            <title>Rupatadine: pharmacological profile and its use in the treatment of allergic rhinitis</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Rupatadine is a once-daily, non-sedating, selective and long-acting new drug with a strong antagonist activity towards both
 histamine H1 receptors and platelet-activating factor receptors. The use of rupatadine is indicated in adult and adolescent
 patients (&amp;gt;12 years of age) suffering from intermittent and persistent allergic rhinitis. In the treatment of these conditions,
 rupatadine is at least as effective as ebastine, cetirizine, loratadine and desloratadine. Avery good safety profile of rupatadine
 has been evidenced in various studies, including a long-term (1-year) safety study. Rupatadine does not present drug-drug
 interactions with azithromycin, fluoxetine and lorazepam, but should not be administered concomitantly with known CYP3A4 inhibitors.

            <description>Conclusion&amp;nbsp;&amp;nbsp;Elongated styloid process causing glossopharyn-geal neuralgia is beginning to be a frequently encountered clinical entity
 nowadays. Of the 28 cases of elongated styloid process who underwent unilateral/bilateral styloidectomy, 27 patients had total
 relief of symptoms. Hence, we advocate styloidectomy, following careful diagnosis of the entity with clinical and radiological
 correlation, if the patient does not respond to medical therapy. The psychological status of the patient would provide clue

            <description>Abstract&amp;nbsp;&amp;nbsp;The incidence of penetrating and lacerated neck injuries has been on the rise recently because of the increase in road traffic
 mishaps. Clothesline injuries are injuries caused by striking against a stationery object like a wire or tree limb. Most of
 the times these end up causing blunt injuries with associated fracture of the laryngeal framework. Here we report two cases
 of penetrating neck injuries with successful outcomes and also the problems encountered in the management of these cases.
 A proper evaluation, rapid airway intervention and proper surgical repair are essential for a successful outcome.

 42 ASA grades I and II patients of either sex aged 18–45 years undergoing elective middle ear surgery were randomly divided
 into two groups of 21 each. Group I received placebo bolus and infusion of saline at a rate similar to DEX in Group II. Group

            <description>Abstract&amp;nbsp;&amp;nbsp;In the practiceof rhinology opacity of maxillary antrum figures frequently and prominently in radiological reports, and often
 the case is treated as chronic maxillary sinusitis but due to presence of wide variety of conditions like anatomical factors
 and technical factors which can give rise to opacity of maxillary antrum, it is dangerous trend to think mostly in terms of
 chronic infection and subject the patient to undue repeated antral punctures or radical antral surgery. An opaque maxillary
 anturm should be treated as a sign and not as a definite diagnosis and an attempt should be made to pin point the cause of
 the maxillary sinus opacity preferably with the help of sinus endoscopy.

            <description>Abstract&amp;nbsp;&amp;nbsp;A prospective analysis of 50 patients with granular myringitis is presented Majority of the patients complained of chronic
 intermittent olorrhoea, without any hearing loss.
 
 Conservative treatment in the form of local ear drops, cauterisation of the lesion and systemic antibiotics was successful
 only temporarily. Period of relief varied from 1 to 6 month. Surgical treatment consisting of excision of myringitic patch
 and replacement of the defect by lemporalis fascia gave permanent analysis of them results and a review of the literature

            <description>Abstract&amp;nbsp;&amp;nbsp;Symptomatic congenital preauricular sinus need surgical excision. Different, approaches had been described for removal. A
 retrospective study of files of 25 patients operated in King Abdul Aziz University Hospital showed that the chance of leaving
 epithelial remnant is common when using the standard technique especially those patients who had prior abscess or surgical
 intervention. The supraauricular approach described by Prasad, offers a better chance for total excision of the sinus and
 its tract.

 immunoglobulin pattern. Diagnosis was clinched by various investigations like ERS. Mantoux test, Chest X ray etc and fine
 needle aspiration cytology or open biopsy tmd thus catgorised to three groups e.g. tuberculous lymphadenitis (Group I) — 64
 cases; chronic nonspecific lymphadenitis (Group II) — 18 cases; and 6 cases, of submandibular and submental salivary gland
 lesions (Grtmp III) which served as control.
 
