Conclusion Autologous augmentation-mastopexy provides a robust augmentation, giving more natural ptotic breasts while avoiding the cost
and potential complications of implant augmentation. The increased lateral flank scarring is well tolerated by these patients,
with the additional benefit of reducing flank fullness.

Conclusion Based on Würinger`s horizontal septum, a safe and well-vascularized mammaplasty is possible even with large resection weights
and long nipple–areola distances, and NAC sensibility can be preserved. The reported technique combining excellent pedicle
perfusion with a wide variety of breast shaping proved to be safe.

Conclusions Transconjunctival blepharoplasty is the first choice for primary eyelid bags. Transcutaneous lower blepharoplasty (skin flap
or skin-muscle flap procedures) is indicated for the senile eyelid with excess skin and muscles. If supporting structures
(skin, muscle, and septum) show laxity and other aging stigma are present, including tear trough deformity and obvious skeleton
rim, Hamra’s procedure (septal reset and fat-replacing technique) is performed.

Abstract
Background Periprosthetic capsules are a common reaction of the body to silicone or polyurethane breast implants. The aim of this study
was to evaluate similarities and differences in the histological features of periprosthetic capsules surrounding silicone
implants and polyurethane foam-coated implants and to correlate those features with the age of the implants.

Methods Tissues were studied from 41 periprosthetic capsules surrounding textured prostheses and from 20 capsules surrounding polyurethane
foam-coated implants. For each sample we evaluated synovial metaplasia, density of collagen fibers or fibrosis, orientation
of collagen fibers, and foreign body reaction (granulomatous reaction).

Abstract A minimally invasive procedure based on a “suspensory system” is presented for the aesthetic correction of the flat nose.
A Goretex monofilament is inserted between the connective tissue and the perichondrium from the septal, upper lateral, and
alar cartilages in a quadrangular fashion. When tightened, it shortens the nose and elevates the tip. The procedure is performed
on an outpatient basis with the patient under local anesthesia. It offers the patient immediate evaluation of the result.
No edema or bruises have been observed. At this writing, 8 months after an application of this procedure, no extrusions or
collateral reactions have been found.

This study evaluated the long-term maintenance of augmented bony projection
when porous hydroxyapatite granules are used on the facial skeleton. Ten female patients aged 28–58 years were studied following
aesthetic augmentation of the facial skeleton at 24 sites using porous hydroxyapatite granules. Postoperative CT scans at
3 months served as the baseline measurement and compared with scans taken at 1 and 2 years, with the thickness of the hydroxyapatite
measured in axial and coronal planes. Thickness of original bone plus overlay of hydroxyapatite, thickness of the overlying
soft tissue, and the overall projection (bone plus soft tissue) were recorded. It was found that 99.7% of the hydroxyapatite
was maintained at 2 years, with no statistical difference (t te...

Conclusions We detected no statistically significant reduction in fibrosis in the periprosthetic capsule after treating the implants with
the inhibitor peptide p144, but we feel that the influence of trauma around the prosthesis is critical in impeding the antifibrotic
activity of the inhibitor peptide.

Conclusion Transaxillary augmentation mammaplasty without routine endoscopic assistance is a safe method with predictable results and
a high rate of patient satisfaction. The transaxillary technique offers the advantage of locating the surgical scar off the
breast. It requires closer supervision during the first few postoperative months compared with the submammary or periareolar
technique because it is more difficult to place and maintain implants at the proper level using the transaxillary approach.

Conclusions The results of this study indicate that the Taiwanese female CS candidates did not have higher body image dissatisfaction
or greater body image investment than the control group. However, body area dissatisfaction was the only significant predictor
for the likelihood of having CS, a feature not previously recognized in Asian CS candidates. The higher self-esteem of the
CS candidates opposes the view that low self-esteem is a principal motivating factor for CS.

Conclusion This prospective comparative trial demonstrates that an 810-nm diode laser treatment, performed immediately after surgery,
can improve the appearance of a surgical scar. The dose plays a great role in scar improvement and must be well controlled.
There is interest in LASH for hypertrophic scar revision. LASH can be used to prevent and reduce scars in plastic surgery.

