            <description>We report an unusual case of hepatocellular carcinoma with three histologically and immunohistochemically distinct components, arising in a noncirrhotic liver, with a pulmonary tumor embolus from one of the three components. The histological patterns of the three components were fibrolamellar, well-differentiated nodular, and pleomorphic. The immunophenotypes were, respectively CK7- /CK20- /Hep Par1+, CK7+ /CK20- /Hep Par1+, and CK7+ /CK20+ /Hep Par1-. The tumor in the pulmonary embolus showed only the morphological and immunohistochemical features of the pleomorphic component. This unusual case ofmetastasizing hepatocellular carcinoma with three distinct histologic components suggests that the histological and immunophenotype of the tumor might be useful in predicting metastatic potential...</description>

            <title>Complications associated with the serological evaluation of febrile transfusion reactions.</title>

            <description>We report a patient with myelodysplasia who was noted to have a febrile response to a unit of red blood cells (RBCs). Testing of the posttransfusion specimen raised concerns for a hemolytic transfusion reaction when the patient's RBCs demonstrated a positive direct antiglobulin test (DAT) and the plasma showed incompatibility with the donor RBCs. Further evaluation revealed that: (1) antibiotic treatment induced a &quot;false&quot; positive DAT in the patient, and (2) donor RBCs were coated with an autoantibody causing them to appear incompatible during postreaction testing. Thus, if donor and recipient incompatibility is encountered during a transfusion reaction evaluation, testing of donor RBCs may help resolve issues of serological incompatibility.

            <description>We report a 21-year-old male track athlete with exercise and rest ischemic symptoms and a right coronary artery (RCA) arising from the left coronary sinus. At surgical reimplantation, his right coronary artery had extensive atherosclerosis despite normallipid levels. This case illustrates anomalous coronary artery origins may produce turbulent flow thereby accelerating atherosclerosis and producing abnormal endothelial function and coronary spasm.

    A telephone interview was conducted with 170 consecutive patients who received a total knee arthroplasty (TKA), a minimum of two years postoperatively. Patients were askedhow satisfied they were with the results. They were asked to compare pain before and after the surgery, and function of the kneebefore and after surgery. Responses were obtained from 160 patients. Ninty-five percent of patients were satisfied, with 46% very satisfied and 40% extremely satisfied. Ninety-two percent had pain which was significantly better, or no pain. Eighty percent could do significantly more or everything they wanted.             <description>CONCLUSIONS: PNI is a relatively rare histologic feature in invasive breast carcinoma occurring 10 times less frequently than LVI. Tumor characteristics associated with PNI include higher T-stage, higher tumor grade and LVI. Despite this, patients with PNI can expect a meaningful survival at five years with appropriately aggressive adjuvant therapy (only one of 13 patients in this study was known DOD after mean follow-up of 5.9 yr). When observed in tissue sections PNI should be reported (for completeness) but its role as an independent poor prognostic feature remains questionable.

            <description>We present a case of a 56-year-old white male with a medical history of hypertension,who initially presented with chest pain, diaphoresis, and electrocardiogram (ECG) changes suggesting myocardial infarction. Initial frontal radiograph showed marked displacement of the cardiac silhouette into the left hemithorax. Cardiac catheterization was performed per hospital protocol showing no evidence of cardiac or coronary disease, but demonstrated marked rotation of the heart and abnormal displacement of the heart into the left hemithorax. As both of these findings could not explain the patient's symptoms, computerized tomography angiography (CTA) of the chest was performed demonstrating congenital absence of the pericardium, a rare entity estimated to occur in one in 10,000 to 14,000 people.

            <description>In this report we describe the clinical presentation, diagnosis, treatment and outcome of a 45-year-old woman with thoracic endometriosis. Four clinical presentations have been described. The majority have presented with catamenial pneumothorax, followed by hemothorax, hemoptysis and lung nodules. Our patient presented with right-sided hemothorax and lung nodules. Video-assisted thoracoscopic aurgery confirmed the presence of endometrial tissue embedded in the diaphragmatic pleura. Talc pleurodesis alongwith atotal abdominal hysterectomy and bilateral salpingo-oophorectomy led to a clinical and radiological resolution.