 Clinical presentations were mainly cervical lymphadenopathy associated with fever, loss of weight, malaise and lethergy. ESR

 in the specimen. This paper describes 78 such cases undergoing surgicad ablation. Uses of CT and modifications of standard

            <title>Role of placental implantation in primary atrophic rhinitis</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Twenty five cases of primary atrophic rhinitis were the subjects of this study. All possible investigations were made to exclude
 syphilis, tuberculosis, paranasal sinus infection etc. The disease was confirmed by biopsy and histopathological findings.
 The age of the patients varied from 14 to 41 years with the maximum incidents between 18 to 30 years. Seventy percent of the
 patients were female. Placental graft was done bilaterally in each case through a sublabial incision and elivation of the
 mucoperiosteum in the floor and lateral wall of the nose. After a follow up period of three months the improvement of symptoms
 and signs were noticed in 88% of 22 patients Three patients were lost to follow up after three months.

 other tests because the stimuli simulates normal head rotation. It is more sensitive (70%) and specific (90%) than alternate
 test specially in the central vestibular disorders. The central disinhibition and compensation inherent in the recovery of
 vestibular lesion makes the alternate test of limited usefulness.

            <title>Transglottic carcinoma with intracranial metastases</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;A sixty year old patient underwent radical Radiotherapy for a transglottic laryngeal carcinoma and the primary site was free
 of disease 6 weeks later. The initial work up showed a clear roentgenogram of the chest and the vertebral architecture was
 normal. The patient had neurological symptoms few months later and a diagnostic CT Scan of brain showed intracranial metastases.
 Locally he was disease free as in the chest. After a palliative course of radiotherapy of the brain, the patient expired.
 The probable route of spread was discussed.

            <description>Abstract&amp;nbsp;&amp;nbsp;Primary mulignant melanomas are rare tumours in the head and neck. The pigmentation of ths mucous membrane with or without
 the pigmentation of skin around it should raise the suspicion and should be confirmed by pathologist from a fresh biopsy specimen
 Radical surgery is the preferred treatment followed by radiotherapy in case of doubt regarding radicality of the surgical
 procedure. Six cases of malignant melanoma of nose and paranasal sinuses who reported within fourteen months are presented.

            <description>Summary&amp;nbsp;&amp;nbsp;An unusual case of haemophilia in an elderly man with extensive intralingual and pharyngeal haemorrhage is presented. The
 patient made uneventfull recovery after tracheostomy and routine emperical treatment. The peculiar behaviour of the disease
 with haemorrhage in certain areas of throat leaving other sites and the probable reasons have been discussed. The role of
 tracheostomy in a hamophilic patient is considerded.

            <description>Abstract&amp;nbsp;&amp;nbsp;Fifty patients of carcinoma of the oropharynx and Hypopharynx who had received a full dose of radiotherapy were followed up
 using laryngoscopic fine needle aspiration technique. The results were compared with those of surface biopsy. 21 cases (43.75%)
 were diagnosed to be harbouring cancer cells using the aspiration method while only 14 cases (29.16%) showed a positive surface
 biopsy.

            <description>This study deals with our experience of 22 laryngectomised cases who had undergone surgical voice restoration by Tracheo-Oesophageal
 Puncture (TEP) procedure and Blom-Singer prosthesis, with follow up ranging from 4 months to 2 years.
 
 A comprehensive team approach pre-operative videofluroscopic assessment, patient counselling and modified TEP technique were
 responsible for a better long term success rate in our series. Indigenised, mycostatin impregnated silicone one way valve
 prosthesis, produced by us, alleviated need for costly imported prosthesis. Comparative evaluation of both prosthesis, proved
 efficacy of our prosthesis beyond doubts.

            <description>Abstract&amp;nbsp;&amp;nbsp;An objective assessment of audiovestibular functions in patients with essential hypertensions has been done. We found that
 uncomplicated hypertension did not affect hearing at low frequencies. In addition, history of vertigo was present in mild
 to moderate hypertension. ENG parameters of MSS, total duration and total amplitude showed more active responses following
 caloric stimulations in hypertensive subjects as compared to normals.

