This study aimed to identify the various silhouette scales that have been developed or adapted since 1983 and to conduct a systematic review of the validation of such scales against objective measures of nutritional status. A total of 33 publications were found and showed moderate to good correlation between nutritional status and both adapted (0.66 to 0.87) and developed silhouette scales (0.59 to 0.94) in adults, but much lower correlation in children and adolescents. Most of the studies used inappropriate statistical analysis. The data indicated that silhouette scales should be used with caution to predict nutritional status with or without anthropometric measures.

    Abstract
The aim of this study was to describe the pattern of trends in lung cancer mortality in Brazil and identify the effects of the factors age, period and cohort (APC) on mortality rates. A time series study was conducted using secondary population-based data. Lung cancer mortality rates by sex were calculated for the period 1980 to 2007. APC models were adjusted to identify the influence of age, period and cohort effects on rates. Lung cancer mortality rates are significantly higher among men. Specific rates for men over the age of 64 and for women of all ages are increasing. There was a greater increase of adjusted rates among women. With respect to the age effect, mortality risk increases with age starting with the earliest age gro...

    Abstract
Leprosy may present acute/subacute inflammatory processes (leprosy reactions). The study characterized the reactional states of patients at health clinics in Vitória, Espírito Santo State, Brazil, and associated them with sociodemographic factors and clinical/nutritional variables. between January and December 2009, longitudinal follow-up of patients with leprosy continued until leprosy reactions occurred or patients completed 6 months of multidrug therapy. Of the 151 patients participating, 78 (51.7%) were females, 48 (31.8%) had 5 to 8 years schooling, 93 (61.6%) worked and earned from 1 to 3 minimum wages, and 55 (36.4 %) had leprosy reactions, but with no statistical association to socioeconomic characteris...

    Abstract
In order to describe trends in traffic accidents, mortality, vehicle types, fleet sizes, and victims' characteristics in Campinas, São Paulo State, Brazil, from 1995 to 2008, this study analyzed vehicle rates, traffic accident rates per inhabitant and per vehicle, case-fatality rates, proportional mortality, mortality rates, and rates ratios. The motorcycle fleet increased 241%. Although the case-fatality rate of motorcycle users from 2000 to 2008 decreased, in 2008 they accounted for 49.3% of fatal accidents on public byways in Campinas. Motorcycles were responsible for the highest run-over rate (66.7 pedestrians/1,000 accidents) and highest pedestrian fatality rate (4 deaths/1,000 accidents). Men sh...

    Abstract
The aim of this study was to describe the medicines prescribed to elderly residents of Florianópolis, Santa Catarina State, Brazil, in the 30 days prior to the interview, and to compare them with the Municipal Medicines List (REMUME). A cross-sectional population-based household survey was conducted, in which 1,705 elderly respondents reported having used a total of 5,458 prescription drugs meeting the study criteria and corresponding to 374 different active ingredients. The most frequently used medicines were for the cardiovascular system, among which the most prevalent pharmacological class was lipid-lowering drugs (although there were no examples of this drug class in the REMUME list). Despite some gaps, the majority of the drugs pr...

This study analyzed the atmospheric conditions on the day of death of 14 sugarcane cutters in São Paulo State, Brazil. Historical data for temperature, humidity, wind, and sunlight were compared with the same meteorological variables on the days the deaths that occurred. The meteorological data were obtained from a reanalysis of the National Center for Environmental Prediction/National Center for Atmospheric Research (NCEP/NCAR). In 10 of the 14 cases, the temperature on the day of death was higher than or equal to the mean plus the standard deviation. In six of these cases, the temperature reached or came close to record levels. These findings suggest that atmospheric conditions can be an important factor in the analysis of overall work conditions for sugarcane cutters.

This study evaluated the validity of self-reported weight and height for determining nutritional status and the implications of their use for analyzing associations with health outcomes. A population-based cross-sectional study in Pelotas, Rio Grande do Sul State, Brazil, in 2007 (n = 2,986) drew a sub-sample of 276 individuals aged > 20 years. Mean self-reported weight was similar to measured weight; height was overestimated in men (1.4cm) and women (2.5cm); real body mass index (BMI) was underestimated by about 1kg/m². Even with small mean differences, data variability was great. The results were influenced by gender, age, and schooling. The use of self-reported measures underestimated prevalence of overweight and obesity, and unpredictable errors were found in the analysis of associ...

This study aimed to identify degrees of exposure to community violence reported by parents and guardians of children from four to 12 years of age and the association with childhood asthma symptoms. A cross-sectional study was conducted with 1,232 parents/guardians in 24 peripheral neighborhoods in Salvador, the capital of Bahia State, Brazil. The authors investigated asthma symptoms in children and acts of violence in the community. More than 75% of parents/guardians had been exposed to community violence in the previous year, with 20% reporting high levels of exposure. Children that were more exposed to violence showed higher asthma prevalence (28.4%) as compared to non-exposed children (16.4%). Children exposed to maximum levels of violence were nearly twice as likely to present asthma s...

    Abstract
The aim of this study was to compare measured height with estimates of height derived from equations using half arm-span measurements and assess how this affects the calculation of the body mass index (BMI) among elderly individuals. Direct height measurements of a subsample of elderly individuals from the baseline sample of the Bambuí Project were compared with estimates of height derived from equations proposed by Bassey and the WHO. The data was analyzed using the McNemar test, Lin concordance correlation coefficient (CCC) and Bland-Altman method (p < 0.05). Estimates of height using the WHO method showed a low CCC in relation to measured height. For BMI, the concordance was greater. H...

This study compared asthma prevalence in two cross-sectional studies with a ten-year interval in Pelotas, Rio Grande do Sul State, Brazil. The first, in 2000, included 1,968 individuals, and the second, in 2010, 2,466 adults (20-69 years). Prevalence of wheezing and shortness of breath in the prior 12 months remained the same after ten years (6% and 6.1%, respectively). In both studies, asthma was more frequent among females and people with low family income. Physician-diagnosed asthma increased by 35.6%, and lifetime incidence of asthma, by 32.2%. There was no percentage change in current asthma symptoms or current asthma. Local socioeconomic improvement between the two studies was consistent with the increase in medical diagnosis, but did not reflect better management of asthma symptoms,...

This study examines the contribution of Workers' Health Referral Centers (CEREST) to the reporting of severe work-related injuries and those involving exposure to biological materials in the Brazilian National Health Reporting System (SINAN), under the Unified National Health System (SUS). The study used data from the FormSUS and SINAN databases, aggregated for the CEREST coverage areas. Valid data were obtained for 125 CEREST (23 State and 102 regional). A majority of the CEREST were assessed as fully installed. The increase in the reporting of severe work-related accidents was greater when staffing was consistent with the demand, and when teams responded to external demands, including those of the media. For exposures to biological material, CEREST with good physical installations, those...

    Abstract
This introduction outlines the Forum on stigma, discrimination, and health: policies and research challenges, including four articles and a final commentary. The first article reviews the development of international research on the relationship between discrimination and health. The second analyzes the recent Brazilian research output on AIDS, stigma, and discrimination. The third article addresses conceptual and methodological aspects of the relations between discrimination and health from an epidemiological perspective. The forth examines the process of affirming sexual rights in Brazil, involving the judiciary system, public policies, and the executive and legislative branches of government, among others. The fifth paper presents comment...

This article reviews the development of international research on the relationship between discrimination and health. It provides an overview of theoretical and empirical work on stigma and prejudice and their impact on discrimination and health. It argues that the literature on these issues has drawn primarily from social psychology and has focused on the impact of attitudes associated with stigma and prejudice on discriminatory practices and consequently health outcomes. It also identifies a growing trend in recent research towards a reconceptualization of stigma, prejudice and discrimination from the perspective of social inequality and structural violence, highlighting relations of power and exclusion that reinforce vulnerability within a complex social and political process. It conclu...

    Abstract
Given the implications of stigma for HIV/AIDS prevention and control of the epidemic, as emphasized by UNAIDS, this study analyzes the Brazilian academic production on health, AIDS, stigma, and discrimination, available in the SciELO database from 2005 to 2010. Brazilian research on the theme is modest as compared to the international literature, but the studies follow the same trend of focusing on individual experiences of discrimination as opposed to analysis of stigma and discrimination as social processes associated with power relations and domination (macro-social structures) and the characteristics of individuals and social groups that shape social interactions. The current study seeks to analyze the reasons for the scarcity ...

