Huddling with nursing and clinical staff is an established principle to improve efficiency and care quality as part of the Patient-centered Medical Home. Resident participation in huddles provides increased interaction with staff and improves office flow and scheduling. Residents acquire a practical skill that they can incorporate into their practice immediately. The huddle provides residents and staff teaching in anticipatory guidance, chronic disease management, and preventive care. It allows the preceptor to manage the flow of patients and residents during the session. Use of the huddle allows for enhanced education, increased satisfaction with precepting, and may generate additional revenue through teaching of practice management and coding

Objective: To study chiropractors graduated in Brazil in the role of practice managers. Methods: A standardized questionnaire with 31 multiple-choice questions and one open-end question was created, with a focus on understanding the factors that interfere with the opening of a chiropractic practice and challenges faced by those professionals to keep their practice active, as well as to verify possible factors related to a successful practice. This instrument was formatted to the program Google Docs and sent by electronic mail to the total population of chiropractors graduated from Brazilian university level chiropractic courses (n=341). Further electronic messages and telephone calls were made to ensure the largest response rate possible. (Source: Clinical Chiropractic)

    Abstract
In recent years, there has been a dramatic increase in the number of adults receiving comprehensive orthodontic treatment. In clinical practice, management of adults may be somewhat different than for most adolescents. Adults are more likely than adolescents to have dentitions that have undergone some degree of mutilation over time, which may necessitate alterations in the treatment strategy. Excessive wear, missing and compromised teeth are some of the differences observed. For adult patients who have experienced bone and attachment los...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

If only I could just focus on treating patients and not have to worry about managing and marketing my practice, I would be so much happier. Virtually every orthodontist has had thoughts like this, particularly during the difficult economic times we have been going through during the last couple of years. The purpose of this issue of Seminars in Orthodontics is to help the practicing orthodontist to successfully address the most common issues in the management and marketing of the orthodontic practice today. To achieve this goal, the guest editors have brought together a wide variety of the country's best-known and experienced experts, both practice management consultants and practicing orthodontists, to provide advice, suggestions, and guidelines that may aid the orthodontist in successfu...

The Financial and Performance Monitors Orthodontists Should Use to Understand the Status and Performance of Their Practice

Flying an airplane without instruments is sometimes called “flying by the seat of your pants.” Although it is possible to fly this way, it is not the most effective and safe way to fly, especially over long distances. Managing a practice “by the seat of your pants” is also possible, but it is not the most effective and efficient practice management philosophy as a long-term approach. Present-day orthodontists must pay the same attention to the performance of the practice as is paid to clinical treatment protocols. This vital monitoring information must be carefully linked to other practice management systems, such as the practice budget, practice goals, staff training, employee performance reviews, salaries, and incentive bonuses.
Historically, gastroenterologists entered into solo, small group, or academic practices. The current economic environment and looming regulatory mandates have led to gastroenterologists integrating into large, single-specialty groups to acquire costly practice infrastructure, gain negotiating leverage with health plans, promote high-quality care, and benefit from professional practice management. Individual gastroenterologists must assess whether a large practice will meet their personal goals, financial needs, and professional visions. The decision to integrate into a large practice will also be affected by local practice patterns and regulatory issues. For these and other reasons, gastroenterologists are going big in private practice. (Source: Gastrointestinal Endoscopy Clinics of North ...

While there is considerable interest in bundling payments to health care providers to encourage them to cut costs, putting the strategy into practice is proving to be more difficult than anticipated. That's the lesson being drawn from a new RAND Corporation study that examined the first three years of a major effort designed to test the bundled payment approach to health care financing. Under bundled payments, doctors, hospitals and other health providers share one fee for treating all aspects of a procedure such as a hip replacement or a chronic disease such as diabetes... Abstract
Venous leg ulcers are a major health issue in terms of financial burden to the NHS, nurses' input, and physical, psychological and social impact to the patient. The best practice management of leg ulcers is the application of high compression, which is a complex task and requires substantial skill and knowledge. Healing and recurrence rates are poor in some cases and this adds to the physical and psychosocial impact, as well as the financial burden. Many of the sequelae of ulceration hinder patients' ability to tolerate treatment. Nurses must acknowledge the difficulties patients face and become skilled in holistic assessment, care planning and the ...

The goal of delivering quality health care at a lower overall cost through accountable care organizations (ACO) as described as part of the Patient Protection and Affordable Care Act of 2010 (ACA) has gained momentum among payers, providers, and the public. The concept includes developing an organization that provides incentives for quality care while lowering the overall cost of the care. If the goals are met, then both providers and payers would be able to share in the financial savings produced by the lower health care costs achieved while improving the quality of care delivered.
CONCLUSION: Despite being an important practice factor, this is the first study to reveal the negative influence of PHNs' autonomy when in interaction with an excessive workload. Significant workload findings and the presence of generational differences suggest the need for development of workload measurement tools and public health human resource strategies tailored to a multi-generational workforce.

