Seroprevalence and risk factors of hepatitis a virus infection in iran: a population based study.

CONCLUSION: The seroprevalence of hepatitis A virus still appears to be too elevated for recommending routine vaccination in the general population. However, the trend towards a lower prevalence in younger age groups and people from urban areas points towards the possible benefit of vaccination in these subgroups.

CONCLUSION: Our findings support the suggestion that a substantial difference between Turkmens and non-Turkmens in terms of genetic susceptibility to ESCC is unlikely. Nevertheless, the moderate effect of genetic factors cannot be ruled out. Further studies to investigate potential environmental and genetic risk factors of ESCC in Golestan and the interaction between environmental and genetic factors are warranted.

CONCLUSION: The results indicate which 5HT3 and 5HT4 receptors are significantly expressed in both colon cancer tissue and the HT29 cell line. Expression for the 5HT3 receptor is more potent. Furthermore, 5HT plays a mitogenic role in colon cancer cells and antagonists of 5HT3, and 5HT4 receptors can inhibit cancer cell growth.

Iran as a developing nation is in epidemiological transition from communicable to non-communicable diseases. Although, cancer is the third cause of death in Iran, it;s mortality are on the rise during recent decades. This mini-review was carried out to provide a general viewpoint on common cancers incidence in Iran and to explain incidental differences that may help us to establish early detection programs and investigate population risk factors. A detailed PubMed, Scopus and Google scholar search were made from 2000 to 2009. The basic inclusion criteria were all relevant studies focused on cancer epidemiological data from Iran. Overall age-standard incidence rate per 100 000 population according to primary site is 110.43 in male...

Asymptomatic hepatitis B infection is characterized as a type of hepatitis in which hepatitis B surface antigen is present in the patient's peripheral blood despite the absence of clinical symptoms. Previous studies have shown that a particular genotype may effect clinical manifestations of hepatitis B infection; hence, the aim of the current study was to determine the frequency of hepatitis B virus genotypes among asymptomatic carriers of hepatitis B.In this experimental study, the plasma samples of 100 asymptomatic carriers were collected and tested for HBsAg and anti-HBs using ELISA. The genotype of hepatitis B virus was determined by the GAP-PCR technique. The results of this study showed that all samples were ...

Foreign body aspiration can be a life threatening event especially for young children with smaller diameters of airway size. The foreign body can result in body response and granulation tissue formation around the object which makes the foreign body removal difficult. In such situations surgical intervention is usually needed but with interventional pulmonology modalities we can restrict the need of surgery.

During 1988 - 1994 a total of 38 cases of human anthrax were admitted to Sina Hospital in Kermanshah (western Iran). There were two cases of gastrointestinal anthrax (5.3%) with culture positive ascitic fluid. Among the many reported gastrointestinal signs and symptoms, unexpectedly one of our patients had only vomiting and ascites whereas the other case had only ascites. Neither had abdominal pain, tenderness, diarrhea, hematemesis, melena, or other expected signs and symptoms of anthrax. Therefore, in contrast to the available reports, these cases presented atypically and despite receiving a sufficient dose of penicillin, the drug of choice at that time, both patients died. Gastrointestinal anthrax is not as rare as reported but due to an...

CONCLUSION: According to our results, it seems that more attention from researchers and health-policy makers towards trauma is necessary in our country. In addition, publishing a journal with the main theme of injury and trauma could be an effective means for increasing the proportion of trauma related articles.

CONCLUSION: Results indicate that arsenic trioxide is a potent inducer of apoptosis in NB4 cells and vitamin D3 significantly decreased the sensitivity of cells to the induction of apoptosis by arsenic trioxide. These findings suggest that 1,25-(OH)2D3 might be involved in anti-apoptotic processes via reactive oxygen species scavenging or other mechanisms not yet known.

This report describes a rare case of secondary carpal tunnel syndrome due to a lipofibromatous hamartoma of the median nerve. Excision of the fibrofatty tissue between the nerve fascicles without risking damage to the fascicles was impossible. The transverse carpal ligament was incised and an epineurotomy was performed. Within six months, the 25-year-old female patient's symptoms were much improved.