            <title>Metastatic renal cell carcinoma presenting with epistaxis</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Renal cell carcinoma is a slow growing malignancy of the kidney that has a high propensity for metastasis. It is the most
 common metastasis reported in the paranasal sinuses. A fifty six year old male patient, an operated case of left renal cell
 carcinoma presented seven years later with epistaxis. Computerized tomography of the paranasal sinuses and of the abdomen
 showed a metastatic lesion involving the left nasosinus region and a lesion involving the right kidney with lung metastasis,
 respectively. Due to the poor prognosis palliative radiotherapy was given.

            <description>Abstract&amp;nbsp;&amp;nbsp;Inflammatory pseudotumour (IPT), also known as inflammatory myofibroblastic tumour, is a rare cause of benign cervical lymphadenopathy.
 In its acute clinical presentation it mimics malignant and infective causes of cervical lymphadenopathy. The combined clinical
 history, histopathological findings, and imaging features of an infiltrating soft-tissue mass should lead to the possibility
 of IPT. Radical surgery should be avoided before there is histological proof of a malignant tumor.

            <description>Abstract&amp;nbsp;&amp;nbsp;Laryngeal cysts, particularly cyst vallecula are generally benign lesions which cause mild dysphagia or change in voice. We
 report here a case of cyst vallecula that caused soreness of throat, dysphagia and change in voice. A 22-year male presented
 with progressive dysphagia and muffled voice secondary to a large cystic mass involving entire vallecula and during coughing
 presented in oral cavity. Per oral marsupilization done and the problem resolved.

            <description>Abstract&amp;nbsp;&amp;nbsp;Schwannomas arising from the supraglottis are uncommon, if not rare. They are known to arise from the head and neck region
 but oropharyngeal presentation is not often seen. Few cases of pedunculated schwannoma arising from the aryepiglottic fold
 presenting as giant tumours in the oropharynx have been reported so far. The tumour in this case has been excised via intraoral
 approach, with nil residual morbidity. A follow up period of one year was uneventful without any recurrence.

            <title>A case study of Plummer-Vinson syndrome</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;The case series presents the pattern of association of different co-morbid conditions of Plummer Vinson’s Syndrome. The commonest
 morbidity is web at post cricoid level. The symptoms of dysphagia are more common and there appears to be a female preponderance.
 The presenting age us usually 5th decade. Although association with post cricoid carcinoma has been reported but only 10%
 of cases in the present series had carcinoma at the time of presentation. The effect of iron and B-complex supplementation
 has been clinically appraised and good symptomatic relief has been seen. Maybe this measure can prevent the progression to
 malignancy. Bouginage was effective in relieving symptoms in all the cases it was tried

            <description>Abstract&amp;nbsp;&amp;nbsp;Cancrum Oris is acute gangrenous stomatitis of oral and para-oral structures in age group between 2 to 16 years particularly
 in sub-Saharan Africa with high morbidity and mortality. Exact bacteriology is uncertain. Acute necrotising gingivitis and
 oral herpetic ulcers are considered to be antecedent lesions. The treatment of cancrum oris is penicillin, tetracycline, metronidazol
 and clindamycin. second step of management includes reconstructive surgery for the treatment of the functional and cosmetic
 problems. The authors are reporting a case of cancrum oris in a 1.5-year-old child.

 rare combination with diptheria which often calls for tracheostomy in children. Various factors of decannulation panic are
 discussed. Where there is no obvious obstruction such as the original condition. Granulation tissue and scar tissue, odema,
 thick mucus, crusts, tracheomalacia and displacement of tracheal flap, a non radioopaque F.B. causing incomplete obstruction
 should be thought of.

 case fatied to maintain his speech subsequently. Further surgery might help this unfortunate patient but he refused it. The
 phonetic stream is effectively diverted through the dermal tube into the oral cavity to be articulated. Speech of better quality
 is developed which is not monotonous and easily understandable. Moreover, the associated expiratory snort and the gulping
 noise that are quite often present in pharyngo-oesophageal speech are conspicuously absent in this laryngoplasty which is

            <title>Benign tumours of larynx</title>

            <description>Summary&amp;nbsp;&amp;nbsp;50 cases of benign tumours of the larynx in whom surgical excision was done form the basis of this study. It has been emphasized
 that the tumour should be removed under general anaesthesia with complete relaxation. The scalping of the tumour should be
 flushed with the surface of vocal cord avoiding any injury to the epithelium of the vocal cord. It has been observed that
 true benign tomours of larynx are whereas the commonumours of the larynx met with are the pseudo tumours.

