Chronic protection against ischemia and reperfusion-induced endothelial dysfunction during therapy with different organic nitrates

Discussion We previously showed that upon repeated administration, the preconditioning-like effects of GTN are attenuated. The present
data demonstrate a gradient in the extent of protection afforded by the two nitrates, suggesting that PETN-induced preconditioning
is maintained after prolonged administration in a human in vivo model of endothelial dysfunction induced by ischemia. Using
isolated human endothelial cells, we propose a mechanistic explanation for this observation based on differential effects
of GTN versus PETN on the activity of mitochondrial aldehyde dehydrogenase.

Conclusion Zotarolimus coated balloons and stents were found to effectively reduce neointimal proliferation in the porcine coronary model.
Inflammation scores were significantly reduced after treatment with the coated balloon. Zotarolimus balloon coating might
be a novel option in preventing and treating restenosis.

Conclusions For adequate risk assessment in ULMCA patients, consideration of both comorbidities and coronary anatomic complexity, is essential.
A combination of angiographic and clinical risk scores improves the prognostic value for the prediction of 1-year MACE risk
and is superior to stand-alone scores.

Abstract The preferred use of dual-coil implantable defibrillator lead systems in current implantable defibrillator therapy is likely
based on data showing statistically lower defibrillation thresholds with dual-coil defibrillator lead systems. The following
review will summarize the clinical data for dual- versus single-coil defibrillator leads in the left and right pectoral implant
locations, and will then discuss the clinical implications of single- versus dual-coil usage for atrial defibrillation, venous
complications, and the risks associated with lead extraction. It will be noted that there are no comparative clinical studies
on the use and outcomes of single- versus dual-coil lead systems in implantable defibrillator therapy over a long-term follow-up.

female gender per se is not an independent risk factor for in-hospital mortality, but close attention should be paid on modifiable
risk factors.

Abstract
Background Novel cardioprotective strategies are required to improve clinical outcomes in high risk patients undergoing coronary artery
bypass graft (CABG) ± valve surgery. Remote ischemic preconditioning (RIC), in which brief episodes of non-lethal ischemia
and reperfusion are applied to the arm or leg, has been demonstrated to reduce perioperative myocardial injury following CABG ± valve
surgery. Whether RIC can improve clinical outcomes in this setting is unknown and is investigated in the effect of remote
ischemic preconditioning on clinical outcomes (ERICCA) trial in patients undergoing CABG surgery. (ClinicalTrials.gov Identifier:
NCT01247545).

Conclusions Late occurrence of bradyarrhythmias should be recognized as a significant contributor to postprocedural outcome after TAVI.
Although this is a well known phenomenon after surgical valve replacement, it is less recognized after TAVI.

Conclusion In patients with paradoxical embolism, recurrent ischemic events are frequent despite medical therapy. These events are not
limited to the early years after the index event; this long-term follow-up revealed a risk of occurrence over the entire follow-up.
These patients have a sustained risk of recurrence, requiring lifetime protection, which should be considered in tailoring
individual therapeutic strategies.

Conclusions This large pooled analysis showed that a high level of guideline adherence is achievable in the context of clinical studies.
Those receiving and tolerating optimal pharmacotherapy experience a better prognosis. Nevertheless, the implementation of
heart failure medication needs further improvement in female and elderly patients, especially those in NYHA functional class
>II and patients with LVEF ≥30%.

Conclusion Our study demonstrated that serum-soluble LOX-1 levels were associated with proximal/mid segment of the LAD lesions. Furthermore,
this study suggested soluble LOX-1 might be a useful biomarker of coronary plaque vulnerability in patients with stable CAD.
Soluble LOX-1, the novel biochemical marker, may provide new insights into not only risk stratification but also therapeutic
strategy for CAD.

Conclusions Altered systemic venous flow return is associated with suboptimal Fontan hemodynamics and seems to progress with patients’
age and long-term follow-up after Fontan operation. Thus, MRI flow volume measurements might help in monitoring Fontan patients
before the onset of clinical signs of suboptimal hemodynamics.