Heck's disease or focal epithelial hyperplasia is a benign contagious disease caused by human papillomavirus types 13 or 32. It occurs with low frequency in the Iranian population. This condition is characterized by the occurrence of multiple, small papules or nodules in the oral cavity, especially on the labial and buccal mucosa and tongue. In some populations, up to 39% of children are affected. Conservative surgical excision of lesions may be performed for diagnostic or aesthetic purposes. The risk of recurrence after this therapy is minimal, and there seems to be no malignant transformation potential. In the present work, we presented the clinical case of a 12-year-old Iranian girl with oral lesions that clinic...

We report a 42-year-old man who presented with obstructive jaundice by an impacted shrapnel splinter in the common bile duct (CBD) 23 years after a combat injury. To our knowledge, this is the first case report of this type in Iranian literature, although there are a few reports of combat related injuries in other countries. This diagnosis should be considered in the assessment of any patient with jaundice and abdominal pain who has experienced a previous combat injury.

Authors: Nayernouri T
This is the first of two articles reviewing the history of medical symbols. In this first article I have briefly reviewed the evolution of the Greek god, Asclepius, (and his Roman counterpart Aesculapius) with the single serpent entwined around a wooden rod as a symbol of western medicine and have alluded to the misplaced adoption of the Caduceus of the Greek god Hermes (and his Roman counterpart Mercury) with its double entwined serpents as an alternative symbol. In the second part of this article (to be published later), I have made a tentative suggestion of why the Simorgh might be adopted as an Eastern or an Asian symbol for medicine.

Smallpox, which currently is only of historical interest, was once one of the most terrible illnesses with high mortality and morbidity. In the late 18th century, the English physician and naturalist, Edward Jenner (1749 - 1823), discovered an efficient preventive technique against smallpox which he termed "vaccination". Afterwards, the practice of vaccination gradually became widespread when finally in 1979, the World Health Organization formally declared the global eradication of this fatal disease.Presented here is a brief account of smallpox eradication in Iran which started on a limited scale in the 19th century by the order of Abbas Mirza (1789 - 1833), the Crown Prince of Fath Ali Shah Qajar (reign from 1797 - 1834), and reinforced in 1848 by Mirza Taghi Khan A...

CONCLUSION: Since the number of S and VS cases would be crowded over a short period of time, the health care system most probably would not be able to provide appropriate services unless special measures are taken in advance. By reduction of R0 and the number of introduced infectious cases the peak of the outbreak might be postponed to the end of 2010. This would provide a golden opportunity to vaccinate a considerable proportion of the population.

CONCLUSION: It would be concluded that opium addicts have an elevated BLL compared to healthy controls. Therefore, screening of blood lead concentration is helpful for opium addicted people especially with non-specific symptoms. In this regard, a similar investigation with a larger sample size of opium addicted patients (including both oral and inhaled) and a control group is suggested to confirm the findings of this research.

Although the global incidence of gastric cancer has been decreased dramatically in recent decades, it is the most common cancer in north and northwest Iran. The wide variation in incidence across different geographical areas and higher proportion of cardia cancer are two main characteristics of gastric cancer in Iran. Current investigations indicate that a high prevalence of H.pylori infection, high dietary intake of salt and smoking are the main environmental factors of gastric cancer in Iran. Gastroesophageal reflux disease is another contributing factor in populations with a higher incidence of gastric cardia cancer. While interventions on modifiable environmental risk factors should be considered as the main modality to reduce gast...

The germline mutations of breast cancer susceptibility genes 1 (BRCA1) and breast cancer susceptibility genes 1 (BRCA2) have been associated with a significant increase in breast cancer risk and certain other cancers. Among the most known mutations in these tumor suppressor genes are 5382insC and 185delAG in BRCA1 and 6174delT in BRCA2. The aim of the current study was to investigate the frequency of these BRCA1 and BRCA2 mutations in southern Iranian familial and sporadic cases with breast cancer. Two hundred fifty women with sporadic breast cancer, 55 women with a familial history of breast cancer in their first degree-relatives and 200 healthy women formed the studied groups. Mitochondrial neurogastrointestinal encephalomyopathy is an autosomal recessive disorder in which a nuclear mutation of the thymidine phosphorylase gene leads to mitochondrial genomic dysfunction. Herein, we report a 29-year-old Iranian man with abdominal pain, diarrhea, hearing loss, ophthalmoplegia, sensorimotor axonal neuropathy, and elevated muscle enzymes. Magnetic resonance imaging showed leukoencephalopathic changes. Metabolite analysis revealed a very high thymidine concentration in the patient's urine consistent with the diagnosis of mitochondrial neurogastrointestinal encephalomyopathy.

