            <title>Unlicensed and off-label drug use in hospitalized children in Croatia: a cross-sectional survey</title>

            <description>Conclusion&amp;nbsp;&amp;nbsp;Unlicensed and off-label drug use is common. It is not illegal and may be clinically appropriate but is associated with a
 number of clinical, safety, and ethical issues. Regulatory authorities should use existing clinical evidence on the use of
 off-label and unlicensed drugs in decision making. Marketing authorization holders and national regulatory authorities should
 monitor for any safety concerns associated with unlicensed and off-label drug use and take appropriate measures as well as
 identify research priorities and mandate clinical studies to resolve important questions.

            <title>Adverse drug reactions: Analysis of spontaneous reporting system in Europe in 2007–2009</title>

            <description>Conclusions&amp;nbsp;&amp;nbsp;The results of this study make several noteworthy contributions regarding national spontaneous reporting systems. The relevance
 of the PBRR for the measurement reporting activity is clearly supported by the current findings. This study also shows that
 there is a general trend towards increased reporting activity. This is maintained by regional centres and encouragement of
 reporting. A further study would be helpful to assess the effectiveness of reporting systems at both the national and European

            <description>Conclusions&amp;nbsp;&amp;nbsp;As intake with a carbohydrate-rich meal is not recommended in the SPC of Zanidip because a twofold difference was considered
 to be clinically relevant, the intake of lercanidipine only 15&amp;nbsp;min before food intake does not seem to be consistent with
 this recommendation. The Marketing Authorisation Holder should clarify the dosing instructions in relation to meals and identify
 a sufficient time-lapse to ensure an exposure similar to that obtained in phase III clinical efficacy studies.

 ultra-rapid CYP2C9-catalysed metabolism. Haplotype based on SNPs 1–4 did not seem to contribute to variation in the MR of

            <description>Conclusion&amp;nbsp;&amp;nbsp;We developed a prediction basis for mean posaconazole concentrations in AML/MDS patients. Patient weight, presence of diarrhea,
 and concomitant medication (chemotherapy and pantoprazole) showed significant effects on posaconazole exposure. Corresponding
 adjustments of the starting dose according to the presence of diarrhea and during the co-administration of chemotherapy or
 proton-pump inhibitors appear justified before therapeutic drug monitoring results are available. Further investigation of
 the interaction between different chemotherapeutic regimens and posaconazole is warranted.

 the safety information provided on the labels of different statin-containing medicines is not standardized. Harmonization

 and “sympathomimetic” clinical features. The information from these three cases is useful to clinical pharmacologists, not
 only in managing individuals with acute methoxetamine toxicity but also in advising the appropriate legislative authorities
 on the risk of acute harm related to methoxetamine use. Further work is needed to determine whether methoxetamine is more

            <title>Pharmacokinetic drug interaction between gemfibrozil and sitagliptin in healthy Indian male volunteers</title>

            <description>Conclusion&amp;nbsp;&amp;nbsp;Although the bioavailability of sitagliptin was increased by 54% when co-administered with gemfibrozil, this interaction may
 not have any clinical significance as sitagliptin has a wide therapeutic index. Hence, in clinical practice, sitagliptin as
 100&amp;nbsp;mg tablets and gemfibrozil as 600&amp;nbsp;mg tablets may be co-prescribed without much threat of sitagliptin toxicity. However,
 these results may not hold if the dose of sitagliptin is increased or if is co-prescribed with other antidiabetic drugs and/or
 cytochrome P450 2C8/human organic anion transporter-3 inhibitors. Further studies are needed to confirm these results in patients.

 pharmacovigilance activities. The Anatomic Therapeutic Chemical for drug classification poses difficulties in many situations;
 the international proprietary name might be more efficient. The variability observed in the time needed for analysis suggests
 that a standardised methodology should be employed. Overall, the findings of this prospective study will contribute to refining
 the signal management procedure to be implemented in the future.

 in our series. As the use of lenalidomide will increase in the future for multiple myeloma and other diseases, clinicians
 should be aware of this entity.

 dosing requirement increases with increasing tacrolimus clearance through concomitant steroid use, undesirable changes in
 tacrolimus levels may occur when steroid doses are tapered, predominantly in slow metabolizers. This often neglected drug

            <description>Conclusions&amp;nbsp;&amp;nbsp;Our meta-analysis indicates that the superiority of ESAs over conventional therapy in patients with acute STEMI is limited
 using current procedures. However, there is evidence to suggest that the timing and dosing of ESA administration may be optimized.
 Moreover, the long-term cardio-protective effect of ESAs in this patient population may be beneficial and worth exploring.

However, additional model validation is needed prior to general use. LSS models to predict AUCOPZ/AUCSUL, and thus CYP3A activity, were not possible, even upon stratification by CYP2C19 genotype and ethnicity.

 volunteers may be predicted using a pharmacogenetic-based dosing algorithm. Simulations revealed that the desired subtherapeutic
 INR level may be achieved by reducing the predicted dose by approximately 15%. Further study is needed to assess the applicability

            <title>Cultural influences on medicine use among first-generation Pakistani immigrants in Norway</title>

            <description>Conclusion&amp;nbsp;&amp;nbsp;This study shows that cultural influences may affect medicine use among first-generation immigrants from Pakistan after having
 lived 10 years or more in Norway. Although access to drugs and basic health care services seems to be problem free, language
 proficiency is a considerable problem that obstructs access to information and is detrimental to the level of communication

 for analysis, the conclusions from the present study still need to be validated with results from large randomized, controlled

 during the drug development process. Consequently, the information available to paediatricians may not always be as detailed

 that children may be receiving drugs at dosages that either lack efficacy or present safety problems. The latter in particular

 CYP2D6 genotypes caused expected differences in pharmacokinetics, but they had no pharmacodynamic consequence. CYP2D6 genotypes did not influence pain control, the adverse symptoms nausea and sedation or the risk for cognitive failure in this
 study of patients treated with oxycodone for cancer pain.

 the implementation of the guidelines alone had some effect, there was an additional effect when reimbursement measures were
 implemented. Furthermore, the practice seemed to have affected the adoption of the national CVRM guideline as the variation
 initially increased after the guideline implementation. By including information on both the proportion of simvastatin prescribed
 and interpractice variation, we achieved a more complete evaluation of how simvastatin prescription developed over time and

            <description>Discussion&amp;nbsp;&amp;nbsp;We here report a case of near-fatal isolated tramadol cardiotoxicity. Because of the inhibition of norepinephrine reuptake,
 excessive blood epinephrine levels in this CYP2D6R UM patient following excessive tramadol ingestion could explain the observed
 strong myocardial stunning. This patient admitted intermittent tramadol consumption to gain a “high” sensation. In patients
 with excessive morphinomimetic effects, levels of tramadol and its main metabolite M1could be measured, ideally combined with
 CYP2D6 genotyping, to identify individuals at risk of tramadol-related cardiotoxicity. Tramadol treatment could be optimized
 in these at-risk individuals, consequently improving patient outcome and safety.
