Patient empowerment as a component of health system reforms: rights, benefits and vested interests

Abstract Different strategies have been developed across countries to foster citizens’ and patients’ involvement, from health policies
to patients’ active participation in decisions regarding their health. The spectrum varies from systems where patients lead
the reform of health care services, to others where a paternalistic approach still limits patients’ autonomy in decision-making.
This paper describes: (1) different interventions for involving patients; (2) experiences to promote consumer evidence-based
advocacy; and (3) barriers to consumer involvement in health system reforms, including vested interests in patients’ associations.
Citizens’ involvement in health systems can vary substantially, but is gaining increasing weight.

Abstract The clinical picture of myocardial ischemia accompanying allergic reactions is defined in the cardiologic literature as Kounis
syndrome (KS) or allergic angina/myocardial infarction. In PubMed, a search for “Kounis syndrome”, “allergic angina” or “allergic
myocardial infarction” retrieves more than 100 results (among case reports, case series and reviews), most of which are published
in cardiology/internal medicine/emergency medicine journals. In allergologic literature, heart involvement during anaphylactic
reactions is well documented, but Kounis syndrome is hardly mentioned. Single case reports and small case series of angina
triggered by allergic reactions have been reported for many years, and involvement of histamine and others mast cell mediators...

Abstract Atrial fibrillation (AF) is intermittent in 30% of patients with cardioembolic stroke and, therefore, might not be seen in
a single standard ECG recording. The aim of this study was to evaluate if prolonged ECG monitoring (96 h) finds episodes of
intermittent AF beyond the 24 h ECG monitoring in patients with cryptogenic stroke or transient ischemic attack (TIA). We
prospectively evaluated consecutive patients affected by cryptogenic stroke or TIA who had sinus rhythm on a 12-lead ECG on
admission, and during ECG monitoring performed in the acute phase (for at least 24 h). Patients had continuous 96 h Holter
ECG monitoring within 30 days from stroke onset. 114 patients were included in the study (mean age 63.1 ± 15.1, 59 males).

Abstract
Clostridium difficile-associated disease (CDAD) is a growing health care problem. Elderly patients with multiple comorbidities and repeated hospitalization
are at high risk for developing the disease. Few data are available on epidemiology of CDAD in Italy and no studies have focused
on CDAD burden in internal medicine wards. We retrospectively analysed all CDAD cases in four internal medicine wards of a
city hospital in northern Italy and reviewed the medical records of patients who developed CDAD during hospitalization. We
identified 146 newly acquired cases, yielding a cumulative incidence of 2.56 per 100 hospitalizations and an incidence rate
of 23.3 per 10,000 patient-days. Main risk factors were advanced age and length of hospitalization. A high proportion ...

Abstract Osteoporosis and osteomalacia are, to date, among the most common metabolic diseases in the world. Lately, an association
between metabolic bone diseases and chronic liver disease has been increasingly reported, inducing many authors to create
a new nosographic entity known as ‘hepatic osteodystrophy.’ The importance of such a condition is further increased by the
morbidity of these two diseases, which greatly reduce the quality of life because of frequent fractures, especially vertebral
and femoral neck ones. For this reason, early identification of high-risk patients should be routinely performed by measuring
bone mass density. The explanation for the association between bone diseases and chronic liver disease is still uncertain,
and involves many factors: f...

Abstract The relationship between overt hypothyroidism and cardiovascular risk has been well documented and some data also suggest
an association between cardiovascular risk and subclinical hypothyroidism. The aim of our study was to investigate, in a large
cohort of euthyroid women, the association of thyroid stimulating hormone (TSH) within the normal reference range with cardiovascular
risk factors. The study was carried out on 744 women with normal thyroid function (TSH 0.3–4.9 μU/mL). Women with TSH above
the median (≥2.1 μU/mL) were more obese, had greater waist girth, were more hypertensive and had higher levels of total cholesterol
(TC), serum triglycerides (TG), blood sugar (BG) and lower levels of HDL-cholesterol (HDL-C) than women with TSH below the

and once again underline the crucial importance of an early and multi-disciplinary intervention in preventing bone complications
in thalassaemic patients.

Abstract “Triage” is a useful tool used in emergency departments (EDs) to prioritize the care of patients. Through a methodical process
of different sequential steps, the triage nurse assigns a color code which goes from red—critical patient with immediate access
to medical examination—to a white code that represents no urgency. Clinical studies have shown that patients can be victims
of errors during the process of care, especially in complex systems such as EDs. To reduce errors it is essential to map the
risks in order to identify the causes (both individual and organizational); the introduction of corrective changes cannot
be postponed. The incorrect assessment at triage represents one of the major errors in EDs. By monitoring this activity, through

