Abstract The diagnostic approach to a patient with polycythemia has been greatly simplified by the introduction of new genetic testing
in addition to traditional tests, such as measurement of red cell mass and serum erythropoietin (Epo) level. Clonal erythrocytosis,
which is the diagnostic feature of polycythemia vera (PV), is almost always associated with a JAK2 mutation (JAK2V617F or
exon 12). Therefore, in a patient with acquired erythrocytosis, it is reasonable to begin the diagnostic work-up with JAK2
mutation analysis to distinguish PV from secondary erythrocytosis. The clinical course of PV is marked by a high incidence
of thrombotic complications that represent the main cause of morbidity and mortality in these patients. Blood hyperviscosity
as well as platelet and...

Abstract In acute pulmonary embolism, patients free from circulatory failure usually present a blood gas pattern consistent with respiratory
alkalosis. We investigated whether the appearance of arterial base deficit in these patients indicates disease severity and
diagnostic delay. Twenty-four consecutive patients with pulmonary embolism were retrospectively evaluated. Twelve patients
had arterial base excess ≥0 mmol/L (Group 1), and 12 patients arterial base deficit <0 mmol/L (Group 2). No patient showed
signs of circulatory failure. Group 1 was characterized by a mean base excess of 2.2 ± 1.7 mmol/L, while in Group 2, the mean
base deficit was −1.9 ± 0.7 mmol/L (p < 0.0001). At 1 week since the embolism, 1...

Abstract Osteoporosis and vascular disease are commonly found together in elderly people. Several common mechanisms and risk factors
have been suggested to contribute to the development of osteoporosis and atherosclerosis. The present cross-sectional study
was performed to determine whether the degree of bone turnover is correlated to carotid intima-media thickness (CCA-IMT),
as a marker of subclinical atherosclerosis. We selected 50 outpatients (mean age 71.7 ± 12.3), underwent to eco-Doppler evaluation
of extracranial carotid tract, without history of calcium and/or vitamin D supplementation, or antireabsorptive therapy. CCA-IMT
was measured by high-resolution B-mode ultrasonography. Bone turnover was evaluated by analysing serum levels of C-terminal
telopepti...

Abstract Inflammation plays a pivotal role in all stages of atherosclerosis from endothelial dysfunction and plaque formation to plaque
destabilization and disruption. Inflammatory biomarkers, originally studied to better understand the pathophysiology of atherosclerosis,
have generated increasing interest among clinicians, because of their utility in the challenging problems of diagnosis and
risk assessment of patients with suspected or proved coronary heart disease. Moreover, in fascinating perspective, they could
be used as therapeutic target, counteracting initiation, progression, and development of complications of atherosclerosis.
In this review, we will provide an overview of the more promising inflammatory biomarkers, focusing on their utility and limitations

Abstract For venous thromboembolism (VTE) prevention in the hospitalized medical patient, no head-to-head trials have been performed
of unfractionated heparin (UFH) 5,000 U subcutaneously thrice (i.e. q8 h or TID) daily versus twice daily (q12 h or BID).
Several meta-analyses have been undertaken in attempts to determine whether one regimen may be more beneficial for safety
and efficacy. Currently, not all international guidelines include a recommended frequency for UFH. Delineation of this frequency
may be helpful to the practicing clinician. Primary studies (with a modified Jadad score of ≥6 to demonstrate a stronger study
design) that compared low molecular weight heparin (LMWH) and UFH, and UFH and placebo were evaluated. Meta-analyses evaluating

Abstract The interaction of ADP with its platelet receptor P2Y12 plays a crucial role in platelet activation and thrombogenesis. This
article reviews the pharmacology and clinical trials of specific antagonists of P2Y12. Clopidogrel is a thienopyridine with
proven antithrombotic efficacy, but it has some important drawbacks: (a) it is a pro-drug that needs to be metabolized to
its active metabolite; (b) it has a delayed onset and offset of action and (c) there is high inter-individual variability
in pharmacological response. Prasugrel is also a thienopyridine, with faster onset of action and a more uniform inhibition
of platelet function compared to clopidogrel, accounting for lower incidence of ischemic events in patients with acute coronary
syndromes (ACS) undergoing per...

