            <description>Conclusions: Corruption is a crime which contributes to undermining the idea of democracy, abiding by the law and, what is more, it hinders the free market economy. The wrongdoer is prosecuted ex officio and liable to severe punishment. The research conducted by the authors shows that these days corruption is a marginal problem in the Polish National Health Service. What seems to be the only chance to completely eradicate corruption from the National Health Service establishments is creating a free market of medical services (devoid of the limits defined by the National Health Fund), as well as privatizing hospitals the way it is happening in the developed countries in the European Union and in the United States of America.             <description>Background: Implantable ventricular assist devices are one of end-state heart failure treatment methods. They are used as 
a bridge to transplantation, a bridge to recovery of heart to have time for cardiac regeneration and as a destination therapy. The aim of this study was to assess the second stage of rehabilitation in patients with implantable left ventricular heart devices of the HeartWare type. 
Methods: 2 patients aged 47 and 63 years underwent a cardiac rehabilitation program: physical exercises and psychotherapy 3 months after implantation of HeartWare devices.             <description>An aortic dissecting aneurysm is an acute life-threatening state with a high mortality risk almost equal to 50% in patients without surgical treatment. In children, an aortic dissecting aneurysm is an extremely rare disease. For children and the youth, one of the risk factors of an aortic dissecting aneurysm is a genetic disorder of the connective tissue like Marfan syndrome. An aortic dissecting aneurysm may coexist with a biscupid aortic valve or a complication of hypertension. In this paper, we present a case of a 13-year-old child with an ascending aortic dissecting aneurysm type 1, treated surgically with a good effect.             <description>Presentation of a clinical case of a 50-year-old female patient admitted emergently to the Second Clinic of Cardiac Surgery and Trans-plantology due to decompensated right heart failure caused by significant tricuspid insufficiency and constrictive pericarditis.
The tricuspid ring diameter and the degree of valve insufficiency were assessed on the beating heart, with extracorpo-
real circulation. Because of the size of the ring (27 mm) and the patient&amp;#8217;s body surface, requiring a 31-mm prosthesis, extra-anatomic implantation of a biological valve prosthesis epic SJM 31 was carried out, leaving the coronary sinus ostium on the side of the right chamber &amp;#8211; Barnard-Schrire&amp;#8217;s procedure.

            <description>The use of the left internal thoracic artery (LITA) has been 
a golden standard in surgical revascularization of myocardium. Analyzing a case of a 52-year-old woman operated non-
electively due to the left main stem stenosis with undiagnosed stenosis of the left subclavian artery, the authors try to answer the question if LITA should be routinely shown on coronary angiography. Due to the worsening clinical state after coronary artery by-pass grafting (CABG), the patient was taken back to the cath lab for the re-do angiography. It revealed a critical stenosis of the left subclavian artery, which was managed by stent implantation. This treatment brought relief from angina and neurological signs.             <description>Conclusion: It seems that the examination of the selected immunocompetent cells can be useful in the routine clinical diagnostics. CPB, as compared to the OPCAB technique, seems to cause major suppression of immunological response which can make the patients more susceptible to infections. However, it should be noticed that in the OPCAB group, the primary number of cells positive to a given receptor is lower than in patients operated with the usage of CPB.             <description>We present a multidisciplinary surgical treatment of a male 4-month-old infant with malignant mediastinal and left lung tumour who deteri-orated rapidly with mechanical ventilation due to symptoms of local compression.
The child underwent life-saving surgical intervention by a multidisciplinary team. Gross total resection of the tumour was achieved via a com-bination of median sternotomy and &amp;#8220;hemi-clamshell&amp;#8221; incision. After an uncomplicated postoperative period the child was transferred to the oncology department for further chemotherapy.

            <title>FORUM EKSPERTÓW Neochordae as the primary technique for repair of isolated posterior mitral leaflet prolapse: a comparison to traditional techniques</title>

            <description>Aim: Posterior mitral valve leaflet (PMVL) prolapse repair using CV5 Gore-Tex neochordae without leaflet resection used as the primary method of repair was compared to traditional quadrangular leaflet resection.
Material and methods: Prospective data were collected in 186 consecutive mitral valve repair patients; January 2003 to March 2006. Bi-leaflet repairs were required in 63 and PMVL only in 54. The latter group formed the study group; 23 patients underwent repair using neochordae prepared to specific intraoperative measurements using a single suture technique and an annuloplasty ring without any resection (group I). The level, measured both before and after the revascularization surgery, did not exceed the value of 2000 kcal/week.             <description>The long-term mechanical assisted circulation is a chance for the treatment of a selected group of patients in whom assisted circulation can lead to myocardial regeneration or as a bridge to transplantation. In this article we present the case of 
16-yeare boy suffering heart failure due to ventricular non-
compaction. Because of significant clinical worsening we 
decided to implant biventricular assisted circulation BIVAD using system POLCAS-RELIGA.             <description>Patients with severe symptomatic aortic stenosis, who from November 2008 to March 2009 were treated with Edwards-Sapien transcatheter aortic valve implantation (TAVI) within the POL-TAVI First Polish Registry, were included in the analysis. Nineteen patients aged 78&amp;plusmn;4,8 years with high operation risk and Logistic EuroSCORE 25 &amp;plusmn; 7.6% were reported (74% were females). In 15 (79%) patients the valve was implanted transapically (TA), in the other four (21%) &amp;#8211; via the femoral arterial access (TF).             <description>Conclusions:  The therapy of pleural empyema requires a differential approach with regard to: general state of patient, dimensions and empyema location, disease duration, coexisting lung pathology and surgical experience. Superior results are achieved with early stages of pleural empyema using antibiotic therapy, pleural drainage along or with the administration of fibrinolytics or videothoracoscopy. Empyemas in the late, fibropurulent or organization stage usually require thoracotomy with decortication. Serious general condition, coexisting conditions and surgical complications are associated with a high percentage of fatal final outcomes. The patient was operated on twice. As a 12-year-old girl she underwent aortic root replacement with composite graft (Bentall-de Bono procedure). After 7 years she was reoperated on. The third operation involved mitral valve replacement and resection of the ascending aorta and the arch due to a large aneurysm located distally to the previous anastomosis. The cardiopulmonary bypass was initiated through the innominate artery. A few weeks after the operation the patient required vacuum-assisted therapy due to sternal wound infection.             <title>TECHNIKI OBRAZOWANIA A case of acute pulmonary embolism in the course of primary pulmonary trunk tumour treated by surgery</title>

