Authors: Petersen PE, Baez RJ, Lennon MA
    Abstract
Dental caries is the most prevalent chronic disease affecting human populations around the world. It is recognized that fluoride plays a significant role in dental caries reduction. Meanwhile, several low- and middle-income countries of Asia have not yet implemented systematic fluoride programs; contributing factors relate to misconceptions about the mechanisms of fluoride, low priority given to oral health in national health policy and strategic plans, and lack of interest among public health administrators. A workshop on the effective use of fluoride in Asia took place in Phang-Nga, Thailand, in 2011. A series of country presentations addressed some of the topics mentioned above; in addition, speakers from countries of the region ...

MedWorm Sponsor Message: Find the best January Sales in the UK.

In 1998, when one of us (BKR) presented the concept for Transdisciplinary Tobacco Use Research Centers (TTURCs) to the National Cancer Institute's (NCI's) Board of Scientific Advisors (BSA) for approval, they questioned whether transdisciplinary was a word.
Background:
Effective malaria case-management based on artemisinin-based combination therapy (ACT) and parasitological diagnosis is a major pillar within the 2007-2012 National Malaria Strategic Plan in the Sudan. Three years after the launch of the strategy a health facility survey was undertaken to evaluate case-management practices and readiness of the health facilities and health workers to implement a new malaria case-management strategy.
Methods:
A cross-sectional, cluster sample survey was undertaken at public health facilities in 15 states of Sudan. Data were collected using quality-of-care assessment methods. The main outcomes were the proportions of facilities with ACTs and malaria diagnostics; proportions of health workers exposed to malaria related health systems support activi...

This Perspectives article offers a series of recommendations to the implementers of the National Alzheimer’s Project Act (NAPA) under the premise that better treatment and preventive strategies targeting neuropsychiatric syndromes (NPS) will improve patient outcomes, lessen caregiver burden, and potentially lead to prevention of Alzheimer’s disease (AD) and related primary dementias. These recommendations arise from the activities of the Neuropsychiatric Syndromes Professional Interest Area (NPS-PIA) of the Alzheimer’s Association International Society to Advance Alzheimer’s Research and Treatment. The NPS-PIA was constituted after an Alzheimer’s Association Research Roundtable meeting devoted to this topic in April 2010. The Roundtable concluded that, despite several decades of ...

Elizabeth Galik It is an exciting time to be involved in the care of older adults. The burgeoning aging population with declining numbers of clinicians with geriatric expertise, health care reform, and the advancement of new models of care, such as medical homes, will require advanced practice nurses who care for older adults to serve as clinical experts, work collaboratively with members of the interprofessional team, and inspire and educate the next generation of clinicians. During the past year, the Gerontological Advanced Practice Nurses Association (GAPNA) completed a strategic planning process to set goals and priorities so that we may continue to grow and be successful in an ever-changing health care environment.
The radiotherapy delivery service in England is emerging from a 25 year blight on strategic planning and forecasting of radiotherapy demand. Two of the most influential documents used as evidence for renovation and expansion in radiotherapy services are the Royal College of Radiologists (RCR) equipment, workload and staffing survey and the National Radiotherapy Advisory Group (NRAG) report . Where the RCR survey quantified significant variation in allocation and consumption of radiotherapy resources across England and Wales, the NRAG 2007 report generated national targets for radiotherapy service provision in terms of fraction burden (40,000 fractions per million of population by 2010 and up to 54,000 fractions per million by 2016) and the proportion of cancer patients receiving radiothera...

Authors: Tuech JJ, Bridoux V, Schwarz L, Oden-Gangloff A, Michel P, Michot F
    Abstract
One of the objectives of the French strategic plan for cancer 2009-2013 is to structure the need for referral surgery, particularly for low rectal carcinoma. However, low rectal cancer is not the only situation in the field of rectal surgery where expert unit are needed for the referral of appropriate patients. We developed the multidisciplinary strategies for low rectal cancer, advanced rectal cancer, recurrent rectal cancer and peritoneal carcinomatosis. Optimal management of these difficult situations can give a chance of long term survival while a non-optimal management could jeopardise the future of patients by changing a potentially curable disease into an incurable one.

