CONCLUSIONS:On average, old, sick, poor, and Medicare patients were less likely to be treated at high‐volume hospitals for uro‐oncologic surgery. Similarly, black patients were less likely to have a radical cystectomy at a high‐volume hospital, and female patients were less likely to have an Nx at a high‐volume hospital. Selective referral of individuals who are less likely to receive care at such institutions may represent a health care priority intended to optimize outcomes across all population strata.
CONCLUSIONS:More fatal cancers, particularly prostate, cervical, and oral cancer in men were detected among blacks, especially in central Georgia, where health behavior and social/economic factors were worse. MIRs are an efficient indicator of survival and provide insight into racial cancer disparities. Additional examination of geographic determinants of cancer fatality in Georgia as indicated by MIRs is warranted. Cancer 2012. © 2012 American Cancer Society (Source: Cancer)

Correlations between the percentage of tumor cells showing an ALK (anaplastic lymphoma kinase) gene rearrangement, ALK signal copy number, and response to crizotinib therapy in ALK fluorescence in situ hybridization–positive nonsmall cell lung cancer

CONCLUSIONS:The strong association between increased copy number of key ALK signals and percentage of positive cells suggests that the <100% rate of cellular positivity in ALK+ tumors is due to technical factors, not biological factors. In ALK+ tumors, neither the percentage of positive cells nor signal copy number appear to be informative variables for predicting benefit from ALK inhibition. The inverse relationship between fused and isolated red copy number suggests ALK+ may be a distinct “near‐diploid” subtype of NSCLC that develops before significant chromosomal aneusomy occurs.
AbstractDespite the use of PSA, Gleason score, and T‐category as prognosticators in intermediate‐risk prostate cancer, 20–40% of patients will fail local therapy. In order to optimize treatment approaches for intermediate‐risk patients, additional genetic prognosticators are needed. Previous reports using array comparative genomic hybridization (aCGH) in radical prostatectomy cohorts suggested a combination of allelic loss of the PTEN gene on 10q and allelic gain of the c‐MYC gene on 8q were associated with metastatic disease. We tested whether copy number alterations (CNAs) in PTEN (allelic loss) and c‐MYC (allelic gain) were associated with biochemical relapse following modern‐era, image‐guided radiotherapy (mean dose 76.4 Gy). We used aCGH analyses validated by fluoresce...

AbstractPersonalized medicine based on predictive markers linked to drug response, it is hoped, will lead to improvements in outcomes and avoidance of unnecessary treatment in squamous cell carcinoma of the head and neck (SCCHN). Recent research has shown that expression of ERCC1 may predict resistance to treatment with platinum agents. Future testing for this marker may help select the optimal type of chemotherapy. Infection with human papillomavirus (HPV) is associated with less aggressive disease and better prognosis in locally advanced SCCHN treated with chemoradiation or radiation alone; HPV‐positive patients may ultimately benefit from less intensive, less toxic therapy. K‐RAS mutations, occurring in about 40% of colorectal cancers and associated with lack of benefit from epiderm...

CONCLUSIONS:The current results indicated that the highest level of lymph node involvement may be used to stratify the outcome of patients who have NSCLC with pN1 status. Patients with pN1 status who had only level 13/14 lymph node metastasis had an intermediate 5DFS rate between that of patients with pN0 status and other patients with pN1 status. Routine examination of level 13/14 lymph nodes is important for accurate pathologic staging and for the predicting clinical outcome of patients with NSCLC.
CONCLUSIONS:After diagnosis, a substantial minority of patients with lung and colorectal cancers continued smoking. Patients with lung cancer had higher rates of smoking at diagnosis and after diagnosis; whereas patients with colorectal cancer were less likely to quit smoking after diagnosis. Factors that were associated with continued smoking differed between lung and colorectal cancer patients. Future smoking‐cessation efforts should examine differences by cancer type, particularly when comparing cancers for which smoking is a well established risk factor versus cancers for which it is not.
CONCLUSIONS:Patients with advanced cancer did not change care preferences after viewing the video, but fewer wanted CPR or ventilation. Documented code status was inconsistent with patient preferences. Patients were more knowledgeable after the video, reported that the video was acceptable, and said they would recommend it to others. The current results indicated that this type of video may enable patients to visualize “goals of care,” enriching patient understanding of worsening health states and better informing decision making.
CONCLUSIONS:Therapy generally followed accepted standards for 2004. Findings in particular tumor sites and stages may reflect the variability that still exists for the treatment of HNC. The use of taxanes and cetuximab is expected to increase based on new evidence of benefit. Reducing treatment‐related toxicities and long‐term functional deficits will be critical and especially important with the increase in human papillomavirus‐related cancers. In addition, further attempts to improve survival for older patients are needed. Cancer 2012. © 2012 American Cancer Society. (Source: Cancer)

