Impact of childhood life events and trauma on the course of depressive and anxiety disorders

Conclusion:  Childhood trauma, but not childhood life events, was associated with an increased persistence of comorbidity and chronicity in adults with anxiety and/or depressive disorders. More unfavourable clinical characteristics at baseline mediate the relationship between childhood trauma and a poorer course of depressive and anxiety disorders.
Conclusion:  Clinicians need to be aware of premenstrual symptom aggravation in a large minority of women with schizophrenia. Treatment strategies will depend on the nature of the symptoms that are exacerbated. Optimal treatment needs to be adjusted to the individual woman.
Moderators of the synchrony of change between decreasing depression severity and disability

Conclusion:  Strong synchrony of change exists between depression severity and disability. Perhaps, because of the strength of this synchrony, few contextual characteristics moderated the association. Clinicians should be aware of the risk of slower or incomplete functional recovery in older people and those without a job or those experiencing low work stress.
Pathophysiology of hypercortisolism in depression: pituitary and adrenal responses to low glucocorticoid feedback

Conclusion:  Classical feed‐forward overdrive and impaired feedback theories of hypercortisolemia in depression were not supported. Depressive hypercortisolemia may result from alternative pathophysiological mechanisms involving irregular basal hypersecretion of cortisol, associated with adrenal enlargement, possibly through splanchnic sympathetic activation of the adrenal cortex.
Conclusion:  Meta‐analytic findings provide evidence for emotion processing and theory of mind deficits in remitted bipolar patients. However, it is not yet clear whether these areas of impairment are related to neurocognitive dysfunctions or to medication effects. The results are discussed with regard to targets for future neuropsychological research on BDs. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  Glutamate seems to be relevant in the molecular processes involved in FM and STD. It also opens the door for Proton MRS (1H‐MRS) in STD and suggests that reducing glutamatergic activity through pharmacological treatment could improve the outcome of patients with FM and STD.
Conclusion:  The HDRS Scale provides unreliable primary data. Low effect sizes can be expected because of the scale’s low precision and low sensitivity to change, particularly in mild and moderate depression. The conclusion of the meta‐analysis by Fournier et al. is therefore unfounded. The clinical value of antidepressants cannot be evaluated from unreliable data. It is urgent that better measurement techniques for depression severity are developed. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  Basal ganglia regions like the right globus pallidus, the right putamen, and the nucleus caudatus are structurally affected in children with ADHD. These changes and alterations in limbic regions like ACC and amygdala are more pronounced in non‐treated populations and seem to diminish over time from child to adulthood. Treatment seems to have positive effects on brain structure. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  Augmentation with a second antipsychotic is modestly beneficial in patients not responding fully to clozapine. Tolerability seems not to be adversely affected, at least in the short term. Longer studies do not appear to increase the probability of showing positive effects for augmentation.
Conclusions:  The present study showed that paranoid symptoms and social phobia share similarities in cognitive profile and inhibited temperament. Avoidance appears to be important in the development of social phobia, whereas cannabis use and traumatic experiences may drive paranoid thinking in vulnerable individuals.
Conclusion:  Of psychiatric patients with depression, one‐fifth were granted a disability pension within 5 years. Future disability pension can be predicted by baseline older age, personality factors, functional disability, lack of vocational education and comorbid somatic disorders. Longitudinally, accumulation of time spent depressed appears decisive for pensioning. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  The traditional 1‐year cut‐point validly identifies increasing BRD severity; DSM’s 2‐month cut‐point does not.
Conclusion:  GABRA6 emerges as a candidate gene involved in the variability of HA. The effect of a significant gene–gene interaction between the SLC6A4 and BDNF genes on HA could explain part of the genetic basis underlying anxiety‐related traits.

