We report here the case a 30-year-old parous, phenotypically normal, reproductive woman. She presented with mature teratoma of the ovary, which contained well formed, grossly visible prostatic gland with a central urothelium-lined duct resembling a urethra. Mature teratomas are characterized by the presence of tissues derived from the embryonic layers of ectoderm, mesoderm, or endodermal lineage. However, prostatic tissue is a very unusual component of teratomas. The rare existence of prostatic tissue in a mature teratoma can be explored through three feasible pathogenetic theories. These origins include teratomatous monodermal differentiation, developmental abnormality, or metaplasia. The coexistence of rare prostatic tissue with a rare central urethral duct in a mature teratoma makes thi...

Early gastric cancer is defined as gastric cancer confined to the mucosa or submucosa. For the majority of early gastric cancers without nodal involvement, endoscopic mucosal removal (EMR) is safer and more cost-effective than are other treatment methods. Here we report a rare case of differentiated mucosal carcinoma with extensive lymphovascular invasion and lymph node metastasis. Endoscopy showed a 5.5 cm non-ulcerated slightly elevated and depressed lesion without indication of submucosal involvement. Although the tumor met the extended criteria for EMR, D2 gastrectomy was performed. The resected specimen showed a 5.5 cm intramucosal carcinoma with extensive lymphovascular invasion of the submucosa, and metastasis to two perigastric nodes, indicating that metastasis can occur in patient...

Primary extracranial meningiomas are rare disease entities, occurring primarily in the head and neck. We herein present a very rare case of primary extracranial meningioma of the retroperitoneum, observed in a 56-year-old man who presented with a retroperitoneal mass measuring 11 cm in its greatest dimension. Histological examination revealed sheets of spindle cells with frequent whorling patterns. The neoplastic cells had abundant light eosinophilic cytoplasm with minimal mitosis or cellular atypism. Immunohistochemical analysis demonstrated diffuse and strong positivity for vimentin and epithelial membrane antigen. Ultrastructural findings revealed abundant intermediate filaments, complex interdigitating cellular processes, and desmosome-like intercellular junctions, all of which are cha...

Gastrinoma is one of the most symptomatic pancreatic endocrine tumors and is associated with recurrent gastric or duodenal ulcer and an induction of chronic gastric acid hypersecretion. This tumor is commonly involved within the gastrinoma triangle. Herein we report a peripancreatic lymph node gastrinoma in a 56-year-old man who was suffered from a recurrent duodenal or jejunal ulcer perforation. Octreotide scan detected a peripancreatic or periduodenal mass and the serum gastrin level was set in upper normal range (107.73 pg/mL). Whipple's procedure was performed. The resected specimen showed multiple duodenal ulcers, hypertrophic gastric body mucosa and an enlarged peripancreatic node, measured 1.5 × 1.0 cm in cross diameter. Microscopic examination revealed a single enlarged lymph node...

Conclusions: PPAR[gamma] expression is related to poor prognostic factors of breast cancer. It is considered that PPAR[gamma] has evident tumor promoting properties in vivo, but receptor-independent effects of PPAR[gamma] ligands may compound its biologic effects in cancer. A better understanding of action mechanisms of the activated PPAR[gamma] is required. (Source: Basic and Applied Pathology)

We report a 62-year-old man who presented with general weakness and poor oral intake for 6 months, and showed multiple masses at the duodenum, jejunum, ileum and the ascending colon. The patient underwent right hemicolectomy with small bowel segmental resection. Microscopically, the tumor masses were characterized by incohesive sheet of neoplastic cells with vesicular nuclei, large nucleoli, and abundant eosinophilic cytoplasm. Immunohistochemical studies showed strong reactivity of the tumor cells for pan-cytokeratin, anti-endomysial antibodies (EMA) and vimentin. This was a case of undifferentiated carcinoma with prominent rhabdoid feautures arising in the ascending colon with multiple gastrointestinal metastases.
Activated charcoal is commonly used in the treatment of patients experiencing an oral overdose of drugs and is considered to be a safe, inert agent. However, various complications associated with the use of activated charcoal have been reported, with acute respiratory failure being a fatal one. In the presenting case, a 29-year-old female arrived at an emergency room having taken an overdose of drugs. When a nasogastric tube was inserted for gastric lavage and charcoal administration, she vomited and respiration ceased. The patient subsequently developed acute respiratory failure and died 19 days after admission. Charcoal deposition in the trachea, bronchi and lungs was observed on the postmortem examination.
Conclusions: The results suggest that the survivin expression is a relatively frequent early event of colorectal carcinogenesis and may play an important role in the adenoma-carcinoma transition sequence. Survivin expression provides important prognostic information and an apoptosis-based therapeutic target in colorectal adenocarcinomas. (Source: Basic and Applied Pathology)