This article addresses conceptual and methodological aspects of the relations between discrimination and health from an epidemiological perspective. Definitions of discrimination and related constructs are reviewed, and the main theories underlying their association with health are presented. Scales developed to assess discrimination are discussed, in conjunction with a new instrument, devised to operationalize the concept in Brazilian epidemiological surveys. As a relatively unpredictable and uncontrollable source of psychosocial stress, discrimination has been consistently associated with adverse health outcomes and behaviors, particularly mental disorders, smoking, and alcohol use. However, progress in the area depends partly on dealing with aspects related to the assessment of the phen...

This article focuses on a politics arena that has been articulated through the impact of ideals of sexual rights on Brazilian sexual politics, namely the affirmation of "LGBT rights". These rights have been constructed both through attempts to extend civil and social rights to the LGBT population that were previously restricted to heterosexuals, and by the enactment of provisions directly banning homophobic discrimination and violence. The focus will be on some of the principal social actors in this process, especially those situated in the three branches of government, since the most decisive clashes are now being waged at this level. Without intending to offer an exhaustive description of what has occurred in the Brazilian courts, Congress, and Administration, we point to the complexity ...

This article estimates the impact of mortality from external causes on the human development index (HDI) along the Brazilian borderland from 2000 to 2005. Data obtained from Brazilian government agencies were combined using the methodology defined by the United Nations Development Program, revealing the HDI according to actual conditions. Subsequently, deaths from external causes were excluded in order to estimate their impact on the index, recalculating life expectancy using the technique of competing causes. HDI showed a gradual increase from North to South, with the most developed regions concentrated in the South, consistent with studies using other sets of economic indicators. By excluding mortality from external causes, the highest gains appeared in regions where the HDI (under actua...

This article presents a meta-analysis of studies on the association between maternal mental illness and congenital malformations. The review consisted of an article search in the MEDLINE, ISIWEB, Scopus, and SciELO databases, using the following key words: "mental disorders" OR "mental health" OR "psychotic disorders" OR "schizophrenia" AND "congenital abnormalities" OR "birth defects". A total of 108 studies were identified, and five articles were selected according to the established criteria. These articles were included in a meta-analysis, involving a total of 4,194 children of mothers with mental illness and 249,548 children of mothers with no such disorders. Pooled relative risk showed a significant association between exposure to mental illness in mothers and risk of malformations i...

    Abstract
Researchers in the health field often deal with the problem of incomplete databases. Complete Case Analysis (CCA), which restricts the analysis to subjects with complete data, reduces the sample size and may result in biased estimates. Based on statistical grounds, Multiple Imputation (MI) uses all collected data and is recommended as an alternative to CCA. Data from the study Saúde em Beagá, attended by 4,048 adults from two of nine health districts in the city of Belo Horizonte, Minas Gerais State, Brazil, in 2008-2009, were used to evaluate CCA and different MI approaches in the context of logistic models with incomplete covariate data. Peculiarities in some variables in this study allowed analyzing a...

This article presents a model for the evaluation of community participation in municipal (county) health systems. The theoretical basis for this evaluative study was the existing Brazilian legislation on community participation in drafting health strategies and overseeing health policy implementation. The indicators and measures were validated using the expert consensus technique. The proposed model has 26 indicators divided into five evaluative dimensions that reflect the legislation's adequacy for the structure, organization, representativeness, health planning participation, and autonomy of the Municipal Health Councils. Its applicability was tested in 24 municipalities in Santa Catarina State, Brazil. The data analysis indicated the best results for the dimension "planning system in th...

    Abstract
The present study aimed to assess mortality rates and related demographic factors among indigenous peoples in the State of Mato Grosso do Sul, Central-West Brazil, compared to the State's general population. Mortality rates were estimated based on data obtained from the Health Care Database for Indigenous Peoples and monthly patient care records as well as demographic data from the Brazilian Unified National Health System (SUS) and mortality data from the SUS Mortality Database. Compared to the overall population, among indigenous peoples there were proportionally more individuals under 15 years of age and fewer elderly, besides higher mortality rates at early ages and from infectious and parasitic diseases. Indigenous men showed si...

    Abstract
In order to evaluate the prevalence of overweight and associated factors in Pernambuco State, Brazil, a cross-sectional population-based study was conducted in 2006, including 1,580 adults 25 to 59 years of age. Overweight was defined as body mass index > 25kg/m2. The conceptual model included demographic, socioeconomic, reproductive, and behavioral variables. Prevalence of overweight was 51.1% (95%CI: 48.6-53.6) and was statistically associated with age over 40 years (PR = 1.27; 95%CI: 1.10-1.46), female gender (PR = 1.29; 95%CI: 1.16-1.43), former smoking (PR = 1.42; 95%CI: 1.20-1.69), higher income (PR = 1.49; 95%CI: 1.30-1.71), and history of early pregnancy (< 18 year...

The objective of this study was to reassess the dimensional structure of a Brazilian version of the Scale of Satisfaction with Interpersonal Processes of General Medical Care, proposed originally as a one-dimensional instrument. Strict confirmatory factor analysis (CFA) and exploratory factor analysis modeled within a CFA framework (E/CFA) were used to identify the best model. An initial CFA rejected the one-dimensional structure, while an E/CFA suggested a two-dimensional structure. The latter structure was followed by a new CFA, which showed that the model without cross-loading was the most parsimonious, with adequate fit indices (CFI = 0.982 and TLI = 0.988), except for RMSEA (0.062). Although the model achieved convergent validity, discriminant validity was questionable, with the squar...

    Abstract
The aim of this population-based cross-sectional study was to investigate access by 20 to 60 year-old women - both black and white - to early detection (pap-smear) exams for breast and cervical cancer in two towns - São Leopoldo and Pelotas - in Rio Grande do Sul State, southern Brazil. Estimates of the association between race/color and access to pap-smear and breast exams were adjusted for income, education, economic class and age. Of the 2,030 women interviewed, 16.1% were black and 83.9%, white. Black women were significantly less likely to have had a pap-smear and/or breast exam than white women. Racial inequalities in access to cancer early detection exams persisted aft...

    Abstract
Despite the existing resources for adequate dengue patient care in the Brazilian healthcare system, the case-fatality rate for the disease is still high in the country. In order to identify factors associated with dengue-related death, this study evaluated quality of care according to the degree of implementation of specific measures, the technical and scientific quality of care, and access to health services in two municipalities (counties) in Northeast Brazil. An evaluative study of the implementation analysis type was performed, with death from dengue as the sentinel event for quality of care. To assess the degree of implementation and quality of care, the study scored ...

This study evaluated the association between adolescent pregnancy and the completion of basic education, mediated by macrosocial indicators. A cross-sectional household survey was conducted with individuals between the ages of 18 and 24 in three Brazilian cities. For the purposes of this study, individuals between the ages of 20 and 24 were selected from this sample survey that included 4,634 people. A total of 29.6% of the girls declared that they had become pregnant prior to reaching the age of 20, while 21.4% of the boys stated that they had made a girl pregnant in adolescence. Girls from households with a per capita family income of US$70 or less and who became pregnant at least once during adolescence were more likely to have not completed basic education; whereas from households with...

    Abstract
Guidelines emphasize that the appropriate time frame for neonatal screening with the heel stick test is from the 3rd to 7th day of life, in 100% of newborns. Treatment for congenital hypothyroidism and phenylketonuria, when initiated in the first two weeks of life, is capable of preventing the neurological sequelae of these diseases. The Rio de Janeiro State Neonatal Screening Program was accredited for Phase 2 of the National Neonatal Screening Program, with two program models (A and B). A performance analysis was conducted for the Neonatal Screening Program, for the years 2005 to 2007. Coverage increased from 2002 to 2007, reaching 80.4%, with 33.8% of the blood samples drawn in the first 7 days of life. The two models showed simila...