Conclusion Data collected in DES.DE revealed that first generation DES used in SVG lesions did not offset the worse clinical outcomes
of bypass-graft intervention. Such sobering results in SVG may suggest to opt for native vessel PCI preferentially or occasionally
for surgical reintervention as decided by the Heart Team.

In conclusion, in STEMI patients reperfused by primary PCI, time-to-reperfusion was an independent predictor for
transmural infarction but not for infarct size and MO. However, transmural infarction was not predictive of death and congestive
heart failure.

distinguish AMI from CMI. Especially in patients with insufficient T2-BB-TSE image quality, the SSFP sequence may be an alternative
for the detection of myocardial oedema.

Conclusion Repeat defibrillation testing after administration of amiodarone therapy rarely fails in patients with left-sided devices.
We observed a higher test failure in patients with a device in the right-subpectoral position although this subgroup was small.
Repeat defibrillator testing is safe as no relevant complications were observed.

Conclusions A heart rate ≥90 bpm at the time of MODS diagnosis is an independent risk factor for increased 28-day mortality. As in patients
with cardiovascular conditions such as coronary heart disease or chronic heart failure, heart rate might constitute a target
for heart rate-lowering therapy in the narrow initial treatment window of MODS.

Conclusions Our results demonstrate a tendency towards a reduction of mechanical ventilation time for patients without severe illness
after sudden cardiac arrest achieved by implementation of a new sedation management, whereby significant longer ventilation
time was observed for severe ill patients. Because of lack of statistical significance of our present study, a randomized
study with sufficient power is necessary to demonstrate positive effects of a standardized sedation management and its influence
on severity of illness.

sample allows screening for heart-focused anxiety in patients with or without cardiac diseases and the measurement of therapy
success after psychotherapeutic or pharmacological intervention.

Conclusion Body weight, patient activity, and the difference between MHR and RHR are mutually correlated and may reasonably contribute
to an algorithm for predicting heart failure deterioration. Blood pressure appears to offer no additional value. As both genesis
and symptoms of heart failure exacerbation are non-uniform and complex, the telemonitoring concepts for heart failure patients
should employ continuous monitoring of multiple diagnostic parameters, rather than rely on a single parameter. Provided that
patient compliance is strictly supervised, reliable data flow from sensors requiring patient involvement is possible.

Abstract Cardiac malformations with impact on loading patterns have the potential to progress to irreversible loss of ventricular function
during human fetal life. N-terminal pro-B-type natriuretic peptide (nt-proBNP) is a marker of cardiac dysfunction and involved
in cardiac remodeling and fibrosis. We evaluated nt-proBNP levels in the circulation of human fetuses with cardiac defects.
A total of 45 cases and 75 controls during the second half of gestation were recruited. Nt-proBNP concentrations were determined
in venous specimens obtained by fetal blood sampling. Results were correlated to echocardiography and Doppler studies. Mean
gestational age was 26.9 weeks (range 21.0–33.4 weeks). Levels of circulating nt-proBNP were elevated in fetuses with cardiac

Conclusion Our study indicates that TAVI under LAPS is as effective and safe as TAVI under GA. Furthermore, total procedure time, intervention
time and labor costs could be reduced by LAPS. Mobilization of patients could be achieved earlier. We therefore consider LAPS
to be favorable in patients undergoing transfemoral TAVI.

Conclusion This study demonstrates the marked disparity between the patient cohorts with chronic systolic heart failure presenting at
secondary and tertiary hospitals. Though patient characteristics—particularly age, aetiology of heart failure and the time
of implementation of pharmacological and device treatment of heart failure—differed significantly, after adjustment for differences
in baseline characteristics, no substantial difference in the combined endpoint of all-cause mortality and HTX was found during
the 5-year follow-up period.

