CONCLUSIONS: The Spanish version of the translated SVHI questionnaire is a validated tool with proper internal consistency and reliability. The average score of the SVHI singers in a healthy population is 20% of the maximum possible questionnaire score, well above the average score in a healthy population for the spoken voice with the 30-item specific VHI.

We present a pilot study to evaluate the benefit of static facial suspension with Silhouette sutures. We operated on a female patient with complete facial palsy secondary to otic tuberculosis. The patient has currently achieved satisfactory facial symmetry, mastication and speech production. As a result, self-esteem and social interaction have also been recovered. Static facial suspension with Silhouette sutures is an alternative to dynamic techniques in patients who do not wish to or cannot undergo those more complex surgeries.

CONCLUSIONS: Conservative management is suggested for AM and subperiosteal abscess in children with cochlear implants. Surgical treatment should be avoided to prevent CI contamination. The first option is intravenous antibiotics and simple puncture of the abscess. If surgical drainage is needed, radiological study should be performed to locate the CI electrodes.

We present a case of spontaneous haemorrhage of a thyroid nodule with diffusion of the haematoma to cervical spaces and mediastinum, which began as a progressive cervical mass affecting the airway and which required surgical treatment for control. The diagnosis was made using ultrasonography and cervicothoracic CT scan, giving high quality images to determine the site of bleeding as well as the extent of the process.

Authors: Visa L, Caballero M, Grau JJ
Formerly, salivary gland cancer was considered to be chemoresistant. Chemotherapy is indicated when distant metastases or inoperable locorregional disease are observed, although the chemotherapy schedule is not well defined. Data on chemotherapy treatment for adenoid cystic carcinoma consist of phaseII trials. Most of these studies analyze therapies with a combination of agents at full dose, although there is no clear evidence that such treatment improves survival. The administration of cytotoxic agents with low doses at frequent, regular intervals with no drug-free interruptions is known as metronomic chemotherapy. Most head-to-head studies show similar or even superior therapeutic results with metronomic scheduling than with a maximum tolerated d...

CONCLUSIONS: The surgical technique of reimplantation and multireimplantation technique has characteristic peculiarities at each step, but in general terms, its complexity is similar to that of implantation in virgin ears and the rate of complications is also similar. The introduction of the new electrodes is simple and complete in the immense majority of cases, given that the cochlea remains permeable, as long as it houses the electrodes; otherwise it tends to become reossified. The functional auditory outcome of the new implant is in relation with its capacities. If they are equal to those of the initial implant then the auditory outcome will be similar and if they are better, then so will the outcome be, with the exception of ears with long periods of auditive deprivation.

Type 2 neurofibromatosis (NF2) is an invalidating, inherited, dominant, autosomal disease. It is commonly confused with type 1 neurofibromatosis, although the two disorders are different. All subjects who inherit a mutated NF2 gene will develop the disease, which is characterised by the growth of schwannomas, generally affecting the vestibular nerve bilaterally, as well as meningiomas and other benign central nervous system tumours, before their third decade of life. It is currently possible to identify the NF2 mutation in most affected families. Up to about 20% of NF2 patients with no family history, apparently sporadic cases, are actually individuals with mosaicism for this mutation. Much of the m...

Singing voice is a special subgroup within the field of voice. In addition to the differences in physiology between singing and speaking voice, singer patients are often regarded as a challenge for the otolaryngologist. The reason for this is probably that the field of voice has not received as much attention as others in our speciality. Moreover, in the case of singers, empathy is vital in the doctor-patient relationship, and, as in many other cases, it forms part of the therapeutic effect. In order to achieve this, the physician has to know what singers are and which are the main pathologies they suffer, how they are formed and how they are expressed. This review offers an overlook of the pathological-physiology of singing voic...

CONCLUSIONS: The induced nasal airflow technique enables an important recovery of olfaction and improvement of taste after total laryngectomy. This technique is easy to learn and to repeat. It does not require expensive materials. The recovery of olfaction and taste implies an improvement in quality of life for the patient, so this technique should be included in all protocols of comprehensive rehabilitation after total laryngectomy.