Mesonephric rests or mesonephric remnants are vestiges of embryonic structures called mesonephric (wolffian) ducts. It usually can be seen in the lateral wall of the cervix uteri. In males, the mesonephric remnants are seen in epididymis and efferent ductules. Prostate is one of the rare locations, on which the mesonephric remnants can be found. Hyperplasia of mesonephric remnants of prostate is rare but important because sometimes it is difficult to differentiate from adenocarcinoma. The lesion is important in the differential diagnosis from the high grade prostatic intraepithelial neoplasia. Here, we report an additional case of mesonephric hyperplasia, epididymal type, in a 66-year-old man. Microscopically, it showed tubular structures lined by columnar cells with pseudostratified elong...

The present paper reports a rare case of granulocytic sarcoma (GS) with plasmacytoid features occurring in the maxillary sinus of a 63-year-old man. The diagnosis of GS has always been difficult, and diagnosis in the present case was made more difficult by the tumor's manifestation as longstanding maxillary sinusitis, the late development of leukemia, unusual location of the tumor, and the plasmacytoid features of the tumor cells. The potential for any lesion of any organ to be GS should be considered in a patient with hematologic abnormalities. (Source: Basic and Applied Pathology)

A 66-year-old woman died during a routine gastroscopic examination. The physician tried to biopsy a submucosal mass-like lesion with tortuous and exaggerated submucosal blood vessels. Massive bleeding occurred due to iatrogenic injury. Isolated gastric varices are less prevalent than esophageal varices, but it has a higher motality rate because of the difficulty of controlling bleeding. Physicians should be mindful of isolated gastric varices, especially during examination of those with liver cirrhosis. (Source: Basic and Applied Pathology)

Pilomatrixoma is an uncommon benign calcifying epithelial neoplasm derived from hair matrix cells, manifested as a firm intracutaneous spheroid mass on the face, neck, and arms, but its pathogenesis is still not clearly understood. This case study aimed to elucidate the downstream effector molecules and apoptotic processes through the Wnt/[beta]-catenin signaling in an ossifying pilomatrixoma. The histological analysis of whole tissue in this case showed the mixture of shadow and basophilic epithelial cells, where the shadow epithelial cells were most abundant in the superficial layer and the transitional epithelial cells were diffusely distributed between the shadow cells and basophilic cells. The basophilic epithelial cells usually distributed around the basal layer were intensely staine...

We report a case of primary non-Hodgkin's lymphoma (NHL) of the uterine body with involvement of bilateral ovaries. A 67-year-old woman had explolaparotomy under the provisional diagnosis of uterine leiomyoma. Enlargements of the uterine body as a leiomyoma and bilateral ovaries were noted in the operation field. The frozen section diagnosis of the uterine wall mass was malignant lymphoma. Total abdominal hysterectomy with bilateral salpingo-oophorectomy was carried out. The pathologic study with immunohistochemical stains confirmed primary NHL of the diffuse large B-cell type of the myometrium with involvement of bilateral ovaries. To the best of our knowledge, the primary lymphoma of the myometrium with involvement of bilateral ovaries has not been reported previously in the literature. ...

Sudden unexpected deaths due to cardiovascular diseases make up almost half of all natural deaths. Practical guidelines are needed for forensic autopsy practice in cases of natural unexpected deaths due to ischemic heart disease or other cardiovascular diseases in Korea. An evaluation of the clinical history is the first step, including the collection of all available information on the subject. As a standard autopsy procedure, every subsystem of the heart and vessels should be thoroughly studied including the coronary arteries, the myocardium, the cardiac valves, and the conducting system. In addition, histological, toxicological, biochemical and molecular analysis need to be applied to the detection of myocardial infarction from the medico-legal point of view. Also important is the asses...

We report a case of metastatic MTC presented as palpable neck and breast nodules in a 39-year-old woman. Fine needle aspiration of breast nodules showed a cellular smear with relatively homogenous, poorly cohesive tumor cells in a relatively clean background. The tumor cells were round to oval, with hyperchromatic nuclei, abundant cytoplasm and plasmacytoid in appearance. A few amorphous materials, possibly amyloid, were also noted. The patient underwent radical thyroidectomy, cervical lymph node dissection and excision of breast nodules. The primary thyroid mass was typical of MTC but metastatic breast tumors showed more homogenous and smaller tumor cells. The cellular origin of the breast tumors was confirmed by their immunoreactivity for calcitonin, chromogranin, synaptophysin and TTF-1...