            <description>Conclusions&amp;nbsp;&amp;nbsp;Thus it appears from our studies that serial section offers a good tool for assessing the biological nature of laryngeal cancer
 as well as radiation reaction. It gives an idea of spread of cancer in all three dimensions which helps immensely in planning
 the management. Non involvement of conus elasticus, intralaryngeal muscles, the submucous glands and cartilages suggests a
 good prognosis. Pre-epiglottic space was found to be invaded by tumour from anterior commissural region and supraglottis.
 By serial section one is able to have a good idea about the immune response that is playing a part. Stage of cell differentiation
 of the growth is not absolutely parallel to the invasiveness of the growth. In addition, radiation reactions can be assessed.

            <title>Hypopharayngeal carcinoma in a child: Case report</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;A 15 years old boy with keratinizing squamous cell carcinoma of the Hypopharynx is being presented because of its rarity.
 The patient presented with pain in throat and difficulty in swallowing since 15 days. Barium swallow and direct Laryngoscopy
 revealed a mass in Hypopharynx. Tobacco smoking since early childhood can be the aetological factor. The patient did not consent
 for surgery and could not afford radiotherapy. Hence, Tracheostomy and feeding gastrosomy were done. Patient died at home
 two months after the diagnosis.

            <description>Abstract&amp;nbsp;&amp;nbsp;Osteoma of the mastoid is a rare, slow-growing and benign tumour. Normally it is asymptomatic, although it may cause cosmetic
 deformities most of the time. A case of osteoma of the mastoid in a 15 years old boy is reported here. Surgical removal was
 done for the cosmetic deformity. There was no recurrence during the two years follow-up period.

 as Ortner’s syndrome or Cardiovocal syndrome. This was first described by Ortner in 1897 to describe left recurrent laryngeal
 nerve palsy secondary to Mitral Stenosis. The common conditions which gave rise to Ortners’ syndrome include mitral stenosis,
 mitral valve prolapse, aortic aneurysm, septal defect, following cardiothoracic surgery, high altitude pulmonary hypertension,

            <title>Spindle cell sarcoma of larynx</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Spindle cell carcinoma of larynx is rare tumors constituting only 0.6% of laryngeal neoplasm with predominance in elderly
 males (average 63 yrs). These tumors were described under various names: pseudosarcoma, Carcinosarcoma, fibromyxosarcoma,
 mixed sarcoma, polypoidal sarcoma. Immunohistochemistry and electron microscopy suggest that spindle cell carcinoma are true
 neoplasm and spindle cells are part of the neoplasm and not benign reactive fibroblast.

            <description>We report a case of Klippel-Trenaunay-Weber Syndrome who presented with craniofacial deformity leading
 to deviated nasal septum, nasal obstruction and intermittent nasal bleed. These patients can have involvement of oral cavity
 and nasal mucosa with angiomatous malformation, which can give rise to epistaxis and excessive bleeding during oral and nasal
 surgeries. We discuss the review and management of such cases from Otolaryngologists point of view.

            <description>We report the case of a middle aged
 man who presented with a 6-months history of progressive nasal obstruction, watering of the eye, headache and occasional epistaxis.
 A diagnosis of esthesioneuroblastoma was established by histopathology and confirmed by immunohistochemistry. On staging the
 mass was classified as a kadish stage C tumor. The mass was excised via a craniofacial resection through a bi-coronal approach
 and the tumor mass was peeled away completely from the frontal lobe dura. The patient underwent postoperative radiation, and
 he was free of recurrence at follow-up 3 months later.