CONCLUSIONS: In the light of these findings we feel that the middle ear does not play a merely passive role in hearing but rather that the contraction of the endotympanic muscles makes possible variations in impedance such that the resonance frequency of the ear shifts towards higher frequencies, thus enhancing sound discrimination in noisy environments.

We report the case of a 67-year-old male diagnosed with prostate adenocarcinoma, who referred dysphagia, dysphonia and noticed the appearance of a laterocervical mass. A CT scan revealed an osteolytic soft tissue neck lesion, which was resected. Its anatomical-pathological study was compatible with metastasis of prostate adenocarcinoma.

The management of voice disorders requires a close collaboration between the throat surgeon and the speech therapy team. The diagnosis of cordal lesions is essential to an accurate therapy. Stroboscopy contributes functional information which is useful to decide between microsurgery, vocal reeducation or a combined treatment. We have reviewed our phonomicrosurgery cases for the past five years and analyzed the correlation between preoperative diagnosis obtained by stroboscopy and intraoperative findings. In 90% of cases there exists a relation between both diagnoses. We consider stroboscopy as an essential means of evaluation to establish an adequate management and guidance for surgical treatment....

Since the beginning of the 80s, numerous clinical trials have shown a significant reduction in the incidence of infections in clean-contaminated upper respiratory tract surgery, due to perioperative use of antibiotics; however, there is no consensus about the best antibiotic protocol. Moreover, there are no universally accepted guidelines about flap reconstructive procedures. In otological and rhinological surgery, tonsillectomy, cochlear implant and laryngo-pharyngeal laser surgery, the use of antibiotics frequently depends on institutional or personal preferences rather than the evidence available. We reviewed clinical trials on different otorhinolaryngological procedures, assessing choice of anti...

We report a case of a 24-year-old patient operated on at our centre for bilateral carotid artery parangliomas who developed baroreceptor failure after their resection. Albeit an infrequent complication, it is important to be aware of it in order to ensure is speedy diagnosis and treatment so as to avoid major post-surgical complications.

An atypical case of deep neck infection is presented with muscular involvement as the main feature. Streptococcus pyogenes was the causal agent and abrupt haemodynamic impairment and severe systemic failure characterized its clinical course, requiring emergency surgical examination and subsequent admission to the critical care unit. After the diagnosis of Streptococcal Myositis was obtained and the antibiotic treatment adjusted, the patient progressively recovered completely.

We report 7 cases of rhinocerebral mucormycosis in a retrospective study of 8 years (2000-2008) in hematologyc patients. An early diagnosis is essential, and therefore there must be a high level of clinical suspicion in patients with predisposing factors. Certainty diagnosis requires fungal cultures or biopsies of the affected areas which prove an invasion of the tissues by the characteristic hyphae. The key to treatment is early and aggressive surgical treatment, together with high intravenous doses of B amphotericyn. Despite this, prognosis is poor and mortality is about 70-80%.

Newborn hearing screening is currently performed routinely in many regional health-care systems in Spain. Despite the remarkable expansion in newborn hearing screening since 2000, its feasibility and the benefits of early identification and intervention, many major challenges still remain. In this article, the Committee for the Early Detection of Hearing Loss (Comisión para la Detección Precoz de la Hipoacusia, CODEPEH) updates the recommendations that are considered important for the future development of early hearing detection and intervention (EDHI) systems in the following points: 1. Screening protocols: Separate protocols are recommended for NICU (Neonatal ...

We report the cases of three patients seen at the Otolaryngology Department after presenting a laryngeal carcinoma that required total laryngectomy, followed by trachaeo-oesophageal puncture and Provox voice prosthesis positioning. In all cases the growth of granulomatous tissue totally incarcerated the prosthesis in the trachaeo-oesophageal mucosa. In two of the cases the prosthesis could be extracted by oesophagoscopy and a new prosthesis was positioned in the same surgery. In the other case an external approach was performed using a peristomal wound to extract the prosthesis. In this case a new trachaeo-oesophageal puncture was performed 3 months after the extraction.