As my year of service as president of APIC comes to a close, I reflect on the tremendous accomplishments of our organization and the terrific infection prevention work taking place each and every day in care settings around the world. This work is reflected in the critical work of infection preventionists, health care epidemiologists, and other scientists who improve our understanding through the pages of AJIC. While there is a long list of accomplishments this year, I’d like to call out three in particular. First, we’ve hired a CEO for the Association, Ms Katrina Crist, and are benefiting significantly from her leadership of the superb team at APIC Headquarters. Second, the APIC Board and a broad array of other key stakeholders convened a Strategic Planning Summit earlier this fall ai...

The problem remains vast. The authors say that "With 9.4 million new cases of tuberculosis (TB) and 1... (Source: Health News from Medical News Today)

CHICAGO—There are pros and cons to beginning a CT emergency department (ED) program for the evaluation of patients with acute chest pain; however, the undertaking requires asking some strategic questions to establish a clear-cut protocol, according to a Nov. 28 presentation by Harold I.
San Pablo's General Plan 2030, adopted in April, contains a 36-page chapter on health. An underlying principle of the health element is that urban planning and design in past eras included a focus on public health, and that it should do so in the modern era as well, by shaping the urban environment in ways that promote public health. Those means include creating opportunities for healthy activities such as walking, jogging, bicycling and other outdoor and sports activities; preventing crime and blight; and promoting access to healthy food and other healthy goods and services. The council meeting included a study session on developing a strategic plan to address a growing childhood obesity epidemic and expand healthy eating and active living policies in the city.
South Africa has the highest number of HIV infected individuals worldwide. Local Voluntary Counselling and Testing (VCT) is one component of the national strategic plan to tackle this. The Mbashe district lies in the heart of the Eastern Cape, a low resource rural community with poor infrastructure and access to health care facilities. In addition to these physical obstacles, HIV stigma is a significant barrier to attendance at clinic-based VCT. Outreach is therefore a vital component of VCT and HIV education programmes. Abstract
Public health arguments for collecting hemoglobin A1c (HbA1c) data, particularly in clinical settings, should be reframed to place more emphasis on nonmedical determinants of population health. We compare individual-with population-level interpretations of HbA1c titers. This comparison reveals that public health researchers need to pay close attention to diagnostic tests and their uses, including rhetorical uses. We also synthesize historical and current evidence to map out 2 possible scenarios for the future. In the first scenario, prevention efforts emphasize primary care and focus almost entirely downstream. The second scenario anticipates downstream interventions but also upstream interventions targeting environments. Our analysis adapts act...

    Abstract
A decade after the sequencing of the human genome, the National Human Genome Research Institute announced a strategic plan for genomic medicine. It calls for evaluating the structure and biology of genomes, understanding the biology of disease, advancing the science of medicine, and improving the effectiveness of health care. Fulfilling the promise of genomics urgently requires a population perspective to complement the bench-to-bedside model of translation. A population approach should assess the contribution of genomics to health in the context of social and environmental determinants of disease; evaluate genomic applications that may improve health care; design strategies for integrating genomics into practice; address et...

MedWorm Sponsor Message: Find the best January Sales in the UK.

    Abstract
At present, traditional fossil fuels are used predominantly in China, presenting the country with challenges that include sustainable energy supply, energy efficiency improvement, and reduction of greenhouse gas emissions. In 2007, China issued The Strategic Plan of the Mid-and-Long Term Development of Renewable Energy, which aims to increase the share of clean energy in the country's energy consumption to 15% by 2020 from only 7.5% in 2005. Biodiesel, an important renewable fuel with significant advantages over fossil diesel, has attracted great attention in the USA and European countries. However, biodiesel is still in its infancy in China, although its future is promising. This chapter reviews biodiesel production from conventional feedstocks in th...

