CONCLUSION: Nebivolol therapy reduced the effects of VR in rats after MI. These beneficial effects were not related to its heart rate and blood pressure reducing effects. Nitric oxide regulatory action of this compound may contribute these beneficial effects on VR developed after MI.

CONCLUSIONS: Patients with atrial fibrillation were mainly with ischemic, hypertensive heart disease and congestive heart failure. Our study, suggests underuse of anticoagulation therapy. The independent predictors of under prescription of anticoagulants in patients with atrial fibrillation were elder age, non-enlarged left atrium, and good left ventricular ejection fraction. Medical treatment with other groups of drugs for atrial fibrillation and comorbidities seems to be according to current guidelines.

Authors: Turgut Celen Y, Peker Y
Obstructive sleep apnea (OSA) is common in general population. There is an accumulating research evidence for an independent relationship between OSA and cardiovascular morbidity and mortality. This relationship is stronger in clinical cohorts compared with the general population, which suggests that concomitant OSA in subjects with traditionally recognized risk factors such as obesity, hypertension, smoking, and hyperlipidemia may provide an additive risk factor for the cardiovascular consequences. In the current article, the clinic-and population-based epidemiologic data will be reviewed in this context.

Several echocardiographic methods have been proposed to assist patient selection for cardiac resynchronization therapy. Color-coded tissue Doppler is one of the most promising methods to quantify mechanical dyssynchrony. However, tissue Doppler data are affected by Doppler angle of incidence and tethering or translational motion. Furthermore tissue Doppler based modalities are good for longitudinal motion analysis but limited in other directions of wall motion such as radial, circumferential, and rotational. Speckle tracking is a more recent technique that allows accurate calculation of regional radial and circumferential strain as well as regional rotation for dyssynchrony analysis. Although no ideal echocardiographic method exists that integrates regional dyssynchron...

Authors: Delaney J, Mittal S, Sherrid MV
Congenital long QT syndrome is a genetic disorder characterized by prolonged QT interval on electrocardiogram and increased risk of sudden cardiac death from ventricular arrhythmias. In long QT syndrome, genes that encode for the various cardiac ion channels or regulatory proteins of these channels are mutated. The various mutations individually lead to a disruption of the normal cardiac myocyte action potential, and thus leading to a propensity for ventricular arrhythmias. Diagnosis can be difficult with patient presentations ranging from palpitations to syncope to sudden cardiac death. The QT interval can also vary over time, often requiring further testing to support the diagnosis. Recently developed genetic testing can be used to identify th...

Authors: Khan A, Mittal S, Sherrid MV
Brugada syndrome is a genetic cause of sudden cardiac arrest characterized by abnormal electrocardiographic (ECG) pattern in the right precordial leads either at rest or after provocation. In this condition, sudden death may occur due to polymorphic ventricular tachycardia or ventricular fibrillation. In approximately 30% of patients, sudden cardiac arrest is the initial clinical manifestation of Brugada syndrome. Treatment strategies for Brugada syndrome are evolving. Currently, the implanted cardioverter defibrillator (ICD) is the only proven treatment for Brugada syndrome. Candidates for ICD include patients include those with the type 1 ECG pattern or who have been successfully resuscitated from sudden death or have had unexplained syncope.

Authors: Wever-Pinzon OE, Myerson M, Sherrid MV
Sudden cardiac death (SCD) in young athletes is generally caused by inherited cardiac disorders. While these events are relatively few compared to other cardiac deaths, they are tragic in that death occurs in a young, otherwise healthy person. The genetic abnormalities most associated with SCD are hypertrophic cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy, long QT syndrome, Brugada syndrome, and catecholaminergic polymorphic ventricular tachycardia. As a result of growing awareness that these deaths can be prevented, guidelines have been issued in both Europe and the United States to help screen and determine qualification for young persons who want to participate in competitive athletics. There remains debate on the how...