In conclusion, acute infection–inflammation could play a role in facilitating the occurrence of CST in a subgroup
with low risk profile for known risk factors. Our findings, if confirmed, could suggest new opportunities for prevention and
treatment of CST.

Abstract According to principles of clinical trial design, the demonstration of efficacy of a new treatment is based on comparing the
response in the treated group with that of a control group receiving placebo or another active treatment. The need for a control
group is also recommended by the major international institutions that govern the ethics and the practice of clinical research.
Despite these principles and recommendations, inspection of a purposive sample of ongoing clinical trials listed in the NIH
registry (http://ClinicalTrials.gov) reveals that as many as one-third of trials are uncontrolled. Since these trials were approved through a formal evaluation
by ethics committees, the lack of adequate control was not perceived as a major deficiency in the study desig...

Abstract General concepts about medical disasters, public health and triage are outlined. Triage is described in the context of public
health emergencies and disaster settings, and the main ethical values at stake in triage are discussed. Possible conflicts
between competing values are outlined. Special attention is given to possible conflicts between the protection of individual
interests (typical of clinical ethics), and the pursuit of collective interests (typical of public health and triage). Hippocratic
ethics is compared to utilitarian ethics and to perspectives that emphasize the principle of justice. Three ethical attitudes
are suggested that may contribute to a resolution of competing values: protection of human dignity, precaution and, especially,
solidarity. Per...

Abstract The use of supplemental sodium bicarbonate for the treatment of patients with septic shock and elevated blood lactate levels
remains a controversial therapy. We conducted a retrospective study between March 2004 and February 2009 of 36 consecutive
patients diagnosed with septic shock who received continuous infusion of bicarbonate therapy. A control group was matched
1:1 for age, site of infection, and predicted mortality by APACHE II. All patients were managed according to standard protocols.
The median time until reversal of shock did not achieve statistical significance between the bicarbonate group (44.5 h [95%

Abstract Combined antiplatelet and anticoagulant therapy has been suggested for those clinical conditions in which conventional antithrombotic
regimens have shown suboptimal efficacy, and in patients with indication for both: antiplatelet and anticoagulant therapy.
Clinical trials aimed at assessing the clinical benefit of the association with respect to mono-therapy have been conducted
in patients with atrial fibrillation, in patients with recent myocardial infarction, and in patients with prosthetic heart
valves. Overall, a favorable benefit-risk profile of combined therapy in comparison to anticoagulant alone has been observed
in patients with mechanical prosthetic heart valves and in those with coronary artery disease while no clear advantage has
been shown in patients...

Abstract Despite its rarity, infliximab-related hepatitis constitutes a cutting edge and challenging problem. In December 2004, a drug
warning was issued by the Food and Drug Administration to alert healthcare professionals to the risk of hepatotoxicity in
course of infliximab therapy. Subsequently, several reports of probable infliximab hepatitis have been published and interest
is growing in trying to elucidate the impact of these events on clinical practice. After discussing our case report, the main
characteristics of infliximab-mediated liver injury are analyzed, coupled with a review of the medical literature. Infliximab
seems to provoke both immunomediated and a direct liver injury, but how this latter happens remains unclear. Moreover, infliximab
immunomediated liv...

consequences on the socioeconomic level. Residual thrombosis is an important predictor of PTS, and severe early symptoms,
old age, obesity, improper anticoagulation, recurrent thrombosis and varicose veins are major risk factors. Diagnosis of PTS
is mainly based on the clinical findings for patients with a history of DVT, while in those without it, instrumental diagnosis
might help in detecting a previous DVT. Prompt administration of adequate compression elastic stockings (ECS) in patients
with symptomatic DVT reduces the frequency of PTS by half. Usually, the management of an established PTS is demanding, and
often discouraging. How...