Gastrointestinal stromal tumors (GISTs) present as a spectrum, from clinically benign to malignant. Metastases usually become clinically apparent within 18[ndash]24 months of diagnosis, though they can develop as many as 30 years after primary tumor removal. The most common metastatic sites are the peritoneal surface and the liver. Pulmonary metastases are less common (approximately 7% of all metastises). Here, we report a case of pulmonary metastasis of a GIST of the rectum. A 50 year-old-woman, having received a Miles' operation for GIST of the rectum 10 years previous, was found to have a tumor in the right lung during a follow-up examination. A 2.5 cm sized well-demarcated mass presented in her lung parenchyma. The tumor was composed of spindle and epithelioid cells showing diffuse and...

There is a controversy on the causal relationship of sexual intercourse as a cause of sudden death. Since sildenafil (Viagra[trade]) has been introduced in 1998, there are more concerns on the safety issue of sildenafil and sexual activity as a forensic determination of the cause of death in those cases. We have collected 68 cases of sudden death occurred within 24 hours after sexual intercourse among 13,824 cases for five years from 2001 at the head office of the National Institute of Scientific Investigation in Seoul, Korea. There were 59 men and 9 women (M:F = 6.6:1). The most common cause of death was ischemic heart disease (20 in 68, 29.4%) in both men and women. Sildenafil (Viagra[trade]) was detected in blood in 8 men and only in gastric content in 2 men (10/59, 17.0%). The level in...

Florid cemento-osseous dysplasia (FCOD) is a benign, non-neoplastic lesion characterized by multiple sclerosing masses only within jawbones. It is frequently confused with chronic diffuse sclerosing osteomyelitis (CDSO) in the literature. Two cases of FCOD are examined to determine the characteristics of their calcifying tissues. The first case was non-infected, while the second case was severely infected, displaying the typical features of CDSO in clinico-radiologic findings. The infected FCOD case showed large bacterial colonies in the main lesion with rare inflammatory reaction. The globular cementum-like materials of FCOD looked like woven bone and showed increased positive reaction to Alcian blue stain, of which peripheral margin was strongly positive for the antibody of ameloblastin,...

We report a case of malignant solitary fibrous tumor in the back of a 27-year-old male patient. Grossly, the mass was well circumscribed, unencapsulated, soft to rubbery, oval shaped and measured 11.0 cm at its greatest diameter. The cut surface showed multinodular whitish-gray to tan solid appearance. Microscopically, the tumor revealed a wide spectrum of histologic features with alternating hypercellular and hypocellular areas, patternless architecture, haphazard arrangement of oval to spindle cells, branching vessels with perivascular hyalinization, frequent keloid-like collagen deposits and stromal hyalinization. Multifocal hypercellular areas revealed moderate to marked cytologic atypia and frequent mitotic figures with atypical mitoses. (Source: Basic and Applied Pathology)

We describe the immunohistochemical findings of the skin lesion of Kikuchi's disease (KD) in two patients. They presented with fever, lymphadenopathy, and eruptions on the skin of palm and face. Each patient had a lymph node biopsy which showed the typical necrotizing lymphadenitis of KD. Skin biopsies from the two patients showed various pathologic findings of a crusted epidermis, a dense lymphohistiocytic perivascular and interstitial infiltrate, and abundant karyorrhectic debris with an absence of neutrophils, paralleling the nodal histology in KD. Immunohistochemically, the lymphohistiocytic cells in the skin and lymph nodes showed similar positivity for CD68, CD3, CD8, granzyme B and negativity for CD20 and CD4. These findings suggest that the cytotoxic T lymphocytes play important ro...

Liver fibrosis is the common scarring response of the liver to most chronic liver damage. Continuous chronic liver damage leads to a progressive accumulation of scarring proteins and finally, alters normal tissue structure and function by inducing fibrosis, cirrhosis and ultimately liver failure. Despite the considerable evolution on the study of liver fibrosis and cirrhosis during the past decade, a full understanding of liver fibrosis and cirrhosis still remains a goal for scientists. Although liver fibrosis and cirrhosis is mediated by various cytokines including tumor necrosis factor-alpha, interleukin-10 and transforming growth factor-beta (TGF-[beta]), it has been shown that TGF-[beta] plays the most significant role in chronic liver disease. In early liver injury stage, TGF-[beta] i...