Abstract Over the last decade, the advent of molecular targeted therapy radically changed the treatment of several forms of cancer.
However, these innovative anticancer drugs, namely monoclonal antibodies and small molecule tyrosine kinase inhibitors were
found to adversely affect cardiovascular function. These “on-target” and “off-target” drug side effects encompass a wide range
of cardiotoxicities, including left ventricular dysfunction leading to heart failure, electrocardiographic abnormalities with
dysrhythmias, hypertension, myocardial ischemia and thromboembolic events. The unclear incidence of drug-induced cardiovascular
events together with uncertainty on their reversibility and long-term safety call for a multidisciplinary effort embracing

Abstract To evaluate cardiac output (CO), both invasive and semi-invasive monitors are used in critical care medicine. The pulmonary
artery catheter is an invasive tool to assess CO with the major criticism that the level of its invasiveness is not supported
by an improvement in patients’ outcomes. The interest in a lesser invasive techniques is high. Therefore, alternative techniques
have been developed recently, and are used frequently in critical care medicine. Cardiac output can be monitored continuously
by different devices that analyze the stroke volume and CO. The purpose of this review is to understand these new technologies
and their applications and limitations.

This study demonstrates higher values of leptin in SLE
patients. Moreover, SLE patients show increased levels of vascular stiffness and PSEM and reduced values of vascular strain
and distensibility. These results globally indicate a decline in arterial elasticity. We find a positive correlation of leptin
with stiffness parameters. According to its atheroprotective action, adiponectin inversely correlates with stiffness parameters.

Abstract The role of pulmonary rehabilitation (PR) in COPD patients with lung hyperinflation has not yet been fully investigated. We
retrospectively evaluated the effect of a standard PR course on exercise tolerance and symptoms according to the presence
or absence of associated lung hyperinflation, as defined by lung function parameters in three Italian rehabilitation centres.
In a cohort of 823 COPD patients (age 71 ± 8 years, FEV1 56 ± 18% pred.) we have systematically recorded: changes (∆) in 6-minute walking test (6MWD) as the primary outcome; dyspnoea
(D); muscle fatigue (F); SO2nadir during effort; perceived breathlessness score (MRC); and specific health-related quality of life (SGRQ). Outcomes were

This study was aimed to estimate the prevalence, risk factors, and
outcome of self-reported hip fracture (srHF) in a cohort of Spanish elderly individuals. Neurological Disorders in Central
Spain (NEDICES) is a census population-based survey of the prevalence and incidence of major age-associated conditions in
three areas of central Spain. Data on health status and several chronic conditions were evaluated in the baseline questionnaire
(1994–1995). Odds ratios for the association between srHF and other comorbidities and health-related variables were assessed
by logistic regression. A Cox model estimated the impact of srHF on 13-year all-cause mortality. The final cohort comprised
5,278 community-living elderly subjects. A total of 166 participants (3.1%) had srHF. Prevalence was as...

This study aimed to evaluate the role of different gastritis patterns on the efficacy of
the currently suggested 14-day triple therapy regimen. One-hundred and seventeen, consecutive, non-ulcer dyspeptic patients,
with H. pylori infection diagnosed at endoscopy, were enrolled. All patients received a 14-day, triple therapy with lansoprazole 30 mg,
clarithromycin 500 mg and amoxicillin 1 g, all given twice daily. Bacterial eradication was assessed with 13C-urea breath test 4–6 weeks after completion of therapy. H. pylori infection was cured in 70.1% at ITT analysis and 83.7% at PP analysis. The eradication rate tended to be lower in patients
with corpus-predominant gastritis as compared to those with antral-predominant gastritis at both ITT (66.1 vs 74.5%) and PP (...

Abstract Most cardiopulmonary diseases share at least one symptom with pulmonary embolism (PE). The aim of this study was to identify
the most common acute causes of dyspnea, chest pain, fainting or palpitations, which diagnostic procedures were performed
and whether clinicians investigate them appropriately. An Italian multicenter collaboration gathered 17,497 Emergency Department
(ED) records of patients admitted from January 2007 to June 2007 in six hospitals. A block random sampling procedure was applied
to select 800 hospitalised patients. Results of the overall 17,497 patients were obtained by weighting sampled cases according
to the probability of the randomisation block variables in the whole population. The case-mix of enrolled patients was assessed

The objective of the study was to determine the predictive value of physical examination findings
for pulmonary edema and elevated B-type natriuretic peptide (BNP) levels in patients with suspected AHFS. This was a secondary
analysis of a previously reported prospective study of jugular vein ultrasonography in patients with suspected AHFS. Charts
were reviewed for physical examination findings, which were then compared to pulmonary edema on chest radiography (CXR) read
by radiologists blinded to clinical information and BNP levels measured at presentation. The predictive value of every sign
and combination of signs for pulmonary edema on CXR or an elevated BNP was poor. Since physical examination findings alone
are not predictive of pulmonary edema or an elevated BNP, clinicians shou...

Abstract Ulcerative colitis, one of the two main forms of inflammatory bowel disease, is characterized by inflammation of the large
bowel with constant involvement of the rectum, and a possible continuous retrograde distribution up to the cecum. Typical
macroscopic lesions are mucosal ulcerations, with immune cell infiltration and cryptic abscesses at histology. Ulcerative
colitis usually manifests with bloody diarrhea, is associated with a number of extra-intestinal manifestations, and may be
acutely complicated by toxic megacolon. Longstanding disease may predispose to the development of colorectal cancer. Therapeutic
options include mesalazine, corticosteroids, immunomodulators and biologic agents; however, if these treatments fail, the
only available therapeutic choice...