Conclusions Women with large breasts try to develop adaptable character properties against chronic troubles. However, the temperament
scale of persistence decreases in this process. Similar temperament and character traits of women with and those without large
breasts suggest that patients desire breast reduction for functional rather than cosmetic purposes.

Conclusion At least one-third of the LLCs analyzed had a medial wall in addition to the lateral wall, which significantly increased the
flare and recoil force of this structure. This makes this cartilage more redundant to reshaping procedures (straightening).
Thus, special attention must sometimes be paid to this common anatomic variation to produce symmetric nostrils and to obtain
a more aesthetically acceptable alar tip.

This report aims to describe the clinical outcomes for a man with buffalo hump
who underwent lipolaser-assisted liposuction. This technique obtained a significant reduction in the size of the adiposity
and an improvement in the neck’s range of motion. At this writing, after 3 years, the patient is satisfied with his cosmetic
and functional results. The authors believe that lipolaser-assisted liposuction using the Smartlipo Deka-Mela neodymium:yttrium-aluminum-garnet
(Nd:YAG) 1,064-mm-long pulse is effective in reducing the cervicodorsal fat pad. The technique is performed using local anesthesia
with low operative risks and minimal surgical trauma. The treated zone shows rapid healing, and the whole procedure requires
a day-hospital recovery, thus reducing the costs.

Conclusion The patients undergoing rhinoplasties preferred new technologies in the preoperative period. The advantages of using the 3D
radiologic viewer included improved preoperative planning, reduction in intraoperative stress, a higher number of patients
undergoing surgery, reduction in postoperative surgical corrections, reduction in surgical time for the functional intervention,
a higher rate of improvement in nasal function, a higher percentage of postoperative satisfaction, and reduced costs.

Conclusions The authors present their early results with SRSM using the dermal barrier flap and silicone implants for early-stage breast
cancer. The dermal barrier flap became a reliable procedure by providing a decreased breast envelope, eliminating the risk
of implant exposure, and forming a double layer of dermal tissue at the incision line.

Conclusions With proper selection of patients and Style 410 breast implants, cases of grade 3 breast ptosis can be managed successfully
by the periareolar approach, thus extending the range of cases suitable for this technique. The different shapes of the Style
410 form-stable, silicone gel implants help to recreate the desired natural-looking breast, achieving stable results with
minimal complications.

Abstract Among the reasons for reoperation after augmentation mammaplasty is the malpositioned implant, especially a lowered inframammary
fold or symmastia, which is difficult to repair. The peri-implant capsule, a physiologic response to a foreign body, is naturally
formed and suitable for use as a flap because of its high vascularity. In addition, it is sufficiently tough for suspension
of the implant. The authors introduce the idea that the capsular flap is very useful for the correction of symmastia or a
lowered inframammary fold. In such situations, the capsular flaps are used to prevent migration of the implant after raising
of the inframammary fold or defining of the midline with capsulorrhaphy. This technique successfully corrected the malpositioned
implants in thi...

Conclusion The use of a deepithelialized double dermal flap is a safe and new way to obtain excellent results in rejuvenation of the
gluteal region. Our technique allows for the creation of a stable and long-lasting infragluteal fold with an aesthetic buttock
curvature and a defined border to the thigh region.

This article reviews the evidence available on the safety of using hyaluronic
acid and raises concerns with regard to its use as a product for breast augmentation, the technique recommended for its use,
and the authorisation process for new implantable medical devices. It is strongly recommended that clinicians review the lack
of data on Macrolane before offering it as a treatment option to patients.

This report describes a technique for treating depressed scars and soft tissue contour deformities. Surgical subcision with
a Beaver tympanoplasty blade is undertaken for depressed or adherent scars to release the fibrous attachments beneath the
scar before autologous fat grafting. Satisfactory results were observed, with an improvement in surface contour for 16 patients
over a 3-year period. The authors recommend the described technique as a safe, minimally invasive, and precise method for
subcutaneous dissection of scar tissue before fat injections.