The sample included 377 schoolchildren 10 to 18 years of age and 338 parents. The following variables were analyzed: level of leisure-time and commuting physical activity, inadequate eating habits, overweight/obesity, smoking, and alcohol consumption. Prevalence rates for smoking and alcohol consumption by adolescents in the previous 30 days were 6.7% and 27%, respectively. Forty-five percent of adolescents failed to reach the target physical activity score. Adherence to steps to healthy eating was low. Physical activity in adolescents was positively associated with that of parents, and adolescent drinking was associated with maternal alcohol consumption. There was no sign...

    Abstract
The subjective concept of health included in the self-rated health process allows identifying factors associated with good self-rated health in populations. This cross-sectional population-based study in 2007-2008 focused on self-rated health and associated factors among adults (n = 1,516) in Rio Branco, Acre State, Brazil. The multivariate analysis used Poisson regression, with fair/poor self-rated health as the dependent variable. Differences in gender, age group, level of schooling, and income negatively affected individuals' self-rated health. After adjusting for age, gender, and self-reported illness, the estimated risks for fair/poor health were higher for the unemployed, obese, and those with some self-reported ill...

We describe the Brazilian context of employment conditions, labor conditions and health, their characteristics and causal relationships. The social, political and economic factors that influence these relationships are also presented with an emphasis on social inequalities, and how they are reproduced within the labor market and thereby affect the health and wellbeing of workers. A literature review was conducted in SciELO, LILACS, Google and Google Scholar, MEDLINE and the CAPES Brazilian thesis database. We observed that there are more workers operating in the informal sector than in the formal sector and these former have no social insurance or any other social benefits. Work conditions and health are poor in both informal and formal enterprises since health and safety labor norms are n...

This study aimed to assess the quality of hospital care for acute ischemic stroke (AIS) in the Brazilian Unified National Health System (SUS), considering the severity of cases and use of computerized tomography. Multivariate analysis was used to adjust the mortality risk, evaluate the use of computerized tomography, and rank the performance of hospitals from April 2006 to December 2007. 16,879 admissions were analyzed, and at least one CT scan was performed in 28.6% of the admissions. The crude mortality rate was 34.3%, and the adjusted rate was 31.2%. Performance of a CT scan showed a protective effect: the adjusted OR was 0.27 for one test and 0.32 for two tests. Under-utilization of computerized tomography is one of the limiting factors for good medical practice in the management of is...

This study evaluated sand fly fauna after insecticide application. Captures were performed in 10 districts from September 2005 to August 2006 with CDC light traps inside and outside each residence. Cypermethrin was sprayed in two cycles during November/2005 and May/2006. The 636 specimens collected, belonging to 10 species, were predominantly Lutzomyia longipalpis (79%), and most frequently males (70%). The highest percentage of specimens were captured in areas surrounding domiciles (85.8%). The main species were observed to be sensitive to treatment with the insecticide. The results showed a reduction in the number of sand flies collected after use of cypermethrin in homes and annexes, and with residual effect lasting from two to four months.

This study focused on the annual cumulative incidence (ACI) of disabling work-related musculoskeletal disorders affecting the neck and/or upper limbs (ULMSD) among workers covered by the National Social Insurance System in the city of Salvador, Bahia State, Brazil. Cases were workers who received disability compensation benefits when unable to work due to ULMSD, during the year 2008. The data were obtained from the administrative systems of the National Social Insurance Institute and Ministry of Labor and Employment. ACI was 15 per 10,000 workers. Increased ACI of ULMSD was associated with female gender, lower income, and work in financial activities or manufacturing. Women earning the minimum wage (US$ 64.00 per month) or less had the highest ACI of ULMSD (123 per 10,000), suggesting ineq...

    Abstract
Data from the Brazilian Surveillance System for Violence and Accidents (VIVA) in 2009 were used to examine socio-demographic characteristics, outcomes, and types of accidents and violence treated at 74 sentinel emergency services in 23 Brazilian State capitals and the Federal District. The analysis included 25,201 individuals aged > 20 years (10.1% > 60 years); 89.3% were victims of accidents and 11.9% victims of violence. Hospitalization was the outcome in 11.1% of cases. Compared to the general population, there were more men and non-white individuals among victims of accidents, and especially among victims of violence. As compared to younger adults (20-59 years), accidents and violence against elderly vict...

This article presents an evaluation model for municipal health planning management. The basis was a methodological study using the health planning theoretical framework to construct the evaluation matrix, in addition to an understanding of the organization and functioning designed by the Planning System of the Unified National Health System (PlanejaSUS) and definition of responsibilities for the municipal level under the Health Management Pact. The indicators and measures were validated using the consensus technique with specialists in planning and evaluation. The applicability was tested in 271 municipalities (counties) in the State of Santa Catarina, Brazil, based on population size. The proposed model features two evaluative dimensions which reflect the municipal health administrator's ...

    Abstract
This paper analyzes the relationship between the financial health and organizational form of private health care providers in Brazil. It also examines the major determinants of customer satisfaction associated with the provider's organ-izational form. An adjusted Altman's z-score is used as an indicator of financial health. A proxy variable based on customer complaints filed at the Brazilian National Agency for Supplementary Health is used as an indicator for customer satisfaction. The study uses a sample of 270 private health care providers and their operations over the period 2003-2005. Panel data analysis includes control variables related to market, operations, and management. Principal results indicate that: (1) private health c...

This study focused on the factor structure of CTSPC scales in an urban population in Northeast Brazil. We conducted a cross-sectional study in a cohort of 1,370 children in Salvador, Bahia State. Factor analysis with promax oblique rotation was performed, and the Kuder-Richardson coefficient was calculated. Factor analysis showed a different distribution of items in the factors as compared to the original instrument. Violence showed a gradual profile in each factor. The Kuder-Richardson coefficient was 0.63 for factor 1, 0.59 for factor 2, and 0.42 for factor 3. The items behaved differently from the original instrument, corroborating international studies. These findings support proposing a resizing of the CTSPC.

This article examines the role of States in mental health policy in Brazil from 1990 to 2009. The methods included the use of an electronic questionnaire on State coordination of mental health in 24 Brazilian States, document review, and analysis based on official data. The results showed that the States use various strategies and tools to conduct mental health policy, especially in monitoring and services delivery. However, the study showed that States have not developed systematic approaches to planning, coordination with other levels of government, or coordination of care. Funding poses a major challenge for management of mental health at the State level. The study suggests that overcoming the challenges in mental health policy depends on the relationship between the Federal government,...

    Abstract
The aim was to assess regional influences on food consumption in infants less than six months of age. A cross-sectional study was conducted in a sample of 18,929 infants participating in the Second Survey on Breastfeeding Prevalence in Brazilian State Capitals and the Federal District in 2008. Consumption rates for tea, fruit juices, formula milk, and porridge were calculated for the State capitals from the five geographic regions of the country. Food consumption was estimated by logit analyses and Poisson models. Differences in food consumption profile were observed between the different regions: tea was more common in State capitals in the South (RP = 2.37), while non-maternal milk (RP = 1....

    Abstract
This paper describes the profile of 95 crack cocaine users attending three community mental health services (CAPS) in Greater Metropolitan Porto Alegre, Rio Grande do Sul State, Brazil, from August 2009 to March 2010. The instruments employed were questionnaires developed by the team, the Self-Reporting Questionnaire (SRQ-20), and inventories of criteria for dependence and abuse (SAMHSA). The data depict a group of users consisting predominantly of young males with elementary schooling, without regular employment but reporting individual income, none of whom living on the streets. They were currently addicted, with heavy daily use of crack for more than two years, and with high SRQ-20 score. This group's characteristics show...