This report describes a 23-year-old man who presented with multiple joint deformities as a consequence of multiple painless intra-articular fractures.Blood counts, biochemistry, and nerve conduction velocity were all normal. X-ray studies showed joint destruction in hips, elbows and knees. We concluded that he is a case of congenital insensitivity to pain culminating in multiple charcot joints.

Historically, stratification in medical profession has followed certain patterns pertinent to the nature of the work and the organ of interest. Evolution of radiology was, however, different insofar as it followed an accidental discovery toward the end of the 19th century. The physical properties of the discovered rays were to set the initial direction for future expansion of the discipline.

CONCLUSION: Our results revealed the advantage of water birth delivery. Those who gave water birth experienced less pain and completed the delivery sooner. Meanwhile, normal vaginal delivery was accomplished more frequently with this group. These all lead to a decreased necessity for medical interventions as well as an increased socioeconomic advantage for the society.

Many families are directly involved in caring of their children with the lifelong autistic spectrum disorders. Sometimes, they are the victims of their children's behaviors. The family, including siblings, may have to explain the children's behaviors to others. Their mothers have poorer mental health with difficulties in family adaptability and cohesion. Its burden is not just limited to psychologic aspects but on the quality of life, economic aspects, and parent-child relationship. This is a narrative review of some of the aspects that families of children with autistic spectrum disorders are facing.

The study was conducted alongside the "Iran's National Noncommunicable Diseases Risk Factor Surveillance survey" of 2005; a cross-sectional study with a multistage cluster sampling approach which targeted Iranians older than 15 years of age. Only 6.4% of the respondents had a HbA1c test during the year before questioning and 25.7% reported to have undertaken a lipid profile examination in the same period of time. Positive responses for eye and foot examinations in the year leading to survey were 39.8% and 20.5%, respectively. About 97.4% of the patients had poor control according to their most recent HbA1c levels. About 25% needed an improvement in diabetes control...

Nonmissile low-velocity penetrating brain injuries are unusual among civilian population. Work-related penetrating head wounds are rarer among this group. Here, we report two rare cases of penetrating head wounds caused by industrial grinder tool. The patients (both men) were struck to the head by detached blades of the grinder tool. Depressed fractures, penetrated bone fragments, and dural lacerations were all treated by the routine fashion. Paranasal sinus damage and further cerebrospinal fluid leakage were also treated conservatively. The reported cases are unexpected variants of injuries that neurosurgeons may come across and their management is still beyond the scope of textbooks. Skill and experience of the general neurosurgeons can handle the ca...

Authors: Sanaat Z, Mohammady G, Esmaili H, Emrani M, Dolatkhah R
Malignant mesenchymal tumors of the larynx constitute 0.3% to 1% of all laryngeal cancers. Of them, osteosarcoma is the rarest. Osteosarcoma of the larynx may be a difficult clinical diagnosis. Pathologic confirmation of osteoid is required for the diagnosis. Treatment is primarily surgical. It is believed that aggressive surgical intervention directed at complete tumor extirpation is the treatment of choice. Outcomes of such patients are generally poor. Most patients die because of pulmonary metastasis. A case is reported here and a review of all available published cases of osteosarcoma of the larynx is presented.

Although prostate cancer is among the most frequent malignancies in the elderly, this tumor may be under-reported, and it seems that its socioeconomic burden is not well-estimated. Chronic urinary obstruction caused by the cancer may cause renal failure, with hemorrhagic tendency, neurologic disturbances, cutaneous disorders, and diverse fingernail changes. Black or brown nail pigmentation has been associated with benign and malignant conditions, including antineoplastic drugs' side effects, subungual metastases, and melanoma. Metastasis of fingernail melanoma can affect lymph nodes from the wrist to elbow and axillary regions. Coexistent melanonychia and wrist lump, mimicking melano...