Abstract Patients with autoimmune rheumatic disorders have an increased incidence of cardiovascular (CV) events and mortality. Despite
this being related to a high prevalence of the traditional CV risk factors, systemic inflammation has been postulated to be
an independent CV risk factor, particularly in patients with rheumatoid arthritis (RA). However, data are still controversial.
We designed a case–control study, in which patients with autoimmune rheumatic disorders were matched with age-, sex-matched
controls. Prevalence of early atherosclerosis was assessed by carotid artery intima-media thickness (IMT) measurement. IMT
values were considered normal (IMT ≤ 0.9 mm) or abnormal (IMT > 0.9). Multivariate analysis was performed to identify p...

Abstract Extended-spectrum β-lactamase (ESBL)-producing bacteria have been spreading from hospitals to communities. Despite this, there
are limited emergency department (ED) patient-based studies about these bacteremias. A retrospective matched case–control
study with a ratio of 1:3 was conducted at a university hospital. The case group consisted of patients aged >16 years with
ESBL-producing bacteremias in the ED. Patients matched for age and sex with non-ESBL-producing bacteremias were sampled as
the control group. Finally, 64 episodes of ESBL-producing Escherichia coli, Klebsiella pneumoniae, and Proteus mirabilis bacteremias were included in our study. The median age of case patients was 71 years, and 29 (45.3%) were males. The most

Abstract New echocardiographic techniques including tricuspid annular plane systolic excursion (TAPSE) and pulsed tissue Doppler imaging
(TDI) of the right ventricular wall have been assessed to better define right ventricular dysfunction (RVD) during the acute
phase of pulmonary embolism (PE) in patients without significant tricuspidal insufficiency. Their application in patients
with a previous history of PE may provide a better estimation of the incidence and clinical significance of long-term functional
impairment of the right ventricle. In a case–control study, we compared the prevalence of RVD in a cohort of consecutive patients
with previous PE and in age and sex-matched controls without PE. Exclusion criteria were moderate–severe left heart failure,

Abstract Atherosclerosis is a complex process that begins with endothelial dysfunction, and continues with several inflammatory processes
leading, eventually, to plaque rupture and formation of arterial thrombus. Increased platelet reactivity and classical coagulation
pathways are not the only players of the whole thrombotic process: microparticles (MPs), irregularly shaped small vesicles
released from the plasma membrane after cell activation, apoptosis, or exposure to shear stress have been demonstrated to
be involved in such a process. MicroRNAs (MiRs), small-non-coding single-strand RNAs acting as post-transcriptional modulator
of target gene expression are expressed in the large majority of eukaryotes. MiRs are implicated in several phenomena: control
of metabolism, c...

Abstract Dyspnea is a common symptom in patients admitted to the Emergency Department (ED), and discriminating between cardiogenic
and non-cardiogenic dyspnea is often a clinical dilemma. The initial diagnostic work-up may be inaccurate in defining the
etiology and the underlying pathophysiology. The aim of this study was to evaluate the diagnostic accuracy and reproducibility
of pleural and lung ultrasound (PLUS), performed by emergency physicians at the time of a patient’s initial evaluation in
the ED, in identifying cardiac causes of acute dyspnea. Between February and July 2007, 56 patients presenting to the ED with
acute dyspnea were prospectively enrolled in this study. In all patients, PLUS was performed by emergency physicians with
the purpose of identifying the ...

The objective of the study is to determine the association between emergency department (ED) crowding and preventable medical
errors (PME). This was a retrospective cohort study of 533 ED patients enrolled in the National ED Safety Study (NEDSS) in
four Massachusetts EDs. Individual patients’ average exposure to ED crowding during their ED visit was compared with the occurrence
of a PME (yes/no) for the three diagnostic categories in NEDSS: acute myocardial infarction, asthma exacerbation, and dislocation
requiring procedural sedation. To accommodate site-to-site differences in available administrative data, ED crowding was measured
using one of three previously validated crowding metrics (ED Work Index, ED Workscore, and ED Occupancy). At each site, the
continuous measure was plac...

