            <description>We report a patient with a type 1 MEN syndrome, with no respiratory symptoms, with hypergastrinaemia, and in whom a 5mm diameter nodule was detected in the wall of the left main bronchus by computed tomography. The bronchial biopsy confirmed that it was a typical bronchial carcinoid and the octreoscan showed a single focus of high uptake coinciding with this lesion. A bronchoplastic (sleeve) was performed with extirpation of 3 bronchial rings, which also demonstrated that it was a typical carcinoid.

This system also allows us to obtain a more cosmetic surgical result, as well as decreasing the mean post-operative hospital stay and the need for analgesics to improve the post-operative pain.

            <title>[Organizing Pneumonia Associated with the Use of Trastuzumab.]</title>

            <description>We present a 60 year-old female patient with a previous history of breast cancer, who was being treated with trastuzumab, an antiHER2 monoclonal antibody. She was diagnosed with casual pulmonary infiltrates that had histological changes compatible with organizing pneumonia. The pulmonary infiltrates disappeared on withdrawing trastuzumab treatment. Due to the increasing use of biological therapies in different medical areas, we believe it is of interest to report this pulmonary involvement attributed to the monoclonal antibody trastuzumab.

    Tracheal bronchus is an aberrant bronchus usually originating from the right lateral wall of the trachea, with an incidence ranging from 0.1% to 5% and usually within 2.0cm above the carina. The incidence of lung cancer with bronchial anomaly is very rare. There are only nine cases of lung cancer developing from the tracheal bronchus reported in the literature. Additional costs for each episode of VAP range from 9,000 euro to 31,000 euro. Thus, its prevention should be considered as a priority. This prevention could decrease associated morbidity, mortality, costs, and increase patient safety.

            <title>[Hypersensitivity Pneumonitis after Exposure to Candida spp.]</title>

            <description>We describe the case of a patient with HP caused by domestic exposure to Candida spp. The diagnosis was made by taking into consideration the, clinical presentation, exposure history, radiological findings, bronchoalveolar lavage, lung function and the immuno-allergy study. The diagnosis was definitively confirmed by performing a specific bronchial provocation test. It has been shown that there is cross-reactivity between different Candida species, and despite making the diagnosis in this case with Candida albicans, we were unable to define exactly which species was responsible for the HP.

            <description>We present the case of a 22-year-old woman with Ewing sarcoma/PNET diagnosed following histologic, immunohistochemical, and in situ hybridization studies of a bronchial biopsy specimen. Radiography, ventilation-perfusion scintigraphy, and a bone marrow biopsy confirmed that the tumor was not metastatic. The patient was started on a chemotherapy regimen of vincristine, actinomycin, cyclophosphamide, doxorubicin, ifosfamide, and etoposide and responded well. She is now being seen regularly at our outpatient clinic.

Each technique has inherent strengths and weaknesses, but when used simultaneously they become far more powerful, to the extent that mediastinoscopy -a more expensive and invasive procedure- is expected to decline in use as the application of endoscopic and endobronchial ultrasound becomes more widespread. The incorporation of these ultrasound techniques has been shown to lead to fewer thoracotomies, benefiting patients and also society, given that costs are thereby reduced.             <description>We present the case of an adult patient who underwent left pneumonectomy and developed postpneumonectomy syndrome 15 months later. Stenosis of the intermediate bronchus occurred between the vertebral body and the right pulmonary artery. Endoscopic treatment with a self-expanding metal stent was successful, and complete remission was observed over the 6 months of follow-up.

Most of the recently introduced treatments have failed to improve the prognosis of patients with ALI or ARDS or to reduce mortality. Several studies have shown improved oxygenation in the prone position during mechanical ventilation in patients with ARDS. However, current evidence strongly suggests that placing ARDS patients in prone position does not improve survival or reduce the duration of mechanical ventilation. Therefore, though in clinical practice this position may improve refractory hypoxemia in patients with ARDS, there is no evidence to support its systematic use.

            <description>CONCLUSIONS: We believe that exploratory EVT is clearly better than computed tomography and/or magnetic resonance imaging for detecting chest wall invasion. In addition to correctly staging a tumor as T3 because of chest wall invasion, the technique can also help decide the best surgical approach in each case.

            <title>[Solitary Pulmonary Nodule: Primary, Metastatic, or Both?]</title>

            <description>We describe the case of a 65-year-old man who underwent low anterior resection for colorectal adenocarcinoma in 2007. Follow-up computed tomography revealed a pulmonary nodule that was classified as metastatic. The patient was referred to our thoracic surgery department, where the nodule was resected. The pathology confirmed that the nodule was neoplastic, and histologic and immunohistochemical examination showed the presence of colorectal metastasis and lung adenocarcinoma. Evidence of metastasis was also found in 2 of the lymph nodes analyzed, 1 with features identical to the primary pulmonary adenocarcinoma, and 1 with features identical to the colorectal metastasis.