Fetus in fetu is an extremely rare condition in which a fetus or fetus-like structure with a vertebral axis is seen in the body of its twin. This paper presents a case of fetus in fetu in a two- day-old female newborn who was referred for an abdominal mass, biliary vomiting, and feeding intolerance. After plain abdominal X-ray and ultrasonography, the patient underwent abdominal surgery with the primary diagnosis of teratoma or fetus in fetu. We found a retroperitoneal mass that consisted of double fetus in fetu and a separate undetermined mass. The pathologic examination confirmed double fetus in fetu and revealed a separate immature teratoma. She was discharged from the hospital after s...

Following the establishment of Gastroenterology and Hepatology Fellowship Programs in 1987, significant developments in research and health care delivery have been achieved. The number of published articles has increased significantly and now more than 10 approved research centers are involved in several longitudinal and population based studies in GI epidemiology, viral hepatitis and GI oncology around the country. Before 1987 less than 50 gastroenterologists were working in the country, but now more than 300 gastroenterologists are involved in public and private health care delivery systems. Advanced diagnostic and therapeutic endoscopic procedures and complex surgical procedures such as liver transplantation are a routine now. These achieve...

CONCLUSION: Problems encountered in the aftermath of the Bam earthquake were related to the lack of prepared action plan and data management system. Here, we present a specifically designed earthquake chart. By following the chart, rescue paramedic personnel and emergency medical teams will be able to recognize high-risk victims, in order to provide timely medical management.

CONCLUSION: Compared with the results sited in Western studies, the prevalence of abdominal aortic aneurysm is lower in Iran (P<0.0001). Its associated factors in our study were similar to previously published studies. Significant carotid stenosis, a factor not reported previously, was an associated factor in our study. Because of the low prevalence of abdominal aortic aneurysm in our population, screening the candidates for coronary artery bypass graft surgery is not recommended.

Cancer-testis antigens are tumor antigens that their expression is almost limited to male germ cells in the testis. Some of cancer-testis antigens are also expressed in the ovary and in trophoblasts. Recently their expression has been seen in different types of tumors. Many pathophysiologic studies suggest that a blood-testis barrier exists in the testis. Because spermatogenesis begins at puberty, new cell-surface antigens are expressed when the immune system has refined the ability to distinguish self from nonself. So, sperms in the testis do not stimulate immune responses. In addition, although antigen-presenting cells are commonly seen in the interstitial spaces of the testis, these cells are scarcely seen within the seminiferous tubules. So, ...

Marjolin's ulcer is defined as a malignant, ulcerating neoplasm occurring in cicatricial tissues. The cancer is usually a well-differentiated squamous cell carcinoma. Wide resection is complicated with a recurrence rate of 20% to 50% and a metastasis rate of 54%. Therefore, we chose amputation as the modality of treatment for heel Marjolin's ulcers in Urmia and presented their results in this study.During the last 10 years in Urmia, nineteen cases of heel Marjolin's ulcers has been detected. About 47% were due to childhood burn. Malignancy was mainly squamous cell carcinoma. The mean latent period of malignant transformation was 11 years. All cases were treated with amputation, without any recurrence or metastasis in an average four-year follow-up period. The squamous ...

This study was performed to reveal the final outcome of our patients and find out the possible risk factors of poor result. The case records of 243 children who were admitted with the diagnosis of septic arthritis in Imam Khomeini and Bu Ali Sina Hospitals, Mazandaran Province, were studied between 1996 and 2005. The diagnosis was based on clinical and ultrasound findings in all patients and positive smear in 67% of them. Among these patients, we had access to 162 cases who had definitely septic arthritis and went through surgical interventions because of the involvement of the hip joint or uncertain response to medical treatments. As four out of six poor outcome cases were related to hip sepsis, hip was the main site of involvement and complications.Six cases of severe complications out o...