Abstract Little information is available on acid–base imbalance in uncomplicated ST-elevation myocardial infarction (STEMI) submitted
to primary percutaneous intervention (PCI). We therefore assessed acid–base imbalance in 257 consecutive uncomplicated STEMI
patients submitted to PCI to determine whether its evaluation could help in identifying patients at higher risk for in-hospital
complications (acute pulmonary edema and dysrhythmias). A basic metabolic profile was performed at hospital admission, that
is before PCI. After PCI, we measured: creatinine, uric acid and NT-pro BNP and serum electrolytes. Peak troponin I was also
considered. Acidemia was present in 11 patients (4.2%), HCO3 < 22 in 62 (24.1%). Base excess < −3 was detectable in ...

Abstract In this review, we have analyzed the role of visceral obesity in the occurrence of metabolic syndrome (MetS). MetS is a common
metabolic disorder that has been related recently to the increasing prevalence of obesity. The disorder is defined in various
ways, but in the near future a new definition(s) should be applicable worldwide. The pathophysiology has been largely attributed,
in the past years, to insulin resistance, although several epidemiological and pathophysiological data now indicate visceral
obesity as a main factor in the occurrence of all the components of MetS. In view of this, relationships among visceral obesity,
free fatty acids, dyslipidemia and insulin resistance have been reported. In addition, the effects of some adipocytokines
and other proin...

Abstract Chronic urticaria (CU) is a skin disorder characterized by the recurrent eruption of short-lived wheals accompanied by redness
and itching for at least 6 weeks. The wheals can be associated with angioedema. CU is considered an autoimmune disease in
about 50% of cases with the presence of circulating histamine releasing autoantibodies mainly directed against the high affinity
IgE receptor FcεRI on mast cells and basophils or against IgE. In several CU cases regarded as idiopathic; the actual pathophysiological
mechanisms are still unknown. Some patients with CU do not respond to antihistamines and require the use of systemic steroids
or cyclosporin, which are, however, not always effective. In CU, several investigators have demonstrated the activation of

Abstract Advances in neuroimaging have modified our knowledge on cerebral vein thrombosis (CVT). This disease is now diagnosed more
frequently, and increasing evidence as to what are the most common risk factors and on the natural history of the disease
is becoming available. Most patients with CVT have a benign prognosis: only a minority of patients die during the acute phase
or in the following months. Most patients surviving CVT recover completely, or have only mild functional or cognitive deficits.
Unfractionated or low-molecular weight heparin is widely used as a first-line therapy of CVT, despite the absence of conclusive
evidence about the safety and efficacy in this setting. Vitamin K antagonists are usually prescribed for secondary prevention,
but the optimal dura...

Abstract Right ventricular dysfunction during acute pulmonary embolism (PE) predisposes to hemodynamic instability and cardiogenic
shock. Aim of this case–control study was to determine the clinical, historical and diagnostic findings associated with right
ventricular dysfunction in patients with acute PE involving the main or segmental pulmonary arteries (central PE) and without
hemodynamic instability on admission to the Emergency Department (ED) (non-massive PE). From January 1, 2002 to December 31,
2005, 211 patients with central PE were admitted to the Department of Emergency Medicine of the “Antonio Cardarelli” Hospital
(Naples, Italy). One hundred eighteen of them had echocardiographic evidence of right ventricular dysfunction on admission

has been described only recently with the demonstration of recurrent mutations in JAK2 or MPL. While life expectancy may not be significantly shortened, arterial and venous thrombosis constitute the major causes of
morbidity and mortality, together with disease evolution to myelofibrosis or transformation to acute leukemia. Therapy is
currently aimed at reducing the rate of thrombosis without increasing the risk of hematologic transformation by inappropriate
exposure to cytotoxic drugs. Nevertheless, the mechanism(s) finally responsible for the increased thrombotic tendency have
not been clearly elucidated, although risk factors for thrombo...

