Lack of effect of short-term treatment with Amlodipine and Lisinopril on retinal autoregulation in normotensive patients with type 1 diabetes and mild diabetic retinopathy

Conclusions: Short-term treatment with amlodipine and lisinopril had no significant effect on retinal autoregulation in young normotensive patients with T1D and mild retinopathy, and this lack of effect was similar for the two drugs. A possible normalizing effect of antihypertensive treatment on retinal autoregulation was not observed; however, it might take longer time to improve autoregulation than to reduce the arterial blood pressure.

Conclusion: Functional results of a translocation of RPE and choroid were heterogeneous and rather disappointing in this study. Results may have been influenced negatively by case selection. We found a relatively high rate of adverse events in the postoperative course. In selected cases, e.g. massive submacular haemorrhage or progressive neovascular AMD unresponsive to other treatment options, autologous translocation of RPE and choroid may still be considered.

Conclusions: Pigmented rats develop less UVR cataract and less ocular inflammation than non-pigmented rats. Pupil size plays a smaller role in UVR cataract development in pigmented rats than in non-pigmented. The role of UVR-induced ocular inflammation in cataract development is still ambiguous.
Intraocular pressure reduction with a fixed treatment protocol in the Early Manifest Glaucoma Trial

Conclusion: With a fixed treatment protocol, the IOP reduction achieved depended very strongly on baseline untreated IOP levels. There seemed to be a lower threshold around 15 mmHg, where therapy did not result in any reduction of IOP. Our results suggest that when effects of IOP-lowering treatment are reported, whether expressed in mmHg or as a percentage of untreated pressure levels, the baseline IOP levels should be specified as well.

Conclusion: The results of our 12-month prospective study indicate that intravitreal bevacizumab alone has similar efficacy and safety to bevacizumab plus PDT for treatment of patients with neovascular AMD, even treatment-naïve patients. The addition of PDT did not assist in reducing the required total number of bevacizumab injections. (Source: Acta Ophthalmologica)

This article reviews the historical and current concepts of 'tobacco optic neuropathy' (TON) a rare disorder of optic nerve function related to the toxic effects of an unidentified constituent of tobacco. It is considered to be an entity distinct from that often described as 'tobacco-alcohol amblyopia', a disorder better described as a nutritional optic neuropathy. It is suggested that 'tobacco-alcohol amblyopia' is an inappropriate term, because the condition to which it refers is not an amblyopia, and there is little evidence to implicate a toxic effect of either tobacco or alcohol in the pathogenesis of that disorder.
Conclusion: The present study confirms experimentally that PA, FPA and POBF are dependent on ocular volume and indicates that the pulsatile component of ocular blood flow is not reduced in myopic patients. Accordingly, the relationship between AEL and POBF described previously appears to be a consequence of different ocular volumes. Our findings have important implications for studies using PA or POBF.
The development of ophthalmology was greatly restricted in Poland in the 19th century, because it was partitioned and occupied by its three dominant neighbours. Polish medical universities were closed, and in Polish hospitals, only clinical work was possible. Those who wanted to study medicine and become ophthalmologists were forced to live and work in exile. Nevertheless, there were some Polish ophthalmologists at that time who had some international influence on retinal research. They contributed to colour vision physiology and pathology, ophthalmoscopy, retinal detachment and gyrate chorioretinal atrophy and congenital choroidal coloboma. The most prominent were Wiktor Szokalski, Ksawery Gałęzowski, Bolesław Wicherkiewicz, Kaziemierz Noiszewski and Michał Bor...

Diabetic macular oedema (DMO) is an important cause of vision loss in patients with diabetes mellitus. The underlying mechanisms of DMO, on both macrocellular and microcellular levels, are discussed in this review. The pathophysiology of DMO can be described as a process whereby hyperglycaemia leads to overlapping and inter-related pathways that play a role not only in the initial vascular events, but also in the continued tissue insult that leads to chronic DMO. On a macrocellular level, DMO is believed to be in part caused by alterations in hydrostatic pressure, oxygen tension, oncotic pressure and shear stress. Three key components of the microvascular pathways include angiogenic factor expression, inflammation and oxidative stress. These molecular mediators, acting in conjunction with ...

