Quantification of trends in radiation oncology infrastructure in Pakistan, 2004–2009

Conclusion:  While there was an increase in number of radiotherapy centers, equipment and human resources available, this was insufficient to comply with international guidelines. An adequate enhancement in radiation oncology infrastructure is needed to cope with the predicted rise in cancer incidence. (Source: Asia-Pacific Journal of Clinical Oncology)

We describe the case of a 46‐year‐old patient who presented with a radiologically proven sphenoid wing metastasis. The patient presented with left‐sided exophthalmos while receiving systemic chemotherapy for relapsed high‐risk colorectal cancer. A left sphenoid wing metastasis was proven by a head computed tomography scan. A metastatic spread to the sino‐nasal tract is regarded as a poor prognosis determinant in colorectal cancer and presents a unique set of diagnostic and therapeutic challenges. The compression of noble structures such as the optic nerve holds serious implications in terms of quality of life but there is insufficient evidence clarifying whether radiotherapy or surgery represent the best option for these patients, leaving the therapeutic plan to be decided case b...

Conclusion:  For patients undergoing annual surveillance mammography at Peter MacCallum Cancer Centre, the transition from film to digital mammography was found to be associated with an increase in both the number of additional investigations performed and in the number of hospital visits. The findings of this study should be considered when conversions from film to digital mammography are planned, specifically in terms of cost allocations and the burden on patient services in radiology and outpatient departments.
eviQ cancer treatments online: How does the web‐based protocol system fare in a comprehensive quality assessment?

Conclusion:  Our evaluation demonstrated the eviQ protocol system is generally of high quality. However, we identified a number of areas in which eviQ could improve either its development processes or the means by which these processes are communicated to end‐users. This research represents the first comprehensive evaluation of oncology protocols and paves the way for continued quality assurance research to ensure that these tools meet their objective of facilitating evidence‐based medicine, standardizing treatment and, ultimately, improving patient outcomes.

Conclusion:  The involvement of the central neck should be a major indication of lymph node biopsy and pathological diagnosis regardless of the imaging findings. In the lateral compartment, hypoechogenicity with loss of hilum, microcalcifications, cystic parts and an index value ≥ 0.51 are indications of a lymph node biopsy to rule out malignancy.
Conclusion:  Patients with locoregional NET had a favorable long‐term survival after curative resection. Distant metastases, hepatobiliary localization and a poor degree of tumor cell differentiation were poor prognostic factors. Further investigational approaches for treatment of advanced disease are needed. (Source: Asia-Pacific Journal of Clinical Oncology)

Conclusion:  Despite this is being a small retrospective study, survival figures and toxicity profiles of low dose weekly cisplatin are comparable to historical controls using high‐dose regimens, hence justifying our approach. In addition, radiotherapy interruptions are minimized and cisplatin is easy to administer in outpatient settings. Future three‐arm studies could include this regimen as the basis of treatment combined with targeted therapies. (Source: Asia-Pacific Journal of Clinical Oncology)

Conclusion:  NT pro‐BNP levels are elevated in a significant proportion of patients with normal LVEF receiving trastuzumab. Troponin I levels are not raised in this group, perhaps reflecting the mechanism of cardiotoxicity. The data provide biochemical evidence of subclinical cardiac strain in women receiving trastuzumab. Results are exploratory and have informed the design of a larger study examining the predictive utility of serial serum NT pro‐BNP levels for TRC in the adjuvant setting.
AbstractOver the past three decades there has been a move toward organ preservation protocols in the management of locally advanced mucosal head and neck squamous cell carcinomas (LAHNSCC) with combinations of radiotherapy (RT), chemotherapy and, more recently, biological agents. Current standard chemoradiation strategies have reached the upper limits of toxicity. In addition, the traditional one size fits all approach of grouping patients according to traditional clinicopathological features fails to take into account the vast underlying biological heterogeneity of tumors and their host. A number of recent advances such as highly conformal RT, molecular profiling and targeted agents, and improvements in treatment response assessment have set the scene for a fundamental paradigm shift towa...

AbstractGastric cancer (GC) constitutes a major cause of cancer deaths worldwide. Recent improvements in both surgical techniques and adjuvant and neoadjuvant radiotherapy and chemotherapy approaches have increased the survival of patients with loco‐regional disease. However most patients with GC have advanced disease either at diagnosis or at follow up. Despite recent advances in the treatment of advanced disease, these patients still have poor outcomes. An emerging understanding of the molecular pathways that characterize cell growth, cell cycle, apoptosis, angiogenesis and invasion has provided novel targets in cancer therapy. In this review we describe the current status of targeted therapies in the treatment of GC. These therapeutic strategies include epidermal growth factor recepto...

