In conclusion, this was an unusual pediatric case of non-occupational allergic contact dermatitis from N-isopropyl-N-phenyl-4-phenylenediamine in black rubber bicycle handgrips. The young age of the patient and the development of contact sensitization within a relatively short period were striking. The release of the sensitizer from black rubber and the skin penetration were possibly enhanced by continuous friction and the accompanying hyperhidrosis, suggesting that the black rubber bicycle handgrip was the primary source of sensitization.

Conclusion:Fifty-seven percent of patients allergic to PPD in this study will likely tolerate newer permanent and demipermanent hair dyes based on PTDS. Most individuals not allergic to PTDS will also test negative to other substances in the dye series. All 10 patients who tested positive to PPD and negative to PTDS who subsequently used a PTDS dye free of PPD tolerated these products. Many individuals allergic to PPD will benefit from the newer PTDS-based products.

Transdermal therapeutic systems (TTSs) are commonly used to treat many diverse medical conditions. There are several advantages to this form of drug delivery; however, adverse cutaneous reactions are common. This review article provides an update on skin reactions due to TTSs containing medications newly available in TTS form, including buprenorphine, diclofenac, lidocaine, oxybutynin, rotigotine, methylphenidate, rivastigmine, teriparatide, and nicardipine.

Photodynamic therapy (PDT) is used to treat certain types of nonmelanoma skin cancer. Metvix cream applied topically in PDT is composed of the active substance methyl aminolevulinate and 14 excipients composing the vehicle. One case of occupational allergic contact dermatitis from methyl aminolevulinate is reported. A 49-year-old nurse's aide working in a PDT unit in the dermatology department developed a dermatitis involving the eyelids and fingers. The lesions began a few months after she started working in that unit. Patch tests were performed with the standard series (Spanish Group for Research into Dermatitis and Skin Allergies [GEIDAC]), cosmetics series, Metvix cream "as is," the Metvix vehi...

Shellac is a common ingredient in cosmetics such as mascaras, lipsticks, nail polish, and hairsprays. As an emerging contact allergen in patients with eyelid dermatitis and contact cheilitis, it was added to the 2009-2010 North American Contact Dermatitis Group screening panel. Consideration of this allergen should be given with patients who present with allergic contact dermatitis in areas where shellac-containing substances were applied.

    Dermatitis. 2011 Jun;22(3):E7-9
    Authors: Ozkaya E, Elinç-Aslan MS
Fixed drug eruption (FDE) is a distinctive drug eruption characterized by recurrent well-defined lesions in the same location each time the responsible drug is taken. Two different clinical forms have been described: the common classic pigmenting form and the rare nonpigmenting form. Nonpigmenting FDE is mainly characterized by symmetrical large erythematous plaques and the dermal histopathologic reaction pattern. Pseudoephedrine is known as the major inducer of nonpigmenting FDE. Pigmenting FDE from pseudoephedrine has not been reported previously. Here, the first case of pseudoephedrine-induced pigmenting FDE is reported, showing the characteristic fe...

Authors: Aakhus AE, Warshaw EM
Methyldibromoglutaronitrile/phenoxyethanol (Euxyl K 400) is a preservative found in both personal care products and industrial sources. Although Euxyl K 400 initially appeared to have low sensitizing potential, increased prevalence of contact allergy to Euxyl K 400 led to regulatory intervention. This review summarizes the history, epidemiology, and management of contact allergy to Euxyl K 400. Issues related to patch-test preparations are also discussed.

Conclusions:Our findings suggest a trend of two new classes of North American patients at our institution being diagnosed with CAD-younger women with skin types IV to VI and older men with skin types I to III. We observed a greater-than-expected number of positive patch-test reactions to para-phenylenediamine. We suggest that patch testing and photopatch testing of individuals may be useful adjuncts in the assessment of CAD.

We report the case of a 51-year-old female who underwent placement of an implantable cardiac defibrillator (ICD) and a biventricular pacemaker and who immediately postoperatively experienced sepsislike symptoms without any identifiable causative pathogens and without any dermatitis. Patch testing revealed an extreme reaction to cobalt confirmed as present in the defibrillator. Removal of the ICD led to the complete resolution of her symptoms. Patch testing helped guide the construction of a gold-coated ICD, which was implanted in the patient. The patient has remained symptom free after implantation of this gilded ICD.