Arrhythmogenic right ventricular dysplasia (ARVD), also known as arrhythmogenic right ventricular cardiomyopathy, is a genetic cause for sudden cardiac arrest. In ARVD, there is progressive replacement of normal myocytes, with fat and fibrous tissue, predominantly in the right ventricle that predisposes the individual to arrhythmias. Patients who are identified with this condition are risk stratified; those at high risk are recommended to have implanted cardioverter defibrillators.

The implantable cardioverter-defibrillator (ICD) therapy is an established intervention for the prevention of sudden cardiac death (SCD) in patients with significant left ventricular dysfunction. Multiple randomized clinical trials have studied the use of ICD for the primary and secondary SCD. These studies were performed in patients with left ventricular dysfunction from coronary artery disease or dilated cardiomyopathy, and the marker of reduced ejection fraction has emerged for selecting patients who would benefit from ICD therapy. Currently, for most of these patients the decision to implant, or not, is determined by relatively straightforward paradigms. The same cannot be said for the genetic cardiac diseases associated with SCD--long QT sy...

Acute heart failure syndrome is a heterogeneous group, which requires distinct therapeutic approaches. Diuretics and/or vasodilators for patients with high blood pressure and inotropic agents for patients with low left ventricular ejection fraction and evidence of hypoperfusion are kept in the forefront. Early initiation of therapy along with accurate and early diagnosis in acute heart failure reduces mortality and morbidity significantly. Therefore, it is critically important to develop algorithms, which could easily be followed by all physicians. In this paper, in parallelism with heart failure guidelines and utilizing data of the clinical studies, performed by colleagues of our country in this area...

CONCLUSION: The results of this study confirm low hypertension risk among adolescents in Manisa, which may be attributed to the low prevalence of "overweight". The relatively low cardiovascular disease risk factor profile of these adolescents needs to be encouraged through adulthood. Thus, a school program of health promotion should be established to prevent the epidemics of cardiovascular diseases in our region.

CONCLUSION: Post-exercise first minute impaired HRR is associated with right ventricular systolic dysfunction in obese patients. Both HRR and right ventricular systolic function correlate well with the exercise distance and METs. Obese patients with impaired HRR should be evaluated with echocardiography to assess their right ventricular systolic function.

CONCLUSIONS: Sinoatrial node artery originating from distal RCA is very rare. This variation, even in patients with severe RCA disease is not associated with severe arrhythmia. Dual blood supply may be a protective factor in this subgroup of patients from arrhythmic events. To be aware of the origin and course of variant SAN artery may provide safe approach to interventional cardiologist and cardiac surgeon during percutaneous and surgical coronary and atrial interventions.

CONCLUSION: Acute kidney injury necessitating RRT makes a small percentage of patients undergoing cardiac surgery and if RRT is not required the survival is excellent. Continuous infusion of furosemide seems to be effective in promoting diuresis and decreasing the need for RRT. However further multicenter studies with different doses of furosemide are required to confirm these results.

The renin-angiotensin system (RAS) can play an important role as the underlying mechanism for the development and recurrence of atrial fibrillation (AF). In addition, AF itself can lead to atrial structural and/or electrophysiologic remodeling by activating the RAS. This remodeling serves as an arrhythmogenic substrate for AF. Thus, the inhibition of RAS with angiotensin converting-enzyme inhibitor or angiotensin receptor blockers may be a strategy to prevent occurrence and recurrence of AF in certain patients such as those with heart failure and hypertension in associated with left ventricular hypertrophy. In addition, it may facilitate the cardioversion of AF. In this review, we overview the effect of RAS blockers on cardioversion success and recu...

CONCLUSION: Circulating endothelial function parameters (plasma ADMA, L-arginine, NOx levels) were impaired in patients with CSX. Nebivolol treatment was associated with better improvements in both circulating endothelial function and exercise stress test parameters than metoprolol. We believe that further studies are needed to evaluate the effects of nebivolol treatment on long-term clinical outcomes in patients with CSX.

CONCLUSION: Both gabapentin and diclofenac are effective in the treatment of chronic PCPP, without obvious side effects. However, gabapentin is found to be superior to diclofenac and its effects sustain longer. The results show that there may be some evidence in PCPP as a kind of neuropathic pain.

