This study addresses the meanings of alcohol/other drug use for those who, during adolescence, moved beyond experimentation to dysfunctional levels of use. Interviews were conducted with 12 students in recovery from alcohol/other drug dependency, all currently participating in a university-based recovery program. Interpretive phenomenological analyses of the interviews revealed that meanings ascribed to use were extensive, complex, and intertwining. Students viewed their alcohol/other drug use in negative ways as an escape, as a mask to feelings, and as leading to a double life. Yet they also saw it as normative, developmentally significant, and as an integral part of their social interactions.

This article briefly outlines issues including the construct of acculturation, measuring acculturation, and understanding the implications that it has with alcohol use in ethnic minorities. A study is presented that illustrates the association between acculturation and drinking patterns in older White and Hispanic veterans. Greater levels of past drinking were associated with more acculturation into the mainstream U.S. society for the Hispanic veterans. For the White veterans, greater levels of past drinking were associated with greater perceived discrimination and less comfort in mainstream U.S. society. Based on the literature, implications for clinical treatment are presented. Clinicians are called upon to understand the worldview of their clients and engage in culturally sensitive alco...

This study explored a model of codependent attitude and behavior as moderators of the relationship between AOD problems in the family of origin (AODF) and offspring self-reports of psychological distress in a counterbalanced multiple-treatment experiment with a small heterogeneous sample of adult males and females. Three-directional hypotheses suggested by the literature were tested. Codependent attitude and behavior buffered Somatization, as predicted by the model, but exacerbated Anxiety,Hostility, and Paranoid-Ideation.Our findings suggest that codependent attitude and behavior may be adaptations that protect AODFoffspring from somatic distress, not the broadband expressions of offspring neuroticism reported in the literature. Owing to the small size and self-selection of our volunteer ...

We describe the Michigan Longitudinal Study (MLS), an etiologic study of family risk for alcoholism over the life course, and provide an overview of findings from the first 12 years of the study. Data suggest that etiology is anchored in family diathesis and shaped by a wide range of experiences that structure differential developmental pathways. One pathway is marked by strong continuity of behavioral dysregulation, evident during the preschool years and continuing through early adolescence. Two discontinuous pathways illustrate the shifting nature of risk and protective factors during early human development. Paternal co-morbid psychopathology, antisocial behavior, and alcoholism, play critical roles not only with respect to parent-child relationships, but also as determinants of family ...

This study compared a Motivational Interviewing inspired group intervention with standard care in naturalistic alcohol/other drug abuse samples. A community-recruited alcohol/other drug abuse or dependent sample (N = 67) was provided up to four sessions of groupadapted Motivational Interviewing (GAMI). Newly admitted patients (N = 64) in two multimodal treatment settings were also recruited. Quasi-experimental comparison of outcomes from GAMI versus realworld standard care (SC) indicated significant within-group improvement on all main outcomes in both groups. SC was associated with significantly more abstinence days, and better family and social outcomes at sixmonth posttreatment compared to GAMI. Differences in reductions in alcohol or drug-related dysfunction were not detected. Thus, gr...

This article provides the psychotherapist with both a practical knowledge about AA as well as an understanding of the tools to maintain the sobriety the patient has acquired in AA. Alcoholics in therapy are viewed as having co-existing disorders of the self and therapeutic strategies are suggested which address treatment of both the disorders of the self as well as the addiction. doi:10.1300/J020v24n03_04
Article: Alcoholics Anonymous: Novel Applications of Two TheoriesThe present paper offers two theories that help to explain why the 12-step program of Alcoholics Anonymous (AA) is such an effective intervention for recovering alcoholics, and provides directions for future research on AA. It is argued that Schlossberg's (1984) transition theory and Rando's (1995) theory of grief and mourni...

This study examines stress and quality of life as a function of time in recovery, and uses structural equation modeling to test the hypothesis that social supports, spirituality, religiousness, life meaning, and 12-step affiliation buffer stress toward enhanced life satisfaction. Recovering persons (N = 353) recruited in New York City were mostly inner-city ethnic minority members whose primary substance had been crack or heroin. Longer recovery time was significantly associated with lower stress and with higher quality of life. Findings supported the study hypothesis; the 'buffer' constructs accounted for 22% of the variance in life satisfaction. Implications for research and clinical practice are discussed.

