Clinical suspicion is infrequent owing to its location and nonspecific manifestations such as abdominal pain, obstructive symptoms or even bleeding.
This article is intended to succinctly review the recent experimental and clinical findings regarding the involvement of oxidative stress and redox signaling in the pathophysiology of ALD and the development of mechanistically‐based antioxidant modalities to target the oxidative stress and redox signaling mechanisms of ALD. The biochemical and cellular sources of reactive oxygen and nitrogen species (ROS/RNS) and the dysregulated redox signaling pathways associated with alcohol consumption are particularly discussed to provide insight into the molecular basis of hepatic cell dysfunction and destruction as well as tissue remodeling underlying ALD.© 2011 The Authors.
Conclusion:  Blood transfusion during LDLT can be predicted using preoperative variables. Massive RBC transfusion may lead to poor long‐term survival, higher postoperative infection rate and prolonged ICU stay. Platelet transfusion is not a risk factor for long‐term survival.© 2011 The Authors.
AbstractIn order to prevent Barrett's esophagus (BE)‐related esophageal cancer development in China and facilitate communications of research results among different centers, we propose using standardized diagnostic criteria and taking a conservative approach to diagnose and manage BE patients. BE Patients without dysplasia need to be treated medically. For low‐grade dysplasia, annual endoscopy with biopsies is recommended along with medical therapy. For high‐grade dysplasia and intramucosal carcinoma, an endoscopic or surgical intervention is suggested. All BE patients should be followed up closely.© 2011 The Authors.
Barrett's esophagus (BE)‐related esophageal adenocarcinoma (EAC) has shown the fastest rise in incidence in Western countries; however, research data on BE‐related diseases from China are inconclusive. We aimed to review and analyze the published results on these diseases in China. We searched PubMed and Chinese medical literature for key words: BE, EAC, Chinese and China. Relevant research papers along with the study results from our own groups were reviewed and analyzed. Using standardized criteria, columnar‐lined esophagus (CLE) was found in as many as 29% of resection specimens in Chinese patients with proximal gastric cancer. However, BE with intestinal metaplasia was rare, ranging from 0.06% in the general population to <2% in referral patients. Risk factors included advanci...

ABSTRACTWhen a patient is hospitalized with acute, severe ulcerative colitis, the primary decision is whether or not to proceed directly to surgery. Absolute indications for immediate colectomy include exsanguinating hemorrhage, perforation and cancer. If medical therapy is undertaken, however, the decision for urgent surgery or non‐operative “salvage” therapy will still be required in the 15–50% of cases in which there is failure to respond within 3‐5 days to a standard regimen of intravenous steroids, antibiotics, decompressive maneuvers, fluid and electrolyte replacement, and other supportive measures. The options for medical salvage therapy are usually cyclosporine or infliximab. There are theoretical and practical arguments on each side; the current GETAID and CONSTRUCT tria...

CONCLUSIONS:  The heterogeneity of the drug transporters may be the reason for the therapeutic sensitivity of 6‐MP in IBD patients. As 6‐MP uptake is a carrier‐mediated and partially sodium‐dependent process, future studies are necessary to evaluate the role of the putative transporters and their correlation with drug sensitivity in patients.
AbstractThe liver X receptors (LXRs) are members of the superfamily of metabolic nuclear receptors that play a central role in regulation of cholesterol absorption, efflux, transport and excretion and many other processes correlating with lipid metabolism. LXRs can also regulate the inflammatory process in vitro and in vivo. The accumulating evidences demonstrate that LXRs involve in the metabolic and inflammatory process in human diseases. Nonalcoholic fatty liver disease (NAFLD) is classically associated with lipid metabolic disorders and inflammatory response, especially in nonalcoholic steatohepatitis (NASH) phase. LXRs’ effects on cholesterol metabolism and anti‐inflammation process make them attractive as potential target for the treatment of NAFLD. Since the ability to synthesiz...

AbstractExtramedullary plasmacytoma (EMP) of the small bowel is an unusual plasma cell neoplasm (PCN) in this anatomic region with only 61 cases described so far. Clinical suspicion is infrequent owing to its location and nonspecific manifestations of abdominal pain, obstructive symptoms or even bleeding. Diagnosis is reached through histopathological examination coupled with immunohistochemistry study of the endoscopic biopsy or surgical resection specimens. Nevertheless, the differential diagnosis between EMP, lymphomas and other kinds of tumors can sometimes be troublesome. Its management include surgery, radiotherapy or chemotherapy. Generally, the prognosis of EMP is favourable, but occasionally it may relapse, as more really progresses to plasmacytic myeloma (PCM). So, EMP should be ...

