Conclusions: We conclude that chronic heart failure is associated with decreased concentration of free sphingoid bases in the plasma. However, despite lower availability of substrates required for synthesis of cardioprotective sphingoid base-1 phosphates, their plasma level remains stable.

Conclusions: Results of our prospective study indicate that in obese patients, there is a significantly greater number of atherosclerotic lesions in coronary arteries found during PCI, as compared to those with normal body weight or overweight. We proved that overweight and obesity did not result in significantly greater damage to the myocardium and left ventricular dysfunction, both in the acute phase and 6 months after myocardial infarction treated with primary coronary intervention, as compared to those with normal body weight. In addition correlation was found between BNP concentration profile and body mass index in the 6-month follow-up after STEMI treated with PCI and bare metal stent implantation.

Conclusions: The high sensitivity and specificity of cervical US make this investigational method sufficient in the assessment of cervical nodal involvement. In esophageal cancer patients with negative cervical lymph nodes on US, three-field lymph node dissection could be avoided. In patients with positive cervical lymph nodes on US one should consider to extend lymph node dissection about lymph nodes of the neck to achieve a curative resection. In patients with negative abdominal US this investigation should be supplemented by more detailed diagnostic methods.

Authors: Leszczyńska A, Buczko P, Buczko W, Pietruska M
    Abstract
Periodontal disease is mainly associated with the activity of bacteria which adhere to the tooth surface and form specific structure of bacterial biofilm. Periodontal bacteria cause inflammation of the gums and aggressive immune response, affecting the periodontium. The first phase of initial therapy - mechanical removal of dental plaque and calculus - is necessary. If this non-surgical therapy has proved to be unsuccessful, an alternative treatment with antimicrobial agents is then considered. Pharmacotherapy is based on systemic or local antibiotics and/or antiseptics, which are applied according to the severity of the disease. A number of recent periodontal studies present some of the pharmacological agents, that ...

Conclusions: Syncope and near-syncope in geriatric inpatients have multifactorial and cumulative aetiology, with blurred, frequently overlapping boundaries between them. The falls, postural hypotension, and/or brain hypoperfusion of different origin seem to be most predictive of the both events, however low systemic blood pressure was predictive for the syncope exclusively.

Conclusions. The performance of G-17 was better after protein stimulation. G-17 was highly specific in the Caucasian, but not in the Asian subgroups. Still the low test sensitivity either at fast or following protein stimulation does not allow us to recommend it for wide screening purpose to diagnose antral atrophy.

Conclusion: The sensitivity of gefitinib resistant cell lines can be restored partially when the two downstream signaling pathways are blocked. However, these cells were still drug resistant, suggesting that the activation of PI3K and MEK pathways is not the only mechanism of EGFR-resistance.

We report a 16-month-old female patient with a 7-month history of periorificial and acral skin lesions. Diagnosing of Acrodermatitis enteropathica was established on the basis of the patient's history as well as clinical and laboratory findings (a lowered zinc level in the child's serum: 17.2μg/dL (N 70-160 μg/dL)). Rapid clinical improvement was observed right after launching zinc supplementation.

Conclusions: PBMC form SSc patients produce increased amounts of VEGF already in the early stage of disease. There is an imbalance in the profile of pro-angiogenic mediators produced by PBMC in SSc which might contribute to the pathogenesis of SSc. Further studies should address clinical significance of our findings.

Conclusions: A decreased blood platelet count, platelet hematocrit and large metabolically active platelet count, which in addition to reduced synthesis and excessive consumption of coagulation factors in states of hiperclotting is characteristic of IUGR, enhances the possibility of bleeding complications and increases the risk of infections. From a clinical point of view, it is important to take into consideration the degree of intrauterine hypotrophy during the evaluation of hemostatic disorders.

