Abstract
This paper describes a local initiative that aimed to resolve difficulties experienced by practice teachers in assessing the practice of students undertaking a BSc graduate diploma in community nursing. Practice teachers initially raised a concern that they were unable to make a holistic assessment of a student, as they had no criteria that enabled them to address issues relating to values, attitude and behaviour. Practice teachers and academic staff worked together over a period of time to update assessment practices, developing a Practice Assessment Grid (PAG). The PAG identifies a common language with which practice teachers can benchmark their own values, enabling them to justify their decisions. Evaluation to date indicates that the PAG enables pract...

    Abstract
This paper reviews the literature on the effectiveness of the debriefing of mothers after giving birth and considers the recommendations in relation to health visiting practice. Studies suggest a correlation between negative birth experiences and depression in women and a small percentage of women meet the criteria for post-traumatic stress disorder (PTSD). Studies on debriefing were examined, some of which suggested that midwife-led debriefing after an operative birth was ineffective in reducing maternal morbidity and the possibility of contributing to emotional health problems could not be excluded. Significant influencing factors were the timing and construction of the debriefing that was carried out. Guidance was found to give clear recomme...

Authors: Woodcock E
    Abstract
The Twf project was established in Wales in 2001 with the overall aim to increase the transfer of the Welsh language within the family. The project has two main strategies, which are to work with midwives and health visitors in order that they can inform parents of the advantages of raising children bilingually, and to raise awareness among parents-to-be, parents and the general public of the advantages of raising children bilingually. The project is funded by the Welsh government through the Welsh Language Board, and its community workers provide health visitors, midwives and other partners within their areas with Twf resources and information about relevant research, and visit antenatal and postnatal clinics on a regular basis to talk to parents.
This study aimed to survey childcare support being undertaken by nursery school teachers in Japan and to identify the theories and skills necessary for nursery school teachers to carry out childcare support in the future. In 2007, a postal questionnaires was sent to 1200 teachers at 850 nursery schools, with responses from 712 teachers from 456 nursery schools. In terms of knowledge and skills considered necessary by nursery school teachers in local childcare support centers, 'knowledge and skills of giving advice' was the most common response, followed by 'understanding of children's growth and development'. Nursery school teachers need to receive sufficient education before and after graduation to gain these skills. It is also clear that there is a need for nursery school teachers to mak...

    Abstract
This paper aims to highlight some working class women's childcare practices in northern industrial areas of Britain during the latter half of the 19th century. It aims to challenge the commonly held belief that 19th century northern working-class factory mothers were irresponsive and neglectful toward their infants, thereby fuelling the high northern infant mortality rate. It will do this by showing that factory mothers were responsible and responsive toward their infants despite being thwarted by the working patterns of industrialisation. It begins by outlining the arguments made by historians that northern working class women were neglectful toward their children. Then key areas such as the working patterns of waged factory mothers will be illustrated...

    Abstract
Lactational amenorrhoea method (LAM) is a reliable form of contraception for up to six months following childbirth, provided the mother's periods have not returned and she is breastfeeding exclusively with sufficient feeds and no dummy or bottle use. This paper considers how LAM works and how it may be supported in practice, and provides a local example involving the use of information provision and training to overcome barriers to its understanding and use. An information leaflet was produced based on national guidance and with input from local practitioners and women. Brief training was also used to help ensure that women receive consistent and up-to-date information from health professionals.

    Abstract
This paper explores the community empowerment aspect of health promotion, where social experience is a key indicator of health, from the perspective of health visiting and other public health nursing. This contemporary model of practice is put into a health promotion context by use of a slightly modified framework and by the inclusion of examples of methods and outcomes of practice that link conceptually with the health assets model and aspects of 'Big Society' thinking. Community development, social capital and capacity building are discussed as key elements of community empowerment with a 'bottom-up' agenda driven by community members. This concerns micro-population health gain, where the process, the quality of the public health nurse-community relatio...