Abstract Upper extremities deep venous thrombosis (UEDVT) is a rare condition. According to the literature, approximately 4–10% of
all cases of venous thrombosis may involve the subclavian, axillary or brachial veins. In the last few decades, the incidence
of UEDVT has increased because of more frequent use of central venous catheters (CVCs) and cardiac pacemaker implantation.
In addition, another common risk factor for UEDVT is cancer. UEDVT is classified as primary, approximately one-third of cases,
which refers either to effort thrombosis or idiopathic UEDVT, or secondary, due to the presence of overt predisposing causes.
The onset of UEDVT is usually characterized by arm swelling and pain, but may also be completely asymptomatic especially in
patients with a long-ter...

Abstract Direct thrombin inhibitors (DTIs) are a class of anticoagulants that bind selectively to thrombin and block its interaction
with its substrates. Dabigatran etexilate and AZD0837, the new generation of DTIs, are now under intense development, and
are potentially of great interest for internists. Dabigatran etexilate is a potent, non-peptidic small molecule that specifically
and reversibly inhibits both free and clot-bound thrombin by binding to the active site of thrombin molecule. It has been
already licensed in the European Union and in Canada for the prevention of VTE in patients undergoing hip- and knee-replacement
surgery. Ongoing trials are evaluating its efficacy and safety for the treatment of deep venous thrombosis and pulmonary embolism,

Abstract The receptor for advanced glycation end-products (RAGE) and its ligands are intimately involved in the pathobiology of a wide
range of diseases that share common features, such as enhanced oxidative stress, immune/inflammatory responses, and altered
cell functions. Soluble forms of RAGE (sRAGE), including the splice variant endogenous secretory (es)RAGE, have been found
circulating in plasma and tissues. Experimental data suggest that these isoforms may neutralize the ligand-mediated damage
by acting as a decoy. Moreover, evidence is mounting to support a role for both sRAGE and esRAGE as biomarkers or endogenous
protection factors against RAGE-mediated pathogenesis. In this review, we will focus on clinical and therapeutical implications
arising from studies inve...

Abstract Chronic liver disease is characterized by a global hemostatic defect including platelet–vessel wall interaction (primary hemostasis),
coagulation and fibrinolysis that may cause abnormalities of the relevant laboratory tests. The causal relationship between
abnormal tests and bleeding has been widely accepted, despite the fact that abnormal tests are poorly associated with the
timing and incidence of actual bleeding. In this article, we review recent evidence from the literature that disputes the
above paradigm, and opens new venues for laboratory/clinical research and patient management in this field.

Abstract The microcirculation is defined as the smallest vessels where gas and nutrient exchange with tissues takes place. One of its
primary functions is to ensure adequate oxygen delivery to meet the oxygen demands of tissue cells. Previous data from clinical
and experimental studies and the recent development of new imaging modalities, such as Orthogonal Polarization Spectral videomicroscopy
and Sidestream Dark Field imaging, have helped to identify the crucial role that microcirculation plays in sepsis. If not
corrected, microcirculatory dysfunction can lead to respiratory distress in tissue cells and subsequent organ failure, even
in the absence of global hemodynamic deficiency. In the present review, we will address past and recent developments regarding
the role of ...