AbstractDespite a survival benefit in the first‐line treatment of castrate‐resistant prostate cancer (CRPC) with docetaxel, the prognosis remains limited. There are increasing options available for patients with CRPC in the second‐line setting, but there is currently little consensus regarding the optimal treatment. There have been numerous phase II and retrospective studies examining second‐line options in CRPC, including retreatment with docetaxel, mitoxantrone, cyclophosphamide and carboplatin, which can be associated with meaningful responses in a significant minority of patients. In 2010 three randomized trials were published or presented which demonstrated a survival benefit in the second‐line setting. These included cabazitaxel compared with mitoxantrone, sipuleucel‐T (i...

The role of poly adenosine diphosphate ribose polymerase inhibitors in breast and ovarian cancer: Current status and future directions

AbstractPoly adenosine diphosphate ribose polymerase (PARP) inhibitors have demonstrated single agent activity in the treatment of patients with recurrent BRCA1‐mutated and BRCA2‐mutated breast and ovarian cancers. They also appear to have a potential role as maintenance therapy following chemotherapy in patients with platinum sensitive recurrent sporadic and BRCA1/2 related high‐grade serous ovarian cancers. The concept of BRCAness raises the possibility that PARP inhibitors may be active in selected patients with homologous recombination (HR) DNA repair‐deficient tumors, even if they do not harbor a BRCA1/2 germline mutation. Further research will be required to identify the subset of patients with sporadic cancers who may benefit from PARP inhibitor therapy. Precise details on t...

AbstractCarcinoids of the ampulla of Vater are infrequent tumors of which a quarter of cases have been detected in patients with type I neurofibromatosis. This hereditary disease is also associated with gastrointestinal stromal tumors (GIST). However, the coincidence of these three entities together have only been formerly detected in five cases. A 53 year‐old female patient, diagnosed with type I neurofibromatosis, with a malignant carcinoid of ampulla of Vater and multiple gastrointestinal stromal tumors in the duodenum and jejunum, was treated with total pancreatectomy and the excision of her intestinal tumors. Five‐years on, a follow‐up showed the patient to be well, and free from tumor recurrence. The coexistence of an ampullary carcinoid tumor, GIST and neurofibramatosis is ver...

Conclusion:  This oxaliplatin and gemcitabine schedule has shown activity in advanced NSCLC with modest toxicity. Neither clinical nor PK outcomes were influenced by the sequencing of these agents, although definite conclusions are limited by small patient numbers. The favorable toxicity profile of this doublet, in light of an encouraging RR, warrants its further investigation in NSCLC. (Source: Asia-Pacific Journal of Clinical Oncology)

This article reviews targeted agents used in solid tumor oncology with an emphasis on clinically relevant details. Overall, the evidence of targeted agents causing surgical complications is limited. The greatest amount of evidence exists for bevacizumab causing perioperative complications, possibly due to its extended half‐life. There are limited data for cetuximab, sorafenib and sunitinib and very little for other solid tumor targeted agents. Our findings suggest that there should be heightened pharmacovigilence around targeted agents with respect to perioperative complications and increased post‐surgical support for patients to aid early detection of postoperative complications until definitive data become available.
This article reviews the available evidence for the use of three monoclonal antibodies: one – ipilimumab – pending approval in Australia, and two – panitumubab and cetuximab – that has been recently approved. Ipilimubab is a fully human monoclonal antibody that targets cytotoxic T‐lymphocyte‐associated antigen‐4 to increase the activation of T cells, while panitumumab and cetuximab both target the epidermal growth factor. The clinical effectiveness of these treatments has been demonstrated, and careful biological studies have shown factors that predict for both response and resistance. This review outlines some of these factors. (Source: Asia-Pacific Journal of Clinical Oncology)

AbstractThere has been much recent attention on immunotherapy for cancer due to the promising results of several randomized phase‐III trials. The management of metastatic melanoma in particular is a significant clinical challenge, and it has long been a desired target of immunotherapies due to evidence of the importance of host immune responses. Cytotoxic T‐lymphocyte‐associated antigen‐4 (CTLA‐4) serves as a negative regulator of immunity. Recent trials administering fully‐human anti‐CTLA‐4 monoclonal antibodies to patients with melanoma have demonstrated survival benefits. However, treatment with anti‐CTLA‐4 antibodies is not without potential toxicities. Autoimmune side‐effects, such as diarrhea, bowel inflammation and dermatitis, were the most commonly reported.
AbstractEffective antitumor responses require communication between antigen‐presenting cells, T‐cell subsets, B cells and natural killer cells. However, tumors have developed a number of mechanisms to avoid detection and elimination by the immune system. This results in a microenvironment that can contribute to the failure of the immune response to the tumor or the prevention of effective immune‐mediated killing. Understanding the barriers to effective tumor immunity allows for the rational design of targeted immunotherapies. The characterization of tumor antigens has led to the development of promising vaccine candidates. Our work has concentrated on the cancer/testis antigen NY‐ESO‐1, which has been an important focus in the development of vaccine‐based therapies against mela...