Conclusions: The highest relative increase of mortality after the discharge of patients with CHD undergoing PCI referred to the patients with NSTE-ACS. However, in the real life PCI practice STEMI patients have the worst hospital and long-term prognosis. Well recognized risk factors for death in patients with CHD are still of great importance in negative prognosis of patients undergoing PCI.

    Abstract
Chronic pancreatitis (CP) is a necroinflammatory process characterized by loss of both exocrine and endocrine function. To date, the disease has been treated symptomatically. Real advances in CP management can be expected once the pathophysiology of the disease is elucidated and individual stages of its development are properly managed. A key role in the CP pathogenesis is played by activation of pancreatic stellate cells (PSCs) that cooperate with the remaining pancreatic cells. All these cells produce cytokines, growth factors, angiotensin and other substances, which paracrinally or autocrinally induce further, persistent activation of PSCs. The activated PSCs are capable of producing and modifying the extracellular matrix. An optim...

Conclusions: Linezolid seems to be the optimal option in empirical therapy of infections caused by GRE strains because of the relationship between its activity (MIC value) and susceptibility breakpoint. There was no correlation between the prevalence of different virulence genes and resistance to the antibiotics tested.

We report a case of a 26-year-old female patient with Fanconi anaemia and severe chronic active hepatitis C virus infection. Her past medical history included treatment with multiple blood transfusions and bone marrow transplantation at the age of 13. The decision to treat the infection was taken, and history of hematologic disease contributed to the introduction of therapy with leukocyte interferon-α n3 and ribavirin combined with a granulocyte - colony stimulating factor. The treatment was well tolerated and after 48 weeks a reduction of the viral load and alanine aminotransferase activity were achieved. No adverse effects on bone marrow functioning were noted.

We describe a case of lupus vulgaris in a 72-year-old male patient with a single lesion localized on his lower extremity, developing for 30 years before correct diagnosis and previously treated with topical steroids. Bacillus infection in other organs was not detected. Diagnosis of tuberculosis was made based on personal history, clinical picture, hypersensitivity to tuberculin, histopathology and polymerase chain reaction. A multidrug therapy with rifampicin, isoniazid and pyrazinamide resulted in regression of the lesion. The common lack of knowledge about the clinical picture of cutaneous tuberculosis causes its late diagnosis and treatment.

Conclusions:Mycoplasma pneumoniae and Chlamydophila pneumoniae are significant etiological factors in pneumonia in children, and as such they should be taken into consideration in differential diagnosis of pneumonia in children. The clinical picture of pneumonia caused by Mycoplasma pneumoniae and Chlamydophila pneumoniae is hardly specific, with basic labs and chest x-ray of little help in differentiation of infection etiology.

Conclusions: In patients with diagnosed and treated arterial hypertension, there are relations among measurements of hs-CRP, ET-1, NE and blood pressure values in spite of treatment, which may improve understanding of mechanisms involving inflammation, endothelial dysfunction and sympathetic nerve activation and may identify patients with refractory hypertension.

Conclusions: The pattern of changes in noradrenalin did not systematically relate to the observed behavioural changes after core injections. The pattern of changes in dopamine suggested that depletion in core mediated the increased conditioning to the trace conditioned stimulus seen in the present study. However, the comparison depletion restricted to the shell subregion was less substantial, and a role for secondarily affected brain regions cannot be excluded.

Pancreatic exocrine insufficiency with steatorrhea is a major consequence of pancreatic diseases (e.g. chronic pancreatitis, cystic fibrosis, severe acute necrotizing pancreatitis, pancreatic cancer), extrapancreatic diseases like celiac disease and Crohn's disease, and gastrointestinal and pancreatic surgical resections. Recognition of this entity is highly relevant to avoid malnutrition-related morbidity and mortality. Therapy of pancreatic exocrine insufficiency is based on the oral administration of pancreatic enzymes aiming at providing the duodenal lumen with sufficient amount of active lipase at the time of gastric emptying of nutrients. Administration of enzymes in form of enteric-coated minimicrospheres avoids acid-mediated lipase inactivation and...

