 period of time to treat certain types of anemias such as, that caused by β-thalassemia, sickle cell disease and myelodysplastic
 syndrome. Deferoxamine, which has been used since four decades as an iron chelator has limited efficacy due to its demanding
 therapeutic regimen, leading to poor compliance. Deferasirox, once daily oral iron chelator provides an effective alternative

 syndromes is discussed in this article as a prototype to know about genetics of malformations: how it is studied and what
 is known. Genetic and chromosomal defects are often associated with congenital malformations. Polydactyly is one of the commonly
 seen malformations and genetic defects of many malformation syndromes associated with polydactyly are known. The role of genetic

            <title>Congenital Chylothorax - Successful management with chemical pleurodesis</title>

            <description>We report a case of congenital bilateral chylothorax presenting as non immune
 hydrops and managed successfully with chemical pleurodesis. A term male baby presented at birth with bilateral pleural effusions
 and subcutaneous edema. It was initially managed with ventilation and intercostals drainage (ICD). After the initiation of
 feeds, re-accumulation of pleural fluid led to the diagnosis of congenital chylothorax. Management with ICD and octreotide
 was unsuccessful but responded to chemical pleurodesis with 4% povidine iodine done on 3 separate occasions.

            <title>Osseous involvement in pediatric Hodgkin’s lymphoma</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Presented here is a report of 3 children, out of 66 total pediatric Hodgkin’s lymphoma (HL) patients (4.5%) with bone involvement
 over a 3 and a half year period. Two patients presented with osseous lesions at the time of relapse and one had concurrent
 evidence of non-osseous disease. The clinical presentation, radiographic findings, histology, treatment and outcome of these
 patients are discussed. Boys and girls are nearly equally affected, local bone pain is the commonest symptom and B-symptoms
 are common. Vertebrae and pelvis are most frequently involved, commonly with an osteolytic picture. Nodular sclerosis is the
 predominant histological subtype. Most children received combined modality treatment and the overall response and survival
 are good.

            <description>Abstract&amp;nbsp;&amp;nbsp;Fulminant hepatic failure due to dengue infection is rare, although mild liver dysfunction is common. Here we report a fatal
 case of fulminant hepatitis in an infant infected with dengue 3 serotype. Attention must be given to the use of hepatotoxic
 drugs in some cases of dengue especially in infants.

            <title>Head injuries in children resulting from the fall of television</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Case records of children admitted with head injury due to TV fall in a pediatric tertiary care hospital in Chennai, during
 March 2007–February 2008 were analysed retrospectively. Out of the 11 children admitted 6 (54%) were in 1–2 yr age group.
 Bleeding from the ear, nose and throat was the commonest, finding followed by a skull fracture, seen on imaging studies. These
 were observed in 9 (81.81%) and 8(72.8%) children respectively. There was no mortality.

            <title>Morbidity among infants in South India - A longitudinal study</title>

 was undertaken in northern part of Karnataka state in India. Birth cohort consisted of all the children born during first
 six months of the study period. They were assessed at the time of enrollment and monthly follow up was done till they attained
 one yr of age. Out of the 194 newborns, 46.4% were boys and 53.6% were girls. 24.8% of newborns were of low birth weight and
 5.1% were preterm. Four (2.1%) had congenital anomalies and 2.5% developed birth asphyxia. Diarrhea (10.8%) and skin diseases
 (8.2%) were the commonest morbidities in the neonatal period. The incidence of morbidity was 3.28 per infant per yr. It was

 was 8.54 +/− 0.33 g% whereas in WGT period was 9.13 +/− 0.14 g% (p &amp;lt; 0.001). The mean blood transfused as packed cells in
 pre WGT period was 326.82 +/− 74.10ml/kg/year whereas during WGT period it was 256.39 +/− 45.47 ml/kg/year. The percentage

            <title>Calculating prevalence of hepatitis B in India: Using population weights to look for publication bias in conventional meta-analysis</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Publication bias can result from the propensity of researchers to document what is unusual. This can distort the inferences
 drawn in systematic reviews. To measure the distortion, it has been suggested that a second analysis be done; using weights
 proportional to the size of the population from which the samples are drawn. We re-evaluate data from a published meta-analysis
 on prevalence of hepatitis B in India, to see how this approach alters the results. Prevalence of hepatitis B among tribal
 and non-tribal populations in different States was analyzed. Weights were then assigned according to population of the State.