Authors: Alamsamimi M, Mirkheshti N, Mohajery MR, Abdollahi M
A 51-year-old woman with neurofibromatosis type 1 presented to our department for investigation of a left breast lump (50x50x30 mm); a mass in the right breast (40x40x20 mm) was also detected on physical examination. The lumps were suspected to be malignant based on physical examination and ultrasonography. Biopsy and frozen sections subsequently confirmed a diagnosis of bilateral invasive ductal carcinoma. A standard bilateral radical mastectomy was performed, followed by postoperative chemoendocrine therapy. Tumor recurrence has not been observed within the first 23 months following the surgery.

This report presents a palmaris profundus tendon anomaly in the carpal tunnel. The case was a 19-year-old female who underwent carpal tunnel surgery. After dividing the transverse carpal ligament, the palmaris profundus tendon was discovered. The abnormal tendon was within the carpal tunnel, laying over the investing tissue of the median nerve, and inserted deeply into the palmar aponeurosis. A segment of the abnormal tendon was excised and the patient's symptoms resolved completely.

During the first half of the 20th century, all subspecialties of internal medicine were managed by general internists and there was no gastroenterologist in Iran.1 The first group of Iranian gastroenterologists who had completed their training program in France started working in Tehran in 1951. Later on, other specialists trained in USA and the United Kingdom joined them. Some of these specialists and some internists with special interest in gastroenterology and hepatology were in charge of gastroenterology education in divisions of internal medicine in the Medical Schools of Tehran, Shiraz, Isfahan, and other universities, while most of them were also practicing gastroenterology in their private offices on a part-time basis.
CONCLUSION: The prevalence of post-disaster post-traumatic stress disorder was high in children. Younger age, female gender, living apart from family, geographic living conditions, being in the center of stress or not after the earthquake, body injury, and loss of family members form the basis for developing post-disaster psychopathology. Due to incomplete presence of post-traumatic stress disorder criteria or high rate of overlooking post-traumatic stress disorder in children, the disorder may be misdiagnosed or remain undiagnosed in them, which warrants more consideration.

CONCLUSION: None of the currently available indexing sites are capable of presenting Iranian research to the international scientific community. The government should intervene by enforcing policies designed to facilitate indexing through a systematic approach. The policies should address Iranian journals, authors, and indexing sites. Iranian journals should be required to provide their indexing data, including references, electronically; authors should provide correct indexing information to journals; and indexing sites should improve their software to meet standards set by the government.

CONCLUSION: Oligoarthritis was the most common type of joint involvement in these Iranian patients. We found no correlation between distal interphalangeal joint involvement and nail changes, although it was seen with more erosive changes and joint deformity. In comparison with other studies, extra-articular manifestations were not common in our patients.

Recent studies have made it clear that Iran has too many road traffic injuries (RTI). This paper explains the reasons for high traffic injury rates and points to solutions. It also provides detailed data on the burden of traffic injury in Iran in 2005. Traffic injury rates have rapidly risen out of control because for a variety of possible reasons such as increasing number of non-standard cars and motorcycles, low gas price, decreasing ratio of travels via public transportation than with private vehicles, and problems with safety design. Besides a high need for a system safety approach, Iran needs to deal with the problem of producing a high number of cars and motorcycles. Providing ...

Major thalassemia is one of the most common hemoglobinopathies in many Asian countries including Iran. Pharmacologic agents such as hydroxyurea have been known to enhance the production of fetal hemoglobin, and also an increase in total hemoglobin level has been repeatedly reported during hydroxyurea treatment in patients with sickle cell disease and in several patients with intermediate beta-thalassemia. We evaluated the long-term efficacy and safety of hydroxyurea in major beta-thalassemic patients. Forty-nine beta-thalassemic patients enrolled in the study. The mean follow-up time was 60 months. The mean dose of hydroxyurea was 10 mg/kg per day (8 - 15 mg/kg). Before starting hydroxyurea, all patients underwent routine bioch...