Abstract Colorectal cancer has recently been associated with an increased atrial fibrillation risk, but evidence is very sparse. So,
we conducted a population-based case–control study in northern Denmark (population 1.7 million) during 1998–2006 to estimate
the atrial fibrillation/flutter risk in colorectal cancer patients. We identified 28,333 atrial fibrillation/flutter cases
and 283,260 sex-, age-, and county-matched population controls. We searched the databases for a prior colorectal cancer diagnosis,
a prior cancer diagnosis other than colorectal cancer, and performance of surgery within 30 days prior to atrial fibrillation/flutter.
We used conditional logistic regression to estimate the OR of atrial fibrillation/flutter in patients with colorectal cancer,

Abstract Chronic Obstructive Pulmonary Disease (COPD) has been recently recognized as a condition involving more than the lungs. The
presence of common factors in COPD and in other chronic extra-pulmonary diseases, as well as the co-existence of these conditions
in the same adult individual, supports the hypothesis of a shared pathogenetic pathway. We will here review the interplay
between coexisting COPD and the metabolic syndrome (MS), based on the most updated knowledge. We will discuss this clinical
condition from the definition, to the pathophysiology and to the clinical implications. Basically, MS is more likely to be
present in a COPD patients, and increased levels of circulatory pro-inflammatory proteins from both the lung and adipose tissue
coincide in these patie...

Abstract Chronic constipation is a frequently complained condition in clinical practice and may be primary (idiopathic) or due to secondary
causes. The definition of the various forms of constipation is presently made according to the Rome III criteria, which recently
incorporated also specific diagnostic algorithms. The diagnosis of constipation relies on the patient’s history, including
use of drugs, physical examination, and specific investigations (transit time, anorectal manometry, balloon expulsion test,
defecography). These will often be useful to start a targeted therapeutic schedule that may include fibres, laxatives, biofeedback
training and, in extreme cases, a surgical approach. This review will analyse the clinical and diagnostic aspects of chronic
constipat...

Abstract Video laryngoscopes allow indirect visualization of the glottis and provide superior views of the glottis compared to direct
laryngoscopes in patients with both normal and difficult airways, but it may be difficult to advance the endotracheal tube
(ETT) through the vocal cords into the trachea, unless a stylet is used. We propose that the Endotrol® ETT may be an effective tool to facilitate video laryngoscope-assisted orotracheal intubation without the use of a stylet.
After obtaining written and oral informed consent, 60-adult patients scheduled for elective surgery requiring general anesthesia
with orotracheal intubation were enrolled. Patients were randomized, respectively, to 1 of 4 groups: Group A1, (15 patients): McGrath® with Endotrol® ETT; Group A2, (15 p...

Abstract Elevated uric acid levels are a common finding in patients with metabolic syndrome and in those with cardiovascular and renal
disease, but the meaning of this elevation is still unclear. In patients with chronic kidney diseases, it could merely reflect
the reduction in glomerular filtration rate: but uric acid levels are known to be elevated in people, also in younger ones,
prior to the development of hypertension or renal disease, independently of several risk factors.
Multiple potential mechanisms suggest a causative role for uric acid in vascular disease. Uric acid has been shown to be involved
in metabolic pathways that lead to oxidative stress, endothelial disfunction, and to a vascular and systemic inflammatory
response. Moreover, the elevation in uric acid...

Abstract The Italian Society of Internal Medicine has developed a voluntary program of professional accreditation of the medical units
run by its constituency. Participation in the program, which is meant to foster staff involvement in clinical governance,
includes all the medical personnel and nurses. Accreditation is awarded provided the candidate unit is able to adhere to a
pre-established set of quality standards, meet a number of clinical and organizational requirements and monitor specific indicators.
Self-evaluation is the first step in the program, followed by a site visit by a team of peer internists experienced in quality
auditing. The program, which has involved so far 19 units, has considered a number of clinical requirements related to the
three most frequent ...

This study analyses the frequency and the potential role of two polymorphisms, the +134del/insA, located in the gene encoding
for Endothelin-1 (EDN1), and the His323His in the gene encoding for Endothelin receptor type A (EDNRA) in a cohort of 98 consecutive
patients with pulmonary arterial hypertension from two different Cardiology Units (Mid-South of Italy), and in 100 healthy
Caucasian subjects randomly recruited from the same area. Cardiac anatomy and function were analysed by non invasive diagnostic
imaging techniques (Echocardiography standard m-mode, 2D, colour-Doppler) and by invasive studies (cardiac catheterization).
Molecular screening of the region of interest was performed by automated sequencing. At univariate analysis, patients with
the His323His TT genotype show a low...

Abstract Acute poisoning is a common medical emergency in Malaysia. Life can be saved if the patient is diagnosed properly and receives
the appropriate treatment such as gastrointestinal decontamination techniques and resources to increase poison elimination
according to clinical guidelines at a reasonable time. The aims of this study were to determine the availability of decontamination,
elimination enhancement, and stabilization resources for the management of acute toxic exposures and poisonings in accident
and emergency departments in Malaysia, and to compare the availability of such facilities among various types of hospitals.
A comparative, descriptive cross-sectional study was conducted using a structured questionnaire. Seventy-four (58.3%) out

In conclusions, our structured physical activity intervention significantly improves the SPPB score in frail elderly
patients who have undergone elective cardiac surgery. An intervention that improves the SPPB score might delay the occurrence
of mobility disability.