Abstract Venous thromboembolism (VTE), which includes deep vein thrombosis and pulmonary embolism, is a major cause of morbidity and
mortality in patients undergoing major orthopedic surgery. Routine thromboprophylaxis has been the standard of care over the
last 20 years. Currently available options for the prevention of VTE in major orthopedic surgery include low molecular weight
heparins, vitamin K antagonists, and, more recently, the synthetic pentasaccharide fondaparinux. Although effective, all these
drugs have several limitations and new oral antithrombotics offering predictable, effective and safe anticoagulation are strongly
needed. This overview focuses on the most advanced oral direct inhibitors to factor Xa rivaroxaban, apixaban, LY517717 and

Abstract Treatments for chronic hepatitis C (CHC) patients intolerant to pegylated interferons (peg-IFNs) are lacking. Thus, such patients
remain at high risk of developing an advanced and decompensated liver disease. Leukocyte IFN-α (Le-IFN-α) seems to possess
a safer profile than other natural and recombinant α-interferons, but no information is available for peg-IFN intolerant patients.
Accordingly, we evaluated the safety and efficacy of Le-IFN-α in patients intolerant to peg-IFNs. Twenty-five consecutive
CHC patients intolerant to peg-IFNs were prospectively enrolled. HCV genotype 1 was present in 80% and cirrhosis in 68% of
cases. Thirteen patients (52%) had thrombocytopenia. Le-IFN-α (3 MU three times a week) was administered for 48 weeks plus

Abstract Essential thrombocythemia (ET) is a clonal myeloproliferative disorder characterized by sustained increase in platelet number
and tendency for thromboembolism. A somatic point mutation that causes a constitutive activation of the JAK2 gene is found
in one in two ET patients. ET is more common in women, its incidence being 0.6–2.5/100,000 patient/year and the median age
at diagnosis is 65–70 years. ET can affect all age groups, including children (0.09 cases/year), and is often diagnosed in
the third–fourth decade of life. Rare cases of familial ET have been reported. Miscarriages are 3–4 times more common among
women with ET than in the general population, especially in patients carrying JAK2V617F. Microvascular disturbances are typical
of ET, but a m...

Abstract Matrix metalloproteinases (MMPs), a family of enzymes that degrade extracellular matrix, are emerging as important modulators
of atherothrombosis. MMPs are produced by inflammatory cells; some of them are also released by activated platelets and play
a crucial role in the remodeling processes, leading to atherosclerotic plaque formation, plaque rupture, arterial aneurysm
development, and critical limb ischemia. Independent from their matrix degrading activity, MMPs also regulate some cell functions
relevant to atherothrombosis, such as platelet activation, neutrophil activation, and vascular reactivity. Plasma levels of
some MMPs are increasingly being recognized as a biomarker of atherosclerosis and cardiovascular risk. In peripheral arterial
disease, MMPs have b...

Abstract Managing oral anticoagulation may be difficult in hemodialysis patients because the antithrombotic effect can be counterbalanced
by an increased risk of hemorrhagic complications. There is insufficient evidence to recommend the routine use of warfarin
for thrombosis prophylaxis of the vascular access in all patients. If a decision for anticoagulation is made, dosing warfarin
to a “therapeutic” level is suggested, although the most appropriate target INR range remains unclear. Many hemodialysis patients
with atrial fibrillation have multiple risk factors for stroke and generally benefit from warfarin, with careful and frequent
laboratory monitoring. Treatment with standard dose warfarin is also recommended in patients with venous thromboembolism provided
that p...

Abstract Procalcitonin (PCT) is known to be a biological diagnostic marker for severe sepsis, or septic shock in critically ill patients.
There are still contrasting data about a role of procalcitonin in patients with acute myocardial infarction or cardiogenic
shock, and in those with acute coronary syndromes, that is, non-ST-elevation myocardial infarction or unstable angina. We
evaluated plasma levels of procalcitonin and C-reactive protein (CRP) in 52 patients admitted to our intensive cardiac care
unit (ICCU): 14 patients with cardiogenic shock (CS) following ST-elevation myocardial infarction (STEMI), 15 patients with
uncomplicated ST-elevation myocardial infarction (STEMI), and 24 with non-ST-elevation myocardial infarction or unstable angina

Abstract Migraine is a complex, neurovascular disorder in which genetic and environmental factors interact. At present, frontline therapies
in the acute treatment of migraine include the use of non-steroidal anti-inflammatory drugs and triptans. Evidence indicates
that calcitonin gene-related peptide (CGRP) plays a fundamental role in the mechanism of migraine. CGRP is a strong vasodilatatory
neuropeptide that is released from activated trigeminal sensory nerves. The development of CGRP antagonists has also been
driven by the fact that triptans are vasoconstrictive and cannot be safely used in patients with cardiovascular risk factors.
Olcegepant (BIBN4096) is the first CGRP antagonist for the treatment of migraine that has been tested in clinical trials,
but because of it...

