The validity of biochemical markers in metabolic bone disease in preterm infants: a systematic review

Conclusion:  Since there is no gold standard for the early diagnosis of MBD it is difficult to establish. There is insufficient evidence that any of the frequently‐used serum measurements are valid biochemical markers of MBD in preterm infants. Increased urinary calcium concentration may be a valid biochemical marker, but more research is necessary to confirm this.© 2012 The Author(s)/Acta Pædiatrica © 2012 Foundation Acta Pædiatrica (Source: Acta Paediatrica)

Conclusions:  A statistically significant association was observed between adenovirus and childhood intussusception. HHV‐6 was a common finding and occurred concomitantly with other viruses. A simultaneous infection with HHV‐6 and adenovirus carried the highest risk for intussusception.© 2012 The Author(s)/Acta Pædiatrica © 2012 Foundation Acta Pædiatrica (Source: Acta Paediatrica)

Conclusion:  Maternal and foetal characteristics influence risk of BPBP, but not extent of injury or reversibility of injury. Due to high risk of permanent disability and modest risk of low Apgar or pH among newborns with BPBP, the recommendation of prompt delivery may need to be re‐evaluated.© 2012 The Author(s)/Acta Pædiatrica © 2012 Foundation Acta Pædiatrica (Source: Acta Paediatrica)

Conclusion:  A mild iron deficiency was found in stunted Nepalese children. The serum ferritin has been identified as a sensitive marker for measurement of iron status in surveyed children. A deficiency of serum protein, serum folate and vitamin B12 were also found in the undernourished Nepalese children.© 2012 The Author(s)/Acta PÃdiatrica © 2012 Foundation Acta PÃdiatrica (Source: Acta Paediatrica)

Conclusions:  Latin American NICUs differ from those in Northern Europe/English‐speaking countries. More deaths are accompanied by full life support and CPR. DNR orders are rare. Withdrawal of life support is virtually non‐existent. Latin American’s doctors are more likely to make decisions without the objections of family about withholding life‐sustaining treatment. (Source: Acta Paediatrica)

Difference in mother–child interaction between preterm‐ and term‐born preschoolers with and without disabilities

Conclusion:  Five years after birth, mother–child interaction of very premature children and their mothers compared unfavourably with term children and their mothers. Mothers with sociodemographic disadvantages, raising a preterm child with severe disabilities, struggle most with giving adequate sensitive support for the autonomy development of their child. Focused specialized support for these at risk groups is warranted. (Source: Acta Paediatrica)

Conclusion:  Overall, the group‐analysis showed no statistical significant difference in QTc duration induced by domperidone. However, 2/45 (4.4%) infants had a prolonged QTc interval (>460 msec) induced by domperidone. As a consequence, QTc measurement should be recommended in routine in infants when domperidone is started (Source: Acta Paediatrica)

Conclusion:  Voiding pattern in the pre‐terms differed in many ways from that of the full‐term newborns. Frequent interrupted and incomplete voiding pattern in the preterm newborns indicates a disrupted coordination of the detrusor‐sphincter and a delayed maturation of the neural micturition center.
Conclusions:  Social capital in the family, school and neighbourhood matters for children’s health and well‐being and the contributions from each context seem to be additive. Besides the family context, investments for improving child health should primarily be in the school, focusing on social relations and on creating safe and cohesive school environments. Neighbourhood social capital is also of importance and so must be taken into consideration when planning child health promotion interventions. (Source: Acta Paediatrica)

AbstractAim:  Evaluation of long term outcome of extremely low birth weight (ELBW) teenagers born in Iceland in 1991‐95.Method:  Participants, 30 of 35 ELBW survivors and 30 full term control teenagers (14‐19 years), were assessed for disabilities, health problems and learning difficulties. Results of national standardized tests in mathematics and Icelandic language were compared to results of neurodevelopmental assessment at five years of age. Results:  A quarter of the ELBW teenagers had disabilities. All were initially diagnosed with neurodevelopmental disorders early in life and neurosensory and/or intellectual disabilities were confirmed later in childhood. Chronic lung disorders, neurological problems and psychiatric disorders were most common health problems. Growth paramet...