Abstract Pain treatment in Italy is far from being optimal. In order to improve this situation, the reporting of a complete assessment
of pain in the clinical record became compulsory by law. Pain-related cancer protocols (143) were selected from the National
Monitoring Centre of Clinical Trials Database and reviewed. Our data indicate that pain management is not being reported as
it should be: treatment has been taken into account in only 36.4% of the protocols, and assessment in 37.1%. Furthermore,
breakthrough cancer pain has never been reported. The main aim of cancer therapy is obviously control the disease, however
Ethics Committees should pay close attention to pain therapy when evaluating clinical protocols.

Abstract Cholestyramine is a bile acid sequestrant, like colestipol and colesevelam. These molecules are positively charged non-digestible
resins that bind to bile acids in the intestine to form an insoluble complex, which is excreted in the feces. They are used
mainly for the treatment of primary hypercholesterolemia and hypercholesterolemia associated with mild hypertriglyceridemia,
in patients not responding to dietary treatment as well as a second line-treatment for pruritus associated with cholestatic
disease, in patients with incomplete biliary obstruction. Several data indicate that modulation of bile acid homeostasis has
a good clinical effect in managing diabetes mellitus and the diarrhea from bile acid malabsorption. In this review, we present

Abstract Since the development of coronary care units (CCUs), telemetry has rapidly become the standard of care in evaluating patients
with suspected acute coronary syndromes, regardless of the probability for ischemia. However, there is no data to support
this practice. Our objective was to evaluate the utility of routine cardiac monitoring in a chest pain observation unit. We
prospectively studied the utility of routine cardiac monitoring in 249 consecutive patients admitted to an observation unit
in an academic Emergency Department over a 6-month period. All the patients presented with chest pain thought to be cardiac
ischemia. Observation included serial cardiac enzymes, ECG cardiac monitoring, and exercise testing in a designated chest
pain observation unit. These pat...

Abstract Amyloidosis comprises a unique group of diseases that share in common the extracellular deposition of insoluble fibrillar
proteins in organs and tissue including the heart. Cardiac amyloidosis could be primary a part of systemic acquired amyloidosis,
or a result of heredofamilial amyloidosis. Although the infiltration of the heart from different types of amyloid results
in restrictive cardiomyopathy that manifests with refractory congestive heart failure and conduction abnormalities, unequivocal
identification of the deposited amyloidogenic protein is mandatory in order to avoid misdiagnosis and inappropriate treatment.
Recent developments in imaging techniques and extracardiac tissue biopsy have minimized the need for invasive endomyocardial
biopsy for amyloidosi...

Abstract Atrial fibrillation (AF) is commonly associated with a significant increase in stroke and thromboembolic risk. Many patients
with AF have associated atherothrombosis given the many risk factors for atherosclerosis that also predispose to AF. Vascular
disease, whether peripheral artery disease or myocardial infarction, contributes to the increased risk of stroke and thromboembolism,
and has been included in stroke risk assessment schema. This review analyzes the interplay between AF and peripheral artery
disease in relation to their associated pathophysiology, as well as epidemiological data linking both conditions and the associated
thromboembolic complications conferred by the presence of both.

Abstract In polycythemia vera, gender has recently been shown to influence the JAK2V617F allele burden, but its effect on the disease phenotype is unknown. This issue was investigated using the database of the

Abstract Patients with chronic obstructive pulmonary disease (COPD) are prone to clinical exacerbations that are associated with increased
airway inflammation, a potent pro-thrombotic stimulus. Limited information is available on the mechanisms underlying the putative
alterations of the endothelial-coagulative system during acute exacerbations. The aim was to investigate whether the activation
of the endothelial-coagulative system occurs in association with the acute inflammatory response of COPD exacerbation. We
monitored the blood levels of surrogate markers of inflammation: interleukin-6 (IL-6); endothelium damage: von Willebrand’s
factor (vWF); clotting activation: D-dimer (D-D), and prothrombin fragment 1+2 (F1+2); fibrinolytic response: plasminogen

Abstract The pathogenesis of hemorrhagic fever with renal syndrome (HFRS) has not been fully clarified. Cell-mediated immunity appears
to play a crucial role in the immune pathogenesis of HFRS. To explore the relationship between Hantaan (HTNV)-specific CD8+ T lymphocytes and the immune pathogenesis of HFRS, the levels of interferon γ (IFN-γ) secreted by HTNV-specific CD8+ T lymphocytes were detected by flow cytometry in peripheral blood mononuclear cells. Levels of HTNV-specific CD8+ T lymphocytes in patients with HFRS were associated with different phases of HFRS. In fever phase, it was significantly elevated.
The levels of HTNV-specific CD8+ T lymphocytes in PBMC of patients with HFRS were negatively correlated with the levels of blood urea nitrogen and creatinine

appropriateness of hospital admission. Data were prospectively collected from all 345 consecutive patients admitted during
the period of 1 month for acute hospital care at a 110-bed division of internal medicine using socio-demographic and medical
information. Statistical analyses included χ2 tests, t tests, and logistic regression analyses. According to the European version of the Appropriateness Evaluation Protocol of
hospital admission, 28.1% of medical admissions for acute care in the Central Hospital of Bolzano, Italy, have been classified
as inappropriate. Factors that reduced appropriateness included female gender, age and chronic ...