    Abstract
You have some reliable and outstanding performers on your team ... but dealing with their idiosyncrasies on a daily basis makes you question whether or not they're worth putting up with. Whether you work in a large corporation or a medical practice, your opportunities are surprisingly similar because it always comes down to having the right people, doing the right job. So how do you live with people who don't always do what you want them to do, when you want them to do it; and worse yet, sometimes don't do it right or simply get under your skin for a gazillion reasons? Read on for advice on how to manage your difficult employees without sacrificing their productivity.

    Abstract
A set of standard operating procedures (SOPs) provides a clinical research department with clear roles, responsibilities, and processes to ensure compliance, accuracy, and timeliness of data. SOPs also serve as a standardized training program for new employees. A practice may have an employee that can assist in the development of SOPs. There are also consultants that specialize in working with a practice to develop and write practice-specific SOPs. Making SOPs a priority will save a practice time and money in the long run and make the research practice more attractive to corporate study sponsors.

    Abstract
It is true that most doctors do not enjoy public speaking. Most physicians, although good with one-on-one conversations with patients, are out of their comfort zone when it comes to speaking in front of other physicians, colleagues, and even lay persons. This three-part article will discuss the preparation, the presentation, and what you need to do after the program is over in order to become an effective public speaker.

    Abstract
Effective provision of physician services and the financial performance of physician practices depend on both cost and price. While there has been much discussion and research on the differences among physician organizations, particularly pertaining to cost and efficiency, little attention has been paid to how prices received for services have changed over time. In order to address this void in the literature, we focus on the trends in prices paid for services rendered by two different organizational structures, namely single- and multispecialty physician groups. In particular, we examine the Producer Price Index for each physician group over the period of 1994 to 2010.

    Abstract
In today's complex business environment, it is more essential than ever to recognize that talent selection is critical and therefore must be at the top of a manager's list of priorities. It is very difficult to ferret out the important behavioral, motivational, and judgment traits that distinguish an inferior choice from a superior performer. Adding people with high performance capability and low turnover potential adds tremendously to the bottom line. Employing modern psychometric tools that accurately measure total human potential have been categorically proven to: (1) enhance overall productivity; (2) reduce employee attrition; and (3) reduce overall hiring costs significantly. Hiring right every time also mitigates potential damage done to th...

Authors: Cassatly MG, Bergquist WH
    Abstract
There has been a long-established covenant in healthcare existing between physicians and society. This agreement, steadfast for decades, has been abrogated by society beginning in the 1990s with the arrival of managed care. Among other things, the broken covenant has resulted in the early retirement of physicians, thereby exacerbating the projected shortage of physicians for the U.S. healthcare system. Executive business coaching of physicians to increase their career satisfaction is presented as a solution to abate physicians retiring prematurely and thus meet society's growing demand for medical practitioners.

    Abstract
Emergency department (ED) utilization for non-emergent conditions is costly and inefficient. We evaluated whether a simple office-based intervention could encourage patients with non-emergent conditions to be seen in the office instead of in the ED. The study compared ED utilization of patients of the intervention family medicine clinic to those of the nearby internal medicine clinic. The intervention consisted of educational posters in the family medicine clinic and follow-up letters after non-emergent ED visits. We measured ED use for the two clinics before, during, and after the intervention. We found a statistically significant decrease in ED utilization for the patients of the intervention clinic. For the intervention office wit...

MedWorm Sponsor Message: Find the best January Sales in the UK.

We examined the effectiveness of this program in improving management in primary care practices in Germany, with a focus on the domain of quality and safety. METHODS:In a before-after study, 102 primary care practices completed a practice assessment using the European Practice Assessment instrument at baseline and three years later (intervention group). A comparative group of 102 practices was included that completed their first assessment using this instrument at the time of the intervention group's second assessment. Mean scores were based on the proportion of indicators for which a positive response was achieved by all of the practices,on a scale of 0 to 100. RESULTS:We found significant improvements in all domains between the first and second assessments in the intervention group. In t...

Over the last 2 decades, a number of attempts have been made to slow the rise in healthcare costs. Among them were Health Maintenance Organizations, which lowered costs through capitation. In most cases, this resulted in quality being sacrificed for cost and ultimately, many patients rejected the Health Maintenance Organization concept due to the lack of choice and access. The economic downturn and the passage of the Affordable Care Act have again focused scrutiny on rising healthcare costs. In the next several years, the healthcare industry will be challenged to provide more care to more people with fewer resources. (Source: Clinical Gastroenterology and Hepatology)

Much of the focus on electronic health record adoption has been on providers and the challenges they
have faced in achieving meaningful use. Now, the government?s attention is turning to patients, many of whom have
expressed concerns about the privacy and security of their health information. How are health information
technology officials going to determine what patients really want in terms of electronic sharing of