The monoclonal antibody against TNFα (infliximab) suppresses cytokines involved in inflammatory reaction. Consequently, infliximab is a potent agent in treating refractory rheumatoid arthritis (RA). There is also evidence showing beneficial anti-TNFα therapy effect on RA-related amyloidosis AA. TNFα inhibition may, however, lead to leucopenia and, eventually, severe sepsis. We discuss a case of RA with RA-related AA amyloidosis and renal impairment which was refractory to disease-modifying anti-rheumatic drug (DMARD). The treatment led to inflammatory complications of two distinct phases: immediately after drug administration and six weeks later. Both phases were linked to an innocuous skin infection.

The influence of dental restorative materials on patients' general and oral health is the main interest of many researchers but the question of their safety is still under consideration. An otherwise healthy 23-year-old patient with no history of oral abnormalities was examined. Dental amalgam restorations were replaced by composite resin material. Salivary smears prepared two days and two weeks after the amalgam removal were compared with those taken before the procedure.

Purpose: Stroke and subclinical "silent infarcts" are major causes of morbidity in children with Sickle Cell Disease (SCD). Ischemic strokes are more common in younger children while hemorrhagic strokes are more frequent in adults. The goal of neuroimaging in acute stroke is to document whether the stroke is ischemic or hemorrhagic, to assess the extent of parenchymal abnormalities and to determine the presence of other cerebrovascular lesions. Computed Tomography (CT) is the primary modality for the assessment of acute stroke patients because of its 24/7 availability and ability to exclude hemorrhagic causes. Magnetic resonance imaging (MRI) and MR angiography (MRA) are recommended to determine precisely extent of infarction and detect cerebrovas...

Atrial fibrillation (AF) is the most common cardiac arrhythmia with a prevalence of up to 10% in patients who are 80 years and older. There has been some significant progress in the understanding and management of AF in recent years. Large-scale clinical trials have provided solid evidence in supporting the role of anti-thrombotic therapies in the prevention of stroke in moderate to high risk patients. Appropriate control of the ventricular rate or the maintenance of sinus rhythm offers long-term benefits in specific groups of patients. Catheter ablation or "Maze" surgery has proven to be curative to some patients. However, the implementation of the evidence-based therapeutic strategies in the day-to-day care of the AF patients have been found to vary greatly f...

Conclusions: Increased TRP degradation and simultaneous altered kynurenines levels were found in plasma of AD patients. It proves activation of peripheral kynurenine pathway in this type of dementia. The alterations of two main KYN metabolites: KYNA and QUIN seem to be associated with the impairment of the cognitive function in AD patients. This appears to offer novel therapeutic opportunities, with the development of new compounds as a promising perspective for brain neuroprotection.

Liver cirrhosis is associated with number of hematological complications and coagulation disturbances. In view of various haemostatic abnormalities it is surprising that many patients do not bleed spontaneously. Severe coagulopathy of liver disease is more frequently seen in acute liver failure, but still remains important complication of liver cirrhosis and chronic liver failure. Decreased production of blood coagulation factors by the liver plays a key role in altered haemostasis in liver diseases. Altered fragile balance of blood coagulation proteins and infection are associated with both worsening coagulopathy and bleeding risk. Additional haemostatic abnormalities in patients with severe liver diseases are thrombocytopenia, chronic dissemin...

The evaluation of gene and protein expression profiles is a promising strategy to drive the therapeutical decision-making in non-small cell lung cancer (NSCLC). Among the several candidate genes that have been proposed, many retrospective studies have indicated excision repair cross-complementing 1 (ERCC1), an endonuclease responsible for the repair of DNA damages, as a reliable biomarker of tumor sensitivity to platinum-based agents. Moreover, the recent evidences showing the clinical efficacy of next-generation multitargeted antifolate drugs, in NSCLC, have highlighted the role of the determination of thymidylate synthase (TS) expression levels. Here is presented a brief overview of the literature regarding these two genes that are currently ...