    Abstract
Smoking of non-cigarette tobacco products is increasing worldwide because of their high social acceptability, misperceptions about their purported harmlessness, and globalization of the tobacco industry. In Brazil, tobacco control experts have recently focused their attention on the importance of monitoring the use of such products. We analyzed data from the Global Youth Tobacco Survey (2009) in three cities. Prevalence rates of non-cigarette tobacco smoking in the previous 30 days among students 13 to 15 years of age were high in Campo Grande (18.3%; 95%CI: 14.4%-22.9%) and São Paulo (22.1%; 95%CI: 19.0%-25.6%), while Vitória showed comparatively lower prevalence (4.3%; 95%CI: 3.1%-5.7%). Abstract
The cohort study was initiated in 1997 to investigate the incidence and predictors of health outcomes in an elderly population with low socio-economic level. The eligible population consisted of all 1,742 residents in Bambuí, Minas Gerais State, Brazil, aged 60 years and over (1,606 participated). During 10 years of follow-up, 641 participants died and 96 were lost, leading to 13,739 person-years of observation. The baseline health profile of participants revealed a double burden of diseases with high prevalence of chronic non-transmissible diseases and widespread Trypanosoma cruzi infection. The most common health condition was hypertension (61.5%), followed by chronic knee or hand symptoms (43.6%), common mental disorders (38.5...

    Abstract
We use data from a population-based cohort of elderly Brazilians to assess predictors of hospitalizations during ten years of follow-up. Participants were 1,448 persons aged 60 years and over at baseline (1997). The outcome was self-reported number of hospitalizations per year. Slightly more than a fifth (23%) experienced no hospitalizations during the 10 year follow-up. About 30% had 1-2 events, 31% had between 3 and 7 events, and about 18% had 8 or more events during this time. Results of multivariable hurdle and Cox proportional hazards models showed that the risk of hospitalization was positively associated with male sex, increased age, chronic conditions, and visits to the doctors in the previous 12 months. Abstract
Few population-based studies have examined long term cognitive trajectory, and these studies were conducted in high income countries. We investigated the association of age, gender and education with 10-year cognitive trajectory in a well-defined population of elderly using data from the Bambuí Cohort Study of Aging, in Brazil. Cognition was measured using the Mini- Mental State Examination (MMSE). Cohort members underwent annual follow-ups. This analysis was based on 12,206 MMSE measurements from 1,461 (91%) baseline participants. We used mixed effects models to study MMSE as outcome. The key findings from this analysis are that women and people with a higher schooling level at baseline had h...

This study aimed to examine the prognostic value of lipid parameters for incident hypertension in elderly living in a community. The study included 306 (81% from total) persons aged > 60 years who were free of hypertension and of cardiovascular diseases at the baseline survey of the Bambuí Cohort Study of Aging. The cumulative incidence of hypertension over three years was 37.3%. The relative risk (RR) of incident hypertension decreased 0.92 for each unit of HDL-cholesterol (95%CI: 0.86-0.99) independent of several potential confounding factors. Individuals with HDL-cholesterol in the top tercile (> 55mg/dL) had a risk of hypertension halve that those in the bottom tercile (RR = 0.54; 95%CI: 0.33-0.90). Other lipid parameters had no significant effect on the outcome. High HDL-choles...

    Abstract
This paper explores the contribution of anthropological perspectives for clarifying the mechanisms through which socioeconomic circumstances influence the strategies developed by elderly for addressing their health problems in a small Brazilian city. Interviews with 20 key-informants explored the community's broad perception of the health situation of the elderly. Life histories collected from 30 elderly women examined their own perception of their health status and their health strategies. Narratives converge in emphasising the important role played by financial factors in accessing health services and medication. Life histories also describe some damaging strategies resorted to by the elderly to deal with their lack of ...

    Abstract
The aim of this study was to investigate whether cohort differences exist in the prevalence of cardiovascular risk factors among older elderly from the Bambuí Cohort Study of Aging. Participants were those aged 71-81 years at two points in time a decade apart: 457 in 1997 (earlier cohort) and 553 in 2008 (recent cohort). The prevalence of hypertension (PR = 1.27; 95%CI: 1.19-1.36) and of diabetes mellitus (PR = 1.39; 95%CI: 1.06-1.83) was higher in the recent cohort compared to the earlier one, regardless of sex. The recent cohort had a lower prevalence of smoking (PR = 0.58; 95%CI: 0.42-0.80), and lower total cholesterol/HDL cholesterol ratio level (PR = 0.85; 95%CI: 0.80-0.89). There was a 136% increase in the pharmaco...

    Abstract
The aim of this study was to evaluate the effect of birth cohort on nutritional status among older elderly (71-81 years old) from the Bambuí Cohort Study of Aging, conducted in Brazil in 1997 and 2008. We compared the two birth cohorts - 1916-1926 (older cohort) and 1927-1937 (recent cohort) - considering body mass index (BMI = weight/height²), waist circumference (WC) and prevalence of overweight (BMI ³ 27kg/m²). BMI (β = 0.09, 95%CI: 0.04, 0.15) and prevalence of overweight (PR = 1.02; 95%CI: 1.01; 1.03) were higher in the recent cohort than the earlier cohort, regardless of sex and schooling. No difference was observed in WC. Stratified by sex, similar overall trends were observed for men, and WC was higher in...

We examined for cohort differences in hypertension control between participants in the Bambuí Cohort Study of Aging of aging born 1916-1926 and 1927-1937. Participants included hypertensive elderly aged 71-81 at baseline (n = 313) and at 11th wave follow up (n = 484). Prevalence of hypertension awareness (70.9% and 81.2%, respectively), median medical appointments in previous 12 months (2 and 3, respectively) and antihypertensive drug use (74.4% and 90.7% respectively) increased significantly from older to younger cohort. However, prevalence of appropriately controlled hypertension among those treated (< 140/< 90mmHg) was similar in both cohorts (44.6% and 40.1%, p = 0.255). Multivariate analysis returned schooling as the only factor independently associated with appropriately contr...

This study examined differences in the use of medications in two birth cohorts (born from 1916 to 1926 and from 1927 to 1937) among older elderly in the population-based cohort study in Bambuí, Minas Gerais State, Brazil. The study used data on participants who were 71-81 years of age in the baseline survey in 1997 (n = 492) and in the 11th wave, in 2008 (n = 620). The number of medications currently consumed (mean = 4.6 and 3.4, respectively) and prevalence of polypharmacy (46.6% and 29.1%, respectively) were higher in the more recent cohort as compared to the earlier one. These differences were independent of gender, age, schooling, number of medical visits in the previous 12 months, and number of chronic conditions.
This article aims to contribute to the debate on public policy evaluation, specifically in the Brazilian health sector. The objective is to analyze some of the key issues in this field through a literature search in the SciELO database. Sixty-six articles were selected using the descriptors "health evaluation" and "evaluation of health programs and projects" in indexed Brazilian public health periodicals from 1994 to 2009. The article explores aspects of academic output according to an established typology and in relation to possibilities for developing the theme of policy evaluation, linked to interventions focused on efficacious, efficient, and effective treatment of the population's health problems.

    Abstract
The aim of this study was to carry out a review of scientific literature published in Brazil between 2000 and 2009 on the characteristics of air pollutants from different emission sources, especially particulate matter (PM) and its effects on respiratory health. Using electronic databases, a systematic literature review was performed of all research related to air pollutant emissions. Publications were analyzed to identify the physical and chemical characteristics of pollutants from different emission sources and their related effects on the respiratory system. The PM2.5 is composed predominantly of organic compounds with 20% of inorganic elements. Higher concentrations of metals were detected in metropolitan areas than in biomass ...

This study investigated the disclosure of HIV-positive serostatus to sexual partners by heterosexual and bisexual men, selected in centers for HIV/AIDS care. In 250 interviews, we investigated disclosure of serostatus to partners, correlating disclosure to characteristics of relationships. The focus group further explored barriers to maintenance/establishment of partnerships and their association with disclosure and condom use. Fear of rejection led to isolation and distress, thus hindering disclosure to current and new partners. Disclosure requires trust and was more frequent to steady partners, to partners who were HIV-positive themselves, to female partners, and by heterosexuals, occurring less frequently with commercial sex workers. Most interviewees reported consistent condom use. Unp...

    Abstract
Cancer in children and adolescents is rare and highly curable if treatment is started early, yet it is still the main cause of death from disease in this age group. The aim of this study is to discuss access to health services for cancer patients under 19 years of age in Brazil, mapping deaths and treatment modalities in the Brazilian Unified National Health System (SUS). Data from 2000 to 2007 were analyzed according to health regions. Maps of cancer mortality rates and cancer care indicators - hospitalizations, chemotherapy, and radiotherapy financed by the national health system - revealed inequality in access, based on the small number of procedures for children in poorer regions of the country. Even with the usual concentr...

