The services provided by NHS primary care trusts have reduced significantly
over the last year, and fees for the other three organisations
were disproportionately affected by changes CQC made to our last fees
scheme.
Our aim in consulting is to ensure that the fees these providers pay are
proportionate to the services they deliver and the costs of their regulation.
Members of the public, organisations
affected and other stakeholders are invited to take
part in the consultation online.

The consultation
will run from 23 January to 20 February 2012. (Source: NHS Networks)

Source: Department of Health (DH)
Area: News
The Department of Health has announced the availability of an additional £150m fund to be allocated to Primary Care Trusts in England, for immediate transfer to local authorities for investment in social care services which also benefit the health system.

The one year pilot will begin in April 2012 and will be subject to an independent evaluation. It allows patients for the first time to choose whether to register with a practice close to their workplace or home, without worrying about practice boundaries.
The primary care trusts (PCTs) involved in the scheme pilot are Westminster, City and Hackney, Tower Hamlets, Manchester, Salford and Nottingham City. GP practices in these PCT areas will join the pilot on a voluntary basis. The areas selected have high numbers of commuters travelling into the area each day, and one of the sites, Nottingham, is an existing 111 pilot site.

In the UK, NICE recommends the drugs that are cost effective for the NHS through evidence-based technology appraisals. However, a report by GP magazine suggested that 33 of 71 primary care trusts (PCTs) have separate lists of drugs that they will not fund for use in primary or secondary care (termed blacklisting), 19 of which are recommended by NICE. That suggestion seems misplaced, at least for cancer drugs. Cambridgeshire PCT was singled out in the report for blacklisting erlotinib, trabectedin, and mifamurtide. According to a Pulse analysis of financial data reported by CCGs across 55 primary care trusts, two-thirds of shadow clinical commissioning groups (CCGs) reporting figures are currently missing their financial targets.
The announcement comes as latest research highlights the value of public health action and shows that the majority of public health interventions recommended by NICE are highly cost effective.
The findings are of particular importance for local authorities as they prepare to take over the role of commissioning public health services from primary care trusts from April 2013.
The briefings will raise awareness and provide information about the existing evidence-based recommendations and evidence reviews from NICE that local government can call upon when commissioning public health services. (Source: NHS Networks)

Clinical commissioning - the ethical implications

The Health and Social Care Bill proposes clinically led commissioning. The new Clinical Commissioning Groups will become statutory bodies and replace the old primary care trusts. This primary legislation will transfer accountability for health care spending to clinicians who will have a duty to meet the health needs of a population as well as balance a budget. This change gives rise to a number of ethical issues which are explored in this article. (Source: Clinical Risk)

The Daily Telegraph has today revealed the findings of the latest Dr Foster Hospital Guide. The guide, published annually, closely scrutinises a range of healthcare data to measure hospital performance and detect trends that could save lives.
As well as listing the hospital trusts in England that score above and below average on a range of different mortality measures, this year’s guide also found that:

The rate of patient deaths in England is 20% lower than it was 10 years ago, in part because of improved hospital care.
For certain conditions, patients admitted to hospital at the weekend are less likely to get treated quickly and have a higher chance of dying.
Hospitals that perform certain operations infrequently pose a significantly greater risk to patients than those which c...

There are an estimated 22,000 people living in the UK with HIV who are unaware of their diagnosis. Late diagnosis in Newcastle remains a problem and in 2008, 60% of new diagnoses had a CD4 count CD4 2/1000. Newcastle Primary Care Trust has an HIV prevalence of 3-4/1000. We undertook a prospective audit offering HIV testing to all general medical admissions attending the Emergency Assessment Unit (EAU) to assess feasibility, acceptability and point prevalence.
SummaryBackgroundMost clinicians have no training about domestic violence, fail to identify patients experiencing abuse, and are uncertain about management after disclosure. We tested the effectiveness of a programme of training and support in primary health-care practices to increase identification of women experiencing domestic violence and their referral to specialist advocacy services.MethodsIn this cluster randomised controlled trial, we selected general practices in two urban primary care trusts, Hackney (London) and Bristol, UK.
Cancer survival rates in the NHS are excellent. But without costly meddling they could be even betterA new piece of research linking survival rates from cancer with investment levels in treatment has shown that the NHS provides excellent value for money. Indeed, between 1979 and 2006, of the 10 countries assessed (which also included Germany, the US, France and Japan), survival rates in England and Wales were found to have improved the most, making this the most cost-efficient health service in reducing cancer mortality. So how come the health service has done so well, when all we seem to hear is bad news about cancer treatment? Why, when our politicians proclaim that the NHS is performing poorly, does the objective evidence suggest the opposite? In my 35 years' working as a clinician I have...