This study explored health visitors' perceptions of their role in assessing and supporting parent-infant relationships and their views of their own educational needs. Semi-structured interviews were conducted with a purposive sample of six experienced health visitors. Health visitors used multiple sources of information in assessing parent-child relationships, including direct observation of behaviour, assessment of known risk factors, knowledge of norms and 'intuition'. Strategies to support the parent-infant-relationship included parenting support at home, listening support, encouraging parenting group attendance and referral to other support agencies. Health visitors reported a lack of training in parent-infant relationships. General agreement is required nationally and locally on educa...

Baby-led weaning is an approach to the introduction of solid foods that is being followed by increasing numbers of parents, but what is it, and should health visitors be encouraging it? This paper aims to refresh practitioners' background knowledge of complementary feeding and to outline the key features of baby-led weaning, as well as to explore the evidence that supports this approach as a logical adjunct to the move to six months for the introduction of solid foods. The more common concerns of parents and professionals, such as choking and iron intake, are addressed. Tips for implementing baby-led weaning are included and some of the potential benefits identified.

Child health promotion services in England have been revolutionised in the last decade, with a shift from a low-threshold universal service to a more highly targeted programme. This has affected health visitors' preventive healthcare work with families. This paper draws on findings from the qualitative arm of a mixed-methods study that aimed to explore how child health promotion reform has been implemented. The particular focus of this paper is on health visitors' experiences of the process of policy reform. In-depth semi-structured telephone interviews were conducted with 25 health visitors working in urban areas in England, and data were analysed thematically. Health visitors described policy change as being enforced, but views on the new programme were coloured by ...

A new self-study training programme was delivered to community practitioners as part of an organisation-wide breastfeeding training programme. Knowledge outcomes were measured before and after the training using the Coventry University Breastfeeding Assessment. Interviews with participants found the programme was well received and seen as a key way to ensure consistent evidence-based practice. Practitioners across all professional groups increased knowledge scores after training. Independent evaluation showed that self-study is flexible and acceptable, but requires self-motivation as well as basic computer skills and access. Self-study is potentially more affordable, and could provide a means to regularly refresh the knowledge of staff and...

A perception of low milk supply is often cited as a reason to stop breastfeeding, though most mothers can produce more than enough with the right support and information. Domperidone may be used to promote lactation, and there is evidence to support its use. However, the many non-pharmacological ways in which a mother may be supported should be tried before considering domperidone.

Food allergies are recognised as a major paediatric health problem in Western countries, affecting 6% to 8% of children up to the age of three years. Food allergy symptoms can be very severe or subtle and sometimes overlooked. Following a Department of Health review of allergy services in 2006, the National Institute for Health and Clinical Excellence (NICE) was commissioned to produce a guideline to support GPs and other healthcare professionals in primary care and community settings in recognising the signs and symptoms of food allergy in order to bring about consistency in diagnosis and assessment. This paper has been written by a member of the guideline development group to provide an overview of the guidance. The guidance addresses six key clinical questions abou...

This paper reports on a multi-method research project that explored perceived role satisfaction and professional burnout among community practice teachers (CPTs) while facilitating post-registration education and caseload management. A bespoke Satisfaction Questionnaire and the Maslach Burnout Inventory (Educators) were completed by 23 participants to elicit quantitative and qualitative data. Findings are presented in relation to three themes--aspects of the CPT role leading to satisfaction, aspects leading to dissatisfaction or burnout, and ways to enhance satisfaction and reduce burnout. The majority of CPTs were satisfied with their current role. A number of factors were elicited that affected participants' perceived satisfaction. Respondents s...

Providing high quality, effective services is fundamental to the delivery of key health outcomes for children and young people. This requires a competent workforce. This paper reports on the development of a validated competence framework tool for the children and young people's health workforce. The framework brings together policy, strategic agendas and existing workforce competences. The framework will contribute to the improvement of children's physical and mental wellbeing by identifying competences required to provide proactive services that respond to children and young people with acute, continuing and complex needs. It details five core competences for the workforce, the functions that underpin them and levels of competence required to del...

