Conclusion:  The results of our 12‐month prospective study indicate that intravitreal bevacizumab alone has similar efficacy and safety to bevacizumab plus PDT for treatment of patients with neovascular AMD, even treatment‐naïve patients. The addition of PDT did not assist in reducing the required total number of bevacizumab injections. (Source: Acta Ophthalmologica)

Conclusions:  From a cost‐effectiveness point of view, it seems advantageous to aim for a low intraocular pressure in all glaucoma patients. The feasibility of this strategy should therefore be investigated. Additionally, the cost‐effectiveness outcomes of initiating monotherapy with a prostaglandin analogue and reducing the frequency of VF testing may be acceptable.
Conclusions:  Increased vergence demand to control intermittent distance exotropia for near also drives significantly more accommodation. Minus lens therapy is more likely to act by correcting overaccommodation driven by controlling convergence, rather than by inducing blur‐driven vergence. The use of convergence as a major drive to accommodation explains many clinical characteristics of distance exotropia, including apparently high near stimulus AC/A ratios.
Conclusion:  The prognostic most reliable but investigator‐dependent methods to clinically detect whether the posterior vitreous cortex is detached are B‐scan ultrasonography and BM. The objective technique of the high‐resolution, two‐dimensional time‐domain OCT allows only in a few cases a clear differentiation of preretinal structures.
Conclusions:  Glaucoma patients do not present the linear relationships between RI and PI observed in healthy individuals.
Conclusions:  Twelve‐year incidence of XFS is higher in women than in men and higher in older age groups than in younger ones. Most persons deemed on the slitlamp to be unilaterally affected have converted to bilateral over 12 years. Eyes with XFS at baseline were 3–4 times more likely to have cataract surgery during the 12 years. Our definition of definite XFS generally holds, while our definition of probable XFS is of no prognostic value over 12 years. (Source: Acta Ophthalmologica)

Conclusion:  The results suggest a significant alteration of both ChBVel and ChBF in EMG, which does not appear to be associated with the severity of central retinal dysfunction. These findings may contribute to a better understanding of the pathophysiology of early glaucomatous damage in EMG and have implications for the treatment of this pathologic condition.
Conclusions:  These cases add further anatomical support to the theory of paradoxical innervation in congenital Brown syndrome. CN IV was absent in two patients on the side of the Brown syndrome, but without muscle hypoplasia. SO muscle size did not vary in up‐ and downgaze, which we interpreted as a sign of constant innervation through branches of CN III. (Source: Acta Ophthalmologica)

Conclusions:  Glaucoma screening at the optician shop was feasible, but the specificity of the screening protocol was rather low. With more stringent cut‐off points (30 mmHg; at least two reproducibly abnormal test locations), the specificity could be improved to 96% (94–98%), apparently without a significant loss of sensitivity. This suggests that screening during regular optician visits might be a viable approach. (Source: Acta Ophthalmologica)

Conclusion:  Quit smoking is an important advice to patients to prevent or slow the progress of AMD. There is no recommendation for routine nutritional or vitamins supplementation for primary prevention. However, patients with documented intermediate risk of AMD or advanced AMD in one eye are recommended to take AREDS‐type vitamin supplements. (Source: Acta Ophthalmologica)

Conclusions:  Our study shows that cortical cataract is associated with diabetes mellitus, not necessarily defined by glucose control, whereas posterior subcapsular cataract is associated with glucose levels. Nuclear cataract is not associated with diabetes mellitus, but is more frequent in women and is also associated with higher heart rate. (Source: Acta Ophthalmologica)

Conclusion:  Pterygium occurs in approximately one‐tenth of Norfolk Islanders. Increasing conjunctival UVAF is associated with prevalent pterygia, confirming earlier epidemiological, laboratory and ray‐tracing studies that pterygia are associated with UVR. Protection from the sun should be encouraged to reduce the prevalence of pterygium in the community. (Source: Acta Ophthalmologica)

Abstract. The aim of this article was to provide a comprehensive review of current knowledge regarding ocular hemodynamic alterations affecting the retinal neuroglial cells and optic nerve head (ONH) function during cardiac surgery. Literature indicates that visual loss after heart surgery is a rare but devastating complication provoked by two main causes of optic ischaemia and infarction during on‐pump cardiac procedures: microembolism and/or hypoperfusion. Retinal ischaemia and ischaemic optic neuropathy are two possible major consequences of extracorporeal circulation in cardiac surgery. The hemodynamic modifications within the vascular beds of retina and ONH during cardiovascular operations have been incompletely studied. Consequently, it is of great interest to investigate the hemody...