There may be a good reason why primary care physicians feel as if they often provide more care than is
really needed. They actually do, according to a research letter published recently in Archives of Internal
Medicine. That report said excessive treatment cost $6.7 billion in 2009. Some of the items deemed unnecessary
included blood work and scans. You may be surprised, however, to find out what the largest item was, representing
  86% of the expense.
More than 30 states are seeking to cut Medicaid payments to physicians, and a California case before
the U.S. Supreme Court will determine whether providers can use the courts to block those actions. Medical groups,
including the American Academy of Family Physicians, have filed briefs in support of providers, warning that
payment cuts could have dire consequences. What is likely to happen if the court prevents provider lawsuits and
allows California to go forward with its plans to cut Medi-Cal payments by 10%? (Source: Medical Economics - Practice Management)

As a participant in the American Gastroenterological Association’s Clinical Congress about a year ago, I had the opportunity to carefully review the program and attend many of the presentations. It was overall a splendid educational event aimed at practicing gastroenterologists, but what struck me as the most unique aspects of the Congress were the Practice Management Course and the Practice Skills Workshop. Here were assembled many of the senior thought leaders in gastroenterology providing practical advice to clinicians faced with bewildering choices on how to manage in changing times. I thought this might be the basis for a Gastrointestinal Endoscopy Clinics of North America issue, and I contacted the Course and Workshop Directors, Dr John Allen and Dr Mark DeLegge, to see if they mig...

The practice of Gastroenterology and the practice of United States medicine both are undergoing tectonic changes as a result of increasing economic constraints, patient demographics, the aging of practitioners, and, of course, the Patient Protection and Accountable Care Act (PPACA). Last January, at the American Gastroenterological Association Clinical Congress, Dr Charles Lightdale asked us to publish content from the Practice Management and Practice Skills Workshops. The Practice Management portion of the Congress is a 2-day course designed to update physicians and administrators on current changes in the practice of gastroenterology. The Practice Skills workshop is a course focused on GI Fellows transitioning into practice. (Source: Gastrointestinal Endoscopy Clinics of North America)

A congressional subcommittee held a hearing recently on whether consolidation has the potential to
make healthcare delivery in the United States more or less expensive. Although such consolidation may facilitate
greater efficiencies and deliver higher-quality services by eliminating duplication and excess capacity, many
experts are concerned that some consolidations are being driven primarily by a desire to increase reimbursements.
Find out how some of the issues raised could directly affect the practice of primary care.
California courts have ruled favorably for physicians and their interests in two recent cases that
dealt with medical malpractice issues of national interest. One case dealt with whether plaintiffs can recover more
than damages actually paid or incurred in an injury, and the other upheld the state’s cap on
noneconomic damages. Some of the issues raised by the rulings are likely to be revisited by other state courts
soon. (Source: Medical Economics - Practice Management)

When it comes to medical care, patients aren’t getting too little, too late, but too
much, too soon, according to many primary care physicians (PCPs). A recent survey of PCPs found that, often, many
believe they provide more care than is really needed, and that malpractice reform, realignment of financial

Information technology holds promise for improving communication, increasing quality, and reducing costs for our health care system. In the ambulatory endoscopy center (AEC), procedure report generating software has evolved over nearly 30 years to fit more closely within this paradigm. Many software systems now allow communication with electronic health record and practice management systems, ancillary service electronic solutions, and regional health information exchanges. The opportunity to use such systems to participate effortlessly with quality reporting programs now exists as well. Automation of procedure-related information including the procedure report, nursing and anesthesia documentation, and endoscopy inventory data has been shown to reduce AEC expense and increase efficiency. ...

This was a large and well-conducted trial that has demonstrated a small effectiveness benefit and a small cost saving when people with lower back pain were stratified using a screening tool. Under the workings of this tool those with the highest levels of distress and problems associated with their pain would be placed in the high-risk group and so referred for physiotherapy with a psychological component, those with medium risk features would have a greater number of physiotherapy sessions and those with the lowest risk would have initial physiotherapy with advice on self-management.
It is important to note that this practice does not differ dramatically from standard general practice care of lower back pain, rather that it applies the use of a simple tool (rather than clinical...

It is well documented that African Americans suffer disproportionately higher incidence (68 vs 55 [men], 53 vs 41 [women] per 100,000) and mortality (31 vs 21 [men], 21 vs 14 [women] per 100,000) rates for colorectal cancer (CRC) in comparison with non-Hispanic whites. Younger African Americans (aged 40–49) are more likely than those aged 50–59 to develop high-grade dysplasia, have 10.7 more cases of CRC per 100,000 people than Caucasians, have the youngest age of onset, and the most advanced stage of presentation. African Americans older than age 75 are at increased risk of adenomas, and the detection rate is even higher among those with a history of advanced adenomas. In addition, African Americans have an increased incidence of proximal colon lesions.
Physicians didn’t have the right of facing their accusers until recent changes were made
to Texas Medical Board policies. A new law going into effect this fall limits that board’s powers and
offers additional protections to physicians in Texas, including prohibiting anonymous complaints. The new law could
be a model for other states where physicians have maintained that medical boards wield too much

Primary care physicians in the United States are paid similarly to counterparts in other countries.
The large chasm between their pay and what specialists earn, however, goes a long way toward explaining why overall
spending on physician services is so much higher in the United States. The size of that gap might surprise