We present the case of a 61-year-old man in whom the magnetic resonance imaging performed prior to left cochlear implant surgery revealed a lesion in the right internal auditory canal resembling a vestibular Schwannoma. Left cochlear implant surgery was performed and three months later, the right internal auditory canal lesion was removed. The final histopathological analysis revealed it to be a meningioma.

CONCLUSIONS: The literature reviewed shows that occupational exposure to formaldehyde may be associated with an increased risk of nasopharyngeal and hypopharyngeal cancer. The results regarding other occupational exposures are not consistent, therefore additional studies with more statistical power and better design are needed to ascertain if occupation is really a relevant risk factor for these types of cancer.

Schwannomas are tumours which grow from the most external layer of the peripheral nerves. They can appear in any part of the human anatomy, but are especially common in the head and neck region. Within this area, the most frequent location is the VIII cranial nerve (80%). Treatment is surgical.

CONCLUSIONS: FNA has good sensitivity in clinical suspicion of thyroid papillary carcinoma, although it should be complemented by extemporaneous biopsy due to its better specificity. In the case of follicular carcinoma there is a tendency towards conservative treatment, without carrying out extemporaneous biopsy previously and with reintervention in case of definitive malignancy.

Several concurrent circumstances have created an impression through the media of a presumed lack of specialists in Spain, which has one of the highest densities of doctors per population in the world: simultaneous creation of jobs in many newly built hospitals; accepting garbage contracts (for months, half/thirds of days, shifts) in relevant hospitals rather than moving to unattractive positions; full dedication to the field of public or private healthcare rather than matching them; bad public healthcare working conditions (low wages, excessive healthcare pressure, lack of respect from the public and from healthcare managers, shifts, scarce professional promotion, difficult family reunification); decreased mobility...

CONCLUSIONS: Use of LigaSure((R)) decreased bleeding and surgical time in CBTs. Lesions of the artery are mainly caused by infiltration or by muscular hypotrophy of the artery, which frequently requires vascular reconstruction. Nerve injury continues to be an important postoperative complication, which may result in a reduction in the quality of life for the patient. The rates of nerve injury as a result of surgery were unchanged.

We report the case of a 35-year-old woman presenting hamartoma in the posterosuperior part of the right nasal cavity. Complete surgical resection by functional endoscopic sinus surgery was the treatment of choice to remove it completely. Hamartomas must be included in the differential diagnostic of unilateral nasal masses, along with inverted papillomas and malignant tumours.

Primary ciliary dyskinesia is a genetically inherited syndrome characterized by cilia immotility or dysmotility. Deficiency in mucociliary clearance produces chronic respiratory infections since birth, male sterility by spermatozoid immotility and situs inversus in 40-50% of patients (Kartagener's syndrome). Diagnosis is made by analyzing cilia motility with high-speed digital video and ciliar ultrastructure. The wide distribution and functions of the cilia in the body mean that this dysfunction can generate other ciliopathies apart from primary ciliary dyskinesia.

CONCLUSIONS: Despite the great development and new surgical techniques, CSF fistulae are still a frequent complication after VS surgery. There might be other aetiological factors such as intracranial pressure. There was no significant relationship between CSF fistula and BMI. Adequate management and early conservative measures led to reduced reintervention rates of less than 5% in all patients.

We present the cases of 6 patients (aged between 26 and 62 years) with asymmetric bone marrow distribution or trapped mucous fluid secretions in the petrous apex, collected over a 6-year period. Diagnosis was made with the use of CT scans and MRI. All of the patients were referred for skull base surgery. In all cases a non-pathologic asymmetry was diagnosed in the petrous apex. Certain non-pathologic conditions of the petrous apex must be treated expectantly without any surgery.

Optical coherence tomography is a novel imaging technique providing high-resolution bidimensional images of tissue microstructures. Several studies have been published on the use of this technique in different fields of medicine, particularly ophthalmology. There are very few studies in the field of otolaryngology. This paper presents various applications of optical coherence tomography in the different sub-specialties of otolaryngology, as well as the benefits of this technique over traditional diagnostic methods.