Abstract The widespread use of metal stents and drug-eluting stents has shown the extent to which patients with unstable coronary perfusion
depend on antiplatelet drugs, and how their risk of late thrombosis depends on the long-term use of agents such as clopidogrel.
It has also been shown that the risk of surgical bleeding, if antiplatelet drugs are continued, is lower than that of coronary
thrombosis if they are withdrawn. Thus, except for low-risk settings, the practice of withdrawing antiplatelet drugs 5–10 days
prior to surgical procedures should be changed. The following suggestions are meant to provide a guideline in this respect.
Most of the current surgical procedures may be performed while on low-dose aspirin treatment. Except when bleeding may occur

Abstract Pulmonary arterial hypertension (PAH) is a devastating disease with significant disability and mortality, and it has much
higher prevalence than previously thought. During the past 15 years, we have witnessed remarkable advances in our understanding
of pathogenesis, in diagnostic process and in the development of disease-specific treatments for PAH. Nowadays, the diagnosis
is more clearly defined, non-invasive markers of disease severity can be widely applied, and finally we can adopt evidence-based
treatment. Newer drugs availability has resulted in radical change in the management of this disease. The article reviews
established approaches to evaluation and treatment of this disorder.

Abstract The porphyrias are a heterogeneous group of metabolic diseases resulting from a variable catalytic defect of one of the eight
enzymes involved in the heme biosynthesis pathway; they are mostly inherited diseases, but in some circumstances the metabolic
disturbance may be acquired. The specific patterns of tissue overproduction (and hence accumulation and excretion) of toxic
heme precursors, associated with each enzymatic deficiency, are responsible for the characteristic biochemical and clinical
features of each of these diseases. Moreover, even in the presence of a specific inherited enzymatic defect, many different
environmental factors (such as drugs, calorie restriction, hormones, sunlight exposition, infections, etc.) often play a key
role in triggering the c...

Abstract Anabolic steroids are widely used for doping, in professional and domestic sports. The mechanism of action is not completely
understood. It may differ somewhat depending on the specific molecule due to structural differences that influence the specificity
of binding with steroid receptors. When used by athletes in training, they can improve performance to levels that cannot be
attained by almost any combination of sophisticated nonchemical support by modern sport science. The severity of the undesired
effects of anabolic steroids depends on a variety of factors, from the type and combination of them, the dose and duration
of administration, as well as the gender of the person taking the drug. Younger individuals and women show greater effects
caused by anabolic st...

Abstract We evaluated the acute effect of the application of positive end-expiratory pressure (PEEP) on LV diastolic function in 10
healthy subjects. We assessed load dependent diastolic function by Doppler examination of transmitral flow and load independent
diastolic function by color M-mode propagation velocity of early flow into the LV cavity (Vp). During the application of PEEP
in comparison to the baseline, we found a significant reduction of the E wave peak velocity [79 (64–83) vs. 65 (57–72) cm/s;
p = 0.028] and a significant reduction in Vp [84 (73–97) vs. 53 (48–66); p = 0.012]. Moreover, we found a significant reduction in left atrial area [15 (13–18) vs. 12 (10–14) cm2; p = 0.018] and right atrial area [12 (11–15) vs. 1...