The issue of children missing from education has been of concern both nationally and locally for many years. Following a local audit of health visitor and school nurse caseloads, the existence of unallocated child records or children who could not be found came to light. These records were held and managed by the local child health department. Children who did not attend school were not offered a school nursing service, and were monitored solely by the educational welfare service. This paper reports on a project to identify and investigate the whereabouts of these children and to allocate the remaining records to school nursing teams. This would ensure that all children in the local area were visible and that any unmet health needs were assessed and addressed. The pro...

Authors: Billingham K
The transition to parenthood is a time of great change for mothers and fathers, and also lays the foundation for the long-term health and wellbeing of the child. This emphasises the importance of preparation for parenthood. A Department of Health-commissioned review found provision and uptake of antenatal education to be variable, and so brought a group of experts together to consider Preparation for Pregnancy, Birth and Beyond. This paper provides an overview of the learning from the expert group's work, offering a new framework that professionals may use to plan effective local programmes and services.

Authors: Locyer V, Porcellato L, Gee I
Within the UK rickets is emerging as a national public health issue, particularly among at-risk groups. There is concern that health professionals are overlooking recommendations for vitamin supplementation and opportunities for prevention. This paper reports on a study that aimed to identify current knowledge and practice regarding vitamin D deficiency and supplementation among community midwifery and health visiting teams employed in one NHS trust. A questionnaire was distributed to all health visiting and community midwifery team members (n=96), with a 76% response rate (n=73). Results suggest varying levels of awareness about vitamin D deficiency and groups at increased risk. Only 52% reported that they were aware of health department recommen...

This study explores the use of emotional intelligence (El) as an explanatory factor in the relationship between attachment and mother-infant bonding and communication. With the long-term aim of synthesising these constructs into a useful tool to identify risk for mother-infant bonding and communication problems, the study utilised previously validated questionnaires. The inter-relationships between El and adult attachment anxiety/avoidance were investigated alongside their associations with mothers' self-assessments of bonding problems and with observer-based assessments of maternal communicative behaviour. The study suggests that the El capacity for managing and controlling emotions in challenging situations mediates the link between anxious attachment style and bonding problems. Women wi...

Authors: Regan P
This paper explores practice development and radical policy reforms found in the White Paper Healthy lives, healthy people, which place a higher priority on UK public health by integrating services under the concept of localism. As a method to improve health and reduce health inequalities, localism aims to promote public health services to be responsive, resourced, rigorous and resilient. However, issues of evidence-based practice, staff freedom to innovate against the tensions of reforms and organisational structures are significant within primary care. By promoting local and professionally driven innovation, the experiences of one community service practice development unit are discussed as an effective approach to encourage local change and innovation. Two issues of...

Authors: Irving L
Unintentional injury is one of the leading causes of morbidity and mortality in children aged one to 14 years and puts more children in hospital than any other cause. The National Institute of Health and Clinical Excellence (NICE) has published public health guidance on strategies to prevent unintentional injuries among children and young people aged under 15, along with related guidance on provision of home safety equipment and home risk assessments, and road design and modification. This paper, by a member of the NICE programme development group, aims to provide an overview of the guidance, which focuses on strategies, regulation, enforcement, surveillance and workforce development to prevent unintentional injuries in the home, on the road and during outdoor play an...

This paper will highlight one of the key findings of a qualitative study based on the analysis of in-depth interviews with 14 health visitors describing their experiences working with refugees and asylum seekers. Despite changes in government legislation to improve children's services in order to prevent harm to children, this recent study demonstrated that health visitors were working with the complexities of needs among refugees and asylum seekers related to safeguarding both children and vulnerable women. The health visitors often worked with families and individuals with no support from other professional services, they worked with failed asylum seekers who were unable to access other forms of support and they worked with women and children who were caught in a ...