This study investigated the influence of parental alcoholism on 593 freshman students at an urban, non-residential, southeastern university to determine how collegiate children of alcoholics (COAs), problem drinkers, and non-alcoholics (nonCOAs) differed in their perceptions of family health as indicated by the Family of Origin Scale (FOS). These three groups of offspring differed significantly in their perception of family health as measured by the scale's constructs of autonomy and intimacy and on various items within each construct. Children of alcoholics and problem drinkers scored lower on each construct and its related items, indicating that COAs perceived their families as less healthy when compared to the perceptions of children of non-alcoholics.

This study explores the association between race/ethnicity and alcohol use for a population of 27,117 Massachusetts injection drug users. This population had all entered the Massachusetts drug treatment system between 1996-2002. Through logistic regression methods, controlling for age, gender, education, employment and homelessness, the study identified that Puerto Rican IDUs were close to 40% less likely to have ever used alcohol compared to their African-American counterparts and 60% less likely to have ever used alcohol compared to their white counterparts. To further verify this finding a sensitivity analysis was conducted for a sub-sample (n = 469) of IDUs who were not in treatment and who were interviewed by trained interviewers rather than by administrative intake workers. This sens...

This study examined the relationship between treatment attrition, psychological distress, and interpersonal problems in 120 patients participating in an alcohol and drug intensive outpatient program. Results indicated a relationship between attrition from alcohol and drug treatment and psychological distress and interpersonal problems. Specifically, patients who did not complete treatment scored higher on both depression and anxiety than treatment completers. Further, two types of interpersonal problems, Domineering and Vindictive, were positively related to treatment attrition. These findings suggest that routine assessment and early identification of psychological distress and interpersonal problems may minimize attrition from alcohol and drug treatment by helping clinicians identify at ...

This study examined how parental drinking and family dysfunction are associated with sibling family role differences. Twentynine sibling pairs of college freshmen and their younger siblings participated. Differences on family roles of hero and lost child were significantly related to parental drinking and family dysfunction. Differences in codependency were significantly related to family dysfunction. Implications for dealing with children's family roles in families undergoing addictions treatment were discussed.

This study examined differential treatment attendance for alcohol abusing runaway youth residing at a local shelter. A traditional office-based family systems approach, Functional Family Therapy (FFT), was compared to a non-traditional, home-based, multisystemic family therapy approach, Ecologically Based Family Therapy (EBFT). As expected, treatment engagement and attendance was significantly higher for those assigned to EBFT (N = 37) compared to FFT (N = 40). Predictors of treatment attendance (income, family chaos, externalization problems and level of youth substance use) were examined within each treatment modality. Findings suggest that home-based (compared to office-based) treatment modalities may significantly increase treatment attendance and engagement of runaway youth and their ...

This study of a stratified random sample of 3,395 high school students from a statewide population was designed to investigate many elements of attachment theory, with a special focus on non-residential fathers. It examined several assets such as resilience, spirituality, and ego-identity, as well as many deficits such as sexual and physical abuses, depression, suicidal thoughts, and problems relating to biological father. Almost all study factors were significant predictors of alcohol use: Of particular note were statistical interactions between having a non-residential father and problems relating to him, attachment to him, caregiver monitoring, and spirituality. Clinical implications of these findings were discussed.

This study surveyed 90 patients entering treatment for alcohol problems and found that 54.4% at some time in their lives, had a life-changing spiritual or religious experience, compared to 39.1% in a large national survey. This study's sample also rated their spirituality higher than their religiousness, and higher than did the national sample. About a third, 32.2%, had no religious preference, compared to 13.8% in the national sample. Having had life-changing spiritual or religious experiences was associated with greater use of positive religious coping, some daily spiritual experiences, and higher self-ratings of one's self as spiritual and religious. Such experiences were not associated with patterns of alcohol use at treatment entry, demographic variables (sex, age, ethnicity, educatio...

Findings indicate that dually-diagnosed persons do engage in both types of fellowships; patterns of engagement differed across fellowships, suggesting different comfort levels. Both types of fellowships were used to deal with addiction. Greater difficulty with substance use at baseline was associated with greater likelihood of attending 12SG at followup; the reverse was true for self-reported substance use at baseline. Findings are discussed in light of ...