This study enrolled 60 patients with asymmetric
calf hypertrophy, defined as a differential calf circumference exceeding 0.6 cm. Differential selective neurectomy techniques,
which depend on the sizes of the small and large calves, were performed exactly via a 1-cm popliteal wound. The pre- and postoperative
mean differences between the larger and smaller calf circumferences at the 1-year follow-up consultation were 1.38 ± 0.65
and 0.42 ± 0.38 cm, respectively (p < 0.01). The reduction was significantly greater in the leg that was initially larger. The procedure was effective in reducing
circumference discrepancies so that leg contours were more balanced. All the patients were able to ambulate normally within

Abstract Pectus excavatum, the most common congenital chest wall malformation, has a higher incidence among men. Since 1987, when Donald
Nuss performed his technique for the first time, the minimally invasive approach has become the most widely used technique
for treating pectus excavatum. Few reported studies have focused on the repair of female pectus excavatum. Women with pectus
excavatum often present with breast asymmetry that may require breast augmentation, either before or after pectus excavatum
repair. To the authors’ knowledge, no reports on the Nuss procedure after breast implant surgery have been published. This
report describes the case of a 26-year-old woman who underwent minimally invasive repair after breast implant surgery. The
authors believe that for w...

Abstract The author presents his experience with breast augmentation using a next-generation, form-stable, anatomically shaped silicone
gel breast implant. Rotation is a potential complication for anatomically shaped breast implants. Anatomically shaped saline
implants have been reported to have a rotation rate as high as 14%, while lower rotation rates of 1–2.6% for anatomic cohesive
gel silicone implants have been reported. Currently, these implants are limited in the United States to US FDA-approved clinical
trials. The author reviews the appropriate surgical techniques to prevent rotation when using these devices. A recent innovation,
placement of the superior pole of the implant underneath the superficial fascia of the pectoralis major muscle, is described.

Conclusion In selected patients, effective treatment of the abdominal region demands correction of both the upper and lower abdominal
laxity and contour. This can be performed safely, effectively, and reliably by a reverse abdominoplasty with IMF reconstruction
independently or simultaneously with circumferential abdominoplasty.

Discussions included details
of the blunt-cannula injection technique currently recommended for administration of large-particle NASHA-based gel. With
the aim of optimizing patient comfort and control over administration of NASHA-based gel, the panel members explored an alternative,
sharp-needle technique. In this article we describe the new technique in detail, together with practical recommendations and
precautions. The technique has several advantages over blunt-cannula injection: improved patient comfort, lack of need for
a skin incision, increased precision regarding the implant position, and improved control of injection volume. However, the
sharp-needle technique requires a high level of skill and a good knowledge of facial anatomy.

Poland’s syndrome, with impressive results and at the cost of limited constraints and scar sequelae, opens new perspectives
and suggests extensive potential applications of lipomodeling in all disciplines related to the breast.

Conclusion The proposed percutaneous electrocauterization technique for the treatment of deep hemangiomas achieved four effects: (1)
volume reduction, (2) bleeding reduction, (3) anesthesia time reduction, and (4) cost reduction. The technique can be defined
as a minimally invasive and effective alternative treatment for deep hemangiomas.

Conclusion The subfascial procedure offers a high-grade result for the short and long term and has fewer disadvantages than the submuscular
or subglandular technique. Likewise, there are fewer immediate postoperative symptoms and patients have a much better and
less painful recovery. The morbidity in these patients is less than with the subglandular and submuscular methods.

In this study, 10 patients with newly reconstructed nipples were instructed to use breastfeeding nipple shields as the sole
means of nipple dressing after the reconstruction procedure.

Results No complications were observed overall. Patients reported full adherence to the postoperative dressing regimen as well as
ease of use, availability, low costs, and pleasing aesthetic appearance under garments.

Discussion Silicone breastfeeding nipple shields offer an efficient, affable, cheap, widely available, and aesthetically pleasing form
of postoperative dressing for reconstructed nipples. Their use may enhance patient compliance with the dressing regimen and
lower the postoperative complication rate.

Conclusion A single dose of intravenous antibiotic is adequate for prophylaxis in breast augmentation surgery, and the extra duration
of antibiotic cover does not result in reduced superficial or periprosthetic infections. Infection can be managed in more
than one way depending on the nature, degree, and extent of infection.

A New Postoperative Otoplasty Dressing Technique Using Cyanoacrylate Tissue Adhesives

We present our method of postoperative dressing
in the form of liquid bonding. Cyanoacrylate tissue adhesives as liquid bonding agents are used for fixation of the pinna
at the mastoid area. After 10–14 days the bonding can be easily removed. No huge dressings, tapes, or plasters are necessary.
The patients are satisfied with the light dressing; they do not feel ashamed to appear in public. We have found this dressing
technique to be simple and economical, especially because of the use of the bonding for skin closure before. It can be used
after otoplasty with an anterior or a posterior approach.