            <description>We present a very unusual case of a baby born with antenatally diagnosed large neck swelling causing compression of trachea
 on imaging, who was euthyroid. There was no history of any thyroid disease in the mother and no history of exposure to known
 goitrogens. The goiter gradually reduced in size and baby’s thyroid function as well as neurodevelopment remained completely
 normal during follow-up.

            <description>Abstract&amp;nbsp;&amp;nbsp;Dyskeratosis congenita - X-linked variety was diagnosed in a twelve year old male child with cutaneous pigmentary changes
 and dystrophic changes in nails of hands and feet. His elder brother had similar nail changes and had died at twelve yr of
 age. We demonstrated the A353V mutation in the proband after sequencing the DKC1 gene. The mother was found to be carrier
 for the same mutation. She did not have any clinical manifestations. This is the commonest mutation worldwide responsible
 for X-linked variety of this disease and has been demonstrated for the first time in an native Indian patient.

 age, body weight, length / height, upper thigh (femur) length, head circumference and chest circumference. Mean kidney is
 length best correlated with height, followed by upper thigh (femur) length and chest circumference.

            <description>We report a case of transient hyperinsulinism with hypocortisolemia in a preterm small for date neonate with recurrent hypoglycemia
 and perinatal stress. Investigation revealed increased Insulin level with subnormal cortisol level even during hypoglycemia.
 Hypoglycemia resolved by its own by 22nd day of life with normal insulin and cortisol level thereafter.

            <title>Spontaneous resolution of marked dilatation of cerebral duro-venous system in newborn presenting with fetal hydrops</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;A newborn with antenatal diagnosis of fetal hydrops at 36 wk of gestation, presented with congestive heart failure (CHF) and
 generalized edema. Computed tomographic angiography showed marked dilatation of cerebral duro-venous system including vein
 of Galen (VOG), straight sinus, torcula and transverse sinus without evidence of arteriovenous fistulae at the vein of Galen.
 Dilatation of duro-venous system resolved with concomitant improvement in biventricular function and CHF with decongestive
 therapy. Such entity should be differentiated from more serious conditions like VOG malformation and venous sinus thrombosis.

            <title>Protein C and Protein S: Causative factor for developing a hemorrhagic infarct in a HbE/Beta thalassemia child</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;A 10-yr-old HbE/Beta thalassemia child who developed subacute to chronic occipitotemporal hemorrhagic infarct with smaller
 chronic infarct with gliotic changes in the left frontal periventricular white matter. Genetic tests showed that patient was
 positive for HbE and IVS1-5 mutation and was negative for thrombogenic mutations. Hemorrhagic infarct was confirmed by magnetic
 resonance imaging study. Antigenic levels of Protein C and Protein S were low. Based on these outcomes, it was concluded that
 Protein C and Protein S deficiency were the causative factor for developing hemmoragic infarct in the HbE/ Beta thalassemia
 patient.

            <title>Inflicted neuro-trauma in infancy</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Two infants with non-accidental inflicted neuro-trauma are reported. One presented with sudden onset lethargy, respiratory
 difficulty and unexplained seizures. There were bilateral retinal bleeds and extradural hemmorage. Other was a well thriving
 child who had 2 seizures and was noted to lack visual fixation. Retinal hemorrhages and chronic subdural and intraparenchymal
 hemorrhages were subsequently discovered. We highlight the importance of suspecting child abuse in infants with sudden unexplained
 unresponsiveness, seizures or respiratory difficulty and the unusual occurrence of extradual hemorrhage.

 this syndrome with X and Y polygamy are of rare occurrence. Here we describe a rare case of 48, XXXY Klinefelter’s variant
 from South India with a reported incidence of 1 per 17,000 to 1 per 50,000 male births. The presence of an extra X chromosome/s
 in these individuals has a great impact on the physical and cognitive functions, which could be attributed to gene dosage

            <description>We present a preterm neonate who developed congenital rubella syndrome in a mother who had rubella at 5 months of gestation.
 An amniocentesis was done in the mother at that time but amniotic fluid rubella PCR was negative. Thus, inspite of prenatal
 screening, one cannot definitely conclude absence of perinatal transmission of rubella.