            <description>The authors present a case of a 59-year-old man admitted to the hospital with the diagnosis of acute pulmonary embolism. After an additional diagnostic process including multislice computed tomography of pulmonary arteries, chest and abdomen the patient was sent to surgery where, besides the emboli, a tumour of the pulmonary trunk and pulmonary arteries was removed. On the basis of immunohistochemistry the diagnosis of leiomyosarcoma, high grade, was established. The authors analyzed the pre-surgery symptoms of this rare tumour and concluded that the pulmonary embolism can obscure them. Only surgery can prolong the patient&amp;#8217;s life so the diagnostic process must take the possibility of tumour into consideration.             <description>Conclusions:  Patients&amp;#8217; right to receive information about the degree of risk of their surgery is one of the basic rights appertaining to a patient. A lack of such information results in the fact that a patient has not received full information as to a planned operation, and his/her agreement to the surgery is invalid. A doctor performing an operation without a patient&amp;#8217;s agreement may be sued by the patient for damages on the basis of acts written in the civil code.             <description>Air leak is one of the most common complications occurring after pulmonary resection. Risk factors as well as methods of prevention and surgical treatment are discussed.             <description>Delayed contrast-enhanced magnetic resonance imaging (DE-MRI) with use of gadolinium-based contrast enables assessment of postinfarction scar size in the left ventricle wall. It is known as transmural extent of hyperenhancement (TEI) and is the percentage ratio of scar thickness to total end diastolic wall thickness in a given segment. TEI correlates with probability of improvement in contractility after a revascularization procedure. The greatest chance for improvement is shown by myocardial segments with TEI less than 50%, and it is very low for segments with TEI over 75%. 
The presented case confirmed the usefulness of TEI assessment in DE-MRI before a revascularization procedure in a patient after myocardial infarction with regional myocardial thinning.

            <description>Conclusions: The fundamental treatment of tracheal stenosis is the surgical approach. The endoscopic management (dilatation and laser) need repeated procedures and in some cases tracheotomy had to be maintained. Better recovery, short hospitalization and excellent results were obtained with the cricotracheal segmental resection. Cricotracheal resection is a reliable and versatile technique for the reconstruction of the subglottic airway, almost regardless of the underlying cause. Segmental resection is the treatment of choice in LTS, making the laryngofissure technique obsolete.             <description>Hypoplastic left heart syndrome (HLHS) is a complex cyanotic congenital heart defect with wide anatomical variability. HLHS requires multi-stage operative treatment according to the Fontan principle for single ventricle CHD. The first stage of treatment is Norwood operation performed in newborn age. Coexistence of genetic syndromes, including Turner syndrome, is an independent risk factor for the first stage of HLHS treatment by Norwood operation.             <description>Thoracic aortic aneurysm after heart transplantation is a rare but serious complication. The authors describe 3 cases (0.6%) 
of thoracic aortic aneurysm during 20 years of follow-up on 485 heart transplant patients at their institution. Dissected and 
non-dissected thoracic aortic aneurysms, diagnosed 23 to 38 months post transplantation, were treated with surgical repair 
or repeat transplantation. Two patients died postoperatively of multiorgan failure, including one with septic shock. One patient was discharged in good health. Surgical repair of aortic lesions in cardiac allograft patients, including repeat transplantation

            <description>Conclusions: Patients who have lung cancer in the residual lung after pneumonectomy should not be automatically excluded from surgery.             <description>Conclusions:  The isolation of Candida spp., especially C. albicans, from BAL was frequent in patients with lung cancer but the presence of Candida spp. in quantity ł104 cfu/ml was rare. In all examined patients the isolation of Candida fungi indicated colonization of the bronchial tree.             <description>Stab wounds and gunshots are the most frequent causes of heart injuries. They more rarely are the consequence of fractures of ribs or the breastbone as a condition of imminent danger of life. The authors describe the case of a 61-year old wounded person treated psychiatrically for a long time. This man was admitted to the hospital with a dagger plunged into the left heart chamber. The patient was operated on immediately, making a suture of the muscle of the injured left chamber of the heart. Prolonged pleural drainage was noted in the post-operative course. The patient was discharged in good condition from hospital 23 days postoperatively. He was transferred to the psychiatric unit for further treatment.             <description>Conclusion: The examined relaxants (recuronium, cisatracurium and suxamethonium chloride) provide appropriate and comparable haemodynamic conditions.             <description>Giant cell myocarditis (GCM) is an unusual cause of cardiomyopathy. Because of its fulminant clinical course and specific treatment, it should be taken into consideration in patients with a new onset of left ventricular heart failure, which is difficult to treat with standard therapy. Giant cell myocarditis may present as an atrioventricular block as well. The outcome is fatal, unless the patient undergoes a heart transplantation. Mean survival time from the first symptoms to end-point (transplantation or death) is 5.5 months. We would like to present a case of a patient who was successfully treated with heart transplantation for cardiomyopathy caused by GCM, which was revealed in the explanted heart.