CONCLUSION:  ICOS can enhance the cytotoxic effect of CIK cells against cholangiocarcinoma both in vitro and in vivo. This effect is mediated by ICOS‐augmented cytokine secretion and cell proliferation, and in part through ICOS‐ICOSL interaction. (Source: Chinese Journal of Digestive Diseases)

The metabolic syndrome is a common complex entity that has emerged as a worldwide epidemic and major public health care concern with a prevalence of approximately 25% in the United States. There have been a number of different definitions of the metabolic syndrome but all center around the metabolic abnormalities of central obesity, hypertension, decreased high‐density lipoproteins and elevated triglycerides with insulin resistance as the uniting physiologic factor. The importance of the metabolic syndrome is not just related to its high prevalence rate but also because it predicts the development of diabetes and cardiovascular disease. Nonalcoholic fatty liver disease is now recognized to be the hepatic component of the metabolic syndrome, which along with its individual components – ...

Inflammatory bowel disease (IBD) includes ulcerative colitis and Crohn's disease which is characterized by the recurrent intestinal inflammation and overactive immune responses. The signaling pathways of p38 mitogen‐activated protein kinase (MAPK) play an important role in bowel inflammation. The inhibition of p38 MAPK can effectively suppress the expression of inflammatory mediators. However, due to the obvious preclinical and clinical side effects, p38 inhibitors are unacceptable in safety profiles and cannot be applied in the treatment of IBD. MAPK‐activated protein kinase 2 (MK2), as the direct substrate of p38α and p38β, is a multifunctional signaling protein in the progression of inflammation and several lines of evidence demonstrate that the inhibition of MK2 may produce the s...

Conclusions:  Atrophy and intestinal metaplasia are rare, but activity and chronic inflammation are severe at the antrum, angulus, and corpus. Pangastritis is the commonest type of gastritis. Lymphoid follicles affect all mucosal layers up to the upper corpus, whereas endoscopic protrusion is found only at the angulus. No correlation with pathological features has been found between N and G types. (Source: Chinese Journal of Digestive Diseases)

AbstractAt present, esophageal adenocarcinoma (EAC) in the United States has the fastest rising incidence among cancers of the lung, breast, prostate, colon, rectum, and malignant melanoma1. This cancer has outnumbered esophageal squamous cell carcinomas in the United States2 and several European countries3. A growing body of evidence indicates that Barrett esophagus (BE) may be the only proven risk factor for EAC. In contrast, the published studies in the English medical literature on BE‐related diseases in China have been scarce and inconclusive. Most recently, however, several epidemiologic, endoscopic, and clinicopathological studies showed intriguing results. In this review, we examined the data published recently to provide readers with the updated information on BE‐related disea...

One of typical examples of liver‐gut cross talk is the interaction and impact of inflammatory bowel disease (IBD) and hepatobiliary (HB) abnormalities on each other's disease course. There are several layers of association between IBD and HB diseases: (i) HB diseases and IBD share pathogenetic mechanisms; (ii) HB diseases parallel structural and pathophysiological changes seen with IBD; and (iii) hepatic toxicity is associated with medical therapy for IBD. Interdisciplinary approach, involving gastroenterologists, hepatologists and, in advanced cases, general, colorectal, and liver transplant surgeons, is advocated.
CONCLUSIONS:  Our data showed a significant increase in diagnostic yield with increasing degree of anemia. Relative difference in hemoglobin rather than absolute values may be a better predictor. The ability of VCE to guide further intervention reached significance in severely bleeding patients. The hypothesis that use of VCE prior to EGD or colonoscopy in the severely bleeding group deserves to be evaluated. (Source: Chinese Journal of Digestive Diseases)

ABSTRACTInflammatory bowel disease (IBD) includes ulcerative colitis and Crohn's disease which are characterized by the recurrent intestinal inflammation and overactive immune responses. The signaling pathways of p38 mitogen‐activated protein kinase (MAPK) play an important role in the bowel inflammation. Inhibition of p38MAPK can effectively suppress the expression of inflammatory mediators. However, due to the obvious preclinical and clinical side effects, the p38 inhibitors are unacceptable in safety profiles and can not be applied in the treatment of IBD. MAPK‐activated protein kinase 2 (MK2), as the direct substrate of p38α and p38β, is a multi‐functional signaling protein in the progression of inflammation, and several lines of evidence demonstrated that inhibition of MK2 may...