Abstract It is difficult to trace full details of the path which irregular or illegal immigrants follow when seeking assistance in
the network of the various hospital departments and health structures. The aim of this work was to analyze the health needs
of immigrant people by reviewing the types of treatment given to them in the day-hospital of our Department of Migration Medicine.
Our study analyzed day-hospital admissions between 2003 and 2009. The patient charts used for managing day-hospital activity
were adopted in 2002 in conformity with the “OSI project”. From these it is possible to draw up a scale picture of the distribution
of each pathology in the immigrant population. The sample population consisted of 1,758 subjects, representing 7.4% of potential
users. ...

Abstract In non-ST-elevation acute coronary syndromes (ACS), an early invasive strategy is recommended for middle/high-risk patients;
however, the optimal timing for coronary angiography is still debated. The aim of this study was to evaluate the prognostic
implications of the time of angiography in ACS patients treated in accord with an early invasive strategy. We analyzed the
relationship between the time of angiography and outcomes at follow-up in 517 ACS patients, of whom 482 were revascularized
with percutaneous coronary intervention (PCI) (86.9%) or coronary artery by-pass graft (13.1%). We also evaluated the influence
of clinical, biohumoral and angiographic variables on the patients’ outcomes at follow-up. Among patients submitted to angiography

Abstract Physicians are occasionally faced with difficult situations in the management of vitamin K antagonists (VKA) due to the lack
of sound data available in controlled studies on certain conditions. In this review we would like to address some special
but frequent conditions that can be encountered in daily clinical practice. These include the use of VKA in hemodialysis,
thromboembolism in patients with liver cirrhosis and the thromboembolic risk in patients who bleed in the course of treatment
with VKA. Moreover, two other conditions were examined: what the best way of expressing prothrombin time would be in patients
with liver disease and how to behave when a patient treated with VKA shows a subtherapeutic INR. These topics were discussed

Abstract To investigate the prevalence of significant renal artery stenosis (RAS ≥50%), and to identify clinical predictors for significant
RAS in patients with an elevated cardiovascular risk, such as those affected by ischemic heart disease. In patients with an
elevated cardio-vascular risk, both atherosclerotic renovascular disease and coronary artery disease (CAD) are likely to occur.
Prospectively from April 2007 to March 2008, all consecutive patients with ischemic heart disease undergoing non-emergent
cardiac catheterization were also evaluated for atherosclerotic RAS by renal arteriography. A RAS ≥50% was considered as significant.
A total of 1,298 patients underwent cardiac and renal angiography. Significant RAS was found in 70 out of 1,298 patients (5.4%).

Abstract Congestive heart failure (CHF) worsening is a worldwide cause of rehospitalization and mortality, specially during the early
period after hospitalization. Fluid accumulation plays a key role in the pathophysiology of both acute heart decompensation
and disease progression. The effective use of drugs to maintain restored clinical stabilization in recently discharged patients
is a difficult task, and it relies on matching the most appropriately tailored therapy to specific clinical profiles. However,
no successful treatment has been shown to reduce post-discharge readmission. We evaluated in a case-control study the effectiveness
of an early and personalized congestion-guided ambulatory program on medium-term (6 months) compensation in recently discharged

Abstract Prior longitudinal observational studies have examined the practice patterns and outcomes of anaemia management, including
the use of erythropoiesis-stimulating agents (ESAs). Several dimensions of effectiveness remain unaddressed; especially considering
the revised ESA label (target Hb levels between 10 and 12 g/dL), the recently published TREAT study, and the European approval
of the first ESA biosimilar (HX575). Anecdotal evidence suggests that patient outcomes are influenced by physician-related
variables and whether anaemia management is congruent with practice guidelines, but this has not been studied systematically.
MONITOR-CKD5 is an international, prospective, observational, pharmacoepidemiological study evaluating the multi-level factors

Abstract Chest ultrasonography is a useful tool to assess extravascular lung water at bedside. In presence of interstitial-alveolar
imbibition, vertical artifacts arising from the pleura are detected; these are called B-lines. Although a positive linear
correlation between B-lines and extravascular lung water has been shown in symptomatic heart failure patients, the subclinical
phase of pulmonary imbibition and the clearance of B-lines after rapid body fluid removal have been less investigated. The
aim of this study was to assess if chest ultrasound could detect lung water imbibition and its variations induced by dialysis,
an experimental model of controlled rapid fluid loss. Forty-one patients undergoing hemodialysis were studied. Total number

Abstract The Prolong study shows that continuing vitamin K antagonists (VKA) in patients with abnormal d-dimer (evaluated by a qualitative assay, Clearview Simplify d-dimer) results in a significant reduction of venous thromboembolism (VTE) recurrence. The present study retrospectively analyzes
a subgroup of patients enrolled in the Prolong study with a view to calculate cut-off values for six quantitative d-dimer methods to predict the risk of VTE recurrence. We measured d-dimer levels by VIDAS d-dimer Exclusion (bioMerieux), STA Liatest d-dimer (DiagnosticaStago), HemosIL d-dimer and HemosIL d-dimer HS (Instrumentation Laboratory), Innovance d-dimer (Siemens) and AutoDimer (Trinity Biotech) in frozen plasma aliquots sampled 30 ± 10 days after VKA cessation in ...