There could be nearly two hundred unregistered or fraudulent nurses working in GP (general practitioner) practices throughout the United Kingdom, according to a new report issued by the NMC (Nursing and Midwifery Council). The Council is preparing a letter to be sent to primary care trusts urging practices to properly check nurses' registrations. Nurses should be registered with the profession's regulator. Recently, authorities arrested a woman in Kent for allegedly pretending to be a registered, qualified nurse... (Source: Health News from Medical News Today)

This article explores the decision in the case of W v M, S and an NHS Primary Care Trust [2011] EWHC 2443 (Fam) in which an application to withdraw feeding from a woman in a minimally conscious state was rejected by Baker J in the Court of Protection. The article places the case in the context of the development of case law concerning the withdrawal of treatment from patients lacking decision-making capacity, where death will be the inevitable consequence of the withdrawal of treatment after the decision in Airedale NHS Trust v Bland. It questions whether nearly two decades after the Bland decision there is scope for the boundaries of the decision to withdraw treatment from patients in 'futile' conditions to be reconsidered by the Supreme Court.

Personal health budgets are being piloted in English primary care trusts (PCTs) between 2009 and 2012. This evaluation report looks at the early experiences of a small sub-sample of budget holders and their representatives. It reports in-depth interviews with 58 people from 17 PCTs, around three months after the offer of a personal health budget, about their experiences of personal health budgets so far.

The fourth interim independent evaluation report on the programme has been published by the personal health budgets evaluation (PHBE) team. (Source: NeLM - News)

In 2011, this approach was subject to judicial review on behalf of Mr Condliff, a morbidly obese former policeman. Mr Condliff had a body mass index of 40 and, due to underlying medical conditions, was deemed ineligible for open bariatric surgery. The surgery can be undertaken laparoscopically, but North Staffordshire Primary Care Trust (PCT), in whose catchment area Mr Condliff lives, does not ordinarily provide the surgery to patients with a body mass index of below 50. The NHS Act 2006 allows Primary Care Trusts to charge for NHS pharmaceutical applications under regulations that came into force in April 2008. These specified the type of applications and the level of fees that would be payable. During the passage of the Health Bill 2006 the Government agreed that review of this agreement would be carried out within eighteen months following introduction. After consultation this document provides an overview of the responses that have been received and outlines actions the Government is going to take.
Source: The NHS Information Centre
Area: News
The NHS Information Centre has published Key Performance Indicators for the "Improving Access to Psychological Therapies (IAPT)" programme for the period April to June 2011.

The report showed that (taken directly from source):
. 42% of those who completed treatment under the programme during the period were no longer 'at caseness' - i.e. no longer showing measurable symptoms of depression or anxiety disorder at their last therapy session.
. Recovery rates for those who completed treatment varied between Primary Care Trusts (PCTs); ranging from seven per cent in one PCT to 63% in another. This may be associated with the roll out of the programme, as some sites started delivering services more recently than others.

Abstract: Men, in general, remain less likely than women to seek medical care, and are only half as likely as women to undertake preventive health visits and/or screening tests. There is a great need to increase men's health awareness and reduce this significant gender disparity. Furthermore, marginalised and socially excluded men rarely access health services, even though the reasons for their social exclusion, particularly drug and alcohol dependency, invariably mean that their need for health interventions is greater than in the normal population. The Open Doors Sexual Health Service has been working with female street sex workers (SSWs) in the London Borough of Hackney since 2006, in order to help them address their physical, psychological and social needs.
A new skin cancer drug has been approved for use in Europe, several newspapers have reported today. The drug, called ipilimumab (brand name Yervoy), has been dubbed a “wonder drug for skin cancer” on the front page of the Daily Express. The Daily Mail calls it the “first real advance” in skin cancer treatment since the 1970s.
Newspapers have given varying estimates of how much the drug will cost. The Daily Mail says it will cost £18,000 per course, while The Daily Telegraph puts the total cost at around £75,000 per patient.