Abstract Two sons of a consanguineous marriage developed biventricular cardiomyopathy. One boy died of severe heart failure at the
age of 6 years, the other was transplanted because of severe heart failure at the age of 10 years. In addition, focal palmoplantar
keratoderma and woolly hair were apparent in both boys. As similar phenotypes have been described in Naxos disease and Carvajal
syndrome, respectively, the genes for plakoglobin (JUP) and desmoplakin (DSP) were screened for mutations using direct genomic
sequencing. A novel homozygous 2 bp deletion was identified in an alternatively spliced region of DSP. The deletion 5208_5209delAG
led to a frameshift downstream of amino acid 1,736 with a premature truncation of the predominant cardiac isoform DSP-1. ...

Is there an additional benefit of serial NT-proBNP measurements in patients with stable chronic heart failure receiving individually optimized therapy?

Conclusion To date, this is the largest analysis of serial NT-proBNP measurements in patients with CHF receiving individually optimized

higher predictive value than stratification of change in serial measurements.

Abstract Heart failure is a frequent and life-threatening syndrome which is not only the result of myocardial injury or hemodynamic
overload as commonly perceived, but appears to be the result of an interplay among genetic, neurohormonal, inflammatory, and
biochemical factors, collectively referred to as biomarkers. Biomarkers can become risk factors in case their therapeutic
modification results in an improvement of clinical outcomes. Among those markers identified in patients with heart failure,
a number appears to have direct clinical relevance in aiding diagnosis, risk stratification, monitoring therapy, and treating
to targets in order to improve clinical outcomes. These include brain natriuretic peptides (e.g., BNP, NT-proBNP), inflammatory
markers (e.g., hsCRP), neu...

Conclusions Copeptin has a distinct release pattern in patients with ST-elevation AMI, peaking within the first hour after symptom onset
before conventional cardiac biomarkers and falling to normal ranges within the first day. Further studies are required to
determine the exact role of copeptin in AMI suspects presenting within the first hours after symptom onset.

Conclusions Both non-invasive techniques are associated with low operating costs and require only a few expendable items for the rapid
determination of cardiac function. We found an acceptable agreement between IGR and ICG as well as a high reproducibility,
which was statistically significant higher for ICG. For cardiac output states exceeding the physiological range, we found
a statistically significant difference. Consequently, values of cardiac function determined by either method should not be
used interchangeably in the clinical setting.

Abstract Primary percutaneous coronary intervention (PPCI) is superior to thrombolysis (TL) as reperfusion therapy in ST-elevation
myocardial infarction (STEMI). TL is a rapidly available, but semi-effective therapy (effective reperfusion in 50% of patients
only), whereas PPCI is a potentially delayed, but highly effective therapy (effective reperfusion in >90%). Since TL loses
its efficacy beyond 2–3 h after symptom onset, it is a significant reperfusion alternative to PPCI in early presenters only.
The individual decision to treat an early presenter with PPCI or TL requires the evaluation of the time delay between potential
start of TL or PPCI, the PCI-related delay (PRD). PRD is greatest, if TL is given in the prehospital setting. Until now, prehospital
TL as ...

Conclusions Repair for AV endocarditis yields excellent results. It is associated with low operative mortality and provides satisfactory
early and long-term survival and favorable freedom from recurrent endocarditis and repeat operation. It should be considered
as the primary surgical option in these patients, and AV replacement should be performed only in cases of severe AV destruction
that renders repair techniques impossible.

Abstract Essential hypertension, insulin resistance, heart failure, congestion, diuretic resistance, and functional renal disease are
all characterized by excessive central sympathetic drive. The contribution of the kidney’s somatic afferent nerves, as an
underlying cause of elevated central sympathetic drive, and the consequences of excessive efferent sympathetic signals to
the kidney itself, as well as other organs, identify the renal sympathetic nerves as a uniquely logical therapeutic target
for diseases linked by excessive central sympathetic drive. Clinical studies of renal denervation in patients with resistant
hypertension using an endovascular radiofrequency ablation methodology have exposed the sympathetic link between these conditions.
Renal denervation could ...

Conclusion Bone marrow-derived stem cell therapy yields smaller EDWT when compared with placebo patients suggesting a positive effect
on maladaptive hypertrophy of viable myocardium. This notion is supported by the enhanced regional contractility within the
BMC group which is inversely correlated with EDWT.

