In the late 19th century, the resection of the thyroid was feared because of its high mortality rate. Kocher's innovative techniques resulted in safe outcomes. His observations that radical resection of the thyroid results in &quot;cachexia strumipriva&quot; contributed to the recognition that the thyroid is essential for normal growth, development and metabolism. He made many other seminal contributions to the field of surgery and medicine, and his expertise was internationally recognized. University Hospitals outpatient clinic dynamics have a direct effect on this behavior: physicians do not talk openly with parents who in turn do not talk with their children, whereas patients chose to protect their parents from additional suffering. Thus, some miscommunication is noticeable among these subjects. These observations suggest that a special mindful measure should be built for these patients.             <description>Estudos recentes mostram que restriÃ§Ãµes na ingestÃ£o de sÃ³dio podem aumentar a resistÃªncia Ã  insulina (RI) e induzir alteraÃ§Ãµes nas lipoproteÃ­nas sÃ©ricas e em marcadores de inflamaÃ§Ã£o semelhantes Ã s encontradas na sÃ­ndrome metabÃ³lica (SM). Realizou-se uma revisÃ£o sistemÃ¡tica da literatura sobre os efeitos da restriÃ§Ã£o do consumo de sÃ³dio sobre a SM ou a RI. Nove artigos foram incluÃ­dos na revisÃ£o. A restriÃ§Ã£o no consumo de sÃ³dio associou-se ao aumento da RI em dois artigos e a diminuiÃ§Ã£o em trÃªs outros. Diagnosis of this disease is often challenging. The following case report described a middle-aged man with symptoms of bone pain and severe muscle weakness, who was found to have TIO.             <description>We report the case of a fourteen years old patient in use of PTU for the last three years who presented with fever, hemorrhagic blisters, necrotic ulcers, and that developed purpuric lesions and nodules in lower extremities. Laboratory and histopathologic findings were compatible with skin leukocytoclastic vasculitis, a pattern found in hypersensitivity reaction vasculitis. Suspension of PTU and introduction of prednisone, induced complete remission of symptoms and healing of the skin lesions. The importance of this study is to call attention to the occurrence of serious cutaneous manifestation with a mortality rate that might reach 10%, associated with a systemic drug frequently used in internal medicine.
