            <description>We describe the case of a 44-year-old homosexual man diagnosed with HIV infection and visceral leishmaniasis. He presented nodules on the dorsum of the hands. Histological study of one of the nodules revealed necrobiotic palisading granulomas with abundant Leishmania amastigotes within the histiocytes and in the adjacent extracellular space. Tissue and peripheral blood cultures were positive for Leishmania infantum, zymodeme MON-24. A biopsy of healthy skin did not reveal the presence of Leishmania. A diagnosis of rheumatoid nodulosis with Leishmania was made and treatment was started with intravenous liposomal amphotericin, leading to slight improvement.     Vesicants are a group of chemicals used in warfare. The most representative agent is yperite, also known as mustard gas. The blisters that appeared on those exposed to yperite during combat in the First World War are responsible for the current name-vesicants-for this group of chemicals. Their affects are produced mainly through localized action of liquid or vapor forms on the skin, eyes, and respiratory tract. However, the high absorption of the liquid form through the skin or the vapor form on inhalation may cause substantial systemic effects. Here we analyze these effects, treatment of intoxication, and long-term sequelae, drawing on our experience and a review of the literature.

    For the past 40 years, the Clark classification of cutaneous melanoma has been accepted and used by the vast majority of dermatologists and pathologists throughout the world. However,after careful rereading of the most relevant articles by Clark and his collaborators, we can affirm that the classification was only ever of passing validity. Today, the possible morphological differences between one case of cutaneous melanoma and another are of no proven prognostic implication. In addition, the morphological differences that can be found are much more closely related to the different localization that to the tumor itself.

    Lichen sclerosus is a chronic inflammatory mucocutaneous disease that is highly bothersome for men and women of all ages. The exact etiology is unknown, although genetic and autoimmune factors, as well as infections, have been implicated in its pathogenesis. First-line treatment is highly potent topical corticosteroid therapy for short periods. Surgery is reserved for cases of phimosis, urethral stenosis, synechiae, and squamous cell carcinoma.

            <description>CONCLUSION: Women became sensitized at a younger age than men, and the frequency of positive results in the patch tests increased with age, reaching a maximum at between 60 and 69 years of age, when the greatest rate of sensitization occurred. Comparison of our results with other Spanish data showed a progressive and constant increase in sensitization to nickel sulfate, fragrance blends, balsam of Peru, and rosin, and a decrease in sensitization to potassium dichromate. The inclusion of new allergens such as palladium chloride, diallyl disulfide, and p-toluene sulfonamide formaldehyde improved the sensitivity of the standard battery in the detection of contact sensitization. We therefore recommend further studies of these allergens.

The presence in the skin of diverse cell types which are able to secrete and/or to respond to stimulation by NTs creates a unique network of molecular signaling in the cutaneous microenvironment.     Epidermolysis bullosa is a group of hereditary diseases affecting 1 in 17 000 live births worldwide. It consists of blistering of the skin and mucous membranes in response to minimal trauma. The disorder seriously affects the patient's quality of life. Diagnosis is based on immunofluorescence mapping and electron microscopy. Treatment is symptomatic, although new cellular and molecular therapies are currently under investigation. This review covers aspects of the molecular biology, clinical presentation, diagnosis, and treatment of epidermolysis bullosa relevant to improving the care for affected patients.

            <description>This article offered an extensive description of the clinical and pathological features and time-course of the skin manifestations of toxic syndrome caused by denatured rapeseed oil, also known as toxic oil syndrome. This new condition occurred in Spain in 1981 and was due to the ingestion of rapeseed oil intended for industrial use that had been denatured with anilines and subsequently refined and sold fraudulently as olive oil. In total, 20 000 cases and 400 deaths were reported. The disease affected mainly women, particularly in the late stages. This historical critique aims to explain the merit of our mentors -given the adverse and unstable scientific, economic, and social situation- in merely aiming to copy the best of other more stable countries to create dermatology structures and programs from which we have all since benefited.     Glomus tumor was first described by Wood in 1812 and named as such by Masson in 1924. It is a rare benign vascular tumor of the neuromyoarterial glomus. The triad of cold intolerance, intense paroxysmal pain, and well-defined site of pain is characteristic of the tumor. Approximately 80 % of lesions are found on the upper limbs, mostly under the nails. Between 2005 and 2008, 7 patients with this disease were seen in our department. Diagnosis was confirmed by histology after excision. We review the clinical features, complementary diagnostic tests, and main surgical techniques described.

            <description>We present 3 patients with histologically confirmed actinic cheilitis treated using photodynamic therapy with methyl aminolevulinic acid as the photosensitizer and red light at 630 nm. The clinical response was good, with no recurrences after 3 to 6 months of follow-up. Our experience supports the use of photodynamic therapy as a good alternative for the treatment of actinic cheilitis.

    In 1909, the Spanish dermatologist Juan de Az&amp;#xFA;a published a study of the main features of skin lesions in dermatitis artefacta. In the article, he paid particular attention to the psychological state of these patients, their family situation, and what they were hoping to gain with pathomimicry. Az&amp;#xFA;a directly confronted the patients with the diagnosis, which he demonstrated by applying an occlusive dressing. Written in a literary style typical of the times, the article includes the subjective impressions of Az&amp;#xFA;a, through which he manages to transmit a much more realistic image of these patients than that portrayed with the sterile language we tend to use in current medical literature.

            <description>We report 4 patients with relatively asymptomatic, annular brownish plaques arising in the skin creases.The lesions had remained stable for months despite many topical treatments. Histological examinationrevealed an atrophic epidermis with a dermal lichenoid inflammatory infiltrate showing marked pigmentaryincontinence.These clinical and pathological features were consistent with lichen planus pigmentosus-inversus,a rare, recently described variant of lichen planus, with only 10 cases reported to date. It has been suggested thatthe intensity and speed of onset of the inflammatory response could be modulated by keratinocyte surface markers, which could also determine the typical morphology of the lesions of this disease.