The mental health of mothers is of significant concern to community practitioners. This paper reports on a case study exploring the success factors of a well established, health visitor-led protocol to identify and treat women with mild to moderate depression. Data were collected through interviews with a purposive sample of 12 community practitioners, a focus group of four health visitors and observation of a multidisciplinary steering group meeting. The protocol was described as an evidence-based tool and safety net that could be used flexibly to support clinical judgments and tailored to individual needs. Success factors included frontline clinician engagement and ownership, continuity of leadership to drive development and maintain mom...

Sleep disorders of childhood are common and cause much distress to children and families. Assessment and treatment services are limited in the UK. More multidisciplinary specialist facilities are needed. The professional interest shown in sleep training programmes and the public demand in response to media coverage of sleep issues suggests that these are likely to develop. A sleep disorder service has been in operation in Southampton for 25 years and this paper illustrates the organisational challenges for staff who are contracted on a largely sessional basis and have other healthcare roles. Experience in Southampton suggests that in such services the specialist nurse can play a pivotal role - as practitioner, team co-ordinator, waiting list manager, link person wi...

Authors: Jenkins M, Parylo C
This paper reports on a survey of homeless families in 11 hostels in Leicester that aimed to inform improved services for them. Of 167 families, 49 agreed to participate. Health was important to the families, and respondents had mixed experiences of attitudes from healthcare staff. Some reported very supportive attitudes, while a small number perceived rude or unhelpful behaviour. While families had a high opinion of their health visitor, one-third did not know how to contact the service and 40% (n = 21) did not know where their local children's centre or Sure Start was. When asked about difficulties accessing health care, not being listened to and not having the time to explain themselves were the two most frequently cited problems. When asked what they wo...

Authors: Waite A, McKenzie A, Daman-Willems C
The Care of Next Infant (CONI) programme is offered through health services to provide extra support to families with babies born following a cot death. Audit of centrally collected data about families registered with the programme and feedback from parents show that compared with national data, families on CONI have more factors associated with an increased risk of cot death, confirming that a history of cot death identifies a particularly high-risk group. The programme, first offered to parents in 1988, remains attractive and helpful to parents in the 21st century. The CONI programme, including provision of a movement monitor, enables health visitors to deliver a targeted programme of extra care to a vulnerable group of babies.

Breastfeeding is as a key target in Sefton as rates fall well below the national average. This paper reports on an evaluation that set out to examine the usefulness of an interactive group session designed to explore the health benefits of breastfeeding. The session used a tool called the Breastfeeding Treasure Box, developed in the US but not previously evaluated. It consists of a box containing 14 items, each chosen to indicate a benefit of breastfeeding, together with a lesson plan. The evaluation was conducted in parentcraft sessions. Five staff with experience of delivering the session completed qualitative questionnaires and 48 clients completed questionnaires about their experiences. Overall, the tool was found to stimulate learning and change t...

Authors: Puckering C, Webster J, Wilson P
This case history describes the first application of the Attachment and Bio-behavioural Catch-up (ABC) programme in the UK. It illustrates the key role and value of primary care clinicians in early infancy. The health visitor's careful and reflective observations, and her close links with a GP who shared her understanding of why these were significant, were the only means for this family to access help. The need for interventions in attachment is only likely to be identified by health visitors, with their unique opportunities to see families in their own homes.

Government policy has highlighted the provision of a postnatal service that responds to women's physical, psychological, emotional and social needs. This paper presents the results of a small in-depth qualitative study that aimed to explore the views of first-time mothers with regard to the level of support they would have liked to receive from health visitors during the postnatal period, focusing on the theme of health visitor contact. Homogenous sampling was used to identify seven first-time mothers. Data was collected from semi-structured interviews and analysed using a thematic network approach. Four organising themes were identified--health visitor contact, emotional adjustment to motherhood, infant feeding and other support. Although the first-time mothers value...

