The report by Milliman Inc., an actuarial firm based in San Diego, examined the impact of the state’s support for primary care “medical homes” – a system to coordinate health care for Medicaid recipients. Milliman’s report, which was required by the General Assembly, found that recipients with a medical home get better care and consumed fewer Medicaid resources than those who lack a medical home, according to a news release issued by Gov...
GraceMed Health Clinic is expanding the hours of its recently acquired Mother Mary Anne Clinic sooner than initially planned.

CEO Dave Sanford says the clinic will be open 8 a.m. to 9 p.m. starting Jan. 2.

“Since its inception, the Mother Mary Anne Clinic has exclusively been an immediate or urgent care center,” Sanford says. “With expanded hours, the clinic will now be available as a medical home, a place for residents of the area to receive continuous care from the same provider.”

GraceMed took over operations of the clinic from Via Christi Health earlier this month... (Source: bizjournals.com Health Care:Physician Practices headlines)

You may have heard the term “medical home”—it’s been bandied about recently as something we all should have. No, it’s not a nursing home. Nor is it a house well-stocked with Band Aids and Tylenol, or one where doctors live.
The American Academy of Pediatrics defines medical home as “a model of care that is accessible, family-centered, continuous, comprehensive, coordinated, compassionate, and culturally effective.”
Well, that sounds exceedingly lovely. Of course we’d all want that. But still, what does it really mean?
Let’s get more concrete. You have a medical home if:

You can get the care you need when you need it
You can easily get the health advice or information you need
You can easily get the specialty care, services, or equipment you need

2011 Hot Topics Session

Huddling with nursing and clinical staff is an established principle to improve efficiency and care quality as part of the Patient-centered Medical Home. Resident participation in huddles provides increased interaction with staff and improves office flow and scheduling. Residents acquire a practical skill that they can incorporate into their practice immediately. The huddle provides residents and staff teaching in anticipatory guidance, chronic disease management, and preventive care. It allows the preceptor to manage the flow of patients and residents during the session. Use of the huddle allows for enhanced education, increased satisfaction with precepting, and may generate additional revenue through teaching of practice management and coding

With the growth of the Patient-centered Medical Home (PCMH) as a construct for practice transformation, there has been an increasing need for health care organizations and their leadership to engage in active change management to limit resistance, reduce fatigue, and ensure successful implementation. An effective method of negotiating the change process is known as interest-based negotiation. In this seminar, a community-based family medicine residency program will share its experience with the early stages of PCMH transformation and how interest-based negotiation was utilized to set strategic priorities. Participants will then practice this methodology through role-playing in conflict-based case studies. (Source: Family Medicine Digital Resources Library (FMDRL) Recently Uploaded)

When you hear the word robot, which image comes to mind first?

Those of us raised on Star Wars and Buck Rogers are likely to identify with the first image, but physically speaking, the robots of today have more in common with your computer and microwave than a Hollywood android.
They may look less interesting than your favorite sci-fi film characters, but modern medical robots are still quite helpful. So much so that the Boston Globe recently ran a story about a pilot project that placed a medical robot created by VGo Communications in the home of the Tally family, whose 2 year-old son Aidan is recovering from surgery he received at Children’s Hospital Boston last month to treat his urinary reflux .
The VGo robot’s main function is videoconferencing, which connects the Ashland-based ...

PHILADELPHIA, Dec. 12, 2011 (GLOBE NEWSWIRE) -- Independence Blue Cross (IBC) today announced a new collaboration with the American College of Physicians (ACP) to improve primary care. IBC is offering up to 100 practices free, one-year access to a new on-line tool that will help identify ways to better serve their patients. The tool, known as the Medical Home Builder 2.0, is the most recent example of how IBC is working with area physicians and respected professional organizations to promote patient-centered care. Since the beginning of 2011, IBC has more than tripled the number of recognized practices to nearly 200 in southeastern Pennsylvania. (Source: Medical News (via PRIMEZONE))

This presentation shares information gained from working with more than 275 physicians in more than 100 practices both large and small, including private practices, residency practices and hospital-owned practices. Experts have identified eight key steps that should be initiated when introducing transformative changes into an organization. When some of these steps are overlooked, rushed, not taken or implemented incorrectly, barriers and challenges arise. Information for this presentation focuses on reviewing the eight key steps to change, barriers and challenges that developed in practices on the journey to become a patient-centered medical home when the eight step process was not utilized and interventions that were put in place to assist in overcoming the barriers and challenges that em...