AbstractThe association between the immune system and malignancy is at the forefront of research aimed at identifying and developing effective immunotherapies to treat and suppress cancer. Understanding the mechanisms underlying the biology, differentiation and effector functions of key immune cells in the tumor microenvironment is central to this process. The stimulation of T cells results in their activation, triggering the release of cellular mediators, including cytokines and chemokines. The interplay between these components determines the polarization of T cells into phenotypically‐, functionally‐ and transcriptionally‐distinct subsets (e.g. T‐helper [Th] 1, Th2, Th17 and T‐regulatory cells). Given that antigen recognition, costimulation and the cytokine milieu are critical...

Conclusion:  MDC is not being implemented in line with best practice or applied consistently across Australia. This audit has highlighted gaps in care delivery, despite national recommendations about MDC. Areas being neglected can affect the quality of care provided and may put clinicians at medico‐legal risk. Recommendations to improve uptake and effectiveness of MDC are provided.
AbstractVarious factors have recently prompted a re‐evaluation of the role of non‐anthracycline regimens in early stage breast cancer (ESBC). Since 1990 anthracyclines have been a key component of chemotherapy regimens. However, there is increased understanding of the long‐term, irreversible toxicities associated with these therapies, including cardiac failure and secondary leukemia. The development of the taxanes in the 1990s led to new adjuvant chemotherapy regimens and trials of various combinations in an effort to further increase survival and reduce toxicity. Concerns about cardiac toxicity were reinforced with the emergence of trastuzumab for the treatment of HER2‐positive breast cancer. Trastuzumab alone causes cardiac toxicity and increases the risk of cardiac toxicity when...

We report two cases of the rare neurological paraneoplastic syndrome, limbic encephalitis, as the initial presentation of small cell lung cancer. The first case responded to treatment of the underlying malignancy, while the second required more acute treatment in the intensive care setting. In this case, initial treatment was with immunosuppression to achieve a degree of stability before the underlying malignancy could be treated. Both cases had significant improvement in neurological function. These cases highlight the importance of directed investigation to try and identify an underlying malignancy in patients in whom a diagnosis of limbic encephalitis is made, and the difficulty in managing such patients.
Conclusion:  It is important to consider both pharmacokinetics and pharmacogenetics in optimizing gemcitabine accumulation. This represents a classical interaction between genes and environment and provides support for the consideration of both CDA genotype and infusion duration in development of an individualized dosing strategy.
Conclusion:  These data suggest that this assay in these cell lineages has limited usefulness as a predictive screen for the endogenous DNA DSB repair competency of radiosensitive cancer patients' cells but can usefully characterize major cellular DSB repair phenotypes.
Conclusion:  The biological behavior of early breast cancer with isolated tumor cells on sentinel node biopsy is similar to both micrometastases and macrometastases, i.e. they behave in a node‐positive fashion. This is an early indication that these patients should be treated with more aggressive adjuvant therapy. (Source: Asia-Pacific Journal of Clinical Oncology)

Conclusion:  Premature closure of this study limits the interpretation of results. While there was a trend towards reduced subjective acute sensory neuropathy with Ca and Mg, this was not significant. Ca and Mg failed to reduce the rate of cumulative sensory neuropathy and instead increased the rate of abnormal NCS, suggesting a significant difference in perceived sensory and objective neuropathy.
We report on two patients who developed cranial nerve palsy due to skull base plasmacytoma. Patient 1, a 55‐year old man with multiple myeloma, developed right sixth cranial nerve palsy during a phase of chemotherapy. He had an intracranial plasmacytoma in the clival region and was treated with 30 Gy whole brain radiotherapy. Patient 2, a 40‐year old man, presented with right third cranial nerve palsy and was detected to have a solitary skull base plasmacytoma. He was treated with radiotherapy followed by thalidomide plus dexamethasone. (Source: Asia-Pacific Journal of Clinical Oncology)

AbstractBilateral uveal metastases from papillary thyroid carcinoma are extremely rare. A 36‐year‐old woman with a 12‐month history of papillary thyroid carcinoma presented with sudden loss of visual acuity and fields in the left eye. An examination and B‐scan revealed a large, solid choroidal lesion in the left eye causing exudative retinal detachment. A small solid mass was also observed in the right eye fundus. Following left eye enucleation, immunohistopathology confirmed metastatic papillary thyroid carcinoma. The authors report the third known case of bilateral choroidal metastases.