Abstract. The mucopolysaccharidoses (MPS) are a group of rare lysosomal storage disorders characterized by the accumulation of glycosaminoglycans in several tissues and organs. This accumulation results in an array of clinical manifestations and premature death in severe cases. Ocular problems are very common in children with MPS and may involve the cornea, sclera, trabecular meshwork, retina, optic nerve and also the posterior visual pathways. The aims of this study are to give an overview of ocular problems in MPS and to provide clinical guidelines for paediatric ophthalmologists for early diagnosis and management of ocular manifestations in children with MPS. Diagnostic problems may arise in children with severe corneal clouding, hampering visualization of the fundus. Intraocular pressur...

Abstract.Acta Ophthalmologica appeared as the scientific journal of the Nordic ophthalmological Societies in 1923. The intention was to expose the clinical and experimental developments among the ophthalmological communities of the four Nordic countries: Denmark, Finland, Norway and Sweden. The collaboration within the field of ophthalmology had been attempted with the publication of ‘Nordisk ophthalmologisk Tidsskrift’ in the years 1889–1892. Now, once again, the wish for a tighter Nordic cooperation was explored by enthusiastic ophthalmologists in all four countries. One person stands out as the driving force, K.K.K. Lundsgaard, who in spite of his early death in 1931 was remembered by name on the front page of Acta since 1936 by the Latin phrase ‘A K.K.K. Lundsgaard edi coepta...

Conclusion:  The results show that photoentrainment of circadian rhythm may be significantly impaired in older subjects because of the colour filtering characteristics of the human lens, whereas the effects were small for all three types of IOLs studied. Consequently, the ageing process of the natural lens is expected to influence the photoentrainment of circadian rhythm, whereas IOLs are not expected to be detrimental to circadian rhythm.
Conclusions:  This study provides the evidence that contractions of the anterior lens epithelial cells take place in significant portion of human lens anterior capsule postoperative preparations after non‐specific stimulation. Contractions are at least partially independent of changes in [Ca2+]i. They can be mechanically induced, are localized and reversible and have a fast response and did not differ among different types of cataract. Physiological and clinical significance of this phenomenon remains to be elucidated. (Source: Acta Ophthalmologica)

Conclusion:  We are only able to recommend photopic full spectral radiance incandescent light sources to visually impaired subjects for their domestic surroundings. Furthermore, we recommend the use of full spectral radiance light sources for the illumination of Pelli‐Robson contrast sensitivity charts. Given equal illuminance, as in our study, the findings show that contrast sensitivity was better by illumination with incandescent light with full spectral radiance compared with fluorescent light with interrupted spectral radiance.
Conclusions:  Short‐term treatment with amlodipine and lisinopril had no significant effect on retinal autoregulation in young normotensive patients with T1D and mild retinopathy, and this lack of effect was similar for the two drugs. A possible normalizing effect of antihypertensive treatment on retinal autoregulation was not observed; however, it might take longer time to improve autoregulation than to reduce the arterial blood pressure.
Conclusion:  We conclude that HRT can be used in children between 5 and 16 years of age, and normal values presented in the study can be used for comparison of children with optic nerve diseases. Because the assessment of rim area varied the least, it may be the parameter to use for follow‐up. The normal large interocular difference should be taken into account when comparing eyes in the individual child. (Source: Acta Ophthalmologica)

Abstract.Age‐related macular degeneration (AMD) is attributed to a complex interaction of genetic and environmental factors. It is characterized by degeneration involving the retinal photoreceptors, retinal pigment epithelium (RPE) and Bruch’s membrane, as well as alterations in choroidal capillaries. AMD pathogenesis is strongly associated with chronic oxidative stress and inflammation that ultimately lead to protein damage, aggregation and degeneration of RPE. Specific degenerative findings for AMD are accumulation of intracellular lysosomal lipofuscin and extracellular drusens. In this review, we discuss thoroughly RPE‐derived mechanisms in AMD pathology.
Conclusion:  The data suggest that cataract surgery triggered a use‐dependent structural plasticity in V2 presumably through improved binocular integration of visual input from both eyes. We conclude that activity‐dependent cortical plasticity is preserved in the ageing visual cortex and may be triggered by restoring impaired vision. (Source: Acta Ophthalmologica)

Conclusion:  The use of OCT and US enabled us to detect a partial or total PVD in 80% of the eyes. A sudden increase in late‐stage PVD between the ages of 70 and 75 was observed, correlating with the reported age prevalence of various macular diseases. In contrast to myopics, both genders of elderly emmetropics have a similar prevalence of PVD. (Source: Acta Ophthalmologica)