            <description>We report a 30-day-old baby with subcutaneous fat necrosis and symptomatic hypercalcemia, who developed metastatic calcification
 in the subcutaneous tissue, kidneys, pericardium and brain. The baby also had anemia, hypertriglyceridemia and hypercholesterolemia.
 He was managed with intravenous saline, furosemide, oral steroids and bisphosphonates and improved with treatment.

            <description>We report a case of a two and a half yr old boy who presented with complaint of bilateral asymmetrical breast enlargement
 since infancy. On examination, he had features of neurofibromatosis type 1 (NF1). Complete endocrinological evaluation was
 normal. Trucut biopsy of the breast revealed overgrowth of fibrocollagenous and adipose tissue without hyperplasia of breast
 parenchyma. Thus a diagnosis of NF1 with pseudogynecomastia was made.

            <description>We describe the case of a child suffering from congenital cyanotic heart disease — double outlet right ventricle (DORV) with
 transposition of great vessels (TOGV). She underwent a left Blalock-Tausig (BT) shunt at one month of age followed by a Glen
 procedure with left pulmonary artery augmentation at six months. Following the second procedure she developed extensive cyst
 formation in the upper lobe of the left lung and pneumothorax. She was managed by intercostal drainage of the pneumothorax.

 was present in 18%. In 2 patients, pheochromocytoma was part of VHL and in one it was associated with MEN 2. Twenty four hour
 urine VMA level was elevated in 100% and metanephrine level in 73%. CT/ MRI were showing the tumor in all. Prazosin extended
 release tablets (maximum 30 mg/day) were used in 73% and doxazosin (maximum 12 mg/ day) in 27%. Intraoperative BP fluctuations

            <description>We report a child with acute lymphoblastic leukemia who developed primary cutaneous mucormycosis at the site of lumbar puncture
 during induction chemotherapy. Though high mortality rates are reported with invasive mucormycosis, prompt biopsy, early identification
 and antifungal therapy using a combination regime of amphotericin-B and rifampicin along with extensive surgical debridement

            <description>We report a male neonate who had liver abscess that resolved with intravenous antibiotics and surgical drainage. However,
 the child developed complete thrombosis of portal vein with cavernous formation within 16 days of therapy and portal hypertension
 subsequently. The child is now 2½ years and has extra hepatic portal hypertension but is otherwise asymptomatic.

            <title>Treatment of chronic immune thrombocytopenic purpura with rituximab in children</title>

            <description>Conclusion&amp;nbsp;&amp;nbsp;CR was found in 20% of our patients, PR in 10% and MR in 30% with rituximab. On this treatment, while some series had good
 outcomes with this treatment (72%–100%, remission ratios), but many series, such as ours, had a poor response rate contrast
 to many reported case series in the literature. This condition may be associated with the age of our most patients who were
 young at the time of commenced rituximab. However, we believe that more studies are required to elucidate the reasons for

 difference of 3 years between the age of presentation and the age of diagnosis in Group 2. Out of the thirteen patients in

            <title>Diabetic ketoacidosis, thyroiditis and alopecia areata in a child with down syndrome</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;A 10-year-old girl with Down syndrome and alopecia areata was admitted with severe ketoacidosis. She had high blood glucose
 level: 615 mg/dL and her HbAlc level was 13.3 %. After the control of ketoacidosis and reduction of the plasma glucose level,
 we found low free T4 level and high TSH levels. Thyroid peroxidase antibodies titer was 1383 IU/mL and both gliadin and endomysial
 antibodies were negative. This was the first report of a child with Down syndrome showing type 1 diabetes combined with alopecia
 areata and autoimmune thyroid disease.