 How many of us advise the patient that mast cell stabilisers (Cromolyn) should be used before the exposure to the allergen.
 Similarly most of the time we forget to tell the patients when to stop the use of the nasal drops, resulting in overuse and
 few cases of rhinitis medicamentosa. It is the intention of this communication to discuss the pharmacological basis of the
 use of commonly used nasal drugs with the fond hope that it will improve our understanding and help in better patient care.

            <title>Late onset facial nerve paralysis after fracture of the skull base</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Late onset facial nerve paralysis caused by post-traumatic cholesteatoma is a rare but serious complication of fracture of
 the skull base. Unless careful follow-up is maintained patient may come with extensive disease even many years later. Since
 post-traumatic cholesteatoma is invasive and occurs in a well pneumatised mastoid it is large and extensive. In our patient
 it involved the labyrinth and caused a permanent facial nerve paralysis. Surgical management involves eradicating the cholesteatoma
 with a canal wall down technique.

            <title>Lactic dehydnogenase activity in head and neck cancer</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Serum and tissue lactic dehydrogenase (LDH) levels were estimated in patients with head and neck cancer. The levels were found
 to be significantly higher both in serum as well as tissue of these patients. The rise was nearly to the same extent in all
 the patients irrespective of the site, character, histopathology or stage of the disease. Further after, radiotherapy the
 levels remained unaltered. The rise in LDH levels in patients with head and neck cancer suggests increased rate of glycolysis
 in the tissue and that the rise in serm could be due to its release from the tissue.

            <description>Abstract&amp;nbsp;&amp;nbsp;Previous electron microscopic work (Ahluwalia, 1992) had demonstrated that there is no fungus in rhinosporidiosis. The so
 called fungal sporangium was shown to be a unique body organized in host tissue for elimination of two indigestible carbohydrates,
 starch (possibly from tapioca) and defective proteoglycans (precursor of mucous). Case histories of two patients of rhinosporidiosis
 operated in this institute indicated involvement of dietary tapioca and chronic inflammation.
 
 Therefore, tapioca samples were procured for laboratory tests from Orissa and Trivandrum, home states of the two patients.

 magnetic resonance imaging) delays between presentation and diagnosis were common place. Atypical facial paralysis and hearing
 loss are the most common presenting features. This combination is very suggestive of a facial neuroma and is an indication
 to proceed to computed tomography. This investigation can reveal expansion of the Fallopian canal at any point through the

            <title>Lipid profile studies in inner ear dysfunction</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;In the present paper fifty patients with symptoms of inner ear dysfunction, primarily deafness, vertigo or both, were subjected
 to a detailed history, pure tone audiometry and caloric tests. A detailed serum lipid analysis was done of all the patients.
 
 It was concluded that atherosclerotic disease of the labyrinthine vessels usually affects patients above forty years of age.
 It occurs bilaterally. Commonly both the cochlear and vestibular components are involved. Raised serum total cholesterol,
 L.D.L. cholesterol levels and hypertension, are more contributory as risk factors in its development.

            <title>Prevention of laryngeal stenosis in Co2 laser management of recurrent respiratory papillomatosis</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Laryngeal stenosis is one of the common complications in the management of recurrent respiratory papillomatosis. The authors
 have compared the results with regard to development of laryngeal stenosis, when surgical treatment was given with CO2 laser alone in the early 34 cases plus 7 advanced cases, while in the remaining 13 advanced cases the CO2 laser ablation was combined with the conventional cup forceps removal. Age, extent of disease, duration of the procedure
 and interval between two procedures were also noted. The incidence of laryngeal stenosis was found to be reduced in advanced
 cases when CO2 laser was combined with the conventional cup forceps removal.

            <title>Use of pericranial flap for reconstruction after craniofacial surgery</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Total resection of lesions involving the anterior cranial base results in defect of variable size, leaving a communication
 between the nasal cavity and extracranial space. Potential complications of this include meningitis and CSF leaks. Repair
 by an inferiorly based pericranial flap was performed in 10 patients in each of whom the defect exceeded 1.5 cms in width.
 In 4 of them the repair was supplemented by a temporalis fascia graft for achieving a dural seal. In 8 other patients no repair
 was considered necessary. Use of this flap did not result in any complication.