When these were ruled out, the search for a zebra had begun. The patient underwent sixty four-slice computerizedtomographic angiography (CTA) which revealed a right pulmonary artery aneurysm (PAA).     We reviewed trauma registry data (2004-06) from a Level 1 trauma center to describe the epidemiology of injury. There were 5,900 trauma patients. Most admissions were among white men, age 20-29 years (mean age = 48 years). Most admissions occurred in the summer, on Saturday and Sunday evenings. Blunt injuries (89%) and fractures (49%) predominated. Mean ISS = 9.7. ISS = 9-15 was highest among those 70+ years and lowest among patients 30-39 years (P &amp;lt; 0.01). Motor vehicle crashes and falls were the most common mechanism of injury. Violent assaults ranked highest in the injury prevention priority score.     Becker's muscular dystrophy (BMD) is one of the most common muscular dystrophy syndromes. The heart is always affected by myocardial fibrosis with early involvement of the right side. Cardiac MRI, offering superior assessment of the right side and visualization of the myocardial fibrosis could become the preferred imaging modality for evaluating patients with BMD.

            <description>We describe the experience of Yale's Center for Interdisciplinary Research on AIDS with AHI diagnosis in Connecticut, as a participating center in the National Institute of Mental Health Multisite AHI Study. We sought to identify AHI cases by clinical referrals and by screening for AHI at two substance abuse care facilities and an STD clinic. We identified one case by referral and one through screening of 590 persons. Screening for AHI is feasible and probably cost effective. Primary care providers should include AHI in the differential diagnosis when patients present with a nonspecific viral syndrome.

She is free of disease one year after diagnosis. Clinicians should be aware of the aggressive behavior of this tumor, and adequate investigation should be done to rule out metastasis from another primary site.

    A simulation education course was developed at Hartford Hospital to teach members of the trauma team the initial management of blunt trauma. Five educational scenarios were created using Sim-Man (Laerdal) with injuries to the 1. head, 2. chest, 3. abdomen, 4. extremities, and 5. multiple injuries. Students were assessed on self-efficacy and knowledge before and after participation in the scenarios, debriefing, and PowerPoint lectures. Self-efficacy increased significantly from pre- to posttesting for each of the five scenarios.     A 62-year-old asymptomatic woman was found to have a left pulmonary artery filling defect on a contrast computerized tomography scan of the chest. Workup revealed a diagnosis of left pulmonary artery sarcoma. Pulmonary artery sarcomas are rare tumors with poor prognosis. A high index of clinical suspicion is required to make a timely diagnosis and avoid inappropriate treatment for pulmonary embolism.

There was a suggestion of a possible protective effect of H. pylori positivity against development of gastroesophageal reflux and Barrett's esophagus.

    Falls are the most common cause of trauma in the elderly. Data regarding patterns of injury following geriatric falls are scant. We conducted a retrospective review of falls in patients aged 65 years and older seen at a trauma center over a nine year period. Two thousand eighty three patients met the inclusion criteria. Hip fracture proved to be the dominant injury (55%), 98% of which were isolated. Five hundred seven (21%) were non-hip fractures. It means that the government will subsidize EMRs for your offices, that within five years you will have to have an EMR to be paid by Medicare, that you will be paid at the same rate as doctors in more rural regions of the U.S, and that Medicare will pay you at reduced rates or not all for procedures it deems as of no benefit.

    Use of prehospital electrocardiograms (ECG) by emergency medical personnel may reduce door-to-balloon (DTB) time in patients with ST-segment elevation myocardial infarction (STEMI) referred for urgent percutaneous coronary intervention (PCI). A 79-year-old female awoke from sleep with severe substernal chest pain and called 911 for assistance. The patient was initially evaluated by advanced life support paramedics who performed a 12-lead ECG at the patient's home.             <description>We report the case of a 20-year-old female who underwent urgent liver transplant after presenting with fulminant hepatic failure secondary to Wilson's Disease. She was seen in consultation in the Osteoporosis Clinic eight months after liver transplantation with low bone-mineral density. Risks for osteoporosis in patients with chronic liver disease include hypogonadism, vitamin D deficiency and medications, and fracture rates are twice those of age-matched controls. In patients who undergo liver transplant, bone loss is more pronounced in the first year post-transplant, consistent with aggressive medical therapy, immobilization and hypogonadism secondary to immunosupressants.