We report three patients seen at Mayo Clinic (Rochester, MN) with allergic contact dermatitis reactions to exotic woods. Patch testing was performed and included patient-provided wood samples. Avoidance of identified allergens was recommended. For all patients, the dermatitis cleared or improved after avoidance of the identified allergens. Clinicians must be aware of the potential for allergic contact dermatitis reactions to compounds in exotic woods. Patch testing should be performed with suspected woods for diagnostic confirmation and allowance of subsequent avoidance of the allergens.

Authors: Basko-Plluska JL, Thyssen JP, Schalock PC
Cutaneous reactions to metal implants, orthopedic or otherwise, are well documented in the literature. The first case of a dermatitis reaction over a stainless steel fracture plate was described in 1966. Most skin reactions are eczematous and allergic in nature, although urticarial, bullous, and vasculitic eruptions may occur. Also, more complex immune reactions may develop around the implants, resulting in pain, inflammation, and loosening. Nickel, cobalt, and chromium are the three most common metals that elicit both cutaneous and extracutaneous allergic reactions from chronic internal exposure. However, other metal ions as well as bone cement components can cause such hypersensitivity reactions. To complicate things, patients may al...

We report two patients with atopic dermatitis and chronic actinic dermatitis who had positive patch- and photopatch-test reactions, which suggested both an allergic contact and a photoallergic contact dermatitis from bisoctrizole. Neither patient could identify previous or current contact with the chemical; nonetheless, it is possible that either the allergic contact or photoallergic contact dermatitis from bisoctrizole led to their chronic actinic dermatitis.

Authors: Herro E, Jacob SE
Majantol is a synthetic fragrance with an intensely floral, slightly fruity, and amber scent used in perfumes, cosmetics, and personal hygiene products. It has recently been identified as yet another fragrance contact allergen, with relatively high rates of sensitization. Majantol is not included in fragrance mix I or fragrance mix II, which are common patch-test screening chemicals for fragrance allergy. Moreover, because it is only a supplement to the North American series, there is a paucity of published data on the prevalence of contact allergy to this substance. Patch testing with Majantol could be useful in patients with suspected or proven contact allergy to fragrances; however, avoidance would be challenging, given that Majantol is unlikely to be list...

Prevention of occupational contact dermatitis is of utmost significance for both insurers and legislators because it can preserve the individual's ability to work and result in decreased costs for public health. In the last 20 years, many concepts for educational interventions and a multitude of teaching aids have been developed by insurance associations and public institutions from different countries (eg, the United States, Sweden, Denmark, Switzerland, and Germany). For didactic purposes, terms of different levels of prevention have been inaugurated in northern Europe in the context of occupational dermatology. This review presents different educational interventions in the fields of primary, secondary, and tertiary prevention a...

Conclusions:Patch-testing with hairdressing-specific chemicals (standard series plus supplemental hairdresser series) was appropriate for numerous clinical situations and was not limited to patients in hair care occupations. The supplemental hairdresser series helped identify more patients than would have been identified with the standard series alone.

Atopic dermatitis (AD) is a chronically relapsing eczematous disease, more common in infants and children than in adults and very rare after midlife. The diagnosis of AD is not always easy in adults, especially when the extension of lesions is limited and their distribution atypical. The aim of this study was to investigate epidemiologic and clinical features of adult AD. The medical files of 332 consecutive AD patients were reviewed to ascertain family and personal history of atopy, age at onset, morphology and localization sites of AD lesions, serum total immunoglobulin E levels, skin prick-test and patch-test results. The present study has show that the disease is more frequent in females and during the third decade of life, starts after the 18th ...

We report a case of a gentleman with a 1.5-year history of treatment-resistant pruritus ani. When patch testing revealed a positive reaction to nickel sulfate, he admitted to daily peanut butter consumption. His symptoms resolved with dietary nickel restriction. Patch testing may be useful in patients with pruritus of the anogenital region, not only to elucidate potential contact exposures contributing to the symptom but also to suggest possible dietary precipitants.

Delayed hypersensitivity reactions to natural rubber latex (NRL) have been recognized. These reactions may contribute to allergic contact dermatitis. We therefore have undertaken patch testing of patients with NRL if they are suspected to have allergic contact dermatitis to rubber.

