CONCLUSIONS: Discrepancies between spousal reports were less significant in Turkey than in most developing countries with DHS data, but the differences were not inconsequential to explaining how spousal attitudes and preferences influence contraceptive use. No evidence was found associating interspousal power differentials with method use. Further research is needed to improve the testing and modeling of such dyadic processes.

CONCLUSIONS: Because approval of family planning and discussion of family planning with partners were shown to be the factors most strongly associated with modern contraceptive use in the multilevel model, programs that seek to increase individual approval and those that teach communication between partners could be particularly helpful to increasing contraceptive use in Mali.

Authors: Gribble JN, Sharma S, Menotti EP
Although governments develop family planning policies to guide program design and implementation, these policies can have both intended and unintended consequences. As a result, policies may need periodic revision to achieve the desired outcomes. Over the last two decades, the government of Peru has instituted a series of laws and policies designed to enhance access to family planning services and commodities. In practice, these policies have not always had their desired effect. This paper examines the policies Peru's Ministry of Health has developed and implemented to promote access to family planning for all, and how those policies have affected contraceptive prevalence, method mix and source mix.

Authors: Vernon R, Solórzano J, Muñoz B
The use of contraceptive methods in Guatemala has rapidly increased over the past two decades, from 23% of married or cohabiting women aged 15-49 in 1987 to 43% in 2002. The most frequently used method in the country has been female sterilization, used by 11% of these women in 1987 and by 17% in 2002. In contrast, fewer than 1% of women reported that their partner had received a vasectomy.1,2 From a public health perspective, it would be desirable for a greater proportion of couples to adopt vasectomy rather than female sterilization. Vasectomy has a lower rate of postoperative complications than female sterilization, and the client's recovery time is shorter; it is also a less expensive procedure for the providing institution.
CONCLUSIONS: HIV/AIDS education and prevention programs should consider more carefully how gender and race may intersect to influence risk perceptions and risk behaviors. In addition, possible reciprocal relationships between risk behaviors and risk perceptions should be considered in education and intervention programs.

In a seminal 1993 article, Ruth Dixon-Mueller questioned the reproductive health field's conceptualization of sexuality, arguing that it had treated intercourse as a sanitized, emotionally neutral act. If one were to learn about human sexuality by reading family planning research and program manuals, she suggested, one would have no idea that sex leads to great enjoyment-as well as pain-for human beings. She called for a more gender-sensitive approach to sexuality in research and programming, including greater attention to the ways in which women want to maximize sexual enjoyment and minimize sexual harm, and to how these desires influence their reproductive health behaviors. Such an approach-which Dixon-Mueller called establishing the "sexuality connecti...

CONCLUSION: Despite the high prevalence of contraceptive use in Uzbekistan, the country's reproductive health program may be constraining method choice. Expanded programmatic efforts emphasizing choice from a range of methods are needed, especially among subgroups of women who have little knowledge or experience with methods other than the IUD.

CONCLUSIONS: Despite these gaps in access to skilled delivery and effective emergency obstetric care, some progress has been made in reducing maternal mortality levels. Improved obstetric care and declining levels of fertility and unwanted pregnancy may have played critical roles in addressing the maternal health care needs of Bangladeshi women.

For nearly two decades, the conviction that poverty fuels the spread of HIV in Sub-Saharan Africa has been ubiquitous among epidemiologists and development workers.1-3 In 2005, however, a team of epidemiologists4 published a comment in The Lancet drawing attention to some surprising findings: Demographic and Health Survey (DHS) data from Kenya and Tanzania appear to demonstrate that HIV prevalence is highest among the wealthiest segments of those populations, and lowest among the poor.5,6 In light of these findings, the authors suggested that wealth, rather than poverty, may be the root cause of behavioral risk for HIV in Sub-Saharan Africa.

CONCLUSIONS: Spousal discussions are important in the decision to get a vasectomy, but these discussions should be initiated by the male partner. Programs need to educate men about contraceptive options, including vasectomies. Detailed, culturally relevant knowledge of the barriers and facilitators individuals experience during their decision- making process will enable vasectomy promotion programs to more successfully target appropriate populations.

CONCLUSIONS: Although average program effort scores have risen again, increases in effort, funding and access to contraceptive methods are still needed in many countries, especially in rural areas and among the poor.
CONCLUSIONS: Preference for male children and parental concern over infant and child mortality may partially explain the difference between desired family size and fertility. A reduction in breast-feeding and an increase in use of less-effective contraceptive methods might be responsible for the inconsistency in the relationship between contraceptive use and fertility.

CONCLUSIONS: These results suggest that premarital sex is becoming more common in Vietnam but is still not widespread. Better information about the sexual networks of men in the general population and their contacts with high-risk groups (e.g., commercial sex workers) is needed to ascertain the likelihood that HIV/AIDS will spread to the general population.

Since the late 1980s and early 1990s, we have come a considerable distance in recognizing that successful HIV prevention work among women means the adoption of a woman-centered paradigm, one that is grounded in women's realities and acknowledges gender roles and gender-based power differentials as critical factors in women's ability to make and effect decisions regarding their health and welfare. We have learned that most women around the world cannot control male condom use, and we have begun to understand that women's attitudes toward and use of protective methods are based on personal, relational, sociocultural and structural factors, with a different mix for each woman. HIV prevention has required us to work on two levels: to achieve long-term, structural change ...

CONCLUSION: Country-specific investigations are needed in Asia to better understand the reasons for differences in service use by age. In general, further systematic evidence would help identify long-term interventions that will be most effective in increasing adolescents' use of maternal and child health services.

CONCLUSIONS: Gender inequality constrains women's access to skilled health care in Nepal. Interventions to improve communication and strengthen women's influence deserve continued support. The strong association of women's education with health care use highlights the need for efforts to increase girls' schooling and alter perceptions of the value of skilled maternal health care.

CONCLUSION: New policies and interventions, tailored to the sexual and behavioral profiles of women in each country, are needed to address the vulnerabilities of adolescent wives. In some countries, delaying age at marriage may be an important strategy; in others, making intercourse within marriage safer may be more valuable.

Fertility awareness-based methods of family planning: predictors of correct use.

CONCLUSION: There was no clear profile of clients for whom these family planning methods would be inappropriate. However, programs offering these methods may help couples overcome potential difficulties in correct method use by including male partners and encouraging their participation in counseling sessions.