Breast Prostheses and Connective Tissue Disease (CTD): Myth or Reality?

Abstract Since their first appearance, breast prostheses have been criticized as being both responsible for and giving rise to systemic
disease. The literature contains many reports on the subject, and theories were controversial from the 1980s to the 2000s.
The aim of this review was to gather together the most important studies on breast prostheses and systemic disease, with particular
attention to connective tissue disease (CTD), in order to verify any relationship between silicone breast implants and the
occurrence of pathologies.

Effect of Botulinum Toxin Type A on Transforming Growth Factor β1 in Fibroblasts Derived from Hypertrophic Scar: A Preliminary Report

Conclusion These results suggest that BTXA effectively inhibited the growth of fibroblasts derived from hypertrophic scar and in turn
caused a decrease in TGF-β1 protein, indicating that BTXA-based therapies for hypertrophic scar are promising and worth investigating
further.

Abstract After more than 25 years of research and development, in October 2006 ArteFill® became the first and only permanent injectable wrinkle filler to receive FDA approval. ArteFill is a third-generation polymeric
microsphere-based filler, following its predecessor Artecoll®, which was marketed outside the United States between 1994 and 2006. ArteFill is approved for the correction of nasolabial
folds and has been used in over 15,000 patients since its U.S. market introduction in February 2007. No serious side effects
have been reported to date according to the FDA’s MAUDE reporting database. ArteFill consists of polymethylmethacrylate (PMMA)
microspheres (20% by volume), 30–50 μm in diameter, suspended in 3.5% bovine collagen solution (80% by volume) and...

Abstract Patients ask for procedures with long-lasting effects. ArteFill is the first permanent injectable approved in 2006 by the
FDA for nasolabial folds. It consists of cleaned microspheres of polymethylmethacrylate (PMMA) suspended in bovine collagen.
Over the development period of 20 years most of its side effects have been eliminated to achieve the same safety standard
as today’s hyaluronic acid products. A 5-year follow-up study in U.S. clinical trial patients has shown the same wrinkle improvement
as seen at 6 months. Long-term follow-up in European Artecoll patients has shown successful wrinkle correction lasting up
to 15 years. A wide variety of off-label indications and applications have been developed that help the physician meet the

Conclusion Cosmetic surgery in transplant recipients is being successfully practiced in the USA. Surgical and nonsurgical aesthetic procedures
are being performed safely in organ transplant recipients without a significant increase in the incidence or degree of complications.
If certain precautions are undertaken, these patients may expect a degree of success comparable to that of the rest of the
population.

Abstract To obtain the best surgical results in orthognathic surgery, treatment planning and evaluation of results should be performed.
In these operations it is necessary to provide the physician with powerful tools that can underline the behavior of soft tissue.
For this reason, considering the improvements provided by the use of 3D scanners in medical diagnosis, we propose a methodology
for analyzing facial morphology working with geometrical features. The methodology has been tested on patients with malocclusion
in order to analyze the reliability and efficiency of the provided diagnostic results.

Abstract During septorhinoplasty procedures lacrimal drainage systems are susceptible to injury because of the close relationship of
the nasal bones. Most of the injuries to the system remain asymptomatic. Here we report a case of septorhinoplasty which was
complicated by acute dacryocystitis postoperatively. The patient was treated successfully with appropriate medical therapy.

In this study we investigated the risk of systemic fat mobilization and fat embolus in rats following liposuction
using dry and tumescent techniques.

Methods At the end of the procedures, the rats were sacrificed and specimens were obtained from the lungs, kidneys, liver, brain,
and skin. Histological examinations of the specimens were carried out. Liposuction was not performed in the control group
(n = 8), but blood and tissue specimens for histological examinations were obtained.

Results We found signs of fat embolus in both blood specimens and histological examinations of tissue samples in the study groups.
However, the results of the examinations were normal in the control group. Although there were no fat particles in the blood

Conclusion We have shown that in vivo grafted adipocyte cell preservation can be enhanced by treatment with botulinum toxin as an adjuvant.
We suggest that botulinum toxin further increases this graft preservation rate by enhancing angiogenesis.