In Europe, methyldibromoglutaronitrile (MDGN) was banned because of excessive rates of contact allergy. However, it is unclear whether the concentrations used for testing MDGN are optimal, as different groups have used varying concentrations with quite different rates of positive reactions.

PEPPERMINT (Mentha piperita) is a popular herb that can be used in numerous forms (ie, oil, leaf, leaf extract, and leaf water). Peppermint oil has the most uses, and use data on the oil are considered relevant to the leaf extract formulations as well. This herbal preparation is used in cosmeceuticals, personal hygiene products, foods, and pharmaceutical products for both its flavoring and fragrance properties. Peppermint oil possesses a fresh sharp menthol odor and a pungent taste followed by a cooling sensation. It also has a variety of therapeutic properties and is used in aromatherapy, bath preparations, mouthwashes, toothpastes, and topical preparations. Topical preparations of peppermint oil have been used to calm pruritus and relieve irritation and inf...

Contact Dermatitis 2010 was the sixth biennial meeting held jointly by the Experimental Contact Dermatitis Research Group (ECDRG) and the American Contact Dermatitis Society (ACDS) (Table 1). The joint meeting was first held in 2000, a year after the creation of the ECDRG by Frank Gerberick in 1999. The first meeting was christened by Ponciano (Chito) Cruz as "Blending Science with Best Practice", which is a title that has endured over the last decade. Among the innovations implemented in the most recent meeting held in Dallas, TX, are blending of basic science investigation with clinical material at all sessions of the meeting, the requirement for all presenters to submit abstracts, and the first Contact Allergen Bee.The Organizing Committee was composed of Ponciano Cruz (Di...

CONCLUSION: In AD patients treated with antibiotics and systemic azoles before patch testing, the number of positive patch-test reactions is almost identical to that of non-AD patients. Patch-test reactions to components of topical medicaments may be more frequent in AD as compared to non-AD patients.

CONCLUSION: Relevance varies between allergens. Physician assessment of relevance at the time of final reading is not the ideal method for determining allergen relevance. This has implications for when best to determine the relevance of certain allergens. For allergens with lower agreement, in particular, extended follow-up is recommended to accurately determine an allergen's contribution to a patient's allergic contact dermatitis, especially in those circumstances in which a patient's condition has not improved.

Airborne allergic contact dermatitis caused by gold is rare, often not even suspected and hence not investigated. Our purpose was to elucidate the clinical relevance of gold-induced airborne contact dermatitis. We analyzed and discussed the occupational dermatitis of two women working as restorers. Both patients' test results showed positive reactions to gold sodium thiosulfate only. The stop-and-relapse test confirmed the diagnostic hypothesis of airborne contact allergy. Our patients turned out to be gold sensitized because of occupational exposure to gold dust and gold leaf.

CONCLUSION:Corticosteroid allergy is found to multiple corticosteroids, and concomitant reactions occur across groups. Group D1 corticosteroid esters produced a higher positive reaction rate (61.9%) than groups D2 (52.38%) and A (51.72%). This may be due to different prescribing habits or the easy access to D1 corticosteroids sold over the counter by pharmacies in Thailand.

CONCLUSION:While it is recognized that the availability of retrospective data is limited, a longitudinal review of these data gives confidence that the QRA approach should be an effective tool for primary prevention. This study also highlights the importance of continued active monitoring of clinical patch-test data for fragrance ingredients.

We report an interesting case of a bartender with hand dermatitis who had an allergic contact sensitivity to lime peel, fragrance mix I, and fragrance mix II. Most reported cases of citrus peel allergy are due to d-limonene, which makes up the majority of the peel oil. However, our patient had an allergic reaction to geraniol, which is a minor component of the peel oil and is present in fragrance mix I. It is important to consider a contact sensitivity to citrus in patients who have positive reactions to fragrance mix I and II and who are occupationally exposed to citrus fruits. An initial positive reaction to fragrance mixes should prompt further testing to citrus in these individuals.