Conclusion:  Psychological problems are frequent in refugee children, but the extents are reduced over time in exile. Traumatic experience before arrival is most important for the short‐term reaction of the children while aspects of life in exile are important for the children’s ability to recover from early traumatization.
Conclusion:  No single approach to decision making emerged from the analysis. Approaches to decision making were influenced by the level of clinical experience and external pressures, such as time and treatment availability. Findings were consistent with dual‐processing theory of decision making. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  Most of the variables tested in our study but one did not point at any advances of adding music therapy to pharmacological treatment. The finding of improvement in Brief Psychiatric Rating Scale could be an indicator of music therapy as a useful adjunct to pharmacotherapy during an in‐patient hospital stay for the few patients amongst those suffering acute psychosis that accept to participate in music therapy.
Conclusion:  Drug treatments are widely used for people with PD despite the relatively weak evidence base. Both the type of personality problem and the context in which treatment is delivered appear to have an impact on whether drug treatments are prescribed.
Conclusion:  As in schizophrenia and autism, eye movement recordings during more complex tasks might help to better characterize the cognitive deficits associated with OCD. Such recordings may reveal specific OCD‐related deficits that could be used as reliable diagnostic and/or classification tools. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  Taken together, the results of this study suggest that PTSD is not a chronic disorder for the majority of borderline patients. They also suggest a strong relationship between sexual adversity and the course of PTSD among patients with BPD. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  Evidence supports that MBSR improves mental health and MBCT prevents depressive relapse. Future RCTs should apply optimal design including active treatment for comparison, properly trained instructors and at least one‐year follow‐up. Future research should primarily tackle the question of whether mindfulness itself is a decisive ingredient by controlling against other active control conditions or true treatments.
A latent class analysis of psychosis‐like experiences in the New Zealand Mental Health Survey

Conclusion:  The presence of sub‐threshold events and variation in the number of times a ‘symptom’ is experienced suggest a psychosis continuum. However, the latent classes labelled ‘hallucinatory’ and ‘psychotic’ differ markedly in symptomatology, which suggests some form of discrete clustering.
‘D’ for depression: any role for vitamin D?

Conclusion:  There is currently insufficient evidence to argue strongly for vitamin D supplementation in patients with depression, but such a strategy is worthy of consideration in depressed patients whose lifestyle and geographical residence may indicate a risk of vitamin D insufficiency – or where low vitamin D levels have been quantified.
Conclusion:  Prefrontal hyperactivation during high planning demands is not a trait characteristic, but a state characteristic of MDD without comorbid anxiety, occurring independent of SSRI use. Disturbances in planning or the related activation are probably not a feature of anxiety disorders with or without comorbid MDD, supporting the current distinction between anxiety disorders and depression.
Conclusion:  Acute tryptophan depletion continues to provide a research tool for investigating the relevance of serotonin to depression onset. There is limited evidence that tryptophan loading is effective as a treatment for depression through its action of increasing serotonin production. Most clinical studies are dated, involve small sample sizes and/or were not placebo controlled. The development of the new serotonin reuptake inhibitor drugs seemingly signalled an end to pursuing such means of promoting increased serotonin as a treatment for depression. The evidence for tyrosine loading promoting catecholamine production as a possible treatment for depression appears even less promising, and depletion studies less informative.
Conclusion: More research is required before omega‐3 supplementation can be firmly recommended as an effective treatment for mood disorders. Whereas increased omega‐3 FA intake may alleviate depressive symptoms, there is little evidence of any benefit for mania. (Source: Acta Psychiatrica Scandinavica)

Conclusion:  To adequately treat patients with FEP, clinicians must emphasize treatment of the substance abuse disorder, as well as the psychotic illness.
Conclusion:  GPs have a key role as regards initiating treatment with APs and ADs in Norway, while psychiatrists’ influence seems limited, particularly among older patients. Efforts for quality improvement of mental health care need to involve primary health care. In addition, an increased focus from psychiatrists towards the increasingly ageing part of the population seems requisite. (Source: Acta Psychiatrica Scandinavica)