    Abstract
A 1-day training event for pediatric residents with interdisciplinary staff was held, which was modeled after the Initiative for Pediatric Palliative Care (IPPC). Training included relational communication, cultural humility, pain-symptom management, family-centered care, team problem solving, and strategic planning using didactic, small group, and plenary platforms. Two bereaved parents were co-learners and trainers. Twenty-six interdisciplinary staff participated. A positive impact was measured in new knowledge gained, value in collaborative learning with health care professionals and families, and ability to work with professionals outside participants' own unit. Confidence to advocate for improved pediatric palliative care was also noted. The...

It began with a desire to bring a group of people from multiple EMS locations throughout the world together with a common goal: to create an international, strategic plan for promoting the EMS profession. The result was the birth of a concept, International Paramedic (iParamedic), an open-source, worldwide forum for discussion and recommendations about critical issues affecting EMS practitioners.
Abstract: As the world’s population ages, countries must prepare for the significant impact Alzheimer’s disease will have on their health systems, their economies, and their citizens. In anticipation of major global demographic changes, many countries in the G-20 since 2000 have begun to develop and enact plans to address Alzheimer’s disease as a national priority. However, even with nearly half of these participating countries having plans in place, there has been little research done to quantify the value of enacting a countrywide plan on this disease. In this review, we summarize recent national plans (from the year 2000 and beyond) and any results stemming from their respective recommendations and activities.
Abstract: The range of medicines and technologies that are essential for sexual and reproductive health care is well established, but access to them is far from universally assured, particularly in less developed countries. This paper shows how the pharmaceutical industry plays a major role in the lack of access to essential medicines for sexual and reproductive health care, by a) investing in products for profit-making reasons despite their negative health impact (e.g. hormone replacement therapy), b) marketing new essential medicines at prices beyond the reach of countries that most need them (e.g. HPV vaccines), and c) failing to invest in the development of new products (e.g. microbicides and medical abortion pills). Small companies, some of them non-profit-making, struggle to fill som...

    Abstract
In medical systems, economic issues and means of action are in the course of dwindling human (physicians and nurses) and financial resources are more important. For this reason, physicians must understand basic economic principles. Only in this way, there may be medical autonomy from social systems and hospital administrators. The current work is an approach to present a model for strategic planning of an anesthesia department. For this, a "strengths", "weaknesses", "opportunities", and "threats" (SWOT) analysis is used. This display is an example of an exemplary anaesthetic department.

This article does not address this broad topic but provides a perspective of how a medical school can evaluate and perhaps intervene to mentor medical students more effectively about the satisfaction derived from a career in surgery. As part of this project, we have evaluated each step of the path through medical school and a surgical residency by comparing data for our students, residency programs, and faculty with national data.

This report aims to present a rare intraoperative finding of an anatomic malformation of the right lung in a 19-year old male patient with recurrent pneumothorax who underwent a surgical repair. An accessory fissure which was separating the superior segment of the lower lobe from the basal segments gave to the whole lung the unique image of a four-lobed one. A profound knowledge of the accessory fissures, even if they are incidentally discovered, is of pivotal importance for the thoracic surgeon and leads to optimal operative assessment and strategic planning.
After much partisan bickering, Congress finally enacted a spending plan that keeps the federal government open through mid-November. The Continuing Resolution reduces spending for the next several weeks by 1.5 percent relative to last year. The temporary reduction ensures that discretionary spending does not exceed the cap of $1.043 trillion set in the debt limit law.