Conclusions Patients with AF managed in primary care often receive guideline-conforming therapy including antithrombotic therapy, rate
control and rhythm control (numbers given above). Despite this apparent adherence, almost half of the patients were hospitalized
in the year prior to enrollment, suggesting that the therapies applied do not stabilize patients sufficiently to keep them
out of hospital.

Conclusion Clopidogrel low response is present in 1/10 patients and more frequent in ACS-PCI. MEA-guided optimizing of thienopyridine
therapy is feasible and results in a very low specific MACCE rate. Low response to all thienopyridines is rare.

Conclusion In clinical practice, elderly patients with ACS undergoing PCI receive significantly less intensive antithrombotic treatment.
In terms of successful PCI, there was no significant difference between the age groups in patients with elective PCI, UA and
NSTEMI. The higher in-hospital mortality of the elderly in all patient groups is most likely due to a higher rate of comorbidities.

Abstract Soluble TWEAK is a member of the TNF-alpha family of cytokines that has been shown to predict mortality in patients with heart
failure. Pulmonary artery hypertension is a devastating disease, in which right ventricular function has been shown to be
the major determinant of prognosis. In this hypothesis-generating study, we sought to evaluate the potential usefulness of
sTWEAK in the prediction of disease severity in patients with pulmonary artery hypertension. We therefore conducted a retrospective
analysis of sTWEAK serum levels in 95 stable patients with PAH. For all patients data on hemodynamic parameters, biomarkers
and functional exercise tests were available. Compared to controls, patients with PAH showed significantly decreased levels
of sTWEAK [median 314&...

Conclusion Abrupt clopidogrel cessation after 1 year of clopidogrel treatment results in ADP specific platelet hyperreactivity between
2 and 6 weeks post withdrawal. This mechanism may contribute to explain increased rates of stent thrombosis at this time as
it was observed in clinical studies.

Abstract Both, anemia and vitamin D deficiency are prevalent in patients with heart failure. According to recent evidence, vitamin
D may stimulate erythropoiesis. We measured circulating 25-hydroxyvitamin D (25[OH]D), 1,25-dihydroxyvitamin D (1,25[OH]2D) and hemoglobin (Hb) in a cross-sectional study in 364 end-stage heart failure patients awaiting cardiac transplantation,
of whom 52.6% met the criteria for anemia (Hb < 13 g/dl in males and <12 g/dl in females). None of the patients were on erythrocyte-stimulating

reduced in the lower tertiles of 25(OH)D and 1,25(OH)2D (P < 0.001). In multivariate-adjusted lo...

Conclusion During World Cup Soccer, the number of deaths due to myocardial infarction was not measurably increased compared to a matched
control period. Thus, we could not demonstrate a translation of a stress-induced increase of cardiac morbidity into a noticeable
increase in mortality. However, our findings are based on a public mortality registry, which may be flawed in many ways, regarding
ascertainment of causes of death, in particular.

Conclusions The S-ICD system can be implanted without the use of fluoroscopy by using anatomical landmarks only. Episodes of VF were accurately
detected using subcutaneous signals, and all induced and clinical episodes were successfully converted. The S-ICD system is
a viable alternative to conventional ICD systems for selected patients.

Conclusion Although abciximab reduced the 30-day and 1-year incidence of major adverse cardiac events in patients with NSTE-ACS undergoing
primary PCI after pre-treatment with a 600-mg loading dose of clopidogrel, the agent did not improve or impact on the evolution
of LVEF over 6–8 months of follow-up.

Abstract Arrhythmogenic right ventricular cardiomyopathy/dysplasia (ARVC/D) is a predominantly genetically determined and heritable
form of cardiomyopathy that is characterized pathologically by the replacement of myocytes by adipose and fibrous tissue and
leads to right ventricular failure, arrhythmias, and sudden cardiac death. The estimated prevalence of ARVC/D in the general
population ranges from 1 in 2,000 to 1 in 5,000, men are more frequently affected than women, with an approximate ratio of
3:1. ARVC/D can be inherited as an autosomal dominant disease with reduced penetrance and variable expression, autosomal recessive
inheritance is also described. There have been 12 genes identified which are linked to ARVC/D, encoding several components
of the cardiac desmosome...