CONCLUSIONS: Staging of otitis allows comparisons of the level of involvement and treatment recommendations: aspiration is advisable in stages II, III and IV; placement of a wick inside the canal is necessary in stages III and IV. Ototopical drops seem to be needed in every case and systemic antibiotics almost only in stage IV. Acidifying agents and topic glycerine vehicles access the canal better than aqueous solutions and would be the first choice in advanced stages.

CONCLUSIONS: The response to chemotherapy in oral cavity tumours was greater in patients with stage III and in those receiving bleomycin. Chemotherapy may improve survival in palliative cases. In neo-adjuvant cases, no great influence was seen in the conversion of doubtful cases into candidates for surgery.

Infrequent variants of extrapulmonary tuberculosis pose a challenging public health problem because of the delay in their diagnosis and treatment. Joint involvement is usually encountered in major joints (hip, elbow, knee and ankle). Tuberculous involvement of the temporomandibular joint is extremely uncommon but must be considered in patients with pain, inflammation or joint stiffness not resolved with conventional treatment.

We report a case of mucocutaneous Leishmaniasis, an uncommon illness in our area. Leishmaniasis covers a group of diseases caused by protozoa of the genus Leishmania with several pathogenic species transmitted by Phlebotomus mosquitoes. Leishmania braziliensis is endemic in parts of South America and is responsible for the mucocutaneous Leishmaniasis reported here. The initial lesion is cutaneous and appears as an ulcer on arms or legs, leaving a scar. In some untreated cases, a late phase may affect the ENT area with hard to treat chronic destructive and mutilating lesions.

Several histopathological modalities of tumours can grow in the rhinopharynx. Solitary extramedullary plasmocytoma (SEP) is a low frequent variety of low-grade lymphoma. The aim of this work is to present a new case of SEP localized in rhinopharynx and to perform a review of diagnostic and therapeutic protocols of these neoplasias. They must be taken into account in the differential diagnosis of nasopharyngeal tumours.

CONCLUSIONS: Most of the fistulas can be managed with conservative treatment. Pectoralis major myocutanenous flap is appropriate when conservative treatment has failed. In small fistulas, nasogastric or gastrostomy tube for feeding can be successfully managed in the ambulatory follow up. The cost-benefit relation must be better analyzed.

CONCLUSIONS: Chronic maxillary sinusitis showing FBMS is rare and it must be suspected with a prior history of dental procedures. The most frequent source of FBMS is material of odontogenic origin, and non-odontogenic origin secondary to an external injury in an accident or assault is much more unusual. Salivary gland tumours represent between 2 % and 6.5 %, approximately, of all head and neck tumours. The aim of this paper was to identify the frequency of minor salivary gland tumours among patients in the Oral Medicine Clinic of the Federal University of Paraná during the period from 1997 to 2007. A retrospective study was conducted on 1,923 histopathological analyses of oral lesions. Fourteen cases of salivary gland tumours were found, of which 7 were benign and 7 malignant. The lesions were localized mainly in the palate (71.5 %). By histological type, 50 % of the lesions were characterized as pleomorphic adenoma, 28.6 % mucoepidermoid carcinoma, 14.3 % cystic adenoid carcinoma and 7...

We present the case of a 65-year-old woman with pain and tinnitus in the left ear lasting for one week. Physical examination in the emergency room showed numerous dipteran larvae totally occupying the external auditory canal. After the diagnosis of otic myiasis, the larvae were removed and the patient was discharged after 24 hours of medical observation. The diagnosis of otic myiasis is by means of direct visualization of the larvae in the ear cavities and treatment includes the removal of larvae from the ear. Prognosis is generally good.

CONCLUSIONS: At our centre, myringoplasty achieves anatomical (78%) and functional (56%) success comparable to the results described in the literature. We consider medium-term follow-up to be of the utmost importance because of reperforation phenomena, which may occur as much as 2 years or more after surgery.

CONCLUSIONS: Microtia may be directly or indirectly associated with the early onset of labour. Future studies should include long-term follow up of the patients in order to detect possible anomalies of the oculo-auriculo-vertebral spectrum. It is also important to take anthropometric measurements to increase the likelihood of detecting cases of grade I microtia and mid-face hypoplasia, and to define with greater accuracy whether isolated microtia is the mildest form of the oculo-auriculo-vertebral syndrome.