Conclusions:  Both inflammatory markers and VEGF‐A were higher in CRVO and H‐CRVO than in BRVO undiluted vitreous samples. It seems that monocyte recruitment to the vessel wall, which might underlie the importance of eosinophils in tissue remodelling after RVO, is of special interest owing to the significant difference in MCP‐1 in the older RVO types.
Abstract.Age‐related macular degeneration (AMD) shares several pathological and epidemiological similarities with systemic atherosclerosis (AS). First, an association between AS and AMD is apparent from the analyses of the histological and biochemical structure of atherosclerotic plaques in the vascular walls and retinal drusen, the hallmark of AMD. Second, there is considerable evidence implicating endothelial dysfunction in the pathogenesis of both disorders, and cellular oxidative stress appears to be a common denominator underlying this process. Moreover, there are observations that the complement system (CS) triggering inflammatory response contributes to the onset and advancement of both diseases. The CS plays a role in the generation of drusen and neovascularization in AMD as well...

Abstract.Seasonal allergic conjunctivitis (SAC) is an inflammatory response of the conjunctiva triggered by exposure to seasonal allergens. Treatment options for SAC include artificial tears, antihistamines, decongestants, mast cell stabilizers, nonsteroidal anti‐inflammatory drugs, dual antihistamine/mast cell stabilizers, immunotherapy and corticosteroids. Topical, intranasal and systemic formulations of corticosteroids have traditionally provided the most effective relief of the inflammation and signs and symptoms associated with severe, acute exacerbations of SAC. However, steroid‐induced ocular and systemic side‐effects have limited the prescribing of these agents. This limitation of traditional corticosteroids led to the development of modified corticosteroids that retain the a...

Conclusion:  Both high‐affinity Ni2+‐sensitive Ca2+ channels contribute to transretinal signalling. Based on the results from the double knockout mice, additional targets for NiCl2 must contribute to transretinal signalling, which will be most important for the structurally similar physiologically more important heavy metal cation Zn2+.
Conclusion: The early upregulation of BDNF seems to act neuroprotectively and leads to a delay of ganglion cell loss. Although there is no direct evidence, the decrease of VIP and ADNP – the consequence of the presence of NMDA receptors on these peptide‐expressing cells – might contribute to the breakdown of endogenous neuroprotective mechanisms given that the decrease of the VIP‐related ADNP runs in parallel with the decrease of VIP. Activating and maintaining these mechanisms must be the primary aim in the therapy of diseases with retinal neuronal degeneration. (Source: Acta Ophthalmologica)

Conclusion:  Treatment with a low dosage of fluvastatin sodium or pravastatin sodium reduced the lipid build‐up as well as the macrophages in the choroid and restored the vascular lumens of choroidal vessels independently of the cholesterol effect. The normal ultrastructural features of choroidal EC and VSMC in statin‐treated animals suggest that the endothelial function is preserved and the ischaemia reduced.
Conclusion:  Functional results of a translocation of RPE and choroid were heterogeneous and rather disappointing in this study. Results may have been influenced negatively by case selection. We found a relatively high rate of adverse events in the postoperative course. In selected cases, e.g. massive submacular haemorrhage or progressive neovascular AMD unresponsive to other treatment options, autologous translocation of RPE and choroid may still be considered.
Conclusions:  The evidence that DO, even by ophthalmologists, is sub‐optimal in detecting common abnormalities challenges existing the notions of training medical students. On pending the results of additional studies of the value of DO in detecting emergencies, we suggest that undergraduate teaching of DO should impart the following: (i) an ability to identify the red fundus reflex and optic disc; (ii) an ability to recognize signs of clinical emergencies in patients, mannequins or fundus photographs; and (iii) knowledge about, but not an ability to detect, other retinopathies.
Abstract. This is a biography of Allvar Gullstrand (1862–1930) on the occasion of the centennial of his 1911 Nobel Prize in physiology or medicine. We reviewed pertinent literature and we did archival studies at the Uppsala University Library and the Regional State Archives at Lund as well as the Nobel Archives at the Royal Swedish Academy of Sciences in Stockholm. Allvar Gullstrand was a brilliant scientist with an exceptional personality. He gave mathematical descriptions of the dioptric system of the human eye with unprecedented accuracy, and he invented and designed ophthalmological instruments of far‐reaching importance. The two most valuable ones are the slit lamp and the reflexless ophthalmoscope. Both are in everyday use by any ophthalmologist in the world. Allvar Gullstrand is s...