Conclusion Transaxillary breast augmentation provides consistent, satisfactory results with ease of dissection, when properly indicated.
Although the subfascial augmentation mammaplasty has all the advantages of the subpectoral and subglandular augmentation mammaplasties
and eliminates the disadvantages of increased postoperative discomfort, implant visibility, and distortion, patients of all
three groups had similar rates of satisfaction. Further follow-up is needed in order to compare the long-term effects of the
three different planes of insertion.

Conclusion There are many ways to provide soft-tissue coverage of silicone breast implants in breast reconstruction. These include acellular
dermis slings, polyglycolic mesh, deepithelialized skin, and muscle. The ideal soft-tissue cover would be supple, easily harvested,
of minimal morbidity, of minimal cost, and preferably autologous. We feel that the technique described here has these qualities
and allows for complete coverage of silicone implants. An additional benefit of this technique is that it helps to increase
the definition of the inframammary sulcus. This method is a good alternative in providing implant coverage during breast reconstruction,
especially when there is a large implant or small pectoralis major muscle.

Abstract Nowadays, oncological breast surgery has to be performed to maximize cosmetic results, and even mastectomies, when unavoidable,
should conform to acceptable aesthetics. We investigated surgical approaches to skin-sparing mastectomies. Scar positioning
and volume replacement were evaluated. We also discuss nipple-sparing mastectomy (NSM), probably the most outstanding item
in the current debate on breast shape preservation. We give suggestions for safe admission criteria and effective treatment.

Conclusion Both patients and surgeons were satisfied with the results because the muscle grafts produce a youthful appearance by adding
natural, soft roundness and fullness to the upper lip without an artificial look or the use of synthetic material, providing
long-term augmentation.

Abstract A new liposuction technology for adipocyte lipolysis and uniform three-dimensional tissue heating and contraction is presented.
The technology is based on bipolar radiofrequency energy applied to the subcutaneous adipose tissue and subdermal skin surface.
Preliminary clinical results, thermal monitoring, and histologic biopsies of the treated tissue demonstrate rapid preaspiration
liquefaction of adipose tissue, coagulation of subcutaneous blood vessels, and uniform sustained heating of tissue.

Abstract Vitiligo is an acquired condition that presents as sharply demarcated white macules. It affects 1–2% of people of all races,
regardless of gender or age. Although the disease does not have any systemic complications, it is of great concern, particularly
in darker-skinned individuals. We treated 14 patients with vitiligo involving the upper and lower extremities. Surgical therapies
were used in conjunction with medical therapy to achieve repigmentation after the disease was stabilized.

Autologous Fat Transplantation and Delayed Silicone Implant Insertion in a Case of Mycobacterium avium Breast Infection

Conclusion Although this kind of complication has significantly compromised the aimed result and has caused frustration to both the patient
and the physicians involved, we believe that the combination of autologous fat transplantation and delayed silicone implant
insertion can be a safe strategy and can produce an acceptable aesthetic result.

Conclusion In the last 25 years the results of autologous fat transplantation have been predictable and satisfying on the condition that
the treatment is performed in stages with small quantities of adipose tissue fat injected in each treatment session. To prevent
major complications the final expected result should not be the aim of a single procedure. Mammary lipografting is a procedure
that can be offered to patients for breast reconstructive and cosmetic purposes.

Conclusions Intralesional injection, termed mesotherapy, using phosphatidylcholine is a potentially effective therapy for benign symmetric
lipomatosis that should be reconsidered as a therapeutic option for this disease.

Round Gel Breast Implants or Anatomic Gel Breast Implants: Which is the Best Choice?

Conclusions The use of anatomic implants is suggested for patients with significant differences in chest height and width measurements,
for cases of significant mammary asymmetry, for patients with a small breast volume or a prominent thorax, and for breasts
with a significant deficit of inferior mammary volume or significant shortening of the breast. The authors recommend round
implants for patients with a superior pole deficit or moderate breast pseudoptosis, for patients who have a breast that will
cover the implant, and for patients who present with a small asymmetry.