CONCLUSION: Most children with CP were older than 12 years old. CDs were frequent and about half of them were thought to directly cause pain. So, further investigations according to standard algorithms are needed in the evaluation of children with CP. Other important result is the increased risk of CD found in elder children.

Atherosclerosis is a chronic inflammatory disease, which selectively involves the arteries in the vascular system. Atherosclerosis develops because of reactions occurring in vessel wall beginning with response to endothelial injury. Endothelial dysfunction is characterized with impairment and loss of monolayer cells covering the inside of the vessels, which is endothelium. Endothelial dysfunction is the first stage in atherosclerosis. Coronary angiogenesis and collateral growth are chronic adaptations to myocardial ischemia to restore coronary blood flow and salvage myocardium in the ischemic region. Nitric oxide (NO) which represents the status of endothelial health plays a major role in collateral vessel development. Asymmetric dimethylarginine (ADMA) which is end...

CONCLUSION: ESR1 PvuII polymorphism is not associated with angiographically significant CAD. ESR1 BtgI polymorphism is strongly associated with the presence of normal coronary angiograms in women, which suggests protective effect. Further confirmation of these findings is required.

CONCLUSION: Admission levels of hs-CRP were emerged as the most important parameter for prognosis and the cut-off value of hs-CRP for predicting NCE was found as 1.1 mg/dl in this cohort of the study population. Further studies are required to confirm the most risky cut off value of hs-CRP for predicting long term prognosis among ACS patients and in general population.

CONCLUSION: Coronary slow flow is associated with prolonged P wave duration and dispersion and impaired diastolic filling. Nebivolol may be helpful in restoration of these findings. P wave duration and dispersion may not be associated with left ventricular function parameters in patients with CSF.

CONCLUSION: P wave duration and PWD are found to be prolonged in hyperthyroid patients with GD. Both propylthiouracil and methimazole reduce the P wave duration and dispersion. Thus, we can conclude that improvements in atrial conduction properties are not associated with the type of ATD but with only achievement of euthyroidism.

CONCLUSION: According to our study, we cannot demonstrate any benefit in reconstructing the whole aortic root for discrete SAS. A single-patch technique is easy, safe and appears durable.

Authors: Okmen AS, Okmen E
Since the drug eluting stents have provided better results concerning restenosis and repeat revascularization comparing to bare metal stents, percutaneous coronary interventions are performing increasingly in more complex stenoses. However, drug eluting stents did not show any advantage on prevention of myocardial infarction and survival. Currently there is no any completed controlled randomized study showing the superiority or equality of drug eluting stents over coronary bypass surgery by means of efficacy and safety. While the long-term results of drug eluting stents still unclear, everyday progressively more patients with multivessel disease are undergoing percutaneous coronary intervention with drug eluting stent implantation.
Authors: Naumov LB
Diagnostic interpretation at auscultation of heart murmurs is accompanied by frequent errors. It creates serious clinical, pedagogical, organizational and social problems. The standard nosological principle of a clinical information description from the diagnosis (a disease name) to the description of symptoms/signs contradicts to real clinical practice from revealing of symptoms through differential diagnostics to a diagnosis establishment. The differential diagnostic algorithm or diagnostic algorithm developed by the author, is based on the opposite syndromic principle of thinking - from the signs to the diagnosis. It completely corresponds to the practical purposes of reliable diagnostics of 35 illnesses, manifested by heart murmurs at a heart auscultation.

Functional mitral regurgitation (FMR) is the mitral regurgitation that occurs due to myocardial disease in the presence of normal mitral valve leaflets. This scenario has the different characteristics than the organic mitral regurgitation. Functional mitral regurgitation is a disease of the ventricle and occurs by the deformation of the mitral valve leaflets. This morbid entity is frequent and has bad prognosis. Functional mitral regurgitation is the result of complex pathophysiologic process including left ventricular local and global remodeling, mitral annular and papillary muscle dysfunction, and left ventricular dysfunction. The dynamic behavior of FMR complicates the quantification of the regurgitation. Exercise stress echocardiography is of particular importance ...