Why is the drug in the news now?
The drug ipilimumab has just been granted ‘marketing authorisation’ by the European Commission, meaning that the drug can be sold throughout the European Union. The European Commission has made this decision based on the...

    Abstract
United Kingdom primary care trusts resembled the primary health care organisations (PHCOs) that have been proposed for Australia - for example, Medicare Locals. They resulted in a loss of innovation, creativity, motivation and morale among general practitioners and other front-line staff. English primary care trusts are being abolished and £80 billion will be handed over to GP commissioners. Management theory and practical experience shows repeatedly the dangers of reorganising into larger units. Lessons for Australia are to defer deciding on the size of PHCOs until their purposes are clear, to enshrine the principle of subsidiarity, and to opt for networking of the current Divisions of General Practice over mergers. So far, debate on the functions and...

The August edition of 'Eyes on Evidence', the free monthly e-bulletin from NHS Evidence covering major new evidence as it emerges with an explanation about what it means for current practice, includes an expert commentary on the results of a cross-sectional study that sought to assess the odds of prescribing recommended coronary heart disease (CHD) drugs by age, sex, ethnicity, social deprivation, and co-morbidity.

The study, published in the Journal Ethnicity & Health in April 2011, involved a total of 10,993 patients aged ?35 years with CHD, from two East London Primary Care Trusts. The authors report high levels of prescribing for people with CHD overall, however lower rates were seen in women, Black African/Caribbean patients ...

It was clear as soon as it was announced that UK Health Secretary Andrew Lansley's unexpected decision to subject the National Health Service (NHS) in England to revolutionary change would have profound consequences for public health. The White Paper, Healthy lives, healthy people, proposes that the public health function currently based in Primary Care Trusts be fragmented (). Some elements will move to the Department of Health, to a division called Public Health England, while others will move into local government.
The research reflects a shifting landscape for primary care trusts, in which more than a quarter (28 per cent) of those questioned said that their commissioning functions had merged with those of other PCTs. Although a very small proportion of respondents (eight per cent) of those questioned reported that clustering had had a negative impact on their ability to respond to local needs, most did not, reporting no change or a positive impact or saying that they did not know what the effect of clustering had been. According to a report by the NHS Confederation, a major survey of NHS leaders by the Picker Institute has concluded that leaders are fighting to improve services in the face of unprecedented financial pressures. But most believe that access to care will decline and that cuts in local authority budgets will lead to more people needing healthcare services.

The survey involved 287 chairs and chief executives from 243 different organisations, including acute trusts, primary care trusts, ambulance trusts, mental health trusts and independent sector NHS providers.

    Abstract
The article explores the implications of using human rights principles as a basis to challenging a resource allocation decision by a primary care trust in the courts. It explores the implications of the recent decision in R (Condliff) v North Staffordshire NHS Trust [2011] and the prospects for development of future litigation.

This article reports on a student nurse project carried out on a community placement in respect to patients dependent on medical devices in the event of power cuts. The project was designed to identify relevant literature to establish what emergency and preparatory procedures and protocols are in place in a primary care trust for this type of emergency and to identify what action a nurse in the community should take to ensure the safety and welfare of their patients if a there was a prolonged power failure. The literature and knowledge of community staff for the correct procedures to follow in the event of a power cut in the community established that there was some cause for concern, and identified that a loss of power to a patient with a medical device poses a serious risk. Given the wide and persistent variation in spending on cancer services, this project led by the King's Fund aimed to identify and quantify sources of variation in primary care trust (PCT) cancer spending and provide PCTs and cancer networks with a systematic way of framing decisions about the appropriate share of their total budgets to spend on cancer services.
Undiagnosed chronic obstructive pulmonary disease (COPD) is a major public health issue, as it leads to patients missing out on appropriate preventive and therapeutic interventions.1–3 The ratio of diagnosed/predicted COPD prevalence differs widely between Primary Care Trusts (PCTs), suggesting that there are unacceptable variations in care.4 A National Clinical Strategy for COPD is to be launched in the UK in 2010 and there is an urgent need for evidence to support strategies to increase the identification of patients, particularly those with early disease. In 2008 a locally enhanced service (LES) for COPD was introduced by NHS Kensington and Chelsea (K&C), giving general practitioners a small financial incentive for each individual screened and a larger payment for each patient...