Abstract&nbsp;&nbsp;Providing a medical home to children with Attention-Deficit Hyperactivity Disorder (ADHD) is challenging. Little is known
about the factors associated with having a medical home for these children, or how comorbidities affect having a medical home.
Our study aims are: (1) identify factors associated with having a medical home and five sub-components of a medical home and
(2) determine the effect of medical home on several outcomes for children with ADHD. The sample included 5,495 children with
ADHD from the 2007 National Survey of Children’s Health. Descriptive and multivariate analyses were conducted. Children with
ADHD alone and children with ADHD plus a physical diagnosis had greater frequencies of having a medical home, or meeting the
five sub-components, th...

Around the world, adults with serious illnesses or chronic conditions account for a disproportionate share of national health care spending. We surveyed patients with complex care needs in eleven countries (Australia, Canada, France, Germany, the Netherlands, New Zealand, Norway, Sweden, Switzerland, the United Kingdom, and the United States) and found that in all of them, care is often poorly coordinated. However, adults seen at primary practices with attributes of a patient-centered medical home&mdash;where clinicians are accessible, know patients&rsquo; medical history, and help coordinate care&mdash;gave higher ratings to the care they received and were less likely to experience coordination gaps or report medical errors. By putting together teams, aligning around shared goals, and mentoring team members, collaborative work flows can develop that are self sustaining and don't require "heroic" input from traditional or acknowledged leaders.
Children's social, emotional, and physical health; their developmental trajectory; and the neurocircuits that are being created and reinforced in their developing brains are all directly influenced by their relationships during early childhood. The stresses associated with contemporary American life can challenge families' abilities to promote successful developmental outcomes and emotional health for their children. Pediatricians are positioned to serve as partners with families and other community providers in supporting the well-being of children and their families. The structure and support of families involve forces that are often outside the agenda of the usual pediatric health supervision visits. The program offers three levels of accreditation based on the readiness of the ACO. As with the NCQA's recognition program for patient-centered medical homes, achieving accreditation at each level will be based on accruing a certain number of points by meeting standards and fulfilling some “must-pass” criteria. Abstract
There is general consensus that our current healthcare delivery system will not be able to supply an adequate workforce, contain costs, and meet the ever-increasing chronic-care needs of the growing and aging population in the United States (US). Some of the major challenges to the U.S. healthcare system are faced by those on the front lines, namely the healthcare workers in primary care. Part of the emerging solution for primary care is the adoption of the Patient-Centered Medical Home Model. The intent of this model is to provide coordinated and comprehensive care rooted in a strong collaborative relationship. Carilion Clinic in Southwestern Virginia is implementing this patient-centered model in which a proactive, multidisciplinary care team...

    Abstract
Abstract:Examination of Latino children in aggregate ignores important subgroup differences due to the parents' English language ability. Previous reports of the pediatric medical home have not stratified Latino children by parental language differences to compare the two groups directly. We analyzed the 2007 National Survey of Children's Health to determine medical home prevalence among Latino children, stratified by language of parental interview. Most Latino children with a Spanish-language parental interview had a usual source of care, but only one-quarter had a medical home. Striking medical home disparities persisted for Latino children with a Spanish-language interview, even after adjustment for potential conf...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Find the best <a href="http://www.januarysales.org/" target="_blank">January Sales</a> in the UK.</p></div>

It has been five years since the AAFP adopted a policy stating that every American should have a "personal medical home." In the years since then, the Academy has championed the idea of the patient-centered medical home, or PCMH, with the belief that the PCMH model improves outcomes and lowers health care costs. Although many of you have started -- and in some cases completed -- the long process of transforming your practices into officially recognized medical homes, some of you have decided that the necessary time, money and effort make it too daunting a task relative to an uncertain financial benefit. In short, you want assurances that your sizable investment will pay off. (Source: As We See It: Voices From the AAFP)

Understanding social determinants of health, such as the social and physical conditions under which children and their families live, work, and play, is essential to reducing disparities and improving the quality of primary care. We studied the relationship between perceptions of neighborhood characteristics such as cohesion, safety, physical environment, and whether children receive care from a patient-centered medical home. We found that place matters. Children were less likely to have access to a medical home if they were from less socially cohesive neighborhoods, less safe neighborhoods, or neighborhoods with fewer amenities. These associations persisted even after adjustment for socioeconomic factors and proved to be more strongly associated than race and income. Our findings undersco...

Conclusions: Previous studies comparing ASD prevalence between NHW and Hispanic children based on a composite Hispanic grouping without consideration of parental nativity likely missed important differences between these racial-ethnic groups. Continuing efforts toward improving early identification in Hispanic children are needed. (Source: Disability and Health Journal)

Abstract&nbsp;&nbsp; The patient-centered medical home model has the potential to reduce healthcare disparities among immigrant children. The purpose
of this study is to examine the relationship between medical home (MH) participation and receipt of preventive services among
immigrant children age 0–17. The study employed extant data from the National Survey of Children’s Health, 2007 (NSCH). Logistic
regression analyses were employed to assess the relationship between receipt of preventive services and MH status among immigrant
and non-immigrant children. Due to primarily the lack of family-centered care, only 40% of immigrant children met the medical home criteria versus approximately 62% of non-immigrant children. Immigrant
children have decreased odds of receiving preventive ca...

