    Abstract
    Malignant pleural effusions are a common complication of many cancers, usually representing metastatic disease. Over 50% of cases are the result of primary lung or pleural malignancy (Janssen et al, 2007), but they may also be caused by other malignancies including lymphoma, breast or ovarian carcinoma. An effusion can be the result of direct tumour involvement, or result from indirect effects such as lymphatic obstruction, sometimes referred to as a paramalignant effusion.

    Abstract
    The spine is the third most common site of cancer metastasis after the liver and lung (Sciubba et al, 2010). In the UK approximately 300000 people are diagnosed with cancer per year, of which 60-70% will have spinal metastasis. Early diagnosis and management can have a significant impact on the morbidity and prognosis of these patients. It is therefore vital for doctors to be able to recognize, investigate and be aware of the management of spinal metastasis.

    This year marks the centenary of the death, in 1912, of Joseph Lister, in my opinion the greatest contributor to the science of surgery in this country, if not in the world. Indeed, 12 August 1865, the date of Lister's first operation under antiseptic surgical conditions, represents the watershed between long centuries of almost inevitable sepsis and risk of death following the most minor surgical procedures and the 'modern' era of surgery.

    Abstract
    Pulsus bisferiens is Latin for 'beat twice'. This describes the pulse where two systolic beats are palpated each cardiac cycle with an interposed mid-systolic dip. The earliest reference to the pulsus bisferiens is an edition of Galen's de Pulsibus, published in 1532. Galen was insistent that the pulsus bisferiens is characterized by 'quies media ictus' - a central pause in the stroke. Normally, the character of the pulse is best palpated in central arteries, e.g. the carotid artery. Pulsus bisferiens is the exception, however, as it is most easily palpated in the peripheral arterial system, e.g. the radial artery.

    Splenomegaly is a common finding on clinical examination, and frequently features in postgraduate assessments. The spleen does not normally descend below the left costal margin.     Abstract
    In pregnancy the liver can be affected by diseases specific to pregnancy as well as unrelated conditions. The possible effect of the disease and its management on both the fetus and mother must be considered. Several physiological changes occur during pregnancy as liver metabolism is altered. Serum protein concentrations fall, with a decrease in serum albumin in part the result of the dilutional effect of an increase in plasma volume. Alanine transaminase and aspartate transaminase levels decrease (Table 1), complicating the diagnosis of disorders involving subtle changes in liver function. Alkaline phosphatase is also produced by the placenta, making this an unreliable marker of liver dysfunction in pregnancy.

    A young woman presented with severe chest pain and an inverted takotsubo cardiomyopathy following adrenaline injection for treatment of an allergic reaction. To the authors' knowledge, this is the first reported case of inverted takotsubo cardiomyopathy following systemic adrenaline administration. The case strongly suggests a central role of catecholamines in the pathogenesis of takotsubo cardiomyopathy.

            <description>This article outlines the case of a 62-year-old man who presented with a deep vein thrombosis, pulmonary embolus and right middle cerebral artery territory ischaemic stroke in association with a patent foramen ovale with right-to-left shunting. Despite anticoagulation, he was noted to have desaturation and tachypnoea when upright, without other symptoms or cardiovascular compromise, that resolved on lying down. The article describes the syndrome of platypnoea-orthodeoxia, characterized by breathlessness, hypoxia and/or cyanosis while in the upright position that improves on lying down, its relationship to this case, and its resolution by closure of the patent foramen ovale.

    Guy's Hospital and its medical school rejoiced in a remarkable group of physicians in the 19th century. These included Richard Bright, a pioneer in the study of chronic nephritis, Thomas Addison, who described two diseases that bear his name (Addison's disease of the suprarenals and Addison's, or pernicious, anaemia), Thomas Hodgkin, who gave the first clear account of lymphadenoma and Sir William Gull, who documented myxoedema, its relationship to the thyroid and its resemblance to cretinism in children.

            <description>This article discusses an elderly woman presenting with haematemesis and chest pain who underwent urgent oesophago-gastroduodenoscopy. This revealed a large dissecting intramural oesophageal haematoma, initially mistaken for a varix. This is a rare cause of haematemesis and chest pain but has characteristic endoscopic findings. It is a benign condition that may be managed conservatively and both clinicians and endoscopists should be aware of its classical presentation. Misdiagnosis of the chest pain as cardiac ischaemia may have an adverse outcome if antiplatelet or anticoagulation therapy is commenced. Follow up endoscopy a week later showed complete resolution of the lesion leaving a linear mucosal defect.

A 12-lead electrocardiogram, as shown in Figure 1, had the appearance of torsades de pointes. However, the patient was cardiologically asymptomatic, and the electrocardiogram appearance was thought to be caused by a Parkinson's disease-associated artefact.

    Abstract
    This year is the 250th anniversary of the birth of Matthew Baillie, whose textbook Morbid Anatomy of the Human Body, published in 1793, and its accompanying atlas, published 6 years later, constituted the first textbook which dealt exclusively with system-based pathology. This adopted a new and convenient method of describing pathology according to the organs involved rather than according to symptoms, as Giovanni Morgagni (1682-1771) of Padua had adopted in his De Sedibus et Causis Morborum (On the sites and causes of disease). This, published in 1760, was rightly regarded as the first textbook which correlated symptoms in life with the appearance of the viscera at autopsy.

    The National End of Life Care Strategy for England (Department of Health, 2008) was a blueprint for improving the care of all dying people over the next 10 years regardless of diagnosis. The strategy emphasized the importance of improved end of life care provision in acute hospitals as more than half of all deaths take place there. As well as ensuring that those who die in hospital have a good death, the strategy called for improved discharge arrangements and better coordination with a range of community services so that more people can die at home if this is their preferred choice.

    Colicky abdominal pain is an intermittent spasmodic pain within the abdominal cavity, characterized by multiple recurrent episodes. The severity of the pain can vary from mild abdominal cramps to very intense, acute attacks, which require hospitalization and treatment with intra-venous analgesia. Between recurrent episodes, patients usually feel well and are pain and symptom free.

    The number of road signs on our motorways encouraging drivers to stop driving when they feel tired, highlights that up to a fifth of road traffic accidents may be caused by sleepiness. Drivers may be sleepy for many reasons, such as insufficient sleep following a late night or a night shift, but underlying medical conditions leading to daytime somnolence, such as obstructive sleep apnoea syndrome, are often under-recognized.

            <description>This article describes the case and sequelae of a young man accidently poisoned by carbon monoxide from a barbecue placed in his camper van. This seemingly innocent action, when the barbecue was apparently extinct and cool enough to pick up with his bare hands, resulted in him being discovered unconscious. Recognized as the most common cause of fatal poisoning worldwide (Hopkins and Woon, 2006), many people remain unaware of the dangers of carbon monoxide.