Progress in encouraging and enabling more black and minority ethnic (BME) staff into positions of leadership within the NHS has been slow. This paper reports on a project to develop a DVD in order to portray positive images of BME leaders and the contributions that they make to the NHS. Filmed semi-structured interviews were conducted with 13 participants in leadership positions to explore their experiences, along with audiotaped interviews and electronic questionnaires with those who did not wish to be filmed. Sections of film were selected for inclusion in the DVD based on key themes. Participants described the positive influences, factors and barriers that had affected their careers. Self-motivation was seen as a key factor, while being able to cope with work-life ba...

Community nursery nurses (CNNs) are an important part of the multidisciplinary team. This paper discusses how two students undertaking a foundation degree in early years experienced changes to their personal and professional identities. A 'life history' approach was used to understand and interpret their experiences in depth. Both students would not have entered higher education had it not been for the widening participation drive. The higher education experience had a positive influence on personal and professional identity for the learners. They underlined that the widening participation drive can enable those students from 'non-traditional backgrounds' to enter and benefit from higher education. However, changes to higher education funding and public sector cutb...

Authors: Wootton J, Norfolk S
Bedwetting (nocturnal enuresis) is a common and distressing condition, both for children and young people and their families, and the National Institute for Health and Clinical Excellence (NICE) has produced its first guidelines for its assessment and treatment in those aged up to 19 years. This paper, by two NICE guideline development group members, provides an update on the new guidance. Treatment options include the provision of advice, use of a reward system and/or alarm, and desmopressin medication. These should be considered carefully depending on individual needs. Treatment for most children is effective and successful. Where necessary, referrals may be made to a specialist enuresis practitioner.

Brazil is a rapidly developing country. Modern health services operate in a situation of extreme social change, which resembles some of the conditions that gave rise to health visiting in 19th century Britain. A visit there revealed much interest in the health visiting principles of the search for health needs, the stimulation of an awareness of health needs, the influence on policies affecting health and the facilitation of health-enhancing activities. Explaining these principles to an audience on the other side of the world highlighted three underlying themes. The principles of health visiting are about health promotion, not assistance; they provide an integrated framework, not a list of competences or skills; and they are all underpinned by a particular value and v...

Children's centres aim to improve outcomes for children through the development of integrated services. Health visitors are expected to lead the delivery of child health promotion programmes in geographical areas that include children's centres. During 2007, a group of eight health visitors linked to Phase 2 children's centres in Bristol formed a learning set facilitated by a public health specialist to explore how their role as both caseworkers and public health practitioners could become part of an integrated children's centre team. They developed two linked models intended to help health visitors develop both aspects of their role in practice. First, a proposed management structure for a children's centre which included health visitors at both the strategic and pra...

The first three years of life have been identified as key to promoting children's later wellbeing, and partnership working across the statutory and voluntary sectors has been proposed as one of the best ways to meet the needs of families. Child health clinics are attended by parents seeking reassurance or help and advice from a health professional regarding child health and development. They have been used in Oxford to develop Early Explorer clinics, in which the statutory health visiting service and voluntary sector Peers Early Education Programme work together with the aim of improving outcomes for children and families. These Early Explorer clinics provide the opportunity to engage parents in supporting their child's development through interaction and non-d...

Cow's milk protein allergy is a common food allergy in infants and children, and along with lactose intolerance it requires a dairy-free diet at a time when adequate nutrition is critical. Parents need reliable advice and on-going support from healthcare professionals about appropriate dairy-free options and alternatives for infants and older children. Professionals need to base advice to parents on current evidence to improve client confidence and outcomes.

This paper examines the impact of imprisonment upon family members and illuminates the effects of imprisonment upon family health and wellbeing, the affect of shame and stigma and the lack of formal health and social welfare provision available to this distinct marginalised group. The dilemmas generated by the transition faced by families when someone receives a custodial sentence are significant and include physical and emotional loss, loss of social mobility and income stability, stigmatisation, stress and anxiety. The health visitor has a significant role to play in assessing and assisting families to acknowledge and meet the needs of this marginalised group within society. Research to inform practice is limited, and typically the research that is availab...