Abstract Low bone mineral density, which increases the risk of stress fragility fractures, is a frequent, often persistent finding
in patients with major depressive disorder (MDD). The clinical association between major depressive disorder and osteopenia
is still unclear, although several factors are associated with a loss of bone mass. The aim of our study, therefore, was to
evaluate bone mineral density and bone metabolism in patients with MDD. Bone mineral density was evaluated in fifty postmenopausal
women with MDD, and in 50 matched postmenopausal control women by dual-energy X-ray absorptiometry of the lumbar spine and
femur, and by ultrasonography of the calcaneus and phalanges. Serum levels of 25-hydroxivitamin D, parathyroid hormone, Osteoprotegerin/Receptor

In this study,54 patients (40 men, mean age 44 ± 17 years) affected by IA and 50 healthy
subjects (34 men, mean age 45 ± 9) were evaluated. IA, defined as dysrhythmias with no evidence of other cardiac pathology,
were either supraventricular (SVA, 23 patients; mean age 45 ± 15 years) or ventricular (VA, 31 patients; mean age 42 ± 18 years).
H. pylori infection and gastrointestinal (GI) symptoms were evaluated. H. pylori strains expressing the cytotoxin-associated gene A (cagA) and the vacuolating-cytotoxin A (vacA) were also assessed through western blot. The prevalence of H. pylori is similar in IA patients and in controls (42 vs. 44%; p > 0.05); H. pylori infection is observed in 48 and 39% of the patients are aff...

This study provides cross-sectional and meta-analytic evidence that, in euthyroid patients, high-though-normal
TSH values are independently associated with NASH. Further work is needed to ascertain the role, if any, of lower cholesterol
serum levels in assisting in the diagnosis of NASH.

In this study, we determined the relationship between digestive diseases and the
horB gene of H. pylori infection. Fresh antral biopsy specimens were obtained from 140 dyspeptic patients (67 men and 73 women; mean age 41.5, aged
19–63 years). They were examined for presence of the horB gene of H. pylori clinical isolates. Bacterial DNA content was extracted directly from the antral biopsy. Statistical analysis was performed
with SPSS version 16.0. Prevalence of the horB gene in H. pylori isolated from patients with gastric cancer, gastric ulcer, gastritis and duodenal ulcer is (5/32) 15.6%, (4/25) 16%, (30/43)
70%, and (9/40) 22.5%, respectively. No significant relationship is observed between age, pathologic findings and gender factors
with respect to the four digestive diseas...

Abstract Increased heart rate (HR) is a risk factor for cardiovascular morbidity and mortality in the general population and in some
clinical conditions. Endothelial dysfunction is an adverse prognostic factor for cardiovascular events. The aim of the study
was to evaluate the effect of HR on central hemodynamic parameters and endothelial function in hypertension. We evaluated
forearm blood flow (FBF) response to intra-arterial infusion of acetylcholine (ACh) and sodium nitroprusside (SNP) in 30 patients
with HR ≤60 min−1 and 30 with HR ≥80 min−1. The FBF was measured by strain-gauge plethysmography. Transesophageal atrial pacing was used to increase the HR. Radial
artery applanation tonometry and pulse wave analysis were used to derive central aortic pres...

Abstract Tissues exposed to ischemia and reperfusion develop an inflammatory response. We investigate the morphological and immunological
changes occurring in the mucosa of a jejunal loop transplanted in the oropharynx of a man undergoing circular pharyngolaryngectomy.
Jejunal biopsies were collected during the transplantation procedures (cold and warm ischemia, reperfusion), during the 7
post-operative days through an exteriorized jejunal segment for flap monitoring, and 45 days after transplantation through
an upper endoscopy. Matrix metalloproteinase (MMP)-3 and MMP-12 increase was accompanied by a parallel rise in apoptotic enterocytes,
and by a concomitant reduction of surface area to volume ratio and enterocyte height. Goblet cell hyperplasia is coupled with

Abstract Crohn’s disease is a chronic inflammatory bowel disorder resulting from an inappropriate innate and acquired immune response
to commensal microorganisms in genetically susceptible individuals. This disease has a fluctuating course, with alternating
periods of remission and relapses, and it is characterized by a remarkable clinical heterogeneity; it may be complicated by
perianal fistulas, abscesses, and intestinal strictures leading to obstructions, besides several systemic manifestations.
However, a complete resolution of the disease is currently not possible, yet Crohn’s disease can be managed with established
and novel therapies, which achieve long-term remission and acceptable quality of life. This review is focused on novel advances
in basic and clinical ...

