The Term Breech Trial Ten Years On: Primum Non Nocere?

Abstract:  In 2000, the Term Breech Trial was published, and its authors recommended cesarean section as the safest mode of delivery for breech‐presenting babies. Criticisms of the trial were raised at the time, which the authors dismissed. Since then, maternal deaths have been recorded among women undergoing cesarean sections for breech presentations. Accordingly, those initial criticisms deserve to be revisited. (BIRTH 39:1 March 2012) (Source: Birth)

Fetal Movement Counting—Maternal Concern and Experiences: A Multicenter, Randomized, Controlled Trial

Conclusions:  Women who performed fetal movement counting in the third trimester reported less concern than those in the control group. The frequency of maternal report of concern about decreased fetal activity was similar between the groups. Most women considered the use of a counting chart to be positive.
Conclusions:  Many women who succeeded felt that they had “learned the hard way” and some of those who did not, felt they were perceived as “bad mothers” and women who had in some way “failed” at one of the earliest tasks of motherhood. What women perceived to be staff perceptions affected how they saw themselves and what they took away from their early experience of infant feeding.
Requesting Cesareans without Medical Indications: An Option Being Considered by Young Canadian Women

Conclusions:  Young women are spreading the word about cesarean delivery on maternal request and may influence one another about their preferred delivery method. During prenatal visits practitioners need to address women’s fear of vaginal birth and its consequences for the mother, counseling, and women’s understanding of the consequences of cesarean delivery. This study supports the urgent need to systematically document cesarean delivery on maternal request as a medical procedure and to study its prevalence and related factors.
Web‐based News Reports on Midwives Compared with Obstetricians: A Prospective Analysis

Conclusions:  Although midwives have rising acceptance, they still struggle with recognition. Obstetricians have both acceptance and recognition. Countries where midwifery is a mainstream option have more news reports related to midwifery than obstetrics. Different issues appear more dominant in some countries, such as work force in the United Kingdom and funding, insurance, and legislation in the United States.
Conclusions:  Although most participants did not intend to resume smoking, their intentions may be stymied by their perceptions about second‐hand smoke and by their overestimation of their control over smoking. Further study should quantify these barriers and determine their evolution over the first year after pregnancy with the goal of informing more successful, targeted interventions.
The Patient Observer: Eye‐Movement Desensitization and Reprocessing for the Treatment of Posttraumatic Stress following Childbirth

Conclusions:  Treatment with eye‐movement desensitization and reprocessing reduced posttraumatic stress symptoms in these three women. They were all sufficiently confident to attempt vaginal birth rather than demanding an elective cesarean section. We advocate a large‐scale, randomized controlled trial involving women with postpartum posttraumatic stress disorder to evaluate the effect of eye‐movement desensitization and reprocessing in this patient group.
ABSTRACT:  In recent years nativity or nation of origin has become the focus of numerous pregnancy outcome studies. A recent research synthesis found that, although considerable heterogeneity in study designs hinders the development of broad generalizations concerning differences in pregnancy outcomes, migrant women were more likely to have better low‐birthweight and preterm birth outcomes than women born in the receiving country in most of the studies that could be incorporated in the meta‐analysis. Researchers considering studies of migration and pregnancy outcomes should incorporate more comprehensive measures of the migrant experience, as the dichotomous variable born or not born in the receiving country only opens the door to understanding the meaning of empirical observations c...

How Are Babies Fed? A Pilot Study Exploring Primary School Children’s Perceptions of Infant Feeding

Conclusions:  Children have a range of perceptions around infant feeding. They appear receptive to new ideas on the subject while of primary school age. An opportunity for education in primary schools arises to present breastfeeding to children as a normal way to feed a baby, but it is vital that education is evidence based and any interventions are evaluated.
In‐Hospital Formula Supplementation of Breastfed Babies: A Population‐Based Survey

Conclusions:  The number of factors associated with in‐hospital formula supplementation suggests that this practice is complex. Some results, however, point to an opportunity for intervention, with the BFHI appearing to be an effective strategy for supporting exclusive breastfeeding.
Conclusions:  Fetal movement counting in the third trimester does not stimulate antenatal maternal‐fetal attachment. This result differs from a previous study where fetal movement counting improved maternal‐fetal attachment. Further research with a focus on possible mediating factors such as levels of stress, concern, and other psychological factors is required. (BIRTH 38:4 December 2011) (Source: Birth)