Conclusion Hyperglycemia at admission in STEMI patients who are successfully treated by PCI is independently associated with the presence
and extent of microvascular obstruction on contrast-enhanced CMR. Thus, microvascular obstruction as assessed by CMR may be
a mechanism that relates admission hyperglycemia in acute STEMI to worse outcome.

to iNO for the treatment of perioperative pulmonary hypertension. However, neither treatment alone abolished the occurrence
of PHTCs.

Conclusions This study demonstrated a weak correlation of sE-selectin level with inflammatory markers and prediction of short-term mortality
in diabetic CHF patients. Elevated serum sE-selectin levels and concomitantly increased NT-proBNP concentrations have additive
predictive power in CHF. This suggests that parallel activation of various pathophysiological pathways confers increased risk
of adverse outcome in CHF.

Conclusion This post-hoc analysis suggests the existence of an interaction between statin therapy before PCI and antithrombotic therapy
during PCI. Patients receiving bivalirudin therapy at the time of PCI showed less periprocedural myocardial infarction when
on pre-PCI statin therapy which has to be investigated in further studies.

Conclusion By using the parameter “afterload-related cardiac performance, ACP”, the impairment of cardiac function can be reliably quantified
showing that septic cardiomyopathy occurs frequently and is of prognostic relevance in patients with septic MODS.

Conclusions Hypertension in pregnancy is characterised by abnormalities in the vascular endothelium, with abnormal CECs and vWf that correlate
with BPs. This may reflect dysfunctional processes that are counteracted with reparative attempts at restoring endothelial
integrity.

Conclusion In TTC patients, high-anxiety trait is a common finding but it is not significantly more frequent than in patients with STEMI.
Moreover, a high-anxiety trait seems to be neither associated with a worse clinical outcome nor a predictor of TTC. Our study
do not support the routine evaluation of anxiety trait in patients with TTC.

Conclusion Both single time point cTnT concentrations and CK activities correlate well with infarct size early after primary PCI for
STEMI. Cardiac TnT levels determined 3–4 days after revascularization for acute myocardial infarction allow for a good estimation
of acute infarct size as well as an approximation of LV function and morphology.

of aortic coarctation (CoA). The prospective study concerned 32 pediatric patients after the CoA surgery. Tissue Doppler imaging
parameters including strain and strain rate and the conventional echocardiographic indexes were analyzed in patients and healthy
controls. Analysis of mitral annulus velocities, E–E′ ratio, strain, and strain rate of left ventricular mid-cavity segments and conventional indexes of mitral inflow showed
the worsening of left ventricular diastolic mechanics in the study group compared to healthy controls. The E/E′ ratio was significantly higher in the study group compared to the control group (8.30 ± 3.24 ...

Conclusion Based on the best available data, in patients with symptomatic severe AS, TAVI demonstrates an improved 1-year survival compared
with medical treatment. The survival benefit of TV-TAVI over medical therapy elucidated from this systematic literature review
is +16.8% and therefore, in good congruence with the recently published results from the randomized PARTNER US trial (+20%).

Conclusions Approximately one-fourth of patients with NSTEMI revealed OCA that was more frequently found in coronary arteries supplying
the infero- or posterolateral myocardium. Patients with OCA had larger infarcts and more non-fatal reinfarctions than patients
with non-OCA. Well-developed collaterals may limit the myocardial damage in these patients.

Conclusions Residual stenosis in AoD does not affect the arterial vasodilatation nor stiffness in patients after CoAo repair. Early operation
has no impact on peripheral vascular remodeling or central pressure which supports the claim that coarctation of the aorta
is a systemic vascular disorder which leads to progressive vascular and end-organ damage despite early correction.