            <description>We report a case of familial small supernumerary marker chromosome (sSMC)in a child with translocation Down syndrome (DS)and
 mother. The GTG-banded chromosomal analysis of DS child revealed 47,XY,+21,+mar and mother karyotype was 47,XX,+mar. The GTG-banded
 sSMC had a similar morphology of small acrocentric chromosomes. Fluorescence in situ hybridization (FISH)evaluation of sSMC using centromere probes(13/21,14/22,22)confirmed sSMC as derivative chromosome 14.

            <description>We report a newborn, who had the congenital anomalies including protuberance on the right lower leg, bilateral equinovarus
 deformity of the feet, genu valgum with knee flexion deformity, syndactyly between the first and the second digit on the right,
 in addition with the absence of the fifth digit. Echocardiography revealed a secundum type atrial septal defect. The combination
 of these congenital defects associated with developmental anomalies of lower extremities. We discuss the clinical, radiological
 findings and pathogenesis of this lower extremity malformation.

 mouth giving a ‘whistling’ appearance. Other dysmorphic features include deep set eyes, broad nasal bridge, long philtrum
 and ‘H’ shaped cutaneous dimple on the chin. There was congenital windmill vane hand position and severe talipes equinovarus
 deformity. The above features are characteristic of Freeman-Sheldon syndrome also known as Whistling Face syndrome. Ultrasound

            <title>Transient neonatal diabetes due to activating mutation in the ABCC8 gene encoding SUR1</title>

            <description>We report a 2 month male child presenting with diabetic ketoacidosis (DKA) and seizures treated with intravenous fluids and
 intravenous insulin infusion till the ketoacidosis was reversed, thereafter responding well to sulphonylureas and at age of
 13 months going into complete remission. At age of 11 months developmental delay in the form of negative neck holding and
 inability to sit without support was seen. The child is 3 years of age now, euglycemic without any insulin or oral hypoglycemic
 agents but has severe developmental delay. Genetic analysis was negative for mutations of KCNJ11, 6q24, Glucokinase and IPF-1
 genes. A mutation R1183W was found in the ABCC8 gene encoding SUR1, which was the cause of neonatal diabetes in this case.

            <description>Summary&amp;nbsp;&amp;nbsp;The total haemoglobin, foetal haemo-blobin and nucleated red blood cells were analysed in 34 infants born of toxaemic pregnancies
 and 58 healthy newborns. In the healthy infants, the respective mean values were 16.03 G.% 64.57% and 4.1/100 W.B.C. The corresponding
 figures for infants born of toxaemic mothers were 16.44 G.%, 79.85 % and 7.1/100 W.B.G. These differences were observed irrespective
 of gestational age, birth weight and crown to heel length. The total haemoglobin and foetal haemoglobin were higher in newborns
 of anaemic mothers and normal pregnancy in comparison to newborns of mothers with haemoglobin more than 7 G.%. The total haemoglobin
 and foetal haemoglobin were higher in small-for-date newborns in comparison to those appropriate-for-date.

            <description>Summary&amp;nbsp;&amp;nbsp;110 cases of tetanus, excluding neonatal tetanus, were studied with reference to aetiology, clinical manifestations, prognosis
 and evaluation of an effective combination of drugs. Most of the children belonged to rural (92.8%) areas, with unhygienic
 environmental conditions and a poor socioeconomic status. Otogenic tetanus was the commonest followed by idiopathic tetanus
 (22.6%) and traumatic tetanus (14%). Lockjaw, rigidity, spasm and fever were the most common presenting symptoms. Phenobarbitone
 and diazepam was the best combination of drugs for the treatment of tetanus. The overall mortality rate was 17.1%. This could
 be due to the drugs used in the present study. The incubation period, period of onset and fever had an inverse relationship

            <description>We report a rare case of disseminated protothecosis in a 10-year-old boy with combined immunodeficiency, hitherto unreported
 from India. Even though it is difficult to diagnose clinically, observation of the sporangiospores within the sporangium in
 culture gives the accurate laboratory identification of Prototheca spp. In this patient, failure to eradicate the infection
 with amphotericin B and recurrence with olecranon bursitis along with skin lesions and splenomegaly was observed. Disseminated
 protothecosis in a child with combined immunodeficiency and failure to eradicate the infection with amphotericin B is reported.