Abstract Patients with inoperable chronic thromboembolic pulmonary hypertension (Inop-CTEPH) treated with conventional therapy have
a poor survival. We compare the 3-year survival between those treated with conventional therapy and those treated with conventional
therapy and a combination of novel drugs. We also evaluate the clinical course. A total of 34 Inop-CTEPH consecutive patients
were evaluated from 1991 to 2009 including right heart catheterization (RHC) and perfusion lung scan (PLS): 7 underwent surgical
treatment while 27 were confirmed inoperable. Of these 27 patients, 12 evaluated from 1991 to 2003 (Group 1) were treated
with conventional therapy and 15 evaluated from 2004 to 2009 (Group 2) were treated with conventional and novel therapies.

Abstract Although evidence from epidemiological studies examining a relationship between cholesterol level and stroke is less than
definitive, there is a compelling evidence from the clinical therapy trials primarily designed to examine the coronary benefits
of statins that statin therapy also causes a reduction in the risk of stroke. Even though a stroke does not have the same
exact pathophysiology as a heart attack, specific trials in stroke patients confirm advantages and risks of statin therapy
in this kind of population. In primary prevention, statins are effective both when low-density lipoprotein (LDL) is raised
and when hs-CRP is elevated. In secondary prevention, an absolute reduction of recurrent stroke can be obtained with statins,
with a number needed to treat ...

long-term, in patients with acute heart failure syndromes (AHF). We prospectively assessed in 100 consecutive non-diabetic
AHF patients whether acute glucose metabolism, as indicated by fasting glycemia and insulin resistance (HOMA index) was able
to affect short- and long-term mortality. In the overall population, 51 patients showed admission glucose values >140 mg/dl.
No significant difference was observed in admission and peak glycemia, insulin and C-peptide values and in HOMA-index between
dead and survived patients. At multivariate logistic backward stepwise analysis the following variables were independent predictors

and diabetes and reduction of added sugars is considered between the goals to achieve in order to promote cardiovascular health
and to reduce deaths from cardiovascular causes. Many reliable mechanisms, such as dislypidemia, inflammation and enhanced
oxidative stress, have been proposed to support a causal link between sugar sweetened beverages intake and cardiovascular
risk, but the ultimate underlying pathways remain to be determined in adequately designed studies. Furthermore, while epidemiological
evidence strongly supports an association between sugar sweetened beverages consumption and obesity, type 2 diabetes mellitus

Abstract Elderly patients are at increased risk for peptic ulcer and cancer. Predictive factors of relevant endoscopic findings at
upper endoscopy in the elderly are unknown. This was a post hoc analysis of a nationwide, endoscopic study. A total of 3,147
elderly patients were selected. Demographic, clinical, and endoscopic data were systematically collected. Relevant findings
and new diagnoses of peptic ulcer and malignancy were computed. Both univariate and multivariate analyses were performed.
A total of 1,559 (49.5%), 213 (6.8%), 93 (3%) relevant findings, peptic ulcers, and malignancies were detected. Peptic ulcers
and malignancies were more frequent in >85-year-old patients (OR 3.1, 95% CI = 2.0–4.7, p = 0.001). The presence of dysphagia (OR&n...

Abstract The aim of this study was to assess whether adherence to a restricted-calorie, Mediterranean-type diet improves endothelial
dysfunction and markers of oxidative stress in patients with metabolic syndrome. A moderately low-calorie (600 calories/day
negative energy balance), low-fat, high-carbohydrate diet (<30% energy from fat, <10% from saturated fat and 55% from carbohydrate)
was prescribed to 53 outpatients with the metabolic syndrome. Participants were divided into two groups according to body

In conclusion the results seem to suggest
that the majority of patients who survive for more than 5 years after an operation for colorectal malignancy return to an
almost normal life. The awareness among individuals about their disease, the improvements in surgical techniques and medical
treatments are among the factors responsible for these positive results.

Abstract The Glasgow Coma Scale (GCS) is the most widely accepted tool for the evaluation of consciousness, despite several reported
shortcomings. A new coma scale, named Full Outline of UnResponsiveness (FOUR) score, is now available. The aim of the present
study is to provide and validate the Italian version of the FOUR score. The Italian version of the FOUR score was developed
according to a standardized protocol, and thereafter validated in a series of patients with acute neurological illness. For
each patient, the FOUR and the GCS scores were recorded by two physicians randomly selected. The inter-rater agreement for
the FOUR and the GCS scores was evaluated using the weighted kappa (κw). The receiving operating characteristic curve was also calculated to determine th...

diagnosed in asymptomatic subjects. Our aim was to define the relationship among symptoms at diagnosis, serological markers
[tissue transglutaminase antibodies (tTGA), anti-endomysium antibodies (EMA) anti-actin antibodies (AAA)] and degree of mucosal
damage. A total of 68 consecutive adult patients with CD were enrolled. Intestinal biopsies were scored according to the Marsh
classification modified by Oberhuber: I–II minimal lesions or absent villous atrophy; IIIA partial villous atrophy; IIIB–C
total villous atrophy (TVA). HLA-typing was done for all patients. No association between clinical presentation and severity