We report a rare case of a nonhealing ulcer at the site of an accidental molten aluminum burn that showed changes of eosinophilic cellulitis and panniculitis histologically. This process was followed by a generalized vesicular and pustular id reaction. The reactions resolved after 1 month of systemic corticosteroid therapy. Two months later, there was a relapse with bullae over the burn scar, accompanied by a pruritic oozing rash on the ears and face; the lesions were controlled again with systemic corticosteroid treatment. The clinicopathologic features suggest that the eosinophilic cellulitis with panniculitis and id reaction in the present case represents an unusual allergic reaction to aluminum. Aluminum allergy may be considered as a possible underlying process in the setting of eosin...

Authors: Pascoe D, Moreau L, Sasseville D
Allergic contact dermatitis from cosmetics is a common problem that is occasionally caused by new or rare allergens. When a patient has a positive patch test to a cosmetic product but to none of the common or commercially available allergens, it is important to further patch-test this patient to the ingredients of the product. Thorough testing with the breakdown of ingredients, usually obtained through cooperation with the manufacturer, often allows identification of the culprit allergen in the cosmetic product. In this article, we discuss emerging or rare allergens discovered by this method, including nail lacquer and lipstick allergens, copolymers, shellac, alkyl glucosides, glycols, protein derivatives, idebenone, and octocrylene.

This article summarizes various skin irritations that can occur after an ostomy and also reviews previously published reports of peristomal allergic contact dermatitis. In addition, the clinical importance of identifying these dermatoses (most important, their effects on the patient's quality of life), risk factors for the skin irritations, the importance of patch testing, treatment of stoma dermatitis, and the importance of patient education and patient-doctor communication are also discussed.

Reports of vitamin E-induced allergic contact dermatitis (ACD) and frequent use of vitamin-E derivatives (tocopherol, tocopheryl linoleate, tocopherol acetate, etc) in skin care products deserves further investigation into tolerability and suitability of vitamin E in skin care preparations. A PubMed search was conducted to review the prevalence of vitamin E-induced ACD. It revealed 931 cases of vitamin E-induced ACD mainly from one large study. There were no reported deaths and only three patients required hospitalization for treatment. It appears that vitamin E-induced ACD is an uncommon phenomenon; incidence is low despite its widespread use in skin care products. Given its antioxidant and photoprotective properties, vitamin E shou...

Authors: Reich HC, Warshaw EM
Formaldehyde-based resins have been used to create permanent-press finishes on fabrics since the 1920s. These resins have been shown to be potent sensitizers in some patients, leading to allergic contact dermatitis. This review summarizes the history of formaldehyde textile resin use, the diagnosis and management of allergic contact dermatitis from these resins, and current regulation of formaldehyde resins in textiles.

CONCLUSIONS:Although patch testing is invaluable in identifying individual allergen sensitivities, the identification of the ACD-causative specific chemical allergen and source material remains problematic. All glove brands used within days prior to and during an ACD episode should be considered potential sources of the contact allergen.

Sodium hydroxymethylglycinate (SHMG) is a preservative used in many commercially available products, including shampoos, conditioners, soaps, moisturizers, body sprays, baby wipes, room sprays, cleaning agents, and pesticides. It is in a class of chemicals known as formaldehyde-releasing preservatives. Notably, members of this class have been associated with allergic contact dermatitis, possibly due to the agents themselves, the formaldehyde they release, or both. Studies on SHMG in animals have demonstrated potential for sensitization and dermatitis, and formaldehyde-allergic patients have been reported to improve when products containing SHMG are avoided. Patients and providers need to be aware of this preservative.

We present a case series of four patients with allergic contact cheilitis (ACC) secondary to exposure to peppermint oil contained in a lip balm product. These patients developed eczematous dermatitis involving their lips and perioral skin. They were tested with the North American Contact Dermatitis Group standard series as well as with an expanded series of flavoring agents, sunscreens, plant and fragrance components, and their own products. The lip balm contained potential sensitizers such as propolis, lanolin, coconut oil, almond oil, peppermint oil, and vitamin E. Our patch-test results showed that peppermint oil was the most likely culprit in these patients' ACC. Peppermint oil is less commonly reported as causing ACC than are more common contactants such as balsam of Peru or nickel su...

We report a woman with positive patch-test reactions to carvone (newly added to the North American Contact Dermatitis Group standard series) and dermatitis on the head. She had used a hair conditioner with a "mint" scent, and the dermatitis resolved when she discontinued using this product. While the manufacturer would not confirm carvone as an ingredient, the clinical course, patch-test results, and ingredient list strongly suggest that this was a relevant allergen in this case of allergic contact dermatitis.