Abstract&nbsp;&nbsp;Few studies have examined potential factors that contribute to low influenza vaccination rates among minority children. This
study aimed to assess the prevalence of early childhood influenza vaccination among young black and Latino children, living
in inner-city neighborhoods, and examine the effects of child, caregiver and health system factors. Secondary data analysis
was performed using a survey about medical home experiences conducted from May 2007–June 2008. The study sample was limited
to children ≥6&nbsp;months in any influenza season prior to the 2006–2007 influenza season. Bivariate analyses and multivariable
logistic regression tested associations between influenza vaccination receipt and socio-demographic and health system characteristics.

The patient-centered medical home (PCMH) is a model of practice that has been proposed to address the many ills of our current health care delivery and financing systems. At its heart is a primary care practice that provides comprehensive, coordinated, high-quality, personalized care. Integral to the success of the PCMH model is a “neighborhood” of specialists who subscribe to the principles of the PCMH. Nephrologists will have an opportunity to practice within this framework, either as the PCMH itself or, more likely, as “neighbors” to the “home.” The effective and enthusiastic participation of nephrologists and other specialists will depend on the details of the model, not the least important of which is the financial structure.
CONCLUSIONS Achieving medical home recognition was associated with improvements in quality and patient satisfaction for these clinics, but the rate of improvement is slow and does not always exceed levels in the surrounding community in Minnesota (which are also improving). Expectations for large and rapid change are probably unrealistic.

    Abstract
BACKGROUND AND OBJECTIVES: Oregon Health & Science University Family Medicine implemented a balanced scorecard within our clinics that embraces the inherent tensions between care quality, financial productivity, and operational efficiency. This data-driven performance improvement process involved: (1) consensus-building around specific indicators to be measured, (2) developing and refining the balanced scorecard, and (3) using the balanced scorecard in the quality improvement process. Developing and implementing the balanced scorecard stimulated an important culture shift among clinics; practice members now actively use data to recognize successes, understand emerging problems, and make changes in response to these problems. Abstract
BACKGROUND AND OBJECTIVES: The Patient-centered Medical Home (PCMH) concept, essential functions of primary care, and an appropriate primary care workforce have been shown to improve health outcomes and reduce health care costs. This paper describes a curriculum that uses the PCMH and related health policy concepts as the foundation for didactic, clinical, and self-directed elements. The PCMH-focused curriculum improved overall student clerkship ratings and maintained student performance on the National Board of Medical Education subject exam.

J Clin Hypertens (Greenwich).Team‐based care is one of the key components of the patient‐centered medical home. Studies have consistently demonstrated that teams involving pharmacists or nurses in patient management can significantly improve blood pressure control. These findings have been demonstrated in several meta‐analyses and systematic reviews. These reviews have generally found that team‐based care can reduce systolic blood pressure by 4–10 mm Hg over usual care. However, these reviews have also concluded that many of the studies had various limitations and that additional research should be conducted. The present state of the art review paper will highlight newer studies, many of which were funded by the National Institutes of Health. Newer strategies involve telephone an...

Like many people across the country, I was sad to hear about the passing of Steve Jobs.
I grew up with the technology Mr. Jobs created. My 5th grade final science project entitled “The Moray Eel” was typed on an Apple II computer. My college soundtrack was powered by a first generation iPod and nowadays I keep in touch with my sister who lives in Spain using face-time on my iPad.
The technology Steve Jobs created radically changed how we interact with computers, the way we listen to music, even the way we communicate with each other. Mr. Jobs was an innovator on a grand scale; some say a Thomas Edison or ...

ConclusionsLatinos and African Americans were at highest risk of missing planned primary care appointments. PAK was associated with a greater reliance on same‐day visits and substantively poorer clinical outcomes. These results have important implications for public health and health plan policy, as primary care rapidly expands toward open access to care supported by the patient‐centered medical home model.
Wisconsin Department of Health Services Secretary Dennis Smith and and Deputy Secretary Kitty Rhoades spent Friday at Wisconsin State Fair Park listening to concerns from Milwaukee-area residents who will be affected from the proposed Medicaid cuts.

The town-hall meeting started with an hour-long presentation by Smith who explained the need to cut more than $500 million from the Medicaid program and the reasoning behind the state Department of Health Service's proposal released Sept. 30.

That proposal includes shifting 215,000 Medicaid recipients to a lower-cost plan with fewer benefits; increasing the number of medical homes for at-risk individuals and people with chronic conditions; and pay for performance measures for hospitals...