Abstract The Objective of this prospective observational study was to evaluate the applicability of the simplified acute physiology
score (SAPS II) in patients admitted to an Emergency Medicine Ward in the Emergency Medicine Ward of a tertiary university
hospital. We studied consecutive patients admitted to an Emergency Medicine Ward from the emergency department. The SAPS II
was assessed in predicting overall in-hospital mortality in terms of sensitivity, specificity and receiver operating characteristic
(ROC) curve. A total of 211 consecutive patients were admitted over a period of 2 months. Median SAPS II score was 28 (range
6–93), with a mean risk of in-hospital mortality of 0.17 (range 0.01–0.97) for the whole population, and an observed mortality
of 15%. The area...

Abstract Chest pain is one of the most common presenting complaints of a cardiac ischemic event. As the presenting complaint often
dictates further interventions, it is necessary to identify and stratify the risk of a cardiac ischemic event in cases of
chest pain as the primary complaint. The study also aimed to evaluate the actual number of cardiac ischemic events from all
cases of complaints of chest pain. The study aimed to evaluate the occurrence of chest pain as an emergency attended by our
emergency management service—108 services. During the period of January 1st–December 31st 2007, a total of 9,130 calls with
chest pain were attended to, out of which 6,235 (68.3%) were included who were patients above 30 years of age and for whom
complete data were availab...

Abstract The aim of this study was to evaluate the accuracy of procalcitonin (PCT) in predicting infective endocarditis (IE). 23 adult
patients with IE, 30 patients with sepsis and 30 with tick-borne encephalitis were included in this prospective study. The
PCT serum level, C-reactive protein (CRP), total leukocyte, and immature polymorphonuclear (PMN) cell counts were determined
on admission, prior to the institution of antibiotic therapy, and compared according to the diagnosis. The median PCT level
in patients with IE endocarditis was 0.81 ng/ml, in patients with sepsis it was 43.74 ng/ml, and in the group with viral infection
it was 0.25 ng/ml (P < 0.001). The highest PCT level was found in patients with Staphylococcus aureus endocarditis. Th...

Abstract To analyse the relationship of PAI-1 plasma levels to echographically determined liver steatosis and cardiometabolic risk
factors in a randomly selected sample of 254 adult male participants of the Olivetti Heart Study. Accounting for age and ongoing
pharmacological treatment, PAI-1 levels were directly (P < 0.005) associated with body mass index (BMI), waist circumference (WC), serum triglyceride (TG), total cholesterol, insulin,
homeostasis model assessment index, gamma-glutamyl transpeptidase and peritoneal fat. At multiple linear regression (MLR)
analysis, measures of adiposity and TG exerted significant and quantitatively similar effects on PAI-1 levels. A progressive
rise in PAI-1 level was detected with increasing degree of steatosis. A stepwise...

Abstract Atrial fibrillation (AF) is often associated, more or less indirectly, with an inflammatory acute or chronic process. So it
is probable that the inflammation could contribute to the genesis and the perpetuation of this dysrhythmia. Phlogistic test
indexes in patients (pts) with AF will be positive and have prognostic significance in patients treated with electrical cardioversion
with restoration of a sinus rhythm. We evaluated 106 pts affected by AF of recent onset without known cardiovascular disease.
We measured the plasma concentration of C-reactive protein (CRP) through a high sensibility method, in addition to routine
blood samples. We perfomed an ECG 1 week and a Holter ECG monitoring 1 and 6 months after the electrical cardioversion. The

Abstract Diabetic patients have an increased risk for cardiovascular complications with respect to the general population. Micro- and
macrovascular complications such as nephropathy, retinopathy, atherosclerosis, and coronary artery disease are usually preceded
by endothelial dysfunction, a condition characterized by impaired vasorelaxation resulting from reduced bioavailability of
the endothelial mediator nitric oxide (NO). Nitric oxide is among endothelial mediators released by endothelial cells in response
to insulin stimulation. Therefore, metabolic abnormalities such as insulin resistance, dyslipidemia, compensatory hyperinsulinemia
and overt hyperglycemia may all contribute to impaired NO bioavailability and abnormal vasodilatation in diabetic patients.
