The combined R&amp;D investments of 10 Asian nations, including China, India, and Japan, now match U.S. expenditures on science at about $400 billion a year. Most of the growth in Asia was driven by China, where R&amp;D spending grew by 28 percent between 2008 and 2009. In the U.S., science expenditures declined by 0.3 percent over the same period. However, the technique at our center differs from that of Fu and associates in the following respects. I am pleased to learn that they also found our surgical technique useful in correcting lid margin deformity, lid retraction, and lagophthalmos. Although the proposed surgery starts from lamellar division of the involved eyelid at the grey line and ends with posterior repositioning of the anterior lamella, our splitting of anterior and posterior lamella does not reach the level that they stated, that is, just short of superior fornix. As a matter of fact, ours only reaches the level that is sufficient to allow the anterior lamella to be recessed from the tarsal margin, that is, approximately 3 to 5 mm.     Abstract
    We studied the prevalence of severe crouch gait over a 15-year period in a defined population of children with spastic diplegia and Gross Motor Function Classification System levels II and III, to determine if there had been a decrease following changes to the management of equinus gait. These changes were replacing observational with three-dimensional gait analysis, replacing single level with multilevel surgery, and replacing gastrocsoleus lengthening with gastrocnemius recession. Of 464 children and adolescents with spastic diplegia who underwent three-dimensional gait analysis, 27 had severe crouch gait. These factors tend to decline with economic recessions. Historically, the demand for ophthalmologists has declined 2 to 3 years following a recession, which may mean lower demand in the near future, given the recent recession.

An 8-month-old boy with Gorlin syndrome presented with a large right-face turn and constant exotropia of the left eye. Eight-millimeter recession of the left lateral rectus muscle was performed at 23 months of age without complete postoperative improvement. Orbital imaging revealed bilateral anomalous extraocular muscles inferolateral to the optic nerves. Surgical resection of the tissue confirmed the accessory musculature with postoperative correction of the strabismus. To our knowledge, this appears to be the first reported case in the radiologic literature.