Authors: Kohli N, Habbal S
Thioureas are used as additives in rubber products and as antidegradants for natural rubber latex. They are an uncommon cause of allergic contact dermatitis. This case represents an example of occupational allergic contact dermatitis solely from thioureas in rubber products and no cross-reactivity with other rubber allergens. Standard patch tests may fail to detect thiourea-induced allergic contact dermatitis, so clinicians should consider thiourea sensitization in rubber allergy.

CONCLUSION:In vitro results indicate that geraniol and citronellol have low potentials for skin penetration, which has implications for their ability to induce allergenicity and for more predictive toxicologic profiling of these materials. In vivo studies should be done to correlate the in vitro results.

Authors: Russell K, Jacob SE
Bisabolol is an active plant extract isolated from German chamomile and thought to have antiinflammatory and skin-soothing properties. It is a common additive in many products, including moisturizing creams and ointments, lotions, cleansers, sunscreens, antiperspirants, and makeup products. Contact dermatitis from bisabolol has been reported in Europe and is purported to occur in the United States. Patch testing with bisabolol-containing products or bisabolol may be useful in the work-up of patients with presumptive allergic contact dermatitis or potentially worsening atopic dermatitis. Patients sensitized to bisabolol should be counseled to avoid any bisabolol-containing products.

Authors: Shaw DW
A 34-year-old male experienced severe allergic contact dermatitis from 12-hydroxystearic acid in a lip balm and from hydrogenated castor oil in an underarm deodorant. He also had a positive patch-test reaction to bis-diglyceryl polyacyladipate-2, which is present in the implicated lip balm and which itself contains 12-hydroxystearic acid. He was also incidentally found to have contact allergy to ricinoleic acid and castor oil. Ricinoleic acid is the principal fatty acid in castor oil, whereas 12-hydroxystearic acid is the principal fatty acid in hydrogenated castor oil. These two fatty acids are each 18-carbon 12-hydroxylated fatty acids, differing only in degree of saturation. The lack of patch-test reactivity to the analogous nonhydroxylated fatty acids, stearic acid...

This article discusses the future of allergic contact dermatitis (ACD) diagnosis and management. Due to early landmark discoveries in this disorder, there is a knowledge base that can be refined. Improved patch-test technology, patch-test strategy, and patch-testing clinical relevance are emphasized. Other factors for improvement include complete ingredient identification on Material Safety Data Sheets, knowledge of the relationship of extracutaneous allergic disease to ACD, understanding of the evolutionary implications of ACD, increased chemical identification of allergens in publication, and enhanced knowledge of immunologic contact urticaria and irritant dermatitis syndrome. Further research into in vitro diagnostic methods, skin bioengineering tools, and ribonucleic acid diagnostic me...

Authors: Bremmer M, Deng A, Gaspari AA
Biologic therapies are an efficacious new method of controlling a number of chronic conditions. Data regarding these medications continues to emerge, giving clinicians a greater understanding of their side effects profiles. The biologic agents used in dermatology, particularly the tumor necrosis factor-alpha inhibitors, have a number of varied dermatologic side effects. In this two-part article, we perform a review of literature regarding cutaneous side effects of infliximab, etanercept, adalimumab, rituximab, efalizumab, and alefacept. In this second part, we discuss injection site reactions, infusion reactions, vasculitis, drug-induced lupus erythematosus, psoriasiform lesions, rebound phenomenon, eczema, atopic dermatitis, and hypersensitivity ...

A localized flare of dermatitis may render patch tests uninterpretable in some patients with recently controlled widespread dermatitis.

CONCLUSION: In patients with irritable skin either immediately after widespread dermatitis is controlled or after the cessation of systemic corticosteroid treatment, a flare of dermatitis induced by patch testing may render patch tests unreadable and therefore uninterpretable. To avoid this confounding occurrence, a waiting period between control of widespread dermatitis and initiation of patch testing is advised.

CONCLUSION: While it is optimal for patch testing to be performed when patients are off immunosuppressants, immunosuppressive therapies should not be an absolute contraindication to patch testing.