We report a case of a 36-year-old man who was admitted to Emergency Room for persistent epigastric pain. Serial electrocardiograms (ECG) showed signs of acute myocardial infarction. However, a coronary angiogram demonstrated no coronary artery disease, and serum troponin was undetectable. Later, serum pancreatic enzyme levels were elevated and an ultrasonography scan of the abdomen was consistent with pancreatitis. Physicians should keep in mind the possibility of an attack of pancreatitis in a patient with abdominal pain and ECG modifications who is a heavy drinker.

Conclusions: A DNA database was established that may be used for the purpose of genetic profile comparison in criminal cases and chimerism monitoring after bone marrow transplantation. Significant differences revealed between the autochthonous Poles and the Old Believers by using RxC test and FST estimate support the idea of genetic isolation of this religious minority. Genetic polymorphisms analysed using statistical methods may be informative in differentiation of populations and ethnic groups in northeastern Poland.

Conclusions: This study suggests a relationship between kynurenine pathway activation, endothelial dysfunction and the progression of atherosclerosis in CKD patients. It opens a new idea that the inhibition of kynurenine pathway may provide an effective strategy to slow down endothelial dysfunction and thereby the prevalence of atherosclerosis in this population.

Authors: Morisset J
Studies on the control of pancreatic secretion in humans of all ages have been a difficult task over the years because of patients' availability and ethic committee rules. Nevertheless, studies were performed and the objectives of this review are to summarize our knowledge on the development of secretory process in newborns, on the different phases of the pancreatic responses to a meal, on the pancreatic responses to the different components of the diet, on the mechanisms involved in the control of the pancreatic responses, and finally on the receptors involved in these controls.

Conclusions: The present study demonstrates that calcitriol and tacalcitol are capable of affecting pro-inflammatory cytokine (IL-6 and IL-8) levels in NP cultures. Our data imply a potential therapeutical application of topical VD derivates in NP and warrant further investigation.

Striae distensae or stretch marks are cutaneous changes which may appear everywhere on the body, except for the face, and they usually occur in stretching areas. We want to present unusually large striae that developed nearly all the body of a 14-year-old patient during steroid therapy. Such huge striae are very rare, especially when the doses of steroids are not high. Widely spread striae are not a serious disease or disability, but only a cosmetic problem. However, they may lead to a persistent complex and feeling of inferiority or even cause serious depression states, especially in teenagers, as in this case.

Purpose: This is the first report of a rare genetic tooth and nail syndrome (TNS) diagnosed in a 14-year-old Caucasian girl with a complete absence of the permanent dentition and, additionally, reduced total and lumbar spine bone mineral density (BMD). This coincidence suggests a new clinical manifestation of the disorder in which genetic factors and/or shared mechanisms may be responsible for the deterioration of the stomatognathic system, anodontia, nail phenotype and osteopenia. Low bone mass appears to be a new component of the syndrome. There is a rationale for bone densitometry scans assuming that patients with TNS may have an increased risk of osteopenia. Reduced BMD and, possibly, impaired bone qual...

This report describes two patients hospitalised in Hepatology Unit, Infectious Diseases Department Medical University of Gdansk because of liver damage discovered in family doctor's practice. Hereditary hemochromatosis was diagnosed in both cases. Diagnosis was established basing on medical records review, and biochemical, molecular and liver specimen tests. The analysis of polymorphism of UGT1A1 gene was done in these cases because those patients were a part of the larger study on prevalence of UGT1A1 gene mutations in patients with hereditary hemochromatosis. We discovered rare variant forms of UGT1A1 gene coexisting with HFE gene mutations.

Worldwide, the prevalence of overweight and obesity and associated complications is increasing. Cardiovascular complications are the most important factor determining survival and influencing clinical management. However, obesity is also associated with an increased risk of metabolic syndrome, type 2 diabetes, cancer and other diseases. The development of complications depends on the amount of body fat and its endocrine function. The hormones (leptin, adiponectin, resistin) and cytokines (TNF alpha, IL-6, PAI-1) produced by the adipose tissue are the link between obesity and obesity-related complications. The present article discusses the structure, function and clinical significance of ad...