CONCLUSIONS:In general, EOL RT was received infrequently, was delivered more commonly to younger patients with more advanced disease, and often was not completed as planned. There also was considerable variation in its use among National Comprehensive Cancer Network institutions. Next steps include expanding this research to other cancers and settings and investigating the clinical benefit of such treatment. Cancer 2012. © 2012 American Cancer Society.
AbstractCancer is an emerging public health problem in Africa. About 715,000 new cancer cases and 542,000 cancer deaths occurred in 2008 on the continent, with these numbers expected to double in the next 20 years simply because of the aging and growth of the population. Furthermore, cancers such as lung, female breast, and prostate cancers are diagnosed at much higher frequencies than in the past because of changes in lifestyle factors and detection practices associated with urbanization and economic development. Breast cancer in women and prostate cancer in men have now become the most commonly diagnosed cancers in many Sub‐Saharan African countries, replacing cervical and liver cancers. In most African countries, cancer control programs and the provision of early detection and treatme...

CONCLUSIONS:ER status and multifocality of the residual tumor after NAC were independent predictors of IBTR after BCT. The MDAPI was barely adaptable to the study patients in terms of predicting IBTR. Patients with ER‐positive and HER2‐negative tumors had a favorable prognosis, whereas patients who developed IBTR after NAC had significantly worse overall survival. The authors propose a new IBTR prognostic index using the 2 factors that were identified as predictive of IBTR: ER status and multifocality of the residual tumor.
CONCLUSIONS:Many parents communicate their BRCA1/2 test results to young offspring. Parents' perceptions of offspring responses appear to vary by offspring age and parent test result. A better understanding of how young offspring respond to information about hereditary risk for adult cancer could provide opportunities to optimize adaptive psychosocial responses to risk information and performance of health behaviors, in adolescence and throughout an at‐risk life span.
Prognosis for primary retroperitoneal sarcoma survivors

CONCLUSIONS:This population‐based study demonstrates that the factors which are predictive of survival at baseline lose significance after one year of survival. Conditional survival estimates allow clinicians to provide survivors with more meaningful prognostic estimates that may impact surveillance schedules and streamline adjuvant therapy decisions and design of future clinical trials.
CONCLUSIONS:No difference was observed in the presence or absence of mitoses, a marker of tumor proliferation, in SPM and MPM. Because it has been demonstrated that the presence of mitosis is a powerful prognostic marker, the current findings suggested that the tumors behave similarly in patients with SPM and patients with MPM. The authors concluded that differences in tumor thickness and prognosis between SPM and MPM more likely are caused by factors other than tumor biology, such as increased surveillance.
CONCLUSIONS.The current spatiotemporal prediction model is adequate to provide reasonable estimates of case counts. To project the estimated case counts ahead 4 years, the vector autoregressive model is recommended to be the best temporal projection method for producing estimates closest to the observed case counts.
CONCLUSIONS:The current results indicated that disparities exist for Asian women with breast cancer who reside in the United States compared with NHW groups and among Asian subgroups. Differences in presenting clinicopathologic features may affect DSS rates, suggesting that further investigation of these disparities is warranted to increase early detection and treatment for specific subgroups.
CONCLUSIONS:The serum IL‐10 level was identified as an independent prognostic factor for unresectable HCC. The current findings suggested that an elevated IL‐10 level may be related to hepatic injury caused by cirrhotic processes rather than tumor load. The authors concluded that the IL‐10 level offers additional prognostic value to the existing tumor staging systems. Cancer 2012;. © 2011 American Cancer Society. (Source: Cancer)

CONCLUSIONS:The nomogram described here outperformed current strategies for survival prediction in breast cancer patients with BMs. Two additional advantages of this nomogram are its ability to predict individualized, 1‐, 3‐, and 5‐year survival for novel patients and its straightforward representations of the relative effects of each of 9 covariates on neurologic survival. This represents a potentially valuable alternative to current models of survival prediction in this patient population. Cancer 2012;. © 2011 American Cancer Society. (Source: Cancer)

How does health literacy affect quality of life among men with newly diagnosed clinically localized prostate cancer?