    Abstract
This cross-sectional epidemiological study aimed to calculate the potential years of life lost by female homicide victims in Recife, Pernambuco State, Brazil, in 2003-2007. A database was used from the Operational Division for Information on Births and Deaths under the Recife Municipal Health Department. All death certificates for childbearing-age women were reviewed for the five-year period. The results showed a total of 12,120 potential years of life lost by these women, mostly young, black (88%), with unknown levels of schooling (78.2%), single (80%), in District III of the city, and murdered with firearms in their own homes. The specific mortality rate was 10.8 homicides per 100,000 childbearing-age women. The 43.3 ye...

This paper presents a model for assessing the quality of primary dental care and tests its applicability in municipalities in Santa Catarina State, Brazil. The model features two components: Management of Oral Health and Provision of Primary Oral Health Care. The first consists of the sub-components Inter-Sector Practice, Community Participation, Human Resources, and Infrastructure. The second includes the sub-components Promotion/Prevention and Diagnosis and Treatment throughout the life cycle (childhood, adolescence, adulthood, and old age). Forty indicators were developed in workshops with a group of experts. 207 municipalities participated in the evaluation, yielding a response rate of 70.6%. The model proved to be applicable and highly be...

    Abstract
This cross-sectional study aimed to estimate mammogram coverage in the State of Goiás, Brazil, describing the supply, demand, and variations in different age groups, evaluating 98 mammography services as observational units. We estimated the mammogram rates by age group and type of health service, as well as the number of tests required to cover 70% and 100% of the target population. We assessed the association between mammograms, geographical distribution of mammography machines, type of service, and age group. Full coverage estimates, considering 100% of women in the 40-69 and 50-69-year age brackets, were 61% and 66%, of which the Brazilian Unified National Health Syst...

    Abstract
The construction of Brazil's Unified National Health System (SUS) has raised a set of challenges for the health sector's administrators and personnel, including issues of work management and continuing education for health workers, in view of the financial, political, and organizational constraints in the process of changing the healthcare model. The current study aimed to analyze the process of formulating the Health Work and Education Management Policy by the Bahia State Health Department. Public policy cycle was used as the theoretical framework. The study analyzed data from institutional documents and records of participant observation by one of the authors. The results include mapping the governmental and nongovernmental stakeholders tha...

    Abstract
This cross-sectional study of 831 low-risk pregnancies compared the management of labor and delivery in a birthing center, a hospital that had previously won the "Galba de Araújo" Award (for excellence in obstetric and neonatal care), and a standard-protocol maternity facility. The rates for use of ocytocin during labor were 27.9%, 59.5%, and 40.1%, while amniotomy was performed in 67.6%, 73.6%, and 82.2% of the women, respectively. Episiotomy rates were lower in the first two facilities, which have adopted patient-centered obstetric practices (7.2% at the birthing center and 14.8% at the award-winning hospital) as compared to 54.9% at the standard maternity facility. ...

The objective of this study was to evaluate the feasibility of using computer assisted telephone interviewing (CATI) as a method for obtaining information on reproductive health in Brazil. A total of 998 eligible women for the study were selected to answer a questionnaire through computer- assisted telephone interviewing undertaken by trained interviewers. The outcomes of each telephone contact attempt were described. Differences between groups were assessed using the χ2 test. Phone contact was made in 60.3% of the attempts and 57.5% of the interviews were completed. The success rate improved with the decrease in time from hospitalization to interview and with the higher numbers of telephones available. A total of 2,170 calls were made, comprising of one to sixteen attempts per woman. The...

This study aimed to develop a forecasting model for the incidence of dengue in Ribeirão Preto, São Paulo State, Brazil, using time series analysis. The model was performed using the Seasonal Autoregressive Integrated Moving Average (SARIMA). Firstly, we fitted a model considering monthly notifications of cases of dengue recorded from 2000 to 2008 in Ribeirão Preto. We then extracted predicted values for 2009 from the adjusted model and compared them with the number of cases observed for that year. The SARIMA (2,1,3)(1,1,1)12 model offered best fit for the dengue incidence data. The results showed that the seasonal ARIMA model predicts the number of dengue cases very effectively and reliably, and is a useful tool for disease control and prevention.

    Abstract
We conducted a cross-sectional study of a sample of 6,616 elderly living in urban areas of 100 municipalities in 23 Brazilian states, who responded to questions on the occurrence of falls in the 12 months prior to the interview, and occurrence of fractures due to the falls. The prevalence of falls among the elderly was 27.6% (95%CI: 26.5-28.7). Among those reporting falls, 11% had suffered fractures as a result. Of the elderly, 36% had received guidance about the precautions necessary to prevent falls, and about 1% had required surgery. Falls were associated with female gender, older age, low socioeconomic status, obesity and sedentary lifestyles. The prevalence of falls di...

This study analyzed the association between air pollution and deaths from respiratory diseases, considering differential susceptibility according to gender. The authors used daily deaths from respiratory diseases (ICD-10, J00-J99), PM10, SO2, and O3 levels, and meteorological indicators in Volta Redonda, Rio de Janeiro State, Brazil, from January 2002 to December 2006. The association was estimated by Poisson regression using generalized additive models, where the increase in risk of deaths from PM10 to lag 1 was 10.01% (95%CI: 1.81-18.88%) in the total female population and 10.04% (95%CI: 0.90-20.02%) in elderly women. The increase in risk of deaths from PM10 to lag 9 was 8.25% in the total male population (95%CI: 0.86-16.18%) and 10.80% (95%CI: 2.18-20.15%) in elderly men. For exposure t...

The objectives of this study were to evaluate the resources and capacity of emergency trauma care services in three Peruvian cities using the WHO report Guidelines for Essential Trauma Care. This was a cross-sectional study in eight public and private healthcare facilities in Lima, Ayacucho, and Pucallpa. Semi-structured questionnaires were applied to the heads of emergency departments with managerial responsibility for resources and capabilities. Considering the profiles and volume of care in each emergency service, most respondents in all three cities classified their currently available resources as inadequate. Comparison of the health facilities showed a shortage in public services and in the provinces (Ayacucho and Pucallpa). There was a widespread perception that both human and physi...

This study analyzes the process of producing a situational plan according to a benchmark from the philosophy of language and argumentation theory. The basic approach used in the analysis was developed by Carlos Matus. Specifically, the study seeks to identify the inherent argumentative structure and patterns in the situational explanation and regulatory design in a plan's operations, taking argumentative approaches from pragma-dialectics and informal logic as the analytical parameters. The explanation of a health problem is used to illustrate the study. Methodologically, the study is based on the existing literature on the subject and case analyses. The study concludes with the proposition that the use of the specific references means introducing greater rigor into both the analysis of the...

This study constitutes a first attempt to describe the genetic population structure of Mycobacterium tuberculosis circulating in Salvador, Bahia State, Brazil. A total of 56 confirmed cases of pulmonary tuberculosis, identified between March and June 2008, were analyzed using restriction fragment length polymorphism (IS6110-RFLP). The study population was characterized by a predominance of males (71.43%) over 30 years of age (68.75%). Forty-one isolates were found to belong to a single pattern (73.2%), while 15 (26.7%) were found in group patterns, forming six clusters. The higher level of diversity observed is much more suggestive of endogenous reactivation than recent transmission.

This article serves as an introduction to the others published in this thematic issue of the Cadernos de Saúde Pública. The author makes a brief historical review of the concepts, approaches and methodologies used to study the relationships between social determinants and health outcomes. By analyzing the current global movement around social determinants of health he emphasizes that the distinctive feature of this movement is the explicit commitment to action through intersectoral public policies that are based on evidence and implemented with the support and participation of broad segments of society. As this special issue of the Cadernos de Saúde Pública is dedicated to presenting the results of research related to social determinants of health, the author focuses on an analysis of ...

A systematic review was carried out in six bibliographic sources. The review was limited to the period between 1999 and 2008, without any other type of restriction. Among the 410 papers identified, 29 were included in the review. An increasing number of articles, specifically in the last four years of study, was observed. However, there is a concentration of studies in the South and Southeast regions of Brazil, and many of them are not closely connected to theoretical formulations in...

