    In his famous 1918 book &quot;The sensitive relative delusion&quot; Ernst Kretschmer not only described this specific delusional disorder but also the underlying sensitive personality disorder. Until now, relatively little attention has been paid to this part of his work. The incomplete reception is outlined, followed by a description of the symptoms, psychodynamics and course of sensitive personality disorder. The interpretation of &quot;Anton Reiser: a psychological novel&quot; - the autobiographical work by Karl Philipp Moritz in 1785 - is used as an example since it describes Anton Reiser as a sensitive personality.

    Abstract
    The electroencephalogram (EEG) is a specific diagnostic method for the evaluation of patients with epilepsies. Interictal epileptiform discharges (IED) recorded in the seizure interval have a high association with the clinical diagnosis of epilepsy. IEDs have to be differentiated from normal variants that resemble IEDs. The EEG may help in the localization of the epileptogenic zone and in the syndrome classification, which is important for therapy and prognosis.

            <description>CONCLUSION:            Emergency medical services should be involved in the establishment of admission programs for acute stroke patients to provide the fastest means of transportation to a stroke unit. Coma, symptoms of posterior cerebral circulation and epileptic seizures cause difficulties in ensuring an immediate and correct diagnosis. Sending an emergency doctor to the scene increases diagnostic certainty which is essential to initiate early treatment.

    Status epilepticus (SE) is a medical emergency. For diagnostic purposes EEG is mandatory when motor phenomena are absent or when a single seizure evolves into SE with impaired consciousness. The EEG may show focal or generalized status patterns, which must be distinguished from encephalopathies. Initially benzodiazepines are recommended; lorazepam is the drug of choice. When the SE persists, phenytoin, valproate, levetiracetam, lacosamide, and phenobarbital are administered. The choice depends on the underlying comorbidities. In this phase, only phenytoin is licensed. A generalized tonic-clonic status which is refractory is then treated with anesthetics including midazolam, disoprivan, or thiopental.     Epilepsy surgery is an important therapeutic option for patients with epilepsy since one third of all epilepsy patients will still not be become seizure free despite newly developed antiepileptic drugs. Anterior temporal lobe resection is the most common procedure. Extratemporal resections require more complex diagnostics and often invasive evaluation which is not the case in most temporal epilepsy patients due to improved imaging (MRI, PET, SPECT). Electrical stimulation of the anterior thalamus has been available as a treatment option since last year.

    Vitamins are not uncommonly uncritically prescribed by neurologists and other medical professions. The effects of vitamins are, however, often pharmacologically and biochemically well-defined. This offers the opportunity for a rational scientific approach to treatment. In this article the biochemical and pharmacological mode of action of vitamins B1 (thiamine), B6 (pyridoxine) and B12 (cobalamine) will be discussed and modern approaches to the diagnosis and treatment of clinical states of hypervitaminoses (B6) and vitamin deficiencies (B1, B6, and B12) will be presented.

    Abstract
    The present state of medicolegal evaluation of persons with traumatic brain injury is discussed in this contribution. Both the ICD and German jurisdiction see a strict separation between structural and only functional brain damage that can be reliably evaluated through neurological, neuropsychological and neuroradiological means. However, there is evidence mainly from MRI investigations that there can be structural changes that presently cannot be detected in the single case, e.g. anisotropy. Furthermore, the neurological and neuropsychological differentiation of directly traumatic and psychogenic disorders can be difficult and requires expertise.

    Modern medical treatment of epilepsy renders two out of three newly diagnosed epilepsy patients seizure-free. More than 20 drugs are currently available and even for epilepsy specialists it may not be easy to select the drug which is best suited for the individual patient. Current guidelines recommend drugs with proven efficacy and optimal tolerability that address the individual needs of the patient. Non-enzyme-inducing agents are preferred if possible, to protect the patient from loss of efficacy when taking other medications for life-threatening disorders. In one out of three patients, seizure freedom is currently not possible due to drug-resistance, which requires a prompt diagnostic and therapeutic reevaluation of the epilepsy.

This will likely reduce the number of cases in the EDNOS category. In addition, the DSM-5 provides brief descriptions of several conditions of potential clinical significance without providing detailed criteria in order to stimulate additional research.

    Hirayama disease is a juvenile benign distal upper limb muscular atrophy rarely observed in Europe, usually monomelic involving C7-Th1 innervated muscles. It is characterized by insidious onset and a self-limited course within a few years. The pathogenesis of this mostly sporadic disease is not fully clarified. Cervical flexion myelopathy with mechanical ischemic damage of spinal motoneurons is the best established pathogenetic hypothesis, but neurodegenerative and autoimmune causes are also debated. Typically, young men of Asian origin are affected. Here we describe three German Caucasian patients with Hirayama disease and provide an up-to-date review of the literature.

            <description>CONCLUSION: A review of the current literature reveals that thermoregulatory disturbances as sequelae of antipsychotic drug administration depend on individual disposition as well as various independent risk factors such as environmental temperature, somatic comorbidities, endocrinological abnormalities (e.g. hypothyroidism) and structural damage of the brain. A complex interaction of dopaminergic regulatory mechanisms in the ventral hypothalamus and peripheral vaso- and sudomotor adjustments seems to be causative. Hypothermia following antipsychotic drug administration represents a serious adverse drug reaction and a potentially life-threatening event.

The reasons and motives behind the king's deposition are discussed.             <description>This article summarizes pertinent functional brain imaging studies addressing the neuronal underpinnings of ASD symptomatology (impairments in social interaction and communication, repetitive and restrictive behavior) and associated neuropsychological deficits (theory of mind, executive functions, central coherence), complemented by relevant structural imaging findings. The results of these studies show that although cognitive functions in ASD are generally mediated by the same brain regions as in typically developed individuals, the degree and especially the patterns of brain activation often differ. Therefore, a hypothesis of aberrant network connectivity has increasingly been favored over one of focal brain dysfunction.

Psychopathological, neuropsychological, brain imaging and genetic studies suggest possible pathophysiological links between ASD and ADHD.     Depersonalization represents an unspecific symptom which is to be found across the entire spectrum of psychiatric nosology. Delineating the historical lines of development of the depersonalization concept and reviewing existing psychopathological experiential knowledge reveals that depersonalization is underpinned by highly diverse modes of experience. In terms of differential diagnostics at the symptom level, a distinction can be made between depersonalization as a neurotic phenomenon on the one hand and a psychotic form occurring in schizophrenia and melancholia on the other.
