As part of our recent ongoing effort to give GIDEON a newer, more functional look, we’ve added Updates and Contents Tabs.
Updates
GIDEON is updated every several days and each update includes a summary of what’s new. It consists of 3 sections: Index, What’s New and Maps.

Index: Left side tree with the most current and prior updates sorted by date.
What’s New: The list of items updated with links directly to the relevant content.
Maps: A map that displays the Outbreak (red dot) and Country (blue dot) notes that have been updated. Clicking on the dots and the relevant disease links directly to the note.

Updates was formerly a link called “What’s New”. A summary automatically appears in blog posts and on twitter, so please subscribe for the latest...

GIDEON’s redesigned Microbiology module has been launched (screenshot). Following the update of the GIDEON Diagnosis module, we’ve implemented many of the new features in Microbiology, including

New tabs

Dynamic identification
Usability improvements

New tabs

The new tabs in Microbiology replace the older radio buttons and make it easier to reach each function. For each category: Bacteria, Mycobacteria and Yeasts there is easy access to Identify or Lists.
Suggestions
GIDEON’s Microbiology Compare function, until now, has ranked phenotypic tests which are most likely to impact the Identification list. Now, the top four tests which are most likely to focus and shorten the list of possible organisms are displayed and dynamically updated as each new test result...

When the humanitarian disaster in Haiti began to unfold, we accelerated development of a comprehensive book on the diseases of that country. This will be one in a series of E-books which present the status of all infectious diseases .. in all individual countries. This Ebook, Infectious Diseases of Haiti, will be offered free of charge to all Health Professionals concerned with the current disaster.
There are 347 forms of human infectious disease in the world today – and 198 of these (more than half) are known to occur in Haiti. Sadly, one of the “side effects” of living in a poor and disaster-ridden economy is a lack of useful facts regarding the local status of individual diseases.
Following the earthquake and humanitarian disaster in Haiti over the past couple of weeks has been heart wrenching. At GIDEON we’ve been working on a series of E-books and have decided to offer a promotional free copy for the Infectious Diseases of Haiti to help health practitioners in Haiti.
There are 347 forms of human infectious disease in the world today – and 198 of these (more than half) are known to occur in Haiti. Sadly, one of the side effects of living in a poor and disaster-ridden economy is a lack of detail regarding the local occurrence of individual diseases. GIDEON (Global Infectious Diseases and Epidemiology Online Network) is a Web-based project which follows the status of every individual disease – both on a global level and within each country.
A recent death in Ireland highlights the fact that the incidence of leptospirosis in this country has been increasing in recent years. In fact, during the past decade, leptospirosis rates have expanded well beyond those of those of the United Kingdom, including those for adjoining Northern Ireland.
The incidence of Hepatitis A in Australia has decreased dramatically in recent decades – from a high of 12,876 cases in 1962, to only 274 cases in 2008.
Since the 1970’s, rates of Hepatitis A in Australia have been strikingly similar to those of the United States (see graph):

Graph notes:
1.3. During 1991 to 1997, highest rates (52 per 100,000) were reported in the Northern Territory.
2. During 1991 to 2006, rates in New South Wales declined from 18.9 per 100,000 to 1.4 per 100,000.
 Individual years:
 1991 – 46.7% from New South Wales.

The following review of malaria in Italy was abstracted from Gideon.
Historical background:
- Malaria was eradicated from Sardinia during 1946 to 1950, through massive application (267 metric tons) of DDT.
- A single endemic case (Plasmodium vivax in Palma di Montechiaro, Sicily) was reported in 1956; with sporadic cases in the area of Palermo during 1962.
- The country was declared ‘malaria-free’ in 1970.
- A single case of autochthonous malaria (P. vivax) was reported in Maremma (Tuscany) in 1997 – Anopheles labranchiae was im...

In view of a massive ongoing outbreak in Manila, the following chronology of published large outbreaks (>50 cases) of leptospirosis was abstracted from Gideon. Outbreaks known to be associated with heavy rains or flooding are denoted by * A number of additional outbreaks have been associated with floods, but without specific mention of case numbers. Thus, a major outbreak in Krasnodar (Russia) in 1997 followed local flooding; as did epidemics in China during 1962 to 1963, 1966, 1973 to 1974 and 1984.