AbstractEndoscopic submucosal dissection (ESD) is a safe and effective alternative to surgery for large non‐polypoid colonic lesions and early colorectal carcinoma. In this article, we discuss the development, efficacy and safety of ESD. As the incidence of colorectal cancer is rapidly increasing in Asia, we advocate standardization of ESD program, including patient selection, hardware prerequisites, and training of operators.
Cisapride: What can we learn from the rise and fall of a prokinetic?

Cisapride, the prototype serotonergic agent, evolved from a body of research that defined the key roles of serotonergic receptors in gastrointestinal motor and sensory function. Impressed by its in vitro properties and encouraged by clinical trial data, cisapride became the drug of choice for the treatment of a wide range of motility disorders and clinicians appeared impressed by its efficacy and comfortable with its side‐effect profile. Once serious cardiac events began to be reported in association with cisapride therapy, dark clouds rapidly gathered and soon enveloped the drug, leading to its widespread withdrawal from markets. What lessons can we learn from the story of cisapride? How can its brief but spectacular rise and equally sensational demise inform the development of new drug...

AbstractCisapride, the prototype serotonergic agent, evolved from a body of research which defined the key roles of serotonergic receptors in gastrointestinal motor and sensory function. Impressed by its in vitro properties and encouraged by clinical trial data, cisapride became the drug of choice for the treatment of a wide range of motility disorders and clinicians appeared impressed by its efficacy and comfortable with its side effect profile. Once serious cardiac events began to be reported in association with cisapride therapy, dark clouds rapidly gathered and soon enveloped the drug leading to its widespread withdrawal from markets. What lessons can we learn from the story of cisapride? How can its brief but spectacular rise and equally sensational demise inform the development of ne...

AbstractWhile dietary habits or nutritional intakes continue to rank as a significant factor influencing cancer incidence, there had been considerable scientific uncertainties about who will be benefited, but who will not be benefited by nutrition. This might be due to inadequate knowledge about an individual's genetic background, cumulative effect of nutrients on genetic expression profiles, ambiguous clinical difference between beneficiary and non‐beneficiary, and the lack of information about active protein induction. During the past 200years of nutrition research, we have experienced the revolutionary advancement in either chemical revolution or genomic revolution. According to the high expectation for tailored medicine, nutrigenomic approach harboring tremendous potential to change ...

CONCLUSION: HSP27 phosphorylation with resultant decrease in HSP27 level was one of mechanisms of indomethacin‐induced cytotoxicity, of which post‐translational modifications were prevented with revaprazan administration in the presence of indomethacin. Therefore, acid pump antagonist could be a choice of the prevention for NSAID‐induced gastropathy backed up with distinctive cytoprotective action beyond acid suppressor.
ABSTRACTALDH2‐and ADH gene polymorphisms associating with enhanced acetaldehyde exposure and markedly increased cancer risk in alcohol drinkers provide undisputable evidence for acetaldehyde being a local carcinogen not only in esophageal but also in gastric cancer. Accordingly, acetaldehyde associated with alcoholic beverages has recently been classified as Group 1 carcinogen to humans. Microbes are responsible for the bulk of acetaldehyde production from ethanol both in saliva, H. pylori infected and achlorhydric stomach. Acetaldehyde is the most abundant carcinogen in tobacco smoke, and it readily dissolves into saliva during smoking. Many food stuffs and “non‐alcoholic” beverages are important but unrecognized sources of local acetaldehyde exposure. The cumulative cancer risk a...

AbstractStudies on sphingolipid have become one of the most rapidly progressed fields in the last 2 decades. These highly diverse lipids have been known to have multiple physiological functions and clinical implications in several diseases including tumorigenesis, inflammation, atherosclerosis, and neural degenerative diseases. Differing from other organs, sphingolipids in the intestinal tract are not only present as lipid constituents in the cells, but also as dietary compositions for digestion in the lumen. The present review is to focus on the presence of sphingolipids and their catalytic enzymes in the gut, the metabolism and the signaling effects of the metabolites, and their impacts on barrier functions, cholesterol absorption, inflammatory diseases and tumor development in the gut. ...

AbstractThe development of alcoholic liver disease (ALD) is a complex process involving both the parenchymal and non‐parenchymal cells in the liver. The impact of ethanol on hepatocytes can be characterized as a condition of “organelle stress” with multi‐factorial changes in hepatocellular function accumulating during ethanol exposure. These changes include oxidative stress, mitochondrial dysfunction, decreased methylation capacity, endoplasmic reticulum stress, impaired vesicular trafficking and altered proteosome function. Injury to hepatocytes is attributed, in part, to ethanol metabolism by the hepatocytes. Changes in the structural integrety of hepatic sinusoidal endotheial cells, as well as enhanced inflammation in the liver during ethanol exposure are also important contribu...