Authors: Aghamolaei T, Tavafian SS, Hasani L, Zare S
We aimed to investigate the attitudes of healthcare providers towards patients with human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) in Bandar Abbas, south of Iran. In this cross-sectional study, a random sample of 250 healthcare providers completed a 20-item questionnaire from January first through February 30th, 2007. There were five domains of attitudes towards patients with HIV. The items measured as a five-point Likert scale ranging from strongly disagree to strongly agree. The mean age of the healthcare providers was 31.3 (SD=7.3) years. Of them 24.4% were males, 42.4% had received Bachelor of Science degree, 19.2% were physicians, 63.2% were nurses, and 17.6% were laboratory technicians. The responde...

We present a patient with desmoplastic and conventional variant of ameloblastoma (hybrid lesion) localized in the anterior portion of the mandible. The tumor was expansile and painless. The radiographic finding showed an ill-defined mixed (radiopaque/radiolucent) lesion between roots of the lower left canine and the lateral incisor. Marginal resection was performed. Follow-up examination one year later revealed no recurrence of the tumor. Desmoplastic variant of ameloblastoma tends to infiltrate between bone trabeculae. Curettage leaves islands of the tumor within bone, which later manifest as recurrences. Therefore, for big lesions en bloc excision is the most widely used treatment to avoid recurrence.

Authors: Seyedmajidi M, Feizabadi M
Calcifying odontogenic cyst is a rare lesion representing about 1% of jaw cysts. It may occur in a central (intraosseous) or peripheral (extraosseous) location.A case of peripheral calcifying odontogenic cyst located on the gingiva, appearing as a painless, circumscribed, sessile, and pink to red nodule has been reported.Peripheral, in contrast to central, calcifying odontogenic cyst tends to affect older patients. Peripheral calcifying odontogenic cyst is a less aggressive lesion than the central counterpart, and a simple excision biopsy is curative. The histologic findings of an epithelium rich in ghost cells has helped in making the diagnosis.

Celiac disease is an autoimmune disease associated with increased risk of several diseases including a variety of malignancies. This is the first report of the concomitance of atopic dermatitis and Hodgkin's lymphoma in a child with celiac disease. Here, we report an 11-year-old boy with chronic diarrhea, glossitis, chronic dermatitis, and megaloblastic anemia who later developed Hodgkin's lymphoma.

Nemazee Hospital in Shiraz, Fars Province, in southern Iran is a well-equipped modern hospital, which was founded in 1955. It is one of the greatest memorials of a Shirazi beneficent merchant, the late Haj Mohammad Nemazee (1895 - 1972), who dedicated much of his wealth to charitable affairs in his birthplace. This paper briefly explores the history of the establishment of Nemazee Hospital in Shiraz.

CONCLUSION: In the present study, tendency of most of the antioxidants to rise in diabetes could probably be due to an adaptive response to the pro-oxidant milieu of the diabetic state. Hence, we suggest that supplementation with dietary antioxidants especially antioxidant vitamins accompanied by change in lifestyle might help to reduce damage brought about by free radical toxicity in diabetes mellitus.

CONCLUSION: To increase the survival rate of neonates and effectiveness of early intervention, the above-mentioned risk factors could be considered as valuable clues. Routine neurodevelopmental screening for neonates and infants for early detection of neurodevelopmental delays is highly recommended. If economic limitations prevent mass-screening of neonates, at least high-risk infants should be routinely re-evaluated.

Although the incidence of colorectal cancer in Iranian older age subjects is currently very low compared to Western population, the younger generation is experiencing an accelerated rate approaching the Western rates and the burden of disease will increase dramatically in near future. The high frequency of positive family history of colorectal cancer in Iranian patients indicates that a significant number of colorectal cancers in Iran arise in family members and relatives of colorectal cancer patients. It is clear that the familial clustering of colorectal cancer is more often seen in younger probands and cancer located in the right side of the colon. These epidemiologic findings call for a broader attempt to promote public aw...

This study was designed to detect transcription errors in a teaching hospital in Tehran. Direct observational method was used in this study. Error was defined as any deviation in transcribing medication order from the previous step (order on the order sheet, administration nursing note and/or cardex, documentation of the order in the pharmacy database). A total of 287 charts with 558 opportunities for error were reviewed. Of those opportunities for error 167 (29.9%) resulted in an error. Omission (the patient did not receive the medication that was ordered) was the highest (52%) transcription error type seen in this study. The evaluation clearly showed that errors at transcription stage were not infrequent. To cut these errors down we suggest implementation of surveillance systems, which m...