One Baby, Four Parents: Assisted Reproduction for Two Couples in New Zealand

Preface:  Egg donation by another woman in New Zealand and elsewhere has become a successful solution for childless couples who want to have a baby. In vitro fertilization (IVF) was first undertaken in New Zealand in 1983 (1). Approximately 5 in 100 couples have IVF in New Zealand, and 450 IVF babies are born each year (2). In this article, two women and two men who underwent this assisted reproductive process describe, in question‐and‐answer format, their experiences from their initial meeting and proposal, evaluation, egg retrieval and implantation, and pregnancy, to the birth of a healthy infant. Petra is the egg donor and genetic mother and Adam is her husband, Donna is the recipient and biological mother, and Erich is her husband and the genetic/biological father.
Conclusions:  The siblings’ situation is characterized by having a parent who tries to maintain a balance between grief and everyday life. Parents are present and engaged in joint activities around the stillbirth together with the siblings of the stillborn baby. Although parents are aware of the sibling’s situation, they feel that they are left somewhat alone in their parenthood after stillbirth and therefore need support and guidance from others.
Attitudes of the New Generation of Canadian Obstetricians: How Do They Differ from Their Predecessors?

Conclusions:  Younger obstetricians were more evidence‐based for some issues and less for others. In general younger obstetricians were more supportive of the role of birth technology in normal birth, including routine epidural analgesia, and they were less appreciative of the role of women in their own birth. They saw cesarean section as a solution to many perceived labor and birth problems. Results suggest a need to examine how obstetricians acquire their favorable attitudes to birth technology in normal birth. (BIRTH 38:2 June 2011) (Source: Birth)

Abstract:In Australia in 2007, a woman with two previous normal vaginal deliveries underwent an emergency cesarean section at full dilatation of the cervix with a breech presentation. The woman died after a severe hemorrhage. The official Coroner’s Report attributed the cause of death to postpartum hemorrhage, whereas the breech presentation was barely mentioned, suggesting that complications with breech cesarean deliveries are under‐appreciated and under‐reported.
Barriers to Managing Diabetes During Pregnancy: The Perceptions of Health Care Practitioners

Conclusions:  Educating women with diabetes about the importance of using effective birth control until they have achieved good glycemic control can help reduce the risk for adverse pregnancy outcomes. Motivators and barriers for a woman with diabetes to achieve glycemic control before, during, and after pregnancy should be considered when developing approaches to improve outcomes. Helping practitioners know what and how to address the needs of childbearing women with or at risk for diabetes can be beneficial. Additional efforts to increase women’s knowledge about diabetes and pregnancy and to develop effective strategies to encourage women’s achievement and maintenance of glycemic control before, during, and after pregnancy are needed.

Conclusions:  A substantial portion of women used nonprescribed methods to induce labor, often without discussing them with a physician. Maternity caregivers may want to inquire about such issues, especially where interventions may do more harm than good. (BIRTH 38:2 June 2011) (Source: Birth)

Abstract:  A Swiss study attempted to establish normal values for the uterine involution expected during the first 5 days postpartum. Two methods were used, apparently independently and without validation. One, named the finger method, consisted of estimating the height of the uterine fundus in finger breadths above or below the umbilicus. The other, the tape method, consisted of marking the distance between the uterine fundus and the pubic symphysis on a blank tape, which was subsequently read in centimeters against a measuring tape. The authors contend that normal uterine involution postpartum is governed by different factors according to whether it is measured by fingers or by tape. Thus, gestational age affected uterine involution when measured with fingers, but not when measured w...

Attitudes of Midwives in Sweden Toward a Woman’s Refusal of an Emergency Cesarean Section or a Cesarean Section on Request

Conclusions:  The main focus of midwives seems to be the baby’s health, and therefore they do not always agree with respect to a woman’s refusal or request for a cesarean section. The midwives prefer to continue to explain the situation and persuade the woman to agree with the recommendation of the obstetrician. (BIRTH 38:1 March 2011) (Source: Birth)

Conclusions:  In some communities few opportunities may occur to witness breastfeeding, and thus existing fears concerning the activity as attracting predatory male attention remain unchallenged. Perceptions of breastfeeding as a sexual activity and the dominant mass media emphasis on breasts as a sexual site may present additional obstacles to breastfeeding. Antenatal or perinatal education with men should address not only practical issues but also provide advice on tackling problems generated by wider sociocultural issues of sexuality and masculinity.
Women’s Perceptions and Experiences of Breastfeeding Support: A Metasynthesis

Conclusions:  The findings emphasize the importance of person‐centered communication skills and of relationships in supporting a woman to breastfeed. Organizational systems and services that facilitate continuity of caregiver, for example continuity of midwifery care or peer support models, are more likely to facilitate an authentic presence, involving supportive care and a trusting relationship with professionals.
Prevalence and Predictors of Alcohol Use in Pregnancy and Breastfeeding Among Australian Women