The Healthy Cities and Agenda 21 programs improve living and health conditions and affect social and economic determinants of health. The Millennium Development Goals (MDG) indicators can be used to assess the impact of social agendas. A data search was carried out for the period 1997 to 2006 to obtain 48 indicators proposed by the United Nations and a further 74 proposed by the technical group for the MDGin Brazil. There is a scarcity of studies concerned with assessing the MDG at the municipal level. Data from Brazilian health information systems are not always consistent or accurate for municipalities. The lack of availability and reliable data led to the substitution of some indicators. The information systems did not always ...

The self-rated health in a sample of adults living in the central area of São Paulo, Brazil, was studied by comparing a group of residents in census tracts without social vulnerability, identified using an indicator developed by the SEADE Foundation, and a group of residents in census tracts with middle, high or very high social vulnerability. Subject age and sex were used as control variables while family income, education level, degree of happiness, adequacy of income, satisfaction with the neighborhood and sense of discrimination were the intervening variables selected. After adjustment in the hierarchical model the self-rated health status was inversely associated with social vulnerability, age and sense of discrimination, and directly...

An ecological study was carried out with the aim of analyzing the evolution of inequalities in mortality in Salvador, Bahia State, Brazil, between 1991 and 2006. The city was divided into four social strata from 95 geographic Information Zones. The variables used for social stratification were education level and income of heads of households. Crude and age-standardized mortality rates, age specific mortality rates, proportional Infant mortality and the proportional mortality ratio, were calculated for each zone and social strata. Data was obtained from Death Certificates and the Populational Census. Although differences between strata were smaller in 2000 than in 1991, they persist and are still high, ranging from 28.7% to 65....

Avoidable deaths have been employed as indicators of health care quality. The aim of this study was to identify factors associated with avoidable deaths from birth to four years of age among children from the 2004 Pelotas (Brazil) birth cohort study. From January 1st, 2004, to December 31st, 2005, deaths were monitored on a daily basis and the causes were investigated and classified according to avoidability. After the first year, deaths were monitored through the Mortality Information System. A total of 94 children died during this period. It was possible to classify 92 deaths, 70 of which were preventable. Low family income, fewer prenatal visits and poor-quality prenata...

The aim of the present study was to assess social inequalities in health status, health behavior and the use of health services based on education level. A population-based cross-sectional study was carried out involving 1,518 elderly residents of Campinas, São Paulo State, Brazil. Significant demographic and social differences were found between schooling strata. Elderly individuals with a higher degree of schooling are in greater proportion alcohol drinkers, physically active, have healthier diets and a lower prevalence of hypertension, diabetes, dizziness, headaches, back pain, visual impairment and denture use, and better self-rated health. But, there were no differences in the use of health services in the previous two weeks, i...

This article aims to evaluate the effects of Brazil's recent economic growth on the monetary income, consumption patterns, and risk exposures of families with children enrolled in the public elementary school system in São Gonçalo, Rio de Janeiro State, Brazil. The article analyzes the following information on families of 447 children that participated in two waves in a longitudinal study: social stratum, per capita family income, evolution in income over a three-year period, and psychosocial factors. The findings showed a 74.8% increase in the families' income, accompanied by an increase in the consumption of material assets and access to health services. This increase should not be interpreted as a guarantee of improved living and health conditions, since it was spent on basic products...

This study describes mortality patterns among the Guarani in Southern and Southeastern Brazil, with a focus on health inequalities. The Guarani population structure is indicative of high birth and death rates, low median age and low life expectancy at birth. The crude mortality rate (crude MR = 5.0/1,000) was similar to the Brazilian national rate, but the under-five MR (44.5/1,000) and the infant mortality rate (29.6/1,000) were twice the corresponding MR in the South and Southeast of Brazil. The proportion of post-neonatal infant deaths was 83.3%, 2.4 times higher than general population. The proportions of ill-defined (15.8%) and preventable causes (51.6%) were high. The principal causes of death were respiratory (40.6%) and infectious and parasitic diseases (18.8%), suggesting precario...

This study investigated the relationship between social capital and social support and the adequate use of prenatal care. A follow-up study involving 1,485 pregnant women was conducted in two cities in the Rio de Janeiro State, Brazil. Demographic and socioeconomic characteristics, social support and social capital data were collected during the first trimester of pregnancy. The post-partum period included information on levels of prenatal care utilization, social networks, parity, obstetric and gestational risk and prenatal care attendance. Hierarchized multinomial logistic regression was used in the statistical analysis. Prenatal care use above adequate levels was associated with high social capital at the city level (aggregated social capital), socioeconomic status and working during pr...

The objective of this study was to estimate catastrophic healthcare expenditure in Brazil, using different definitions, and to identify vulnerability indicators. Data from the 2002-2003 Brazilian Household Budget Survey were used to derive total household consumption, health expenditure and household income. Socioeconomic position was defined by quintiles of the National Economic Indicator using reference cut-off points for the country. Analysis was restricted to urban households. Catastrophic health expenditure was defined as expenditure in excess of 10% and 20% of total household consumption, and in excess of 40% of household capacity to pay. Catastrophic health expenditure varied from 2% to 16%, depending on the definition. For most definitions, it was highest among the poorer. The high...

The objective of this study was to describe the characteristics of neonatal and pediatric intensive care units (ICU) and beds in Rio de Janeiro, correlating with population demands in 1997 and 2007. All neonatal and pediatric ICUs were visited, identifying the availability and type of beds. Comparisons were made between: supply and demand using projected need for beds for the population; public and private ICUs; and geographical regions. In 2007, 95 units were included totaling 1,094 beds (74 units and 1,080 beds in 1997): 51% public and 48% private (47% and 52% in 1997); 47% neonatal, 18% pediatric and 35% mixed units. Most units were located in the metropolitan area. The distribution of public and private beds was similar in the metropolitan area in both periods; in the interior, public ...

This cross-sectional study aimed to investigate the presence of inequalities in the access and use of dental services for people living in the coverage area of the Family Health Strategy (FHS) in Ponta Grossa, Paraná State, Brazil, and to assess individual determinants related to them. The sample consisted of 747 individuals who answered a pre-tested questionnaire. Data analysis was performed by chi-square test and Poisson regression analysis, obtaining explanatory models for recent use and, by limiting the analysis to those who sought dental care, for effective access. Results showed that 41% of the sample had recent dental visits. The lowest visit rates were observed among preschoolers and elderly people. This review aimed to investigate risk and protective factors for breast cancer and to analyze whether scientific evidence from the World Cancer Research Fund and American Institute for Cancer Research, published in 2007, was confirmed by new research. In May 2010 we reviewed cohort and case-control analytical studies from 2007 to 2010 in the PubMed, LILACS, and SciELO databases. We selected 27 articles (14 case-control and 13 cohort studies). Breastfeeding and physical activity were protective factors against breast cancer, and alcohol consumption was a risk factor. A direct proportional relationship was observed between larger waist circumference, weight throughout adulthood, and height and risk of breast cancer in postmenopausal women. The a...

This study aimed to determine whether the triple border region between Brazil, Argentina, and Paraguay is an area of high tuberculosis transmission. An ecological study was conducted with incidence data and population estimates for the three countries. Mean incidence rates were calculated for 2001 to 2007. Spatial analysis techniques identified high-incidence areas in the region using maps with Bayesian smoothing of rates and spatial averages. During the target period, Foz do Iguaçu, Brazil, had the highest incidence, followed by Ciudad del Este in Paraguay, and Puerto Iguazú in Argentina. The analysis showed a spatial cluster of municipalities with high tuberculosis risk in the triple border region. Tuberculosis in the tri-border area shows increasing or stable incidence rates and munic...

This study estimated and compared the performance of cytology and hybrid capture II in screening for precursor lesions of cervical cancer among HIV-infected women. The study population consisted of women from the open prospective cohort at the Evandro Chagas Clinical Research Institute, Oswaldo Cruz Foundation (IPEC/Fiocruz). Colposcopy and histology were considered jointly in defining the gold standard. Cytology showed 31.8% sensitivity and 95.5% specificity, while hybrid capture II showed higher sensitivity (100%) and lower specificity (52%). The positive likelihood ratio was 7.1 for cytology and 2.1 for hybrid capture II, while the negative likelihood ratio was 0.7 for cytology and 0.0 for hybrid capture II.