This study provides an overview of the natural history of these tumours and the main therapeutic options: observation, surgery and radiosurgery, with comments on their indications, advantages and disadvantages. Due to the completely different approach and peculiar features of bilateral VS in patients with type II neurofibromatosis, these are excluded.

We report a case of desmoplastic small round cell tumour of the sub-maxillary gland, with an evolution of 8 months, affecting a 36 year old male. He suffered chronic lymphatic leukaemia five years ago and needed a bone marrow transplant. There was a 4x3 cm tumour. There were no signs of malignancy on the CT scan. A right sub-maxillectomy was performed. The pathology analysis gave a diagnosis of DSRCT. Post-surgical radiotherapy was given. The definitive diagnosis was reached using immunohistochemical techniques, such as polyphenotypical differentiation (epithelial, mesenchymal and neural), and by demonstration of translocation (11;22)(p13;q12). Sub-maxillary location is very rare.

The development of knowledge of paragangliomas has been inextricably linked to the development of all fields of medicine. Throughout history, both knowledge of diseases and technical improvements in diagnosis have served to understand these tumors, although unanswered questions remain. From the XVIII century to the XXI century events sometimes unexpected and other times intensely sought have occurred that have helped to identify the characteristics of these tumors. These events have ranged from description of paragangliomas to their genetic diagnosis. The incidence of paragangliomas is not high. Nevertheless, the variability of these tumors in terms of their localization, forms of presentation, possibilit...

Paragangliomas are tumors that arise in the extraadrenal paraganglia and result from migration of neural crest cells during embryonic development. Based on their anatomical distribution, innervation and microscopic structure, these tumors can be classified into interrelated families: branchiomeric paraganglia (related to the branchial clefts and arches), intravagal, aortic-sympathetic and visceral-autonomic. Head and neck paragangliomas belong mainly to the first two of these families. The present article is divided into two parts. The first part reviews the embryological origin of these tumors. Special emphasis is placed on the process of neurulation or neural...

Paragangliomas arise from the extra-adrenal paraganglion system. Histologically, paragangliomas are usually easy to diagnose, with well-defined characteristics. These lesions are clearly delimited and highly vascular and are composed of cell balls (Zellballen) separated by thin fibrous septa. These cell balls are composed of two types of cells: chief cells and sustentacular cells. Other, less frequent patterns, which are nearly always focal, can also be found and hamper diagnosis: angiomatoid, fusocellular and clear cell. Some paragangliomas show intense fibrosis, which can compress and distort the cell balls, giving rise to a pseudoinfiltrative appearance (sclerosing paragangliomas). With...

Paragangliomas and pheochromocytomas are neuroendocrine tumors arising in the extraadrenal and adrenal medulla, respectively. These tumors appear in certain familial syndromes, such as multiple endocrine neoplasia types 1 and 2, Von Hippel Lindau disease, neurofibromatosis type 1 and familial paraganglioma syndromes. The latter syndromes show a strong association with germline mutations of succinate dehydrogenase subunits, such as SDHB, SDHC and SDHD. This enzyme complex is related to tumorigenesis through mechanisms involved in cell proliferation, apoptosis, and alterations in oxygen sensing function, although none of these factors has been clearly identified as a cause of tumoral development. In the last few years, several ...

The terms glomus or chemodectoma arose with knowledge of the histological structure of these tumors; their paraganglionic cells, together with autonomic ganglion cells, form the paraganglia, and consequently the most appropriate term to describe these tumors is paraganglioma. Classification of these tumors varies according to the parameter chosen: patient age, secretory capacity or biochemical behavior, whether the tumor is isolated or syndromic, or benign or malignant, etc. Moreover, there are other classifications based on other features such as localization, extension, the recommended surgical approach, immunohistochemical characteristics of the tumor, whether the tumor is hereditary, etc. None of these c...