Conclusions: Patients with worsening cataracts occurring at a pre-senile age should be examined for galactosaemia. We describe for the first time the molecular genetic findings in congenital ectopia lentis et pupillae. Early treatment in conjunction with a galactose-free diet is mandatory in patients with galactosaemia. Members of a family with heterozygosity for galactosaemia should be advised to attend a human genetic consultation. (Source: Acta Ophthalmologica)

Conclusion: Contrary to clinical expectation, we found neurodiscs that appeared to be small and dense in eyes with high myopia. We further discussed the eventual peaking of the angle between the temporal vessel arcades that is sometimes observed in high myopia. Occasionally, literature on myopia reports the presence of small and tilted discs whereas no mention was found of the issue of temporal vessel arcade peaking.
Conclusions: PPV with AVS is a safe procedure, and adjunctive IVT had no additional effects on vascular perfusion. Successful decompressive surgery was followed by disappearance of collateral vessels at the BRVO blockage site and was a clinical marker for intravascular reperfusion. Long-term epiretinal gliosis and subfoveal photoreceptor atrophy limited functional and visual recovery. (Source: Acta Ophthalmologica)

Conclusion: When OCT is used, macular abnormalities are present in 7.4% of patients with RP, which is lower than the prevalence reported earlier. Although vitrectomy can improve the macular morphology in some patients with RP, improvement of visual function may be limited most likely because of the long-standing retinal dysfunction.
Conclusions: Laser in situ keratomileusis treatment for high myopia can effectively reduce high degrees of myopia. Under correction was observed in both treatment groups but Group B has a slightly better predictability. Significant loss of BSCVA occurs infrequently after LASIK for even considerable grades of myopia (0.6% in each group). (Source: Acta Ophthalmologica)

The influence of injection volume and capsular bag contraction on the refractive power of polymer refilled lenses – a finite element modelling simulation study

Conclusion: The simulations show that overfilling during polymer injection leads to a refractive power increase of the lens. Capsular bag contraction also results in a power increase. The calculated accommodative amplitude of the lens is minimally affected by capsular bag contraction but decreases significantly with increased capsular bag stiffness as a result of fibrosis.
Conclusion: No differences between ANN and SVM were observed in this study. Both MLCs performed very well, with similar diagnostic performance. Input parameters have a larger impact on diagnostic performance than the type of machine classifier. Our results suggest that parameters based on transformed A-scan thickness measurements of the RNFL processed by machine classifiers can improve OCT-based glaucoma diagnosis.
The volume of cells that a length of capillary supplies with O2 is called a Krogh cylinder. This geometric 'tissue unit' was named after the Danish zoophysiologist and Nobel laureate August Krogh who made important discoveries in the fields of external and internal respiration in the first half of the last century. Krogh's ideas concerning tissue O2 distribution can be extrapolated to retinal oxygenation by larger vessels (including arterioles, arteries and even veins) and by vessel groups within higher-order 'microvascular units' (including the choroid). During retinal development, for example, the difference in pO2 levels within arteries and capillaries determines Krogh cylinders of different radius and establishes the periarterial capillary-free zone of His. The O2 supply to the venous ...

Conclusion: The early upregulation of BDNF seems to act neuroprotectively and leads to a delay of ganglion cell loss. Although there is no direct evidence, the decrease of VIP and ADNP [ndash] the consequence of the presence of NMDA receptors on these peptide-expressing cells [ndash] might contribute to the breakdown of endogenous neuroprotective mechanisms given that the decrease of the VIP-related ADNP runs in parallel with the decrease of VIP. Activating and maintaining these mechanisms must be the primary aim in the therapy of diseases with retinal neuronal degeneration. (Source: Acta Ophthalmologica)

Conclusion: Treatment with a low dosage of fluvastatin sodium or pravastatin sodium reduced the lipid build-up as well as the macrophages in the choroid and restored the vascular lumens of choroidal vessels independently of the cholesterol effect. The normal ultrastructural features of choroidal EC and VSMC in statin-treated animals suggest that the endothelial function is preserved and the ischaemia reduced.
Conclusion: We identified certain possible risk factors for age-related cataract. In a Mediterranean Greek population, we found that smoking, use of cortisone drops, cardiovascular heart disease and sunlight exposure increase the risk for cataract, while use of hat and vision repair spectacles act protectively.
Conclusion: A mfERG component related to bipolar and Müller cell function was accelerated by a short intermittent exposure to HBO. The response developed after the end of the HBO exposure and lasted for at least 3 weeks, suggesting that it was prompted by the withdrawal of HBO rather than the onset and subsequent brief exposure to HBO.
Conclusions: Based on a variety of clinical ophthalmic preterm appearances, we propose the hypothesis that, in addition to increased structural vulnerability, a reduced functional reserve may pave the way for further abiotrophy or involution over time. This may manifest as reduced resistance to later ophthalmic events, such as disease or trauma, or simply the impact of age. (Source: Acta Ophthalmologica)

Conclusions: These results suggest that the diabetes-mediated increase in CTGF upregulates VEGF and TGF-[beta]2 expression and induces apoptosis in the retina. This elevation may be inhibited by treatment with CTGFsiRNA. Connective tissue growth factor may serve as a potential target for the prevention and treatment of DR.
This article, based on a report prepared for the International Council of Ophthalmology (ICO) and the International Society for Low Vision Research and Rehabilitation (ISLRR), explores the assessment of various aspects of visual functioning as needed to document the outcomes of vision rehabilitation. Documenting patient abilities and functional vision (how the person functions) is distinct from the measurement of visual functions (how the eye functions) and also from the assessment of quality of life. All three areas are important, but their assessment should not be mixed. Observation of task performance offers the most objective measure of functional vision, but it is time-consuming and not feasible for many tasks. Where possible, timing and error rates provide an easy score. Patient resp...

