    Although transrectal ultrasound-guided biopsies (TRUSB) of the prostate gland are generally considered to be low-risk procedures, a study from Canada reported that there had been a significant increase in the percentage of hospital admissions following TRUSBs between 1996 and 2005 (1.0% to 4.1%). The authors speculated that the increase may be secondary to the emergence of antibiotic-resistant enteric bacteria or the result of an increasing number of cores taken with each TRUSB.             <description>We present an interesting case of tracheobronchial foreign body aspiration. A 29-year-old healthy female, with no history of pulmonary disease, presented on multiple occasions to healthcare providers with wheezing and cough. She was repeatedly diagnosed and treated for asthma with acute exacerbations. Upon further evaluation, the patient was subsequently found to have a tracheobronchial foreign body causing her symptoms. This case report highlights a clinical approach to wheezing illnesses and reviews the diagnosis and management of tracheobronchial foreign body aspiration.

    Abstract
    Duodenal diverticulitis has been considered a rare entity. The diagnosis with computed tomography has become a fast and noninvasive means of detection and guide to management. Cases of surgically resected duodenal diverticula reforming and reinfecting are rarer yet, often presenting with similar symptoms. Duodenal diverticulitis can present with a wide range of symptoms mimicking anything from gastritis to acute abdomen, or as in the case of our patient, as mid abdominal pain with newly developed liver abscess. According to the literature, duodenal diverticula are incidentally discovered at a rate of 5% to 10% in living adults and in up to 22% at autopsy.

            <description>We report an atypical presentation of sporadic Creutzfeldt-Jakob disease (CJD) in a 74-year-old woman that illustrates the difficulty in diagnosing this rare, but important, cause of rapidly progressive dementia. Despite well-established criteria, this diagnosis is often missed or substantially delayed (Table 1). In this case, a precipitous cognitive decline associated with a urinary tract infection initiallysuggested delirium.             <description>We report a case of a 39-year-old man who presented with a nonfunctioning pituitary macroadenoma which extended into the suprasellar region. He underwent a transcranial resection of the tumor followed eight months later by transsphenoidal surgery for the residual tumor. Postoperatively he developed massive subarachnoid and intraventricular hemorrhage. A cerebral angiogram revealed a leaking anterior communicating artery aneurysm which was not seen on the computed tomography angiography and magnetic resonance angiography before the surgery. Complications of transsphenoidal surgery, particularly vascular hemorrhagic complications, and risk of rupture of undetected aneurysms are discussed.

    Abstract
    Paratesticular leiomyosarcoma is a rare entity. Greater than 95% of tumors arising within the scrotal sac are testicular in origin. Of the paratesticular tumors, rhabdomyosarcoma accounts for nearly half of malignant masses. Paratesticular leiomyosarcoma is infrequently encountered, resulting in a relative paucity of literature regarding this tumor type. Here we present a case report of a leiomyosarcoma arising from the spermatic cord in a 71-year-old man and review the literature regarding paratesticular leiomyosarcoma presentation, diagnosis and treatment.

            <description>We present a unique patient with refractory diabetic ketoacidosis (DKA) as a first presentation of acromegaly. In addition to an insulin drip and intravenous fluids, our patient was managed with octreotide therapy. As he developed acute renal failure in the context of renal hypoperfusion, continuous venovenous hemofiltration (CVVH) was instituted. After only three days of therapy, the growth hormone (GH) level dropped circa fourfold and insulin growth factor 1 (IGF-1) level dropped ninefold. We postulate a hypothetical role of CVVH in removal of plasma GH and IGF-1, similar to the clearance of other medium size molecules such as brain natriuretic peptide and procalcitonin. If this is confirmed in future studies, CVVH may have therapeutic implications for the above category of patients.

            <description>The objective of this study is to determine the current availability and utilization of capnography by Connecticut prehospital providers. A web-based survey was distributed to prehospital providers throughout Connecticut. The majority of respondents who provided advanced levels of care had capnography available for both intubated and nonintubated patients. Capnography is often used to confirm endotracheal intubations, monitor ventilation, and during cardiopulmonary resuscitations. Prehospital providers are using this monitoring device increasingly in the field for critically ill patients. However, it's use could still be increased in other clinical situations.

    Brain lesions as heralding manifestations of systemic disease can pose a diagnostic problem, particularly when other organ system involvement is not apparent and neuroimaging is nonspecific. Computerized tomography (CT) may identify additional lesion sites. However, when chest roentgenogram is negative chest CT is not routinely considered. A patient is presented in whom chest CT led to lung biopsy and diagnosis of cerebral cryptococcoma when clinical and neuroimaging features were nonspecific, and chest x-ray and serology were negative. We suggest that chest CT is warranted in patients with abnormal brain imaging not typical of a primary brain process, even in the presence of a negative chest x-ray.

            <description>We describe a case and review the literature on this malignancy. Our patient was asymptomatic and presented with a left upper-lung field lobulated mass. Primary PMCN is a spectrum of mucinous cystic lesions that should be considered as a separate lung tumor entity due to its unique morphologic features and its distinct clinical behavior. Prognosis is generally better than other more common lung malignancies.

    This paper describes a project designed to increase computerized physician order entry (CPOE) at a community hospital staffed by voluntary and employed physicians. Computerized physician order entry increased from 13% to 80% of inpatient medications ordered by voluntary doctors and from 42% to 87% of medications ordered by all physicians. Voluntary doctors did not shift medication ordering to employed staff. A weak correlation was found between total medication orders and use of CPOE by voluntary doctors. The rate of medication errors was reduced. The use of CPOE shortened time to pharmacy verification of orders and improved identification of ordering physicians.

The authors report the case of a 62-year-old woman with a cecal mass that was surgically removed and the histopathologic diagnosis was adenocarcinoma of the colon. Meanwhile, aretroperitonealpelvic nodalmasswas detected as questionable metastasis of the primary tumor. A subsequent computed tomography-guided fine-needle biopsy was carried out to establish the tumor stage. Surprisingly, the fine-needle biopsy revealed a benign schwannoma. This unusual case posed a dilemma in postoperative staging of the colon cancer.

    Shoulder instability is common in young athletes due to acute and repetitive trauma, as well as overuse in athletes with generalized ligamentous laxity. In overhead athletes, instability may present as impingement or rotator cuff tenderness. Fortunately, most symptomatic instability can be treatedwith rehabilitation. In athletes with recurrent symptomatic instability despite conservative treatment, surgical stabilization can restore function and allow return to sport.

            <title>Intrathoracic lymphadenopathy: sarcoidosis or metastatic seminoma? The diagnostic dilemma revisited.</title>

            <description>We report a young patient with stage I testicular seminoma followed by close surveillance after radical orchidectomy, who developed hilar and subcarinal lymphadenopathy more than two years after the original diagnosis. A mediastinal biopsy was consistent with noncaseating granuloma with no evidence of tumor. Our case highlights the importance of histologic confirmation of the etiology of lymphadenopathy in these cases. We reiterate that histological examination remains the cornerstone for establishing a definite and accurate diagnosis of testicular seminoma relapse.

            <description>CONCLUSION: With the complexities of cancer genetic counseling and testing, it may be unrealistic to expect all clinicians to provide these services. A more realistic approach is better provider education and a framework in which healthcare providers identify patients who would benefit from a referral to a certified genetic counselor or experienced cancer genetics professional.