Authors: Jahanzad I, Amoueian S, Attaranzadeh A
Familial lecithin-cholesterol acyltransferase deficiency is an uncommon autosomal recessive disorder from a heritable defect in esterification of plasma cholesterol. In 1968, the disease was described by Gjone and Norum in Norway. Our case was a 38-year-old woman. Her disease was manifested by presence of lower extremities edema, proteinuria, corneal opacities, increased plasma cholesterol, and hemolytic anemia. Suspicion of the disease was based on renal biopsy, which revealed mesangial expansion and capillary wall widening with clusters of foamy cells in the mesangium. Immunofluorescence study was nonspecific, but specific findings of electron microscopy showed deposition of lipid in the glomerular basement membrane and mesangium. This ...

Malignant transformation in a mature cystic teratoma of the ovary is rare. The most common malignancy is squamous cell carcinoma, which consists of about 75% of malignant transformations. In the present report, we describe a case of advanced-stage squamous cell carcinoma arising in a mature cystic teratoma. A postmenopausal 63-year-old woman with squamous cell carcinoma arising in a mature cystic teratoma is presented. The initial investigation by ultrasound showed a left adnexal mass with mixed echo pattern, which arose the suspension of malignancy. She underwent a laparotomy and left oophorectomy. Histopatholog was compatible with squamous cell carcinoma arising in a mature cysti...

We report a patient who presented with two episodes of severe hypertension after intramuscular injection of betamethasone. The first attack was associated with pulmonary edema, while the second attack was associated with high anion gap metabolic acidosis, renal failure, hyperglycemia, and hypokalemia. The attacks led to the diagnosis of pheochromocytoma, which was confirmed by appropriate diagnostic tests. The tumor was excised successfully and the patient is presently asymptomatic. We believe that these episodes were initiated by glucocorticoid injection, an event reported in a few cases. We briefly review potential mechanisms resulted in hypertensive crisis in such patients.

The history of the publication of biomedical journals parallels the development of modern medicine in Iran. Modern medicine was introduced to Iran in the mid-nineteenth century, particularly after the establishment of the Dar al-Fonun School in 1851. The foundation of this collage of higher education was instrumental in further advancement of modern medicine together with the publication of medical textbooks, but the biomedical journals were published later i.e. at the beginning of twentieth century and the first weekly public health periodical was established in 1901. From that point onwards, especially following the inauguration of the Faculty of Medicine of Tehran in 1934, the number and diversity of biomedical journals was gradually inc...

CONCLUSION: The data reported by the Tehran Cancer Registry provide information on the cancer profile in Tehran metropolitan area. It clearly shows the breast and stomach, prostate, and tobacco-related cancers as major cancer in this population. The observed cancer rates indicate that Helicobacter pylori eradication, tobacco control measures, and early detection of breast cancer are of importance for cancer control in this population.

Several pain assessment tools have been proposed for use in different settings, but neither have been validated for all patients, especially Iranians. We conducted this study to compare the accuracy of two most commonly used tools for evaluation of pain intensity in a group of postoperative Iranian patients. All postoperative patients admitted to the surgical wards of Masih Daneshvari and Rasoul Akram Hospitals, Tehran, Iran were studied. During a two-month period, patients were evaluated for pain intensity within 24 hours of operation. Visual analogue scale and faces rating scale were used for this purpose. Eighty-two patients were enrolled into the study. Forty-eight patients underwent obstetrics and 34 had general surgeries. Using Spearman ana...

Autoimmune liver diseases are different entities that sometimes have overlapping features. They share many of the general characteristics of autoimmune diseases, with systemic involvements. Here, we report a 30-year-old woman with ophthalmic symptoms of conjunctivitis and dacryocystitis who was treated completely. After a few months, the patient presented with autoimmune cholangiopathy. Sequelae of autoimmunity have been considered in different organs in this case.