This study examines the role of religiosity in AA involvement and long-term sobriety in a representative sample of 587 men and women interviewed upon entering treatment and re-interviewed 1 and 3 years later. Religiosity is defined as spiritual, religious, secular (atheist or agnostic) and unsure, using the Religious Beliefs and Practices Scale employed in Project MATCH. Similar proportions within each religiosity group reported prior 12-month AA exposure at baseline; and over 40% of the unsure, spiritual and religious respondents and 25% of the secular respondents reported having gone to at least one AA meeting in the 12 months before the year 3 interview. Those who reported a spiritual awakening at year 3 were at the highest odds of continuous sobriety for the last year; notably, religio...

This study consisted of a stratified random sample of 3,550 adolescents selected from 55 public school districts from a statewide population in the Midwest. The purpose was to replicate a previous study that tested the delinquency syndrome construct by examining the amount of overlap between alcohol consumption, use of other drugs, and crime using log-linear analysis. The overlap in various forms of delinquency also was examined according to age, gender, and race, with the hypotheses that these factors interact with the amount of overlap between the behaviors examined. In agreement with the previous study, the results support the delinquency syndrome, but they indicate less overlap than the delinquency syndrome construct suggests. Moreover, overlap between these forms of delinquency is mod...

This study explores methods of improving the treatment of alcoholism among Native American people by integrating culturally relevant practices into treatment methodology. Primary to the findings of this study are the opinions and experiences of four Lakota people living on the Rosebud Reservation in South Dakota, individuals who have worked with alcoholism in their community for a number of years. Also important is an understanding of the history of alcoholism among Native American people, including forms of cultural breakdown that lead to increased levels of alcohol problems. Findings suggest that alcoholism treatment programs that integrate Native American culture and traditions into the recovery process will be more likely to be accepted by Native American alcoholics than mainstream reh...

This study evaluated service utilization, including medical visits, psychological, alcohol and drug counseling and 12-step attendance, and substance use. Runaway youths (n=51) between the ages of 12 and 17 with a diagnosis of alcohol abuse or dependence were recruited from two southwestern runaway shelters. Results showed that psychological counseling and medical visits were associated with less alcohol use while 12-step attendance was associated with more alcohol use at pretreatment. Service utilization was not associated with illicit drug use. The findings suggest that alcohol and other drug use are affected differently by service utilization in this sample of youths. More research is required to evaluate and develop treatments which can effectively intervene with this high-risk group.

This study compares the academic performance of two distinct types of students enrolled over a five year period in a Sociology class entitled ?Alcohol, Drugs, and Society.? Roughly 30% of enrollees take this course as part of their training to become certified addictions counselors through the College of Human Sciences. The remainder come from a wide range of other courses of study, largely through the College of Arts and Sciences. These majors include, but are not limited to, sociology, social work, psychology, history, political science, and journalism. Data reveal that the difference in average grades between the pre-professionals and all others is not statistically significant. (Source: Alcoholism Treatment Quarterly)

This article seeks to stir interest in lost chapters of the history of alcoholism recovery in America by cataloguing and comparing pre-A.A. alcoholic mutual aid societies with A.A. and post-A.A. mutual aid groups, and by describing those characteristics that pre-A.A. alcoholic mutual aid societies shared in common. In addition, it will identify the factors that led to the demise of pre-A.A. alcoholic mutual aid societies, discuss what the collective histories of these groups reveal about the sources of A.A.'s resilience and the potential fate of post-A.A. alcoholic mutual aid societies, and provide valuable insights into the process of recovery.

This study examined 12-step group preference in relation to demographic, psychological, or substance-related variables among low-income persons who identified alcohol as their drug of choice on admission to a substance abuse treatment program. Results indicated that African-Americans and respondents with both an alcohol and illicit drug problem did not prefer Alcoholics Anonymous over Narcotics Anonymous. These findings can guide clinicians in making referrals to 12-step groups.

This study examined participation in 12-step programs and attitudes toward 12-step meetings in an outpatient sample of 81 severely mentally ill patients with comorbid substance use disorders. The study found that dual diagnosis patients attended 12-step programs at rates comparable to those reported for patients in primary addiction treatment settings. Diagnosis and attitudes toward 12-step meetings each had independent effects on 12-step participation. The difficulties that some dual diagnosis patients report experiencing at 12-step meetings may need to be addressed to maximize 12-step attendance and potential to benefit from 12-step programs.