Conclusions:  Anti‐Xa concentrations fluctuate during maintenance enoxaparin therapy, with therapeutic levels being achieved only sporadically in young infants. Despite this, enoxaparin appears efficacious in thrombosis resolution. Further studies on the impact of stringent control of concentrations on outcomes in this population are warranted.
Health outcomes among Swedish children: the role of social capital in the family, school, and neighbourhood

Conclusions:  Social capital in the family, school, and neighbourhood matters for children’s health and well‐being, and the contributions from each context seem to be additive. Besides the family context, investments for improving child health should primarily be in the school, focusing on social relations and on creating safe and cohesive school environments. Neighbourhood social capital is also of importance and so must be taken into consideration when planning child health promotion interventions. (Source: Acta Paediatrica)

Conclusion:  Cardiac evaluation, including 2DSTE, detects frequent alterations in myocardial systolic function in children diagnosed with PWS, whose conventional echocardiographic findings did not indicate ventricular systolic dysfunction. Because cardiovascular morbidity and mortality is substantial in PWS, especially adults, we emphasize the need for cardiac assessment in PWS. (Source: Acta Paediatrica)

Discussion:  Only 43.8% of the units provided therapeutic hypothermia. Even in centres where hypothermia was offered, adherence to the standard protocols was extremely poor. To secure the safety and efficacy, further promotion of the standard cooling protocols is required; an efficient cooling centre network has to be established by optimising the work forth distribution and transportation system.

Conclusions:  In the absence of a genetic comorbidity, children with congenital diaphragmatic hernia have a favorable cognitive outcome, but motor outcome may be affected. This needs to be taken into account for parental counseling. Further, long‐term neurodevelopmental assessment in children with congenital diaphragmatic hernia is important to provide early therapeutic interventions. (Source: Acta Paediatrica)

Conclusions:  Criticisms of the expense of NICU care find little support in these data. Rather, NICU care is remarkably well targeted to patients who will survive, particularly when contrasted with care in adult ICUs. We continue to search for better prognostic tools for individual infants. (Source: Acta Paediatrica)

Conclusions:  Within the size limits of our study cohort, the ABCA3 missense mutation E292V had no remarkable effect on pulmonary outcome in VLBW infants. Present results do not rule out the possibility that E292V phenotype is associated with minor difference in the morbidity. (Source: Acta Paediatrica)

AbstractClinical algorithms can be powerful tools for identification of sick newborns at risk for neonatal mortality. Several studies have evaluated clinical signs for newborns age 0 to 60 days to identify severe illness; however, few studies have focused specifically on the most vulnerable time period for neonatal death, the first week of life. Therefore, we reviewed the studies that evaluated clinical signs in newborns 0 to 60 days, focusing on infants 0 to <7days. Based on a comparison of relevant studies, we then identified the common, important clinical signs shown to be useful for identification of at risk newborns by health workers in community based and low‐resource settings. We concluded that further work is urgently needed to develop a clinical algorithm for widespread valid...

AbstractFollowing a perinatal hypoxic‐ischaemic insult, term infants commonly develop cardiovascular dysfunction. Troponin‐T, troponin‐I and brain natriuretic peptide are sensitive indicators of myocardial compromise. The longterm effects of cardiovascular dysfunction on neurodevelopmental outcome following perinatal hypoxic‐ischaemia remains controversial. Follow‐up studies are warranted to ensure optimal cardiac function in adulthood.Cardiac biomarkers may improve the diagnosis of myocardial injury, help guide management, estimate mortality risk and may also aid in longterm neurodevelopmental outcome prediction following neonatal hypoxic‐ischaemia.
Conclusions:  In this study, cognitive and motor function in preterm children with early postnatally acquired HCMV infection transmitted via breast milk was within the normal range. However, the findings suggest that their outcome is poorer than outcome in preterm children without HCMV infection. These findings need to be replicated in larger scale studies.
Conclusions:  Respiratory morbidity associates with high TF in lungs and plasma. In sick newborn infants upregulation of TF may be mediated by IL‐6 and IL‐8. High TF and proinflammatory cytokines may together participate in the pathogenesis of pulmonary and extrapulmonary injury in preterm infants through proinflammatory mechanisms.
Permanent and Transient Congenital Hypothyroidism in preterm infants

Conclusions:  The incidence of permanent CHT in preterm infants is similar to term infants. Profound abnormalities of thyroid function can occur in preterm babies with transient hypothyroidism but both categories of hypothyroidism can be detected by a ‘once‐only’ TSH screening strategy with a relatively low cut‐off.
Conclusions:  The differences in BMD between patients with Duchenne and Becker as well as between different bone measurement sites demonstrated in the present study point out the importance of preserving muscle strength and motor function in patients with muscular dystrophy. Moreover; it highlights the value of performing region‐specific analysis of the bone quality in these patients. (Source: Acta Paediatrica)

Conclusion:  Early mother‐infant skin‐to‐skin contact immediately after Caesarean Section should be promoted until the occurrence of the first breastfeed. If the mother is unable to provide skin‐to‐skin contact immediately after birth, the father‐infant skin‐to‐skin contact is a valuable alternative since it enhances paternal interaction.
Multiple psychosomatic symptoms can indicate child physical abuse ‐ results from a study of Swedish school children

Conclusion:  This study shows that multiple psychosomatic symptoms are associated to child physical abuse. Health professionals therefore need to pay special attention to school children who complain about the co‐occurrence psychosomatic symptoms and pattern of symptoms which could not be easily explained by other causes.