The horseshoe kidney is the most common renal fusion anomaly. Approximately one-third is associated with other congenital anomalies. The combination of horseshoe kidney and bilateral ureteral duplication is a very rare entity. The horseshoe kidney anomaly discovered incidentally in ultrasound examination in our patient when he was being detected for urinary incontinence. Because he had dilated renal pelvis he had undergone intravenous pyelography (IVP). Bilateral double collecting system and bilateral partial hydronephrosis was detected on IVP. Magnetic resonance urography was also performed in the same patient for better delineation.

Conclusions: A decreased count of blood platelets, platelet hematocrit and increased platelet distribution width may result from a low gestational age or a dysfunction of megakaryocytes and the placenta. Blood platelet indices may be vital in the diagnosis of haemostatic disorders.

One of the key factors determining the function of all types of stem cells is their location in a specific microenvironment called a niche which is understood as a system of adjacent cells directly influencing their ability to carry out self-renewal divisions. The cells which compose the niche influence cytophysiological processes of stem cells both directly via the intercellular junction system and via the synthesis and release of many protein regulatory substances which are ligands of specific receptors in a particular stem cell. These proteins are often the products of distinct genes whose expression tends to be specific for niche-composing cells. The niches formed of a few cells only observed in Drosophila gonads may become a valuable funct...

Breast neoplasm may develop in ectopically located glandular tissue. This paper presents an interesting and rare case of a 50-year-old female who despite regular mammography screening examination developed an invasive accessory breast cancer. Clinical examination revealed a 2 cm - tumour localized 4 cm below the left inframammary fold. The lesion was immobile, the skin and the atrophic nipple were retracted, the tumour infiltrated the thoracic wall. Oligobiopsy and additional examinations showed an invasive stage IIIB ductal breast cancer (Bloom II, G-2). The receptor status was: ER(+), PGR(+), HER2(-). The increased level of cancer antigen 15.3 was found. The patient was submitted to pre-operative chemotherapy. She also underwent surgery ...

Idiopathic retroperitoneal fibrosis (IRF) entirely confined to the pelvic cavity is an extremely rare clinical entity. Herein, is described the case of a 36- year old male who presented with clinical and imaging manifestations of a large pelvic tumor. Exploratory laparotomy revealed a large mass in the right pelvis originating from the retroperitoneal space, displacing the right iliac vessels, the right ureter and the urinary bladder completely to the left. A laborious resection of the mass measuring 14x10cm was performed. Histopathological examination and detailed immunohistochemistry analysis were suggestive of idiopathic retroperitoneal fibrosis with no evidence of malignancy. This is a very rare case regarding localization and clinica...

Conclusions: Since the mean MMPs levels were found to be growing it can be assumed that aprotinin has no significant effect on the regulation of MMPs in the saliva of CP patients. It thus seems that aprotinin application after scaling has no additional therapeutic effect.

Pancreatic cancer is one of the most aggressive and devastating human malignancies. Despite new knowledge in the molecular profile of pancreatic cancer and its precursor lesions, survival rates have changed very little over the last 40 years. Therefore, a better understanding of the detailed mechanisms underlying the pathogenesis of this disease is critical if we expect to develop new and effective strategies for prevention, early diagnosis and treatment of pancreatic cancer. The review herein focuses on a distinctive signaling pathway, the Notch pathway, which has recently been associated with carcinogenesis, including pancreatic cancer. It is aimed at summarizing key results which support a role for this pathway in the initiation, progression and main...

Conclusions: Our results showed that the recurrence probability rates are different for particular categories of unfavorable pregnancy outcomes. How much they are dependent on the size of 9p chromosome segments taking part in the imbalance needs further studies based on a larger number of observations.