Public comments are now being accepted on the National Global Change Research Plan, a 10-year strategic plan for the federal program that coordinates U.S. climate change research. Four goals frame the plan: advance science, inform decisions on adaptation and mitigation, build sustained assessment capacity, and advance communications and public education. Comments are being accepted through 29 November 2011.
AbstractGiven the growing field of Pediatric Hospital Medicine (PHM) and the need to define strategic direction, the Society of Hospital Medicine, the American Academy of Pediatrics, and the Academic Pediatric Association sponsored a roundtable to discuss the future of the field. Twenty‐one leaders were invited plus a facilitator utilizing established health care strategic planning methods. A “vision statement” was developed. Specific initiatives in 4 domains (clinical practice, quality of care, research, and workforce) were identified that would advance PHM with a plan to complete each initiative. Review of the current issues demonstrated gaps between the current state of affairs and the full vision of the potential impact of PHM. Clinical initiatives were to develop an educational ...

Blacks have persistently higher rates of high blood pressure, or hypertension, compared to whites, resulting in higher health costs and mortality rates. Recent research has shown that social and environmental factors—such as high levels of stress and exposure to lead—may explain racial disparities in hypertension. Based on these findings, we recommend a fundamental shift in approaches to health disparities to focus on these sorts of cumulative risks and health effects. Federal and state agencies and research institutions should develop strategic plans to learn more about these connections and apply the broader findings to policies to reduce health disparities.
Strategic planning is most often associated with organizations with multiple priorities, numerous activities, and many stakeholders such as employees or—in the American Dietetic Association's case— members. But strategic thinking and planning for the future is vital for individuals as well. And many of the same strategic planning techniques that are used by organizations like ADA can help each of us increase our own personal and professional potential. Responses to slow institutionalization of inter‐professional healthcare practice have failed to describe change variables and to identify change agents relevant to inter‐professional healthcare practice.
In 2010, WHO's Director General, Margaret Chan, formed an independent monitoring board to assess progress towards achievment of the milestones of the Global Polio Eradication Initiative strategic plan 2010–12. The board's report in April this year showed that the number of poliomyelitis cases has decreased from 350 000 in 1988 to 1294 in 2010. Endemic transmission of wild poliovirus types 1 and 3 continued only in Afghanistan, India, Nigeria, and Pakistan, the four countries in which poliovirus transmission has never been stopped. At that time, I solicited input from members of the Society and submitted comments on behalf of SP4M on one section of the Plan, “Goal IV: Empower Individuals with Health IT to Improve their Health and the Health Care System” – See: SPM Comments on Goal IV of Federal Health IT Strategic Plan 2011-2015 – http://bit.ly/pIgqDk

Cloud computing is a new way of delivering computing resources and services. Many managers and experts believe that it can improve health care services, benefit health care research, and change the face of health information technology. However, as with any innovation, cloud computing should be rigorously evaluated before its widespread adoption. This paper discusses the concept and its current place in health care, and uses 4 aspects (management, technology, security, and legal) to evaluate the opportunities and challenges of this computing model. Strategic planning that could be used by a health organization to determine its direction, strategy, and resource allocation when it has decided to migrate from traditional to cloud-based health services is also discussed. Abstract
This paper describes the Costa Rican health system which provides health, water and sanitation services. The health component of the system includes a public and a private sector. The public sector is dominated by the Caja Costarricense de Seguro Social (CCSS), an autonomous institution in charge of financing, purchasing and delivering most of the personal health services in Costa Rica. CCSS is financed with contributions of the affiliates, employers and the state, and manages three regimes: maternity and illness insurance, disability, old age and death insurance, and a non-contributive regime. CCSS provides services in its own facilities but also contracts with private providers. The private sector includes a broad ...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

In this paper, we provide the rationale for establishing PREVNet, a description of the work of the network, and an assessment of the success of PREVNet. PREVNet’s strategic plan focuses on enhancing the practice of those who work with children and youth through knowledge mobilization under four strategy pillars: education and training, assessment and evaluation, prevention and intervention, and policy and advocacy.
This study analyzes the process of producing a situational plan according to a benchmark from the philosophy of language and argumentation theory. The basic approach used in the analysis was developed by Carlos Matus. Specifically, the study seeks to identify the inherent argumentative structure and patterns in the situational explanation and regulatory design in a plan's operations, taking argumentative approaches from pragma-dialectics and informal logic as the analytical parameters. The explanation of a health problem is used to illustrate the study. Methodologically, the study is based on the existing literature on the subject and case analyses. The study concludes with the proposition that the use of the specific references means introducing greater rigor into both the analysis of the...