Head and neck paragangliomas are slow-growing tumors and the initial symptoms are sometimes non-specific, often hampering and delaying diagnosis. These tumors may be asymptomatic, even when they have reached a considerable size. Symptomatology is highly varied in terms of anatomical location, stage (degree of invasion, local involvement and the presence of metastasis) and catecholamine secretion (pheochromocytoma), which not only produces a series of systemic manifestations but also serves as a guide to the search for specific genetic diseases, of which these tumors may be a component. Thus, in addition to identifying their anatomical location, excess catecholamine production must ...

Cross-sectional imaging techniques [magnetic resonance imaging (MRI) and computed tomography (CT)] are essential in the presurgical evaluation of paragangliomas. These techniques provide data on tumoral hypervascularity, tumoral margins, invasion of neighboring structures, tumoral multifocality and the presence of metastasis. These imaging techniques are also essential for sequential volume assessment in non-surgical cases, postoperative evaluation and screening of familial forms. MRI is the technique of choice in paraganglioma assessment due to high contrast resolution among tissues, optimizing visualization of tumoral delimitation and the degree of local invasion. In addition, with the new dynamic MR angiogra...

The present article aims to describe the current role of interventional neuroradiology in the diagnosis and treatment of head and neck paragangliomas, based on our experience and a literature review. The cellular polymorphism of head and neck paragangliomas confers these tumors with their characteristics hypervascularization (angioarchitecture) with characteristic arteriographic signs and hemorrhage that justify presurgical embolization. The main indications of digital angiography in head and neck paragangliomas are to confirm diagnosis and identify functional involvement of the large cervical vessels, as well as their possible vascular replacement. Head and neck paragangliomas are neuroendocrine tumors that express somatostatin type 2 receptors and can consequently be visualized through nuclear imaging techniques, using radionuclide-labelled somatostatin analogs, specifically 111In-pentetreotide. 111In-pentetreotide scintigraphy is a safe and non-invasive technique that can be used to explore the entire body; thus, multifocal paragangliomas as well as malignant paragangliomas with local and distant metastasis can be detected. Because this technique is functional, it is highly useful to confirm recurrence or residual tumors, as well as to follow-up patients undergoing surgery. Paragangliomas can be familial and consequently this technique can be used for screening ...

Head and neck paragangliomas are infrequent tumors that, for various reasons, may pose a particular challenge to anesthesiologists. It is especially important to rule out vasoactive amine secretion by the tumor and, when this occurs, to establish appropriate medical treatment before surgery as well as to maintain hemodynamic stability in the perioperative period. Balanced anesthetic techniques that allow early awakening and neurological evaluation are recommended. Bleeding, which can be profuse, must be closely monitored. In the postoperative period, special attention must be paid to the risk of bronchial aspiration and the development of compressive cervical hematoma, as well as t...

The present article reviews the current panorama of the surgical treatment of carotid paragangliomas. The article begins by discussing surgical indications and preoperative aspects (Shamblins classification, the value of genetic study, informed consent, the utility of embolization, etc). Intraoperative aspects (surgical team) and features of the surgical technique (the patients position, incision and exposition, vessel control and identification of nerves, subadventitial or periadventitial tumoral excision, the need for arterial resection and reconstruction, etc.) are discussed in depth. Postoperative aspects, morbidity (neurological and vascular) and mortality, as well as how to follow-up these patients, are discussed at the end of...

Vagal paragangliomas are much less frequent than carotid and jugulo-tympanic paragangliomas and, like these other forms, can be sporadic or familial. The familial forms usually occur as isolated tumors or within syndromic entities such as multiple endocrine neoplasia type IIA-B, Von Hippel- Lindau syndrome and neurofibromatosis type 1. The genes causing the disease are located in three loci: SDHD (11q23), SDHC (1q21) and SDHB (1p36.1p35). Approximately one-third of apparently sporadic paragangliomas also show germline SDH mutations. In familial forms, the rate of multicentric tumors is up to 30-40%. Vagal paragangliomas arise from arise from paraganglia associated with the vagus nerve. Lower cranial nerve...

