States have considerable flexibility in how they design and implement these exchanges.             <description>This article describes why the Affordable Care Act could lead to favorable or adverse risk selection across plans. It reviews provisions in the act and recent proposed regulations intended to mitigate the problem of risk selection. We performed a simulation that showed that under the premium rating restrictions in the law, large incentives for insurers to attract healthier enrollees will be likely to persist&amp;mdash;resulting in substantial overpayment to plans with very healthy enrollees and underpayment to plans with very sick members. The Small Business Innovation Research (SBIR) program and Small Business Technology Transfer (STTR) programs would receive a larger share of federal research dollars under the bicameral compromise.

Currently, the programs are funded by the 11 federal agencies that spend more than $100 million a year on research. The SBIR receives 2.5 percent of each agency&amp;#8217;s research budget; STTR receives 0.3 percent. On 23 June, the House of Representatives passed a bill that would change the ability of an inventor to patent a discovery. HR 1249 would give priority to the first inventor to file for a patent. Currently, patent law favors the first person who invents or discovers a new technology, device, process, or material. Some worry that this change would favor large corporations that have the resources to undergo the lengthy and expensive process of filing for a patent, and that individuals or small businesses may be at a disadvantage. Supporters of reform claim that this is an important step towards harmonizing the U.S. patent system with the process used by the rest of the world.

We need to be cognizant of our expenses and the payment for our services. Evaluating the explanation of benefits (EOB) is part of that responsibility for any medical practice. In this article, we will describe the importance of the EOB and how to monitor EOBs on a regular basis.

By adopting inexpensive prevention efforts, such as promoting the state-sponsored tobacco cessation quitline, employers can help employees quit smoking and, thereby, assist in improving employee health and lower medical costs.