Conclusion: We are only able to recommend photopic full spectral radiance incandescent light sources to visually impaired subjects for their domestic surroundings. Furthermore, we recommend the use of full spectral radiance light sources for the illumination of Pelli-Robson contrast sensitivity charts. Given equal illuminance, as in our study, the findings show that contrast sensitivity was better by illumination with incandescent light with full spectral radiance compared with fluorescent light with interrupted spectral radiance.
Comparing intravitreal triamcinolone acetonide and bevacizumab injections for the treatment of diabetic macular oedema: a randomized double-blind study

Conclusions: Intravitreal triamcinolone proved to be more efficient in reducing DMO, providing longer lasting visual improvement, relative to bevacizumab. Eyes treated with triamcinolone had the highest percentage increase in IOP. Further studies are needed to corroborate these findings. (Source: Acta Ophthalmologica)

Conclusion: The presence of Terson's syndrome was associated with an increased mortality rate (50% versus 9%; p < 0.01). Therefore, patients with the diagnosis of intracranial haemorrhage should be submitted to a funduscopic examination, because the presence of intraocular haemorrhage is an important life-threatening prognostic factor.
Conclusions: SMRL and primary retinal lymphoma present with similar clinical manifestations. Systemic T-cell lymphoma invades the retina and vitreous more aggressively than systemic B-cell lymphoma. A diagnosis of SMRL is made when there is a clinical history of systemic lymphoma (particularly from nasopharynx, testis and skin), and lymphoma cells are identified in the vitreous or retina. Molecular analysis is more useful than vitreal cytokine measurement for SMRL diagnosis. (Source: Acta Ophthalmologica)

Conclusion: The between-group distinctions are consistent with the possibility (derived from the women's health literature) that phyto-oestrogens may counteract a decline of short-wavelength-sensitive cone-mediated response among postmenopausal women. These results suggest another potential application for SWAP outside its original intended purpose as a glaucoma test. Future studies should assess whether phyto-oestrogen consumption is most beneficial for women who are sufficiently young and/or not too far beyond menopause.
Conclusion: Emmetropia was not the predominant outcome for refractive development in children. Instead, populations were predominantly mildly hyperopic or substantial amounts of myopia appeared in them. This suggests that mild hyperopia is the natural state of refractive development in children and that emmetropia during childhood carries the risk of subsequent progression to myopia.
Many studies have suggested an association between blood flow dysregulation and glaucomatous damage to the optic nerve. Confocal scanning laser Doppler flowmetry (CSLDF) is a technique that measures the capillary blood flow of the retina and optic nerve head and provides a two-dimensional map of ocular perfusion in these areas. This review discusses the anatomy of the anterior optic nerve vasculature and the capabilities and limitations of the CSLDF. Methods to minimize error and to acquire more reliable measurements of capillary blood flow are also outlined. (Source: Acta Ophthalmologica)

Conclusion: This 40-year postoperative follow-up of patients with childhood ET who underwent strabismus surgery by a single surgeon in Sweden showed that multiple surgeries and presence of postoperative adduction deficit were the most important factors influencing the incidence of consecutive XT after surgery. Presence of uncorrected amblyopia did not alter the prognosis for long-term development of consecutive XT. (Source: Acta Ophthalmologica)

Conclusion: The vast majority of adverse reactions to fluorescein are mild and not attributed to immunological mechanisms. Allergy skin tests cannot predict non-immunological reactions but their utility remains substantial in predicting anaphylaxis during SFAs and must be performed in patients reporting risk factors in their past medical history.
Conclusion: Damaged RPE area was more evident and better delineated by FAF imaging compared with fundus examination and fundus photography alone. Autofluorescence imaging might be a useful exam to show the length and severity of post-traumatic retinal lesions and it may add relevant information in the global evaluation of blunt ocular trauma complications. Moreover, OCT added valuable information to the diagnosis and progression of choroidal rupture. Further studies to determine the predictive value of FAF in ocular blunt trauma are warranted.
Trabeculectomy with OloGen versus trabeculectomy for the treatment of glaucoma: a pilot study

