            <description>A previously fit 12-yr-old boy, who had no previous history of anaesthesia, underwent general anaesthesia using isoflurane for an elective circumcision. After uneventful surgery and anaesthesia, he suffered a cardiorespiratory arrest in the recovery room. Prompt oxygenation and cardiopulmonary resuscitation (CPR) were instituted. The initial serum potassium was &amp;gt;13 mmol litre&amp;ndash;1 and prolonged CPR was required while potassium levels were reduced. Further investigation demonstrated a creatine kinase (CK) &amp;gt;70 000 U litre&amp;ndash;1 which was consistent with a diagnosis of rhabdomyolysis.             <description>The authors performed a well-designed, randomized, double-blinded study demonstrating decreased postoperative need for analgesia in 8- to 12-year-old boys after circumcision under local anesthesia with 5% tramadol (2 mg/kg) plus epinephrine, compared with those receiving prilocaine (1.5 mg/kg) plus epinephrine. The authors noted that a higher percentage of patients in the tramadol group required additional injections (45% vs 30%) for analgesia during the procedure. Although this difference did not reach statistical significance, the study is limited by the relatively small number of only 20 patients in each group. Predictors of decreased support included being of Hispanic ethnicity and believing that the uncircumcised penis was more culturally normal.             <description>The American Academy of Pediatrics (AAP) male circumcision policy states that while there are potential medical benefits of newborn male circumcision, the data are insufficient to recommend routine neonatal circumcision. Since 2005, however, 3 randomized trials have evaluated male circumcision for prevention of sexually transmitted infections. The trials found that circumcision decreases human immunodeficiency virus acquisition by 53% to 60%, herpes simplex virus type 2 acquisition by 28% to 34%, and human papillomavirus prevalence by 32% to 35% in men. Among female partners of circumcised men, bacterial vaginosis was reduced by 40%, and Trichomonas vaginalis infection was reduced by 48%. Antischistosomal therapy was the main cause of contamination there, followed by procedures performed by informal providers and traditional healers such as dental care, wound treatment, circumcision, deliveries, excision and scarification. It is also highly prevalent in sub-Saharan Africa and in the Middle East. In Europe, its prevalence has recently increased particularly among intravenous drug users and in immigrants.             <description>CONCLUSIONS: The implications of this analysis are that common interpretations of clinical trial interventions are insufficient for understanding the true efficacy of an intervention in some circumstances. Estimates of effectiveness in trials may misrepresent effectiveness per exposure event and effectiveness over a lifetime of risk. Care should be taken when designing protocols for analysis of trial results when the expected incidence is high. No change to the current practice of designing randomized control trials is suggested but analysis of trial data could be extended to calculate other statistics of effectiveness. A type of extrapolation and interpolation method for estimating levels of effectiveness is proposed.

            <description>Abstract&amp;nbsp;&amp;nbsp;Adult male circumcision reduces a man’s risk for heterosexual HIV acquisition. Infant circumcision is safer, easier and less
 costly but not widespread in southern Africa. Questionnaires were administered to sixty mothers of newborn boys in Botswana:
 92% responded they would circumcise if the procedure were available in a clinical setting, primarily to prevent future HIV
 infection, and 85% stated the infant’s father must participate in the decision. Neonatal male circumcision appears to be acceptable
 in Botswana and deserves urgent attention in resource-limited regions with high HIV prevalence, with the aim to expand services
 in safe, culturally acceptable and sustainable ways.