According to a report by the Daily Mail, Health Secretary Andrew Lansley is suing Servier for £220 million, on the basis that the company has overcharged the NHS for blood pressure medicines.

The report notes that Servier's patent on perindopril ran out in 2001, but that Servier paid rivals millions not to make generic versions. The Health Secretary is now 'spearheading' legal action at the High Court with ten strategic health authorities and 147 primary care trusts to reclaim the £220 million that the NHS is believed to have been overcharged between 2001 and 2007. (Source: NeLM - News)

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Private prescriptions Dentists must use the private prescription form FP10PCD* when prescribing Schedule 2 and 3 controlled drugs. A dentist can prescribe privately any Schedule 2 or 3 controlled drug on an FP10PCD form, as long as it meets the dental needs of the patient. Pharmacists may challenge private controlled drug prescriptions if they feel that the controlled drug has no recognised dental use. FP10PCD forms should be obtained from Primary Care Trusts (PCTs), even if the dentist has no NHS contract with the PCT.
NHS prescriptions The standard FP10D** prescription form should be used to prescribe controlled drugs. Dentists can only prescribe controlled drugs included in the DPF list (e.g. temazepam...

DiscussionThe recent announcement by government regarding the proposed abolition of Primary Care Trusts may pose challenges for the research team regarding their engagement with the research study. However, whichever commissioning organisations are responsible for resource allocation for dental services in the future; resource scarcity is highly likely to remain an issue. Wider understanding of the complexities of priority setting and resource allocation at local levels are important considerations in the development of dental commissioning processes, national oral health policy and the future new dental contract which is expected to be implemented in April 2014.
AbstractCurrent health policy in England regarding the management of childhood illness advocates for care to be delivered as close to home as possible. The aim of this article is to report findings from a qualitative component of a larger study evaluating models of care closer to home (CCTH) for children and young people who are ill. The focus is on parents’ psychosocial experience of receiving CCTH for their ill child. A qualitative case study design was used, with four Primary Care Trusts in England as the case study sites. In‐depth, semi‐structured interviews were conducted with 27 parents and one extended family caregiver of children using CCTH services within the case study sites. Interviews were conducted face‐to‐face and audio‐recorded with permission. Data were collecte...

More than two-thirds of general practitioners (GPs) believe that they will require specialist advice if they are to effectively commission cancer services, according to a new report by the Cancer Campaigning Group (CCG), a coalition of UK cancer charities. NHS Cancer Networks—of which England has 28—currently offer such advice. They work across Primary Care Trusts, bringing together experts to make decisions about developing treatment within a certain area. But the Department of Health has not guaranteed funding beyond April, 2012. (Source: The Lancet Oncology)

Perhaps as a result of the furore, in some areas less than a third of children are vaccinated by the time of their fifth birthday.Data from the Health Protection Agency (HPA) reveals 334 confirmed cases of measles to the end of April in England and Wales, compared with 33 cases for the same period last year, and 374 in all of 2010.The numbers are still q...

Increasing HIV patient numbers, emerging comorbidities and an ageing patient population support the need for HIV shared care service delivery models in the UK.1–3 The British HIV Association recommends that HIV patients register with a general practitioner (GP) and disclose their status, but the limited data available suggest disclosure is variable.4 5 Successful shared care planning depends on understanding the local HIV-infected population and the proportion of HIV infections disclosed to GPs. To support emerging GP consortia, a method similar to the data extraction for Quality and Outcomes Framework6 was used to estimate the proportion of HIV infections disclosed to primary care and identify the size of individual-practice HIV-infected populations in Lambeth Primary Care Trust (PC...