Today's imaging studies accurately delineate the extent of glomus tumors of the temporal bone. Microsurgical techniques allow total removal of even the largest tumors with acceptable morbidity. While surgical management is the principal treatment of glomus tumors, stereotactic fractionated radiation therapy may be used as an alternative treatment when there is a risk of cranial neuropathy with surgical management. We should be aware of the full range of management options and base treatment choice on the age and medical condition of the patient and location and size of the tumor.

The most common sequelae in the treatment of head and neck chemodectomas are cranial nerve lesions; of these, the most frequently affected are the trigeminal, facial, glossopharyngeal, vagus and hypoglossus nerves. The lesions of the lower cranial nerves (IX, X, XI and XII) affect the oral and pharyngeal phases of swallowing and frequently lead to aspiration. The frequency with which these nerves are affected varies according to several factors, being 30% in glomus vagale tumors and 20% in carotid tumors. If the V and VII cranial nerves are affected, defects in oral preparation, bolus transport and drooling may occur. Lesions of the XI and X cranial nerves are associated with de...

Paragangliomas are rare tumors and most are benign: less than 10% show criteria for malignancy. In the head and neck, the main locations are carotid, jugulo-tympanic and vagal. Surgery continues to be the mainstay of treatment but both external beam radiotherapy (EBRT) and stereotactic radiosurgery (SRS) have been shown to be effective therapeutic alternatives. EBRT requires large volumes to cover the lesion, while radiosurgery allows administration of a high dose in a smaller volume, with high precision, which should reduce the number of long-term complications. The results in terms of local control with both treatment modalities are good, as reflected in published series. The role of chemotherapy is restricted to...

A closed separation between the anterior cranial fossa ant the sinus is necessary after the resection. The reconstruction was performed using a pedicled pericranial flap and titanium mesh in most of the cases.

The clinical profile of the patient type is a middle-aged individual from rural areas, with a 1:1 male/female ratio, asymptomatic but affected by growing laterocervical lymphadenopathies, often appearing in summer, and notable on palpation, without any demonstrable entranceway. However, the profile is subject to variable patterns. The suspected diagnosis was confirmed on the basis of serological determinations (93 %). Two groups were observed: one formed by those with less evident clinical manifestations and an early ganglionar stage that mostly evolved well with specific antibiotic therapy, and another one with major clinical manifestations or advanced stage in which surgical procedures were required although subsequent evolution was good.
We report our experience in eight patients with oropharyngeal tularaemia with cervical adenopathies. The mean age was 45 years (11-76), with 5 males. All suffered oropharyngeal pain, fever and asthenia. The time elapsed between the onset of symptoms and attending our clinic varied from 8 to 60 days. Mean duration of oropharyngeal symptoms was 11.7 days, and adenopathies appeared in the first three days. Resolution of adenopathies occurred at 2 to 3.5 months. Three cases needed surgical drainage and two fistulized spontaneously. Seven patients were treated with ciprofloxacin and one with erythromycin. A correct and precocious antibiotic therapy, and sometimes surgical drainage, could accelerate the healing process.

Following a dramatic fall in tuberculosis, the last two decades have seen not only an increase in its overall frequency but also the extrapulmonary forms, including head and neck tuberculosis, have increased in proportion. Despite this rise, head and neck tuberculosis is still very rare, except for cervical adenitis. This fact and the non-specific nature of its presentation explain that tuberculosis is often an unexpected finding in biopsy results. Notwithstanding the advances in the diagnosis of tuberculosis, with ever speedier and more reliable tests such as polymerase chain reaction, its detection almost always requires the performance of invasive procedures such as punctures or biopsies. On the other hand, the diagnostic gold standard is still the ...

We present a case of frontal mucocele secondary to ostium drainage obstruction due to herniated orbital content. This entailed difficulties for intranasal surgical resolution so a combined approach was used. The different surgical options for treating such cases are discussed.

Chondromyxoid fibroma is an infrequent bone tumour in the craniofacial bones and exceptional in the paranasal sinuses. This unusual location, its non-specific clinical manifestation and aggressive behaviour with local destruction and a high recurrence rate can complicate precise diagnosis and treatment.