The following background data on brucellosis in Mexico are abstracted from Gideon
Time and Place:
- Highest rates are found in Queretaro, Coahuila, Guanajuanto, Nuevo Leon, Chicuahua, Jalisco, Tamaulipas and Michoacan.

LOS ANGELES, CA–(June 12, 2008) - More than one million cases of foodborne salmonella are reported each year, accounting for 9.7 percent of all foodborne illnesses and 30.6 percent of all food-related deaths, according to GIDEON Online, an online database of infectious disease information for medical professionals.
Specifically, foodborne salmonella resulted in 15,600 hospitalizations and 550 deaths, over the last year.
For more information on salmonella, Dr. Stephen Berger, MD, infectious disease expert, is available for interviews. Dr. Berger, cofounder and Medical Advisor for GIDEON, has published more than 180 articles and books, including “Introduction to Infectious Diseases” and “The Healthy Tourist.” He is currently affiliated with the Tel Aviv Medical ...

The following background information on Trypanosomiasis in Angola is abstracted from GIDEON:
Trypanosomiasis was first identified in Angola in 1871, in the Quixama region (Kwanza River area, south of Luanda).
A resurgence of disease rates during the 1980’s does not represent a general phenomenon for Africa, and appears to be related to civil unrest in the country. See graph

Trypanosomiasis is endemic to 7 of the country’s 18 provinces.
- Risk areas include the northern provinces of Bengo, Kwanza north and Kwanza south, Luanda, Malange, Uige, and Zaire.
- Widespread disease in the southern provinces of Kunene, Huambo, Namibe, and Huila was first reported in 2003.
- 2.5 to 4 million persons are considered at risk in 6,000 villages north of the Kuanza River.

“The bottom line is it’s actually nothing to worry about,” says Steve Berger, director of microbiology and tropical medicine at the Tel Aviv Medical Center. “We’re living in a sea of bacteria. The germs that you’re dealing with are normal bacteria, and nobody’s going to catch anthrax or Ebola or anything from a keyboard.”

As America once again enters the “Lyme disease season” information regarding this major infectious disease continues to evolve rapidly. The following material has been generated using GIDEON, an interactive on line program that can be used to diagnose or explore the status of all infectious diseases: signs and symptoms, status in every country, ongoing epidemics, antibiotics, vaccines, parasites, virus … in real time. So, let’s test your knowledge about Lyme disease:

True or false: Lyme disease is a relatively new infection that is primarily limited to the United States.
You have probably heard that Lyme disease is transmitted by ticks. How many tick-borne diseases occur in the world ? In the United States ?

Recent outbreaks highlight the fact that Hand, foot and mouth disease (HFM) is the second most commonly reported viral disease in Singapore. In fact, since 2001 rates of HFM have approached those of varicella. See graph
The following data regarding Enteroviral infection in Singapore are extacted from Gideon www.GideonOnline.com [primary reference available on request]
Enteroviruses are responsible for 0.2% of lower respiratory infection in the age group 0 to 20 years (1990 to 1994).
During 1996 to 1997, 44% of school children ages 5 to 12, and 50% of their mothers were found to be seropositive toward Enterovirus 71.
- Enterovirus 71 accounted for 71% of enteroviral isolates in 2000; 44.3% in 2001; 3.5% in 2002.
Epidemics of hemorrhagic conjunctivitis due to Enterovirus 70 were reported...

from Dr. Steve Berger, GIDEON’s medical advisor:

If you are planning a trip to any country, become aware of the local health situation, ongoing disease outbreaks, weather, emergency telephone numbers, availability of medicines you may be taking.
If you will be traveling to a tropical or developing country, consult a Travel Medicine Clinic for advice, vaccines and preventive medications.
Eat only well cooked foods, preferably in “clean” or “modern” facilities. Avoid eating fresh vegetables, exotic plants and animals and non-bottled water.
In areas where mosquito-borne diseases (malaria, dengue, etc) are endemic, use insect repellents and wear long-sleeved clothing appropriate to local weather.