Conclusion: In this pilot study it appears that trabeculectomy with OloGen does not seem to offer any significant advantages compared with trabeculectomy alone. Additionally, even though there were no statistical differences between the two groups as far as complications were concerned, one eye from the study group developed endophthalmitis 10 days after surgery and two eyes presented with positive Seidel test and flat anterior chamber and required additional suturing. Studies with larger numbers of patients and longer follow-ups are required to confirm these findings and to examine the safety and long-term outcomes of trabeculectomy with OloGen.
This article reviews molecular mechanism of intraocular inflammation in animal models and in humans, and the immunological defence system of the eye with particular attention to ocular pigment epithelium. In experimental autoimmune uveitis (EAU), T lymphocytes, particularly CD4+ T lymphocytes, play a central role in its immunopathogenic mechanisms. In humans, activated CD4+ T cells also play a central role in the immunopathogenic mechanisms. This notion is demonstrated in two human diseases: one is Vogt[ndash]Koyanagi[ndash]Harada disease, and the other is human T-cell leukemia virus type 1 (HTLV-1) uveitis. Activated CD4+ T cells infiltrating the eye are harmful to vision-related cells and tissues in the eye and cause sight-threatening conditions. However, the eye has regional defence sys...

Conclusions: Molteno aqueous shunt as the first glaucoma procedure decreased IOP effectively in uveitic glaucoma. Even after 4 years, the survival estimate was quite high. The IOP decreased continuously during the first year after the surgery, and the medication was slowly tapered even up to 3 years postoperatively. It is suggested that it may be possible to postpone further surgical intervention during the first postoperative year after Molteno implantation even if the IOP is not quite optimal.
Conclusion: Optical coherence tomography-derived retinal thickness measurements are subject to measurement variability in patients with active nAMD. The results suggest a change criterion of more than 59 [mu]m in central 1 mm (A1) subfield macular thickness is necessary to distinguish true clinical change from measurement variability in these patients.
Conclusion: Elevated MMP-2 and MMP-9 levels have been implicated in the failure of corneal re-epithelialization, and enhanced MMP-2 and MMP-9 levels in CDK patients suggest that these MMPs may play a role in corneal scarring in CDK. Elevated levels of MMP-8 suggest a defensive role for this MMP in inflammatory reactions associated with recurring corneal traumas. Decreased expression of TIMP-1 in CDK patients suggest deficient antiproteolytic shield likely to render the corneas of CDK patients vulnerable to enhanced MMPs. Overall, these data suggest a mechanistic link between MMPs and TIMP-1 level in cornea and tears with corneal scarring in CDK. (Source: Acta Ophthalmologica)

Conclusions: MF were more easily identified following immunohistochemical staining with anti-PHH3. This resulted in higher mitotic counts than obtained with H&E sections. For PHH3, the mitotic count determined by a MA was virtually the same as those obtained by an experienced pathologist. PHH3 is now routinely used in our centre, and new prognostic thresholds for uveal melanoma will be defined in further studies. (Source: Acta Ophthalmologica)

Purpose: To identify differences in corneal hysteresis (CH) and central corneal thickness (CCT) between healthy and glaucomatous patients. Methods: Retrospective observational study. One hundred and thirty-three eyes of 75 healthy and 58 glaucomatous patients were included. CH was measured in each patient using Ocular Response Analyzer. CCT was determined by ultrasonic pachymetry. For each patient, one eye was randomly selected. We used a Student t-test to search for significant differences between the different groups (p (Source: Acta Ophthalmologica)

Conclusion: Cannulation of the anterior chamber measures the actual IOP; however, the IOP measured by cannulation in the vitreous cavity using the same catheter diameter was different, possibly because of the viscosity of the vitreous. The viscosity of the vitreous probably makes the cannulation method of IOP evaluation in the vitreous cavity inaccurate. (Source: Acta Ophthalmologica)

Influence of suture regularity on corneal astigmatism after penetrating keratoplasty

Conclusion: The origin of corneal astigmatism after penetrating keratoplasty is multifaceted. Regular stitch length and stitch advancement on the surface appears to improve the optical quality of the graft after suture removal. Factors such as stitch depth, suture tension and variations in wound construction might also be important predictors of corneal astigmatism after penetrating keratoplasty.
Conclusion: In adult Chinese, body height is associated with higher level of education, higher income, lower systolic blood pressure, higher diastolic blood pressure and lower body mass index, in addition to living in an urban region and being male. From an ophthalmic point of view, tall individuals had deeper anterior chambers. There was a tendency towards lower body height and marked hyperopia. Because the socioeconomic, systemic and ocular parameters examined influence the occurrence and therapy of ocular diseases, these data may be helpful in the assessment of risk factors and in the diagnosis and treatment of ocular diseases.