Parents of young children often seek the support of professionals when experiencing difficulties in managing their child's behaviour. Parents of children with complex neurodevelopmental disorders, including autism spectrum disorder (ASD), are no exception. This paper describes a pilot Incredible Years parenting programme that was offered to a group of parents of children with ASD. A service evaluation suggested positive changes within a 12-week period, particularly in parental mental health, but also in child behaviour.

This study aimed to examine whether practitioners were supported in fulfilling the development of these competencies and to identify strategic resources within the local health service that would either help or hinder these developments.A qualitative approach and purposive sampling were utilised, resulting in five participants in two focus groups. Findings were set within the continually evolving context of government policy and local health agendas--the potential facilitation or conflict of competence afforded by the local health strategies, priorities and requirements of the local and national health economy, the capabilities of health visiting staff and their service as a whole and the role of historical and cultural issues, and the future service and workforce development. The primary purpose of practice improvement is to improve clinical practice through changing the behaviour of healthcare professionals. Breastfeeding is a key public health issue, conferring benefits associated with both infant and maternal health, yet breastfeeding rates in the U.K. and Ireland are among the lowest in Western Europe. In this paper, the ways in which practice improvement can be utilised to enhance both efficiency and effectiveness are described, using a case study of the potential contribution of health visitors to increasing breastfeeding duration in primary care in order to illustrate this in clinical practice.

Recent months have seen great emphasis on leadership within the U.K. Unlike politicians, leaders of community nursing have little support from aides and advisors as they grapple with the implementation of policy agendas. This paper gives insight into some of the emotions involved in leading community nursing to meet some of the recent NHS policy agendas, such as shifting the balance of care. The focus of this paper aims to examine emotions in leadership, particularly collegial emotional labour within community nursing. Qualitative interviews with 12 leaders of community nursing pointed to the current trials and tribulations of undertaking a leadership role in community nursing. The nurse leaders indicated how they undertook surface acting t...

This paper reports on the development, delivery, content and student evaluation of a comprehensive elder abuse and self-neglect workshop for public health nursing and social work students. The workshop provided an interdisciplinary shared learning experience for the students to prepare them for their critical role in safeguarding vulnerable adults. The aim of the workshop was to increase knowledge, awareness and understanding of roles and responsibilities and critical practice problems in the prevention and management of elder abuse and self-neglect. The shared learning approach provided clarity on roles and responsibility, valuing and respecting the contribution of each team member. The importance of building communication and trust with team m...

According to the National Service Framework, children have a right to participate in the development of healthcare services and yet research suggests that young children are at risk of exclusion from user involvement initiatives. This paper outlines the findings of a participatory action research project conducted with families attending a health visitors' parent-baby group. A combination of participatory research methods were used to ascertain the infants' perspectives of the service and this led to a number of changes in terms of professional attitudes, service provision and working practices. Changes in professional attitudes included acknowledging the importance of social interaction to the children, recognising young children's views as embodied an...

The quality of early relationships is an important factor in the development of emotional health and wellbeing. Yet in the UK, we do not integrate support for the relationship between the baby and parents into antenatal parentcraft. Neither do we provide enough focus on the relationship in basic and post-qualification training or support for practitioners to integrate working with the relationship into their practice. The Solihull Approach provides one model for working with the relationship between parents and the child and between the parents and practitioner. It integrates concepts from disparate academic fields that can focus a practitioner's work on supporting the relationship between parents and baby or child, whether individually or in a parenting group.

This study explores whether a novel parent-supported emotional literacy programme called Parents And Children Together (PACT) is associated with improved social and emotional development for children compared to a standard curriculum. In eight schools in Cornwall, 686 children from Years 3 to 6 and their families participated in the PACT programme, and were compared with 212 peers from four demographically-matched schools who received a standard curriculum. Parents in intervention schools believed that the PACT programme had a significant positive impact on the social and emotional development of their children and improved the partnership with their child's school. Parental ratings of their child's emotional literacy was significantly higher after participation in the programme, and the c...