Conclusion The preliminary results show that fat injection in irradiated tissue prior to breast alloplastic reconstruction may reduce
the radiation-related complications on implants. Benefits from fat grafting are in keeping with the theoretical basis of this
surgery. Larger studies are needed to confirm our observations.

Conclusions A wide range of complications seen among our patients showed that polyacrylamide hydrogel may not be as safe and biocompatible
as it was thought previously. Both patients and physicians must be aware of the potential side effects of polyacrylamide hydrogel
before gel administration.

Abstract The goal of a mastopexy is to restore the shape and volume of the breast after ptosis of the breast. Ptosis occurs commonly
in response to aging and breastfeeding. A low nipple position on the breast mound can be corrected by addressing the skin
envelope, but maintenance of upper pole fullness and a youthful shape of the breast is the challenge to surgeons. To address
this, techniques using local glandular flaps and implants have been suggested. A new technique has been created with regard
to a detailed knowledge of the breast gland’s vascular anatomy. The lower breast gland is raised as a large vascularized flap
and rotated into a pocket beneath the upper pole. The operative procedure is presented together with the experience of the

Abstract The gold standard treatment for the many aesthetic aspects of aging has for many years been surgery in its many forms. However,
with increasing patient demand for cosmetic rejuvenation and with the strong desire and drive by patients to attain aesthetic
enhancement with minimal risk and rapid recovery, there has been a strong surge inspiring the field of nonsurgical skin rejuvenation.
Traditionally, most of the nonsurgical methods have centered around those that destroy the epidermis and cause a dermal wound,
with resultant dermal collagen remodeling and secondary skin tightening and rhytid improvement. Currently, there is growing
interest in a wide range of nonablative interventions that, predictably, are claimed to rejuvenate skin and subcutaneous tissue

Abstract Lymphedema typically occurs on the extremities and affects millions of people throughout the world. Although currently there
is no single treatment proven effective for lymphedema in every patient, suction-assisted lipectomy has been shown to be effective
in some patients. Suction-assisted lipectomy offers patients with lower-extremity lymphedema a less invasive, less morbid
surgical option compared with traditional excisional techniques. In this article we present a case of lymphedema reduction
with suction-assisted lipectomy in a patient with bilateral lower-extremity lymphedema.

Abstract
Background Synmastia following mammoplasty is an uncommon complication. The true incidence is not known and the condition is underrecorded
and undertreated. Medial capsulorrhaphy is the treatment of choice and can be accomplished in a single or staged procedure.
Muscle-splitting submuscular biplane is used to correct synmastia following subglandular mammoplasty. The procedure allows
the use of undisturbed submuscular anatomy and obviates the need for capsulorrhaphy.

Methods A retrospective single-surgeon record of over 1900 implant-related surgeries included five patients treated for synmastia.
Four had their primary surgery done by the author and one was a secondary referral. Only one patient was aware of the condition
and revision was done excl...

This report summarizes the opinions advanced by an international panel of acknowledged expert plastic surgeons convened to
discuss women’s attitudes about aesthetic breast surgery. To inform the discussions, a survey was undertaken to explore women’s
attitudes and opinions. The survey showed that significant numbers of women take the initial steps but do not proceed to surgery.
This represents a failure to meet the needs of many women who stand to benefit from surgery. It also represents a waste of
resources. Cost and fear are important factors, but a crucial consideration for women is whether they can find a surgeon they
can trust. The panel recommends using standard consultation procedures, ensuring good clinic organization, managing expectations,

Abstract Three main techniques are used to excise sebaceous cysts: conventional wide excision, minimal excision, and punch biopsy excision.
A new method with two steps is proposed. First, a laser is used to make a small hole for removal of the content. Then the
cyst wall is removed entirely with a minimal excision about 1 month later. With this method, the cyst is completely removed
with only a small scar. It offers a good alternative for eradication of uninfected cysts, especially large cysts or cysts
located in areas of thick skin or cosmetic concern.

Conclusion The decrease in MMP-to-TIMP expression can cause increased synthesis and deposition of collagen surrounding alloplastic breast
implants, leading to a profibrotic state. The higher expression of TIMPs in capsular tissue of patients with smooth silicone
gel implants might be a reason for the observed higher rates of capsular contracture. In the future, a nonoperative treatment
that decreases TIMPs but increases the activity of MMPs may be an appropriate therapy for patients with capsular contracture.