Conclusion:  Under a ranibizumab regimen with substantially fewer injections than with fixed four‐weekly injection regimens, BCVA was improved compared with the natural history of neovascular AMD, but did not achieve the visual gain observed in randomized clinical trials using fixed 4‐week retreatment. Visual acuity at month 3, after the initial fixed‐interval injections, was the strongest predictor of BCVA at month 12. (Source: Acta Ophthalmologica)

Abstract. The main objective of this report is to encourage consistent quality of testing and reporting within and between centres that use colour Doppler imaging (CDI) for assessment of retrobulbar blood flow. The intention of this review is to standardize methods in CDI assessment that are used widely, but not to exclude other approaches or additional tests that individual laboratories may choose or continue to use.
Conclusions:  African normal subjects and POAG patients had an altered CH, which is associated with a significant underestimation of GAT IOP. This may potentially contribute to the earlier development and greater severity of glaucoma damage in Africans compared with Caucasians at diagnosis. (Source: Acta Ophthalmologica)

Conclusion:  TNFα inhibits IFNγ‐induced MHC II expression in RPE cells. This inhibitory effect was reversed by infliximab and was not because of a global inhibition of IFNγ ‐mediated RPE cell activation but rather to a specific down‐regulation of CIITA expression. Those findings are consistent with the role of TNFα in the resolution of inflammation and might help to elucidate the complex development of autoimmune uveitis.
Conclusions: Intravitreal bevacizumab significantly reduced VEGF plasma levels until 28 days after intravitreal injection in patients with exudative AMD. Ranibizumab did not achieve a significant plasma VEGF reduction at the same time‐point. These findings alert to the potential systemic safety differences between the two drugs after intravitreal administration. (Source: Acta Ophthalmologica)

Conclusions:  Retinal vessel densitometry assessment in the presence of cataract results in the erroneous elevation of the diameter measurement and thereby the calculation of blood flow. The bidirectional Doppler assessment of blood velocity appears to be more robust to light scatter induced by cataract. Care needs to be exercised in the interpretation of studies of retinal vessel diameter or blood flow that utilize similar densitometry techniques. (Source: Acta Ophthalmologica)

Conclusion:  Intraocular pressure measurements after ppV are important to prevent unintentional high IOP, especially within the early phase (3 hr post‐treatment) in eyes with silicon oil filling. Gas filling leads to prolonged IOP increase (24 hr post‐treatment). Long‐lasting hypotony (≥6 hr) is very rare after 20G vitrectomy. (Source: Acta Ophthalmologica)

Conclusion:  Both initial treatment regimen lead to improved functional results after 1 year. However, patients, not suffering from adverse events, who underwent initial treatment with rtPA and gas showed better results. To maintain VA, controlling neovascular AMD by anti‐VEGF treatment regime after initial treatment with rtPA+gas is important for all cases. (Source: Acta Ophthalmologica)

Conclusion:  Expert training and execution of a specific protocol for CDI of ocular blood flow velocity measurements provide highly reproducible results in healthy volunteers. This is important for long‐term studies assessing ocular hemodynamics, where multiple examiners may be involved. (Source: Acta Ophthalmologica)

Combining refractive and topographic data in corneal refractive surgery for astigmatism

Conclusions:  The advantage of the optimization method described in this article is a consistent reduction in corneal astigmatism towards sphericity. No new corneal astigmatism is carved on the cornea, and probably less tissue is ablated. The optimization method may also be used to combine refractive and topographic data for higher order aberrations with sinusoidal components. However, compared to the traditional purely refractive driven treatment, more refractive astigmatism will remain in the eye in most cases. A controlled clinical trial is necessary for comparing these two treatment modalities. (Source: Acta Ophthalmologica)

Conclusions:  Our genetic case–control study did not indicate an association between AQP4 gene variants and IIH. However, the theory of an etiopathogenic link between IIH and AQP4 is tempting, and discussed in this article. Association studies with large sample size are difficult to perform owing is the rarity of the condition. (Source: Acta Ophthalmologica)

Conclusion:  Severe visual loss after SiO use was observed in 1/3 of patients with otherwise good visual potential. The visual loss was associated with a significant reduction in inner retinal thickness indicating neuronal cell loss in the macular area as a possible explanation.
Conclusion:  IOPcc was significantly higher in NTG eyes than in normal eyes. The ORA may be useful for distinguishing between the IOPcc of NTG eyes with normal IOP and that of normal eyes. In addition, the ORA enables CRF and CH to be measured in vivo, and weakness of the lamina cribrosa may be clinically inferred from the fact that CRF and CH were reduced in NTG eyes in our study. Low CRF and CH may be clues to the pathology of NTG. (Source: Acta Ophthalmologica)

Conclusions:  Corneal culturing affects the metabolic profile of the tissue. The increases in the levels of some metabolites within the second week of culturing likely result from variations in tissue metabolic or enzymatic activity caused by changed (organ culture) environment. As the mechanisms responsible for these changes are not clear, further research is indicated. (Source: Acta Ophthalmologica)