As of 2008, mankind is confronted by 346 generic infectious diseases, distributed in a seemingly haphazard fashion across 220 countries. An average of three new diseases are described every two years - and a new infecting organism is published every week ! Over 1,600 human pathogens have been reported, each with a specific set of phenotypic, genomic and susceptibility characteristics which must be confronted by diagnostic laboratories and clinicians. The pathogens are in turn confronted by 276 generic anti-infective agents and 67 vaccines - marketed under 10,493 proprietary names.
Table 1, below, lists the major infectious diseases and pathogens which have been reported since 1972. Many conditions on this list (ie, Lyme disease, Legionellosis, Cyclosporiasis) are in fact old diseases whic...

Regarding recent outbreaks of human metapneumovirus infection in Saskatchewan, a large number of prevalence surveys have been published for newer respiratory tract viruses in Canada. The following are abstracted from the GIDEON Database
Human metapneumovirus (HMPV)
4.1 percent of adults with community-acquired pneumonia (2005 publication)
9.5 percent of respiratory specimens from the community and 25 percent of outbreaks - median age 24 months (Calgary, 2007 publication)
14.8 percent of patients with acute respiratory tract infection (4 provinces, 2001 to 2002 influenza season)
8 percent of acute lower respiratory tract infections (Montreal, 2001)
3.9 percent of pediatric inpatients and outpatients (Alberta, 2002 to 2003)

Although botulism is commonly associated with canned foods and vegetables, tomatoes are rarely implicated. A single botulism outbreak among 116 listed in the GIDEON database is ascribed to tomatoes. In fact, only three papers could be identified through a a PubMed search using the terms “tomatoes” and “botulism.” [1-3] Interestingly, two of these papers originated in Russia, the country which reported the most recent outbreak in ProMED.
One explanation for the relative rarity of botulism in tomatoes is related to the acidity of this vegetable. A lowered pH is thought to act through two mechanisms: enhanced susceptibility of C. botulinum spores to heat sterilization, and inhibition of germination of surviving spores. [4]

Quoted in ProMED:
Visceral leishmaniasis was 1st described in Brazil in 1934 and 1st reported from Rio de Janeiro state in 1977.
90 percent of cases are reported from the north (mouth of the Amazon) and eastern regions. The disease is endemic to Alagoas, Bahia, Ceara, Espirito Santo, Goias, Maranhao, Mato Grosso do Sul, Minas Gerais, Para, Paraiba, Pernambuco, Piaui, Rio Grande do Norte, Roraima and Sergipe. Urban transmission was 1st reported in 1981, from Teresina (Piaui State). A large focus extends from Roraima into Venezuela and Guyana.
The reported incidence has increased from 164 cases (35 fatal) in 1980 to 3220 (223 fatal) in 2005; see graph . A total of 44 289 cases were reported during 1980 to 2000, 39 823 of these from the northeast. Brazil accounts for over 90 percent of cases ...

Marjorie Pollack, an editor at ProMED, recently speculated about the cause of deaths in Zambia using GIDEON:
Speculation on a differential diagnosis with the symptoms given above — vomiting and backache — is a “fun” challenge. Checking the possible list of infectious agents that would produce the clinical spectrum of a cluster of severe, fatal infections associated with vomiting and backache in Zambia using the GIDEON (Global infectious disease and epidemiology network) database, comes up with a list of the following possible diagnoses (and probability of occurrence) — which includes viral, bacterial, and parasitic diseases:
Rift Valley fever (30.8 percent probability), leptospirosis (27.5 percent), influenza (2.4 percent), malaria (11.5 percent), meningitis &...

An interesting piece by Steve in ProMED, regarding tick borne diseases in the US:
Although Rocky Mountain spotted fever and Lyme disease are better known to the lay public, the list of additional tick-borne diseases has grown in recent years. The incidence of Lyme disease in the United States is greater than 5-fold that of all other tick-borne infections combined:

If we eliminate Lyme disease, we find that tularemia was the most common disease in this group until 1965, but has since been superseded by Rocky Mountain spotted fever — and more recently Ehrlichiosis:

Most significantly, although deaths due to other tick-borne diseases have declined in recent years, the number dying from tularemia has remained surprisingly constant for over 4 decades: (Source: GIDEON blog)