Conclusions: The results of our study show that IL-10 serum concentration correlates neither with the disease activity nor with the levels of anti-mannan Candida antibodies and the fungal colonization of the gastrointestinal tract in ulcerative colitis. It seems that IL-10 serum concentration cannot be a universal marker for the assessment of ulcerative colitis activity. Moreover, anti-mannan Candida antibodies and significant fungal colonization are present in the minority of patients with ulcerative colitis suggesting that yeasts have minor, if any, influence on the clinical course of the disease.

Conclusions: The short term enzymatic replacement therapy together with a high fat meal has immediate beneficial effects on diet digestibility and on the growth retardation observed in EPI pigs. The turbidimetry method used to measure lipaemic index is a reliable, quick and efficient technique in measuring plasma lipid profiles and thus a good tool for assessing fat absorption.

We present a case of fungaemia caused by Cryptococcus laurentii in a young man with membranoproliferative glomerulonephritis on aggressive immunosuppressive therapy. We also considered a tick-borne infection because of the endemic area of ticks' occurrence. Most cases of fungaemia caused by Cryptococcus laurentii were successfully treated with fluconazole. We still observed septic fever and positive microbiological blood tests after 3 weeks of treatment with fluconazole in our patient. Therefore, among the others, a computer tomography of abdomen was done, which revealed an inflammatory (presumably mycotic) focus near right lobe of the liver. Accordingly, we started treatment with itraconazole. Controlled microbiological blood tests after 5 weeks of itraconazole therapy were negative. Unti...

Hepatitis C virus (HCV) infection is a common problem. Combined treatment with interferon and ribavirin improved treatment efficacy, but still high percentage of infected patients has not reached virus elimination. It was found that HCV infection course, but also treatment efficacy among other things can depend on patient's individual factors, including MHC genes structure for HLA tissue compatibility antigens. Many connections between HLA system and HCV infection course were noted. There are some reports concerning connections between MHC structure and results of chronic hepatitis C treatment with interferon. In future, results of investigations connected with this problem can allow to verify qualification criteria for treat...

Pasteurellosis is a zoonosis often caused by cat or dog bites or scratches, or by direct exposure to their secretions. Pasteurella multocida is the main pathogen involved in infections through domestic animal bites; generally a local infection characterized by its particular virulence with consequent rapid onset. Serious infection has also been reported in persons affected by comobirdity without domestic animal bite injuries. Here we report the case of a woman with lower limb exudating vesicular skin ulcers affected by liver cirrhosis, bilateral knee arthritis, septicemia with positive blood culture and synovial fluid culture for Pasteurella multocida. The etiology of Pa...

Conclusions: TNF-R1 has been identified to be a prevalent form of the TNFalpha receptor in nasal polyps which may reflect the apparent dominance of this form in TNFalpha signalling. The findings raise the possibility that the eosinophils from NP may influence biological responses through TNFalpha-dependent mechanisms. The differences between ENP and NNP relating to TNFalpha and the expression of its receptors may reflect the distinct character of those diseases.

Conclusion: The results of our study confirmed the presence of a high diversity of carotenoids in the physiologic, benign and malignant tissues of the breast and the reproductive tract in women. The differences observed among the frequency of isolation of some carotenoids do not allow to make straightforward conclusions. The frequent isolation of provitamin A carotenoids in the examined material and the lack of their occurrence as major carotenoids may be connected with using them in the cellular biological processes. However, this requires further investigation.

Diagnosis and Therapy of Gestational Breast Cancer: A Review.

Conclusion: The evaluation and management of women with gestational breast cancer requires a multidisciplinary approach. A chemotherapeutic regimen should be individualised to a maximum reduction of risk, if applied in the second and third trimester. Surgical therapy may include mastectomy and sparing operative procedures. Sentinel node biopsy should be considered in node negative patients. Radiotherapy should be postponed to the postpartum period.