Nowadays, age-related degenerative etiologies have largely replaced the rheumatic ones and as a natural result of this etiologic change, coronary artery disease has become associated with valvular heart disease to a greater extent. Degenerative aortic valve disease has an important pathophysiological similarity to atherosclerosis and is the leader in this association. There is a general consensus that severely stenotic aortic valve should be replaced during bypass surgery for severe coronary artery disease. For moderate degree aortic stenosis, aortic valve replacement is usually performed during coronary bypass surgery. Ischemic mitral regurgitation has recently received great attention from both diagnostic and therapeutic points of view. Ischemic mitral regurgitati...

Authors: Uluçam M
Prosthetic valve replacement replaces a heart disease with another one. There is no ideal prosthetic valve. All prostheses need to be followed by caution and treatment depends on the type of the prosthesis. Mechanical prostheses add anticoagulant drugs to patient's life and degeneration of bioprostheses requires reoperation. Prosthesis dysfunction includes a group of pathological processes like thrombosis, thromboemboli, valvular or paravalvular regurgitation, patient-prosthesis mismatch, infective endocarditis and bioprosthesis degeneration. Correct diagnosis and treatment of these problems will decrease the mortality and morbidity due to prosthetic valve disease.

The tricuspid valve is the right-sided atrioventricular valve, and it is composed of three leaflets (i.e., anterior, posterior, and septal) attached to a fibrous annulus. The cause of tricuspid regurgitation (TR) is more often functional rather than due to a primary valve lesion. Functional TR is due to annular dilatation and secondary to mitral and aortic valvular diseases. Presence of the TR is associated with poor early and late prognosis. Patients with TR have the presenting symptoms of fatigue and weakness related to a reduction in cardiac output. Echocardiography is the best diagnostic method to evaluate TR and associated abnormalities. Tricuspid regurgitation is generally well tolerated. However, in the presence of pulmonary hypertension cardiac output ...

Authors: Aydin M, Cetiner MA
Valvular heart diseases still continue to be an important health problem. Surgical replacement of cardiac valves keeps a widely used treatment method for the present. However, the efficiency of minimal invasive and percutaneous methods targeted to repair and replacement of the diseased valves has been searched for nowadays. The first clinical experiences and early stage outcomes on the applicability of these methods are encouraging. Nevertheless, it should be kept in mind that percutaneous valvular interventions are at their development stages. Long term confidence and efficiency studies of these treatment modalities are needed. The present review emphasizes the studies on percutaneous techniques initiated in the treatment of valvular heart diseases.

CONCLUSION: Conventional training for heart auscultation is ineffective. Auscultation skills acquired by medical students do not grow up during their program of study. Even graduated cardiologists and instructors clinicians themselves make frequent errors in diagnostics of heart murmurs. The proposed diagnostic algorithm decreases the number of errors many times. The programmed teaching with constant feedback is the optimal method for flawless recognition of acoustic signs.

Platelets play a critical role in pathogenesis of atherothrombotic diseases such as acute coronary syndromes and ischemic stroke. Clopidogrel, a thienopyridine derivative is an effective antiplatelet drug mostly used in combination with aspirin or as a single drug in aspirin intolerant patients. However, despite its proven efficacy in various clinical trials, some patients exhibit impaired response to clopidogrel and have activated platelets while on usual clopidogrel treatment. Although definition and mechanism(s) of this therapeutic failure are poorly understood, it is associated with higher morbidity and mortality as in aspirin resistance. Various causes have been implicated in clopidogrel resistance and alternative therapies are recomm...

Type 2 diabetes mellitus (DM) is a common disease affecting people in the world and its incidence is increasing rapidly. Cardiovascular disease (CVD) is the most important cause of mortality and morbidity among patients with type 2 DM. Patients with diabetes have a poorer prognosis than patients without diabetes. It is well known that other cardiovascular risk factors such as hyperlipidemia, hypertension and obesity that usually accompany to diabetes mellitus or impaired glucose tolerance as a metabolic syndrome component or not, can contribute to accelerated atherosclerosis and increased risk of cardiovascular event in diabetes mellitus. The management of patient with type 2 DM is not only glucocentric but focuse...