MedWorm Sponsor Message: Please support the Doctors In Chains campaign for the medics tortured and sentenced for up to 15 years in Bahrain. #FreeDoctors

Hawaii’s health-care leaders intent on crafting a 12- to 18-month strategic plan to fill the growing employment gaps in the health-care industry have taken their efforts to the Neighbor Islands.

The Hawaii Workforce Development Council recently contracted with the Hawaii/Pacific Basin Area Health Education Center to host Health-care Industry Skill Panel meetings in each county to get feedback on the strategic plan. Meetings took place earlier this month in Hilo on the Big Island and on Kauai.

Additional meetings will take place July 25 at the University of Hawaii’s John A... (Source: bizjournals.com Health Care:Physician Practices headlines)

Neuroprosthetic devices based on brain-machine interface technology hold promise for the restoration of body mobility in patients suffering from devastating motor deficits caused by brain injury, neurologic diseases and limb loss. During the last decade, considerable progress has been achieved in this multidisciplinary research, mainly in the brain-machine interface that enacts upper-limb functionality. However, a considerable number of problems need to be resolved before fully functional limb neuroprostheses can be built. To move towards developing neuroprosthetic devices for humans, brain-machine interface research has to address a number of issues related to improving the quality of neuronal recordings, achieving stable, long-term performance, and extending the brain-machine interface a...

Your Editorial (June 18, p 2055) sees a new leadership role for UNAIDS in global health, with AIDS at the leading edge of a new movement for integrating health responses to disease. This Editorial is obviously based on the self-serving press releases and reports of UNAIDS. The UNAIDS Strategic Plan on which these are based was itself prepared by consultants charged with saving UNAIDS in the light of rapidly increasing awareness of its irrelevance to global health. Only when the writing was so clearly on the wall for UNAIDS did the organisation commission this work to try to reposition UNAIDS given that international funding is shifting from HIV to health systems development.
This report updates a previous report with the latest TUTD findings. In 2010, the number of onscreen tobacco incidents in youth-rated (G, PG, or PG-13) movies continued a downward trend, decreasing 71.6% from 2,093 incidents in 2005 to 595 in 2010. Similarly, the average number of incidents per youth-rated movie decreased 66.2%, from 20.1 in 2005 to 6.8 in 2010. The degree of decline, however, varied substantially by motion picture company. The three companies with published policies designed to reduce tobacco use in their movies had an average decrease in tobacco incidents of 95.8%, compared with an average decrease of 41.7% among the three major motion picture companies and independents without policies. This finding indicates that an enforceable policy aimed at reducing tobacco use in y...

J Clin Hypertens (Greenwich). The Hypertension Initiative began in 1999 to help transition South Carolina from a leader in cardiovascular disease (CVD) to a model of heart and vascular health. Goals were to reduce heart disease and stroke by 50% by promoting healthy lifestyles and access to effective care and medications. Continuing medical education was used to train providers, encourage physicians to become American Society of Hypertension (ASH)–certified hypertension specialists and recruit practices into the community‐based practice network (CBPN). Practice data audit with provider specific feedback is a key quality improvement tool. With ASH support, the CBPN has grown to 197 practices with approximately 1.6 million patients (approximately 700,000 hypertensives). Clinical data ar...