Conclusions:  More research is needed to ease uncertainty about “safe” levels of alcohol use during pregnancy and while breastfeeding. A high proportion of the sample reported alcohol use during pregnancy or lactation, despite uniform international government guidelines recommending that no alcohol should be consumed during the prenatal and postnatal periods. These results indicate that public health education campaigns about the risks of alcohol during these periods are needed. (BIRTH 38:1 March 2011) (Source: Birth)

Women’s Preferences for Aspects of Labor Management: Results from a Discrete Choice Experiment

Conclusions:  Women appear to dislike being turned away from the labor ward before admission for delivery. Extra visits before admission only appear to be a price worth paying if they result in reductions in the duration of time spent on the labor ward, reductions in the chance of being immobilized in hospital during labor, or a lower chance of requiring an instrumental or operative delivery.
Abstract:  Home birth has attracted a great deal of attention of late, culminating in a meta‐analysis to assess its risks for mother and baby. Mothers were estimated to be 2.6 times more likely to die and babies 3 times more likely to die from a planned home birth than from a planned hospital birth. The actual data on which these estimates were based demonstrate that meta‐analysis can be developed into an art that suits whatever purpose its authors hope to achieve. Combining studies of home versus hospital, without differentiating what is inside them, where they are, and what is around them, is akin to producing a fruit salad with potatoes, pineapples, and celery.
Abstract:  Several studies have reported a good pain relief effect from sterile water injections, especially for low back pain, during labor. The aim of this paper was to illustrate the use of the method in a clinical childbirth situation by means of a descriptive case report. The woman in the report described that the method provided a powerful pain relief effect, measured by a visual analog scale, and that her experience was highly positive.
Conclusions:  Misconceptions about the appropriateness of oral health care during pregnancy may affect women’s access to and use of this care. Given the implications of poor oral health on possible adverse birth outcomes and its larger connection with the general health of mothers and babies, attention to oral health misconceptions and barriers is warranted.

Conclusions:  Women who planned a home birth and women who had a cesarean section based on maternal request are significantly different groups of mothers in terms of sociodemographic background. In a birth context that promotes neither home birth nor cesarean section without medical reasons, we found that those women who had a planned home birth felt more involvement in decision making and had a more positive birth experience than those who had a requested, planned cesarean section.
Examining Autonomy’s Boundaries: A Follow‐up Review of Perinatal Mortality Cases in UK Independent Midwifery

Conclusions:  Information about clinical processes (and outcomes) is essential if informed decisions are to be made. The women in this review had reportedly accepted the potential consequences of their high‐risk situations. If reality is to match rhetoric about “patient” autonomy, such decision making in high‐risk situations must be accepted. (BIRTH 37:4 December 2010) (Source: Birth)

Elective Cesarean Delivery: Does It Have a Negative Effect on Breastfeeding?

Conclusions:  Emergency and elective cesarean deliveries are similarly associated with a decreased rate of exclusive breastfeeding compared with vaginal delivery. The inability of women who have undergone a cesarean section to breastfeed comfortably in the delivery room and in the immediate postpartum period seems to be the most likely explanation for this association.
Conclusions:  A history of childhood abuse significantly increased the risk of experiencing severe fear of childbirth among primiparas. Fear of childbirth among multiparas was most strongly associated with a negative birth experience. (BIRTH 37:4 December 2010) (Source: Birth)

A Mirage of Change: Family-Centered Maternity Care in Practice

Conclusions: Despite positive changes in recent years, family-centered maternity care in Canada still needs to be improved. Women's ability to use their acquired prenatal knowledge to feel satisfied by their birth experience continues to be undermined by a system of care that does not prioritize women's informed choice. Further systemic change is required to align maternity care with the needs of Canadian birthing women and their families. (BIRTH 37:2 June 2010) (Source: Birth)

Conclusions: Women responded as individuals and despite different clinical circumstances, the role of the staff and the institutions in which care was provided were key factors in the way most women constructed their cesarean section experience. The themes described present a powerful argument and reminder about why health professionals working in maternity care need to continue to listen to women.
Conclusions: Breastfeeding was viewed as a morally inappropriate behavior by most of these teenagers, with formula feeding being perceived as the appropriate behavior. Existing breastfeeding promotion activities are likely to continue to fail to reach teenagers experiencing deprivation in England in the absence of effective strategies to change the underlying negative moral norms toward breastfeeding.