The aim of this study was to estimate the prevalence of self-reported hearing loss among elderly people in São Paulo, Brazil, according to socio-demographic characteristics and to identify associated factors. Data were from the São Paulo Health Survey (ISA-Capital 2003), and the subgroup of elderly (n = 872) was analyzed. The χ2 test was used to verify the association between self-reported hearing loss and socio-demographic characteristics. The statistical analyses used Poisson regression. Prevalence of self-reported hearing loss in the elderly was 11.2%, and was higher in men (PR = 1.86; 95%CI: 1.19-2.92). There was an important lack of knowledge among the elderly regarding the causes of their hearing loss (4...

Statistical reasoning in health practice was disseminated by epidemiology and is widely used in various health fields. This paper revisits the historical development of epidemiology and the concept of risk, responsible for the use of statistical chance as part of causal thinking in health sciences. The study covered the period from 1872 to 1965, and the documental base consisted of scientific articles related to the development of epidemiology as a field, particularly in the American Journal of Hygiene, as well as scientific books. Three phases were identified in the development of epidemiology: constitution, exposure, and risk. The article proceeds to discuss epistemological and social and health aspects required for the historical understanding of each phase. It con...

This study focused on coverage of screening for cervical cancer and associated factors in women 20 to 59 years of age in Florianópolis, Santa Catarina State, Brazil, in 2009. This was a cross-sectional study using cluster random sampling. Two outcomes were examined: a history of at least one Papanicolaou test and delaying the test (never performed or performed more than three years previously). Among 952 women, 93% (95%CI: 91.5-94.7) had ever had a Pap smear, whereas 14% (95%CI: 11.8-16.2) had delayed the test. According to Poisson regression, both outcomes were associated with marital status, schooling, presence of chronic disease, and consulting a physician for other reasons. Age, income, and hospitalization in the previous year were only associated with ever having a Pap smear. Althoug...

This study identifies geographic, demographic, and socioeconomic differences in the quality of data on underlying causes of death in the Brazilian elderly population. We used three quality indicators: the proportion of ill-defined causes, the proportion of unspecified causes, and the two proportions combined. We analyzed the main ill-defined and unspecified causes and the association between these indicators and individual characteristics on death certificates (DC) and characteristics of the municipalities. Unspecified causes exceed ill-defined causes, and both increase with age. Schooling, race, size of municipality, and per capita GDP were associated with quality of data (especially with ill-defined as compared to unspecified causes). Having received medical care decreased the odds of ca...

This study aimed to analyze NHL mortality trends for 1980-2007 in individuals 20 years and older in State capitals in Southeast Brazil. Population data were obtained from the Mortality Information System and the Health Statistics Division of the Unified National Health System (DATASUS). Age-related mortality trends were analyzed using polynomial regression models. In the 60 and older age group, a statistically significant upward linear trend was observed for Belo Horizonte and São Paulo in 1980-2007. When analyzed in two different periods, 1980-1995 and 1996-2007, statistically significant increases in NHL mortality rates were only observed in the former period. These results suggest that the increase in 1980-2007 may have resulted from the rising mortality rates from 1980 to 1995, since ...

This article aims to investigate the impact of the Baby-Friendly Hospital Initiative (BFHI) on exclusive breastfeeding (EBF). This was a cross-sectional study with 811 mothers of infants under five months of age, randomly selected at five health centers in Rio de Janeiro, Brazil. The exposure variable was hospital of birth, categorized in accredited hospitals (BFH), certified hospitals (CBFH), and hospitals without accreditation. The data were analyzed by complementary log-log transformation models, which capture cohort longitudinal experience (current status data). Even after adjusting the analysis for maternal socio-demographic, lifestyle, and psychological factors, health services use, and infants' age and health status, duration of EBF was longer in infants born in BFH and CBFH. It is also necessary to define which parameters should be used for evaluating the quality and efficacy of such services in an intercultural context. Based on a case study - the implementation of health services in the Upper Xingu - the author addresses some issues related to the political uses and "dangers" associated with "health spaces" and the distinct concepts (indigenous and non-indigenous) of what constitutes health and quality of health services. These issues affect not only health services but also the local political situation.

This study used probabilistic record linkage to estimate underreporting of stillbirths and infant deaths in Maranhão State, Northeast Brazil, in 2008. Records were linked between the Hospital Information System (SIH) (N = 374,418) and the Mortality Information System (SIM) (N = 26,597), using data extraction, identification, and processing software. Under-recording rates were 9.7% for stillbirths, 12% for neonatal deaths, and 5.3% for post-neonatal deaths. In the larger municipalities, the correction of the infant mortality rate was greater than 19%. The superiority of information on infant deaths in the SIH (as compared to the SIM) was 6.5% in municipalities with < 25 thousand inhabitants, 2% in municipalities with 25-80 thousand inhabitants, and 8.3% in those with > 80 thousand in...

This study describes the context of a large outbreak of chickenpox in the Cauca Valley region, Colombia (2003 to 2007), with an emphasis on macro-determinants. We explored the temporal trends in chickenpox incidence in 42 municipalities to identify the places with higher occurrences. We analyzed municipal characteristics (education quality, vaccination coverage, performance of health care services, violence-related immigration, and area size of planted sugar cane) through analyses based on set theory. Edwards-Venn diagrams were used to present the main findings. The results indicated that three municipalities had higher incidences and that poor quality education was the attribute most prone to a higher incidence. Potential use of set theory for exploratory outbreak analyses is discussed. I...

This study used dynamic linear models to analyze the dynamics of tuberculosis (TB) mortality in São Paulo State, Brazil. The study confirms previous findings of a decline in TB mortality in State in the last decade, while pointing to an increase in deaths beginning in 2005. Analysis of the two most frequent underlying causes pointed to HIV/TB co-infection. Forecasts for the next two years were performed, providing little evidence to support a continuing downward trend in the number of TB-related deaths.

This study highlights the advantages of Bayesian estimators for viewing and interpreting maps. For the problem at hand, EB and FB methods showed quite similar results and had the great advantage of easy use by health professionals, since they evenly highlight the main spatial patterns in the mortality rate in the State during the target period.

This study focused on breast cancer prevention practices adopted by young women in Maranhão State, Brazil. This was a population-based descriptive study conducted from June 2007 to January 2008. The majority of the women had low income (42.1%) and fewer than eight years of schooling (62.6%). Some 30% reported breast self-examination. Among women older than 35, 71.6% had never had a mammogram. The most common preventive measure was clinical examination (35.2%), which had increased by 11.5% in the previous ten years. Such information on opportunistic breast cancer screening in Maranhão should help produce specific public health policies for the State.

This study aimed to estimate thyroid cancer incidence in Brazil in 2006 using polynomial models that take age and birth cohort effects into account in the estimation process. Estimates were based on new cases of thyroid cancer in nine Brazilian cities (Manaus, Palmas, Natal, Recife, Cuiabá, Belo Horizonte, São Paulo, Porto Alegre, and Curitiba) and the Federal District from 1997 to 2001. We estimated 6,066 new cases, of which 1,065 in men and 5,001 in women. The incidence rates were, respectively, 1.16 and 5.27 per 100,000 inhabitants. Rates increased with age in both genders, especially after 30 years. The size and profile of rates according to age and gender indicate that the method yields plausible estimates.

The objective of this study is to describe preliminary results from the cross-cultural adaptation of the Quality of Life Assessment Questionnaire, used to measure health related quality of life (HRQL) in Brazilian children aged between 5 and 11 with HIV/AIDS. The cross-cultural model evaluated the Concept, Item, Semantic and Measurement Equivalences (internal consistency and intra-observer reliability). Evaluation of the conceptual, item, semantic equivalences showed that the Portuguese version is pertinent for the Brazilian context. Four of seven domains showed internal consistency above 0.70 (α: 0.76-0.90) and five of seven revealed intra-observer reliability (ricc: 0.41-0.70). This first Portuguese version of the HRQL questionnaire can be understood as a valuable tool for assessing chi...