            <title>Even a single third trimester antenatal fetal screening for congenital anomalies can be life saving</title>

            <description>We report two cases of malrotation with extremely different outcomes based on the availability and non-availability of antenatal
 scanning. The first one had a single third trimester ultrasound which detected malrotation and helped in saving baby’s life
 whereas the second baby without antenatal screening succumbed to midgut volvulus and its sequelae because of delayed presentation.
 In developing countries where most of the pregnancies may go unsupervised, a single third trimester scan for fetal anomalies
 by an experienced sonologist, in areas where routine antenatal screening program has failed or not feasible can help save
 the baby’s life at least in surgically correctable anomalies. This may be an important strategy to reduce the infant mortality
 rate in the country.

            <description>Abstract&amp;nbsp;&amp;nbsp;A 15-mth-old male child of consanguineous parents, presented with classical features of congenital hypothyroidism. Serum total
 thyroxine (T4), total triiodothyronine (T3) and TSH were low. There was no evidence of deficiency of other pituitary hormones.
 Magnetic resonance imaging of the pituitary was normal. TSHB gene sequencing revealed a homozygous missense mutation due to
 single base substitution G?A at codon 85 resulting in change from Glycine to Arginine. This mutation in TSHB gene has been
 reported earlier in three cases with similar phenotype from Japan.

            <title>Diagnostic dilemmas, course, management and prognosis of traumatic lung cysts in children</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;The patients were 8 and 9 years old respectively. Both were passengers in a vehicle and suffered multisystem injuries. Case
 no.1 suffered a fractured occipital bone, lung contusions and a small pneumothorax in addition to the traumatic lung cysts
 in the left lung. Case 2 sustained contused and lacerated liver and right lung cysts. CT examination showed cystic areas in
 the lung which were diagnostic in case 2. In case 1 a traumatic rupture of diaphragm could not be ruled out and the patient
 underwent an exploratory laparotomy to deal with the same. The lung cysts in both the patients were treated conservatively
 and both showed resolution in repeat CT scans done at 6 months.

            <title>Vincristine induced neurotoxicity in cancer patients</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Ten out of 20 children, treated with usual doses of vincristine for various types of childhood cancers, developed neurotoxicity
 during treatment. Peripheral neurotoxicity (mixed motor-sensory 4/10, pure motor 3/10, pure sensory 3/10) was seen in the
 form of weakness of lower limbs, areflexia, neuropathic pain, or sensory loss. Autonomic neuropathy presented as constipation
 and urinary retention in 2 children, while 2 children developed encephalopathy in form of seizures, confusion, aphasia, and
 transient blindness. In children with severe neuropathy, vincristine administration was withheld/dose reduced till clinical

 of a known case of Hemophilia A presented with recurrent, spontaneously occurring echymotic spots having prolonged PT, APTT,

            <title>Clinical profile of adrenoleukodysrophy</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;X-linked Adrenoleukodystrophy (ALD) is the most common of the peroxisomal disorder and is associated with functional defect
 of the very long chain fatty acid (VLCFA) oxidation leading to the accumulation of VLCFA in the white matter and adrenal cortex.
 Retrospective evaluation of medical records of ALD patients were carried out. In all the 5 patients the duration of the symptoms
 varied from 1–7 years. Most of them presented with Addisonian crisis (4/5) and hyperpigmentation (5/5), white half of them
 (3/5) had neurological symptoms. All patients had biochemical evidence of the adrenal insufficiency. All siblings of patients
 should be screened for the possibility of ALD with VLCFA.