Abstract Patients who miss clinic appointments make unscheduled visits which compromise the ability to plan for and deliver quality
care. We implemented Electronic Medical Records (EMR) and same day patient tracing to minimize missed appointments in a community-based
HIV clinic in Kampala. Missed, early, on-schedule appointments and waiting times were evaluated before (pre-EMR) and 6 months
after implementation of EMR and patient tracing (post-EMR). Reasons for missed appointments were documented pre and post-EMR.
The mean daily number of missed appointments significantly reduced from 21 pre-EMR to 8 post-EMR. The main reason for missed
appointments was forgetting (37%) but reduced significantly by 30% post-EMR. Loss to follow-up (LTFU) also significantly decreased

Summary: The recent publication of national men’s health policies in Ireland and Australia marks the first attempts by state governments anywhere in the world to target men as a specific population group for the strategic planning of health. The impetus for policy action in both countries can be traced to an increasing concern about sex differences in health status between men and women; a growing awareness of the need for a more gender-specific approach to health policy; and an expanding men’s health field at a research, advocacy and community/voluntary level. This paper will describe the background to men’s health policy development in Ireland and Australia; outline the aims, methodologies and key principles used for policy development; and highlight the principal priorities for po...

In September, 2010, the Association of Pediatric Program Directors (APPD) included in their strategic plan a commitment to improve global child health (GCH) by collaborating to advance curriculum, improve training, and advocate for better support (institutional, programmatic, financial) of GCH education, with an agreement that outcomes for best training practices be studied. Significantly, both the Federation of Pediatric Organizations and The Institute of Medicine have recently committed to improving global health care and education as well.
Researchers studied strategic planning and problem solving among 890 10-30 year-olds using a computerized test called the Tower of London. Older test takers did better on the tower test, showing a greater ability to plan ahead and solve problems. On the hardest problems, mature performance wasn't seen until at least age 22. These findings have implications for programs that target adolescents' still-emerging capacity to plan ahead, control impulses, regulate emotions, and resist peer pressure.
This paper reports the planning efforts and preliminary results of increasing fuel economy in the current fleet of medium-duty tactical truck. A strategic plan was developed through investigation of current and future technology offerings from original equipment manufacturers and after market suppliers. Research efforts consisted of an initial phase where a broad range of integration candidates were collected and a secondary phase where in-depth analysis was conducted to target those to be considered for inclusion in the strategic plan. The strategic plan lays out the integrated technologies in the near term including auxiliary electrification of engine cooling fan and hydrogen injection. For the mid-term time frame, the plan involves implementing an engine stop/start system and electrifyi...

The present study examined age differences in performance on the Tower of London, a measure of strategic planning, in a diverse sample of 890 individuals between the ages of 10 and 30. Although mature performance was attained by age 17 on relatively easy problems, performance on the hardest problems showed improvements into the early 20s. Furthermore, whereas age‐related performance gains by children and adolescents (ages 10–17) on the hardest problems were partially mediated by maturational improvements in both working memory and impulse control, improved performance in adulthood (ages 18+) was fully mediated by late gains in impulse control. Findings support an emerging picture of late adolescence as a time of continuing improvement in planned, goal‐directed behavior. (Source: Chil...

The LTPAC (Long Term Post Acute Care) Health Information Technology (HIT) Collaborative consists of an alliance of long-term services and post-acute care stakeholders. Members of the collaborative are actively promoting HIT innovations in long-term care settings because IT adoption for health care institutions in the United States has become a high priority. One method used to actively promote HIT is providing expert comments on important documents addressing HIT adoption. Recently, the Office of the National Coordinator for HIT released a draft of the Federal Health Information Technology Strategic Plan 2011-2015 for public comment. As a Pro-Health activity, the use of this management tool in collective health care was analyzed. From the prioritized problems, the following goals were established: professional education and performance which are coherent with the family health care strategy, emphasizing the situational diagnosis and the schedule of inter-sector actions. Despite the resistance of some workers concerning changes, favourable aspects were emphasized: weekly team meetings, the existence of local health care councils, and the initiatives of community educational actions. This exercise strengthened the students' communication, leadership, and decision-making skills,...