Authors: Andreucci CB, Cecatti JG
The Program for Humanization of Prenatal and Childbirth Care (PHPN) was launched in Brazil in 2000, with quantitative criteria for obstetric care and SISPRENATAL as the database. The current study pooled the national data on prenatal care using SISPRENATAL and other data sources. This was a systematic review of prenatal care with an online search of articles in MEDLINE, EMBASE, and SciELO. The study compiled publications since 2001 that used PHPN process indicators. A meta-analysis was performed, estimating the mean proportion of each process indicator with its respective 95%CI. Process indicators increased over the target period, but SISPRENATAL showed low coverage for PHPN as compared to other data sources. The PHPN faces the challenge of proper data...

This study aimed to assess pre-hypertension and hypertension-related factors in 1,125 seven- to-14-year-old subjects enrolled in the public school system in Salvador, Bahia State, Brazil. Exposure variables, namely body mass index, waist circumference, gender, age, physical activity, environmental and housing status, family income, diet, schooling, and maternal age were analyzed by polytomous logistic regression, and the outcome variable was categorized as normal, pre-hypertensive, and hypertensive. Prevalence of high blood pressure was 14.1%, including the prevalence of both hypertension (4.8%) and pre-hypertension (9.3%). An association was shown between pre-hypertension and overweight (OR: 3.13; 95%CI: 1.75-5.57). Hypertension was associated with overweight (OR: 3.02; 95%CI: 1.45-6.28),...

Population studies can help identify the complex set of risk factors for neonatal mortality among very low birth weight infants. A cohort (2000-2001) of 213 live newborns with birth weight < 1,500g in the southern region of São Paulo city, Brazil, was studied (112 neonatal deaths and 101 survivors). Data were obtained from home interviews and hospital records. Survival analysis and multiple Cox regression were performed. The high mortality in the delivery room and in the first day of life among neonates < 1,000g and < 28 weeks gestational age and the absence of survival in neonates < 700g suggest that care was actively oriented towards newborns with better prognosis. The aim of this article is to discuss the social representations of "health in sexual relations" as reported by women treated under the Unified National Health System (SUS) in Natal, Rio Grande do Norte, Brazil. A total of 150 women were tested using the free word recall test, with "health in sexual relations" as the stimulus. Women were also asked about their sources of information on the subject. The results were obtained with content analysis and the EVOC software. We identified three representational dimensions: prevention, relationship with the partner, and quality of life. The central nucleus of social representation consisted of the elements "prevention" and "condoms". Likely sources for representation were television, health services, an...

The objective of this study was to evaluate the association between income inequality at a lagged time of 2 and 11 years with two short latency outcomes (untreated dental caries and gingivitis) and two long latency outcomes (edentulism and periodontal attachment loss > 8mm). We used data from the Brazilian oral health survey in 2002-2003. Our analysis included 13,405 subjects aged 35-44 years. Different lagged Gini at municipal level were fitted using logistic and negative binomial multilevel analyses. Covariates included municipal per capita income, equivalized income, age, sex, time since last dental visit and place of residence (rural versus urban). Crude estimates showed that only untreated dental caries was associated with current and lagged Gini, but in adjusted models only curren...

This study investigated whether men's and women's work status is associated with current smoking, after adjusting for schooling, income, and health, besides discussing hypotheses for such associations. The sample included individuals 15 to 64 years of age living in metropolitan areas, currently in the work force, and interviewed in the Brazilian National Household Sample Survey (PNAD) in 2008. Prevalence ratios (PR) and 95% confidence intervals (95%CI) were obtained by Poisson regression. After adjusting, smoking prevalence was statistically higher in individuals with informal jobs (men, PR = 1.31, 95%CI: 1.24-1.38; women, PR = 1.22, 95%CI: 1.12-1.31) or who were unemployed (men, PR = 1.31, 95%CI: 1.18-1.44; women, PR = 1.16, 95%CI: 1.03-1.32). A cross-sectional study was performed to assess knowledge and performance on the prevention of risk factors for chronic non-communicable diseases (CNCD) in adults, among primary care physicians in the urban health system in Pelotas, Rio Grande do Sul State, Brazil. The study included observation of 422 consultations by 61 physicians. 58.8% of the patient consultations for CNCD and 25.3% of those for other reasons lacked any preventive recommendations and/or measurement of weight, height, and blood pressure. For patients with CNCD, the most frequent recommendations were weight loss, more physical activity, and reduced fat and salt intake. Among patients without CNCD, only 39.1% received any recommendations related to primary prevention. Physician perform...

Cognitive impairment is a neurodevelopmental sequela that is more prevalent in very low birth weight (VLBW) premature children. There are few Brazilian studies on this group's cognitive development at school age. The current study proposes a theoretical discussion on the determinants of cognitive development at school age in VLBW preterm children, using a hierarchical analytical model. According to this model, biological and environmental factors interrelate on several levels (distal, intermediate, and proximal), resulting in changes in cognitive development. The aim is to investigate the possible mediation of variables and their interrelationships and the resulting events that could lead to cognitive impairment as the outcome. Sel...

This study explored age-standardized cancer mortality rates according to anatomical site in Rio Branco, Acre State, Brazil, by constructing linear regression time trend models. Cancer mortality showed an increasing but inconstant trend in men and stability in women. At the end of the time series, the highest cancer rates among women were for the cervix, lung, liver and intrahepatic biliary tract, stomach, and breast. Among men, the highest rates were for cancer of the lung, prostate, liver and intra-hepatic biliary tract, stomach, and esophagus. The study showed an increasing mortality time trend for cancer of the prostate, breast, and lung and declining mortality rates for cervical cancer in women, lung cancer in men, and stomach cancer in both sexes. The high mortality rate from liver ca...

The objective of this study was to identify factors related to food intake, anthropometry and biochemical measurements associated with chronic non-communicable diseases. It involved a cross-sectional study among a representative sample of adults and elderly people in Bambuí, Minas Gerais State, Brazil. Anthropometric, biochemical and nutrient intake data were evaluated. The study included 409 individuals (55.3% of whom were women), with a mean age of 40.6±16.0 years. There was found to be a 14% rate of obesity associated with high/very high triglycerides and arterial hypertension. Arterial hypertension was present in 22.4% of subjects and correlated with age > 60 years, insufficient protein intake, obesity and high cholesterol. Diabetes was found in 3.7% and was associated with large ...

This study aimed to evaluate the completeness of data on nine items included in both the certificate of live birth (CLB) and death certificate (DC), as well as the retrieval of data on early neonatal deaths. The study assessed live birth and early neonatal and fetal death records in municipalities in the Ribeirão Preto Region, São Paulo State, Brazil, from 2000 to 2007. Database matching for live births and deaths allowed retrieving data on early neonatal deaths. Data completion was good on the CLB and poor on the DC. Retrieval of data related to mortality was satisfactory, except for maternal schooling, number of live born children, and stillbirths. The findings highlight the need to improve completion of the DC and showed that the retrieval process improved the completeness of data fro...

Authors: Vazquez DA
The article aimed to analyze the impacts of earmarking revenues and conditional transfers on the supply of health financing in Brazil. After analyzing the role of these Federal regulation mechanisms on decentralized healthcare administration, the article verified the effects on total expenditure in health and disaggregated by level of government, evaluated whether transfers by the Unified National Health System (SUS) were consistent with the evolution in the decentralized supply, and measured the inequalities in per capital health spending by municipalities. The conclusions showed the complementary relationship between earmarking revenues and conditional transfers according to supply, which: (1) increased the share of State and Municipal governments in health financ...

This study evaluated the impact of an infant feeding update program on exclusive breastfeeding and the incidence of diarrhea and respiratory illness in infants. A randomized cluster field study was conducted in the city of Porto Alegre, Rio Grande do Sul State, Brazil, with 20 healthcare centers. Health professionals received information on the Ten Steps to Healthy Feeding for Children up to Two Years of Age. We evaluated 619 infants 6-9 months of age. The results showed longer duration of exclusive breastfeeding (p = 0.02) in the intervention group but no significant differences in the incidence of diarrhea or respiratory symptoms. Complementary analyses showed that exclusive breastfeeding was longer in the group of children without the occurrence of diarrhea (p = 0.001) or respiratory sy...