CONCLUSIONS:Among patients with newly diagnosed localized prostate cancer, those with low health literacy levels were more vulnerable to mental distress than those with higher health literacy levels, but physical well‐being was no different. These findings suggest that health literacy may be important in patients managing prostate cancer and the effects of treatment, and provide the hypothesis that supportive interventions targeting patients with lower health literacy may improve their HRQOL.
CONCLUSIONS:Pemetrexed has single‐agent activity in relapsed/refractory PCNSL. Toxicities were seen likely because of the higher than standard dose used. Further investigation of this agent or other multitargeted antifolates in PCNSL is warranted to determine optimal dose and efficacy in a more homogeneous population.
CONCLUSIONS.SSE and PSE were identified as complementary early detection strategies, particularly in men aged >60 years, in whom both partial and full‐body PSE were associated with thinner tumors. Given the high rates of physician access, PSE may be a more practical approach for successful early detection in this subgroup with highest mortality. Cancer 2012;. © 2011 American Cancer Society.
CONCLUSIONS.Decreased melanoma risk perception and knowledge among low‐SES individuals and decreased physician communication regarding skin examinations of these individuals may be key components of the consistently observed socioeconomic gradient in mortality. The current findings suggest the need to raise melanoma awareness among lower‐SES patients and to increase physician awareness of socioeconomic disparities in clinical communication and care.
CONCLUSIONS:Cancer patients experienced numerous geographic and health care system challenges, resulting in significant delays in receiving diagnosis and treatment, even for cancers highly amenable to early intervention. This unacceptable and unethical situation is likely explained by limited knowledge about cancer among patients and health care professionals, government neglect, poverty, and reliance on traditional healers.
CONCLUSIONS:DCE‐MRI was identified as a prognostic factor for EFS and overall survival before treatment on this trial and was indicative of a histologic response to neoadjuvant therapy. Further studies are needed to verify these findings with other treatment regimens and establish the potential role of DCE‐MRI in the development of risk‐adapted therapy for osteosarcoma.
CONCLUSIONS:Diagnosis of 2 or more primary cancers (excluding nonmelanoma skin cancers) is associated with increased risk for poorer physical and mental health status over and above that associated with diagnosis of a single primary cancer. Survivors of multiple and single primary cancer diagnoses should be considered as distinct subgroups, and increased attention should be devoted to the unique status and needs of survivors of multiple primary cancer diagnoses.
CONCLUSIONS:Further studies need to be performed to examine CD52 expression and function in HS as well as the role of alemtuzumab in a larger cohort of patients. However, the clinical impact of single‐agent alemtuzumab in the 2 patients described in this report was very encouraging. Given the toxicity and frequent inefficacy of conventional chemotherapy, patients with incompletely resected HS should be strongly considered for alemtuzumab therapy.
CONCLUSIONS:The current results indicated a significant increase in the incidence of EATL in the United States, which may reflect the increasing seroprevalence of CD and better recognition of rare types of T‐cell lymphomas. The incidence may continue to rise given the large ratio of undiagnosed‐to‐diagnosed individuals with CD in the United States. Cancer 2011;. © 2011 American Cancer Society.
Although several single‐nucleotide polymorphisms have been associated with radiation‐induced toxicity in candidate gene studies, there are many important limitations to such analyses. The primary limitations include the approach (candidate gene vs genome wide), small patient numbers, lack of a validation set, and subjective endpoints.See also pages 000–000.
CONCLUSIONS:BCR‐ABL1 expression up‐regulated HO‐1, a survival factor for CML cells. This up‐regulation was more pronounced in blast crisis CML relative to early stage disease and was mediated by the NADPH oxidase components Rac1 and p47phox. The expression of p47phox was increased in BCR‐ABL1–expressing cells. Cancer 2011. © 2011 American Cancer Society. (Source: Cancer)

Prognostic and therapeutic impact of the chromosome 20q13.2 ZNF217 locus amplification in ovarian clear cell carcinoma

CONCLUSION:These findings indicate that ZNF217 overexpression is critical to growth and survival of OCCCs with ZNF217 gene amplification. Furthermore, they suggest that ZNF217 siRNA‐induced phenotypes depend on amplification status of OCCCs. Therefore, ZNF217‐targeted therapy may benefit OCCC patients with ZNF217 amplification.
