Conclusions Elevated MIF levels in the sera of PV patients could participate in disease induction by activation of T cells as well as induction of autoantibody production by B cells. Given that MIF counter-regulates the effects of steroids, MIF antagonists may prove to be very effective, novel steroid-sparing agents for this life-threatening conundrum.
Conclusion In contrast with male AGA, no association was found between type II AGA in Egyptian women and the AR gene. Therefore, the genetic study of this gene does not serve as a biomarker for the identification of women with a predisposition to AGA. (Source: International Journal of Dermatology)

An open-label, randomized, 64-week study repeating 10- and 20-U doses of botulinum toxin type A for treatment of glabellar lines in Japanese subjects

Conclusion Repeated treatments of glabellar lines with 10 or 20 U of BoNTA provided long-term safety and efficacy in Japanese subjects. The 20-U dose provided longer duration, greater subject satisfaction, and greater subject-rated improvement. The 20-U dose was identified as optimal for improving glabellar lines in Japanese subjects. These results apply only to the formulation of BoNTA used in this study.
Conclusions Our results support the hypothesis that imiquimod activity in the treatment of BCC is partly a result of a pro-inflammatory action mediated by CD3+/CD4+ lymphoid cells and of a pro-apoptotic activity associated with decreased Bcl-2 expression.
A 23-year-old pregnant woman at 38 weeks of gestation presented with a 2-day history of a red eruption on her abdomen and upper thighs. The patient had applied baby oil to her abdomen a few days earlier, but she denied using any other topical products. Her only medications were prenatal vitamins and iron, which she had been taking since the beginning of her pregnancy. The clinical impression was contact dermatitis, and she was started on topical Sarna lotion (containing 0.5% camphor and 0.5% menthol) after she declined topical steroids. Three days later, she presented to the dermatology clinic with worsening pruritus, mild diarrhea, and subjective chills. On physical examination, there were diffuse erythematous plaques studded with 1[ndash]2-mm pustules on the upper extremities, chest, bac...

A 45-year-old woman presented with a 3-year history of cutaneous lesions. Clinical examination revealed red[ndash]yellow to brownish infiltrated papules coalescing into annular-shaped plaques of several sizes with a hyperpigmented center affecting the abdomen and lower limbs, as well as multiple firm nodules on the right palm, elbows, and knees (Fig. 1a[ndash]c). The patient also reported sporadic arthralgia and low fever. She had been treated for leprosy for 2 years with multidrug therapy (clofazimine, dapsone, and rifampicin), with complete remission of the lesions during treatment, but recurrence after discontinuation. Histologic examination of a biopsy specimen taken from the cutaneous lesions showed an interstitial inflammatory infiltrate with the presence of many neutrophils and occa...

Conclusion The global number of cases of cutaneous leishmaniasis in Sicily has increased in recent years, and such increases can be explained, in part, by the fact that, in this region, sandflies are present during a large part of the year. This is a result of the climatic variation in recent years (increasing temperature and humidity). There has also been an increase in the number of new and rare variants of cutaneous leishmaniasis. A knowledge of the unusual clinical variants of cutaneous leishmaniasis, as well as classical forms, allows early detection. (Source: International Journal of Dermatology)

Plexiform neurofibroma in neurofibromatosis type 1, an autosomal-dominant genetic disorder, is characterized by a combination of interlacing components or a network. The prominent enlargement of a nerve with tumor nodules results in the gross pathologic appearance termed "bag of worms." Plexiform neurofibroma was found in two of seven family members with neurofibromatosis type 1 in three generations. Ultrasonography/color doppler and magnetic resonance imaging, in addition to microscopic pathology, were used as diagnostic tools, and their indications for future use in the diagnosis of plexiform neurofibroma are highlighted.
Paraneoplastic pemphigus is the term used for an exclusive subset of pemphigus. The clinical lesions may resemble pemphigus, pemphigoid, erythema multiforme, graft-vs.-host disease, or lichen planus. A common denominator in all patients is the concomitant occurrence of either occult or confirmed systemic neoplasm. It is imperative to confirm the diagnosis through microscopy, where intraepidermal suprabasal cleavage, epidermal acantholysis, dyskeratotic keratinocytes and vacuolar changes in the basal epidermis, interfacial dermatitis, and epidermal exocytosis can be seen. Furthermore, the deposition of immunoglobulin G (IgG) and complement in the epidermal intercellular spaces, detected by direct and/or indirect immunofluorescence, is equally crucial for confirming the diagnosis.
We report a case of pulmonary actinomicosis with secondary cutaneous involvement which led to the diagnosis. A 51-year-old man presented with an indurated, erythematous plaque on his right chest wall. He had been diagnosed with pneumoniae one month prior ago and received antibiotic treatment but symptoms persisted. Fibrobroncoscopy was normal and bronchoalveolar lavage samples were negative. The cutaneous plaque evolved with fistulization and drainage of serohematic material with white grains. Actinomyces meyeri was cultured from bacteriologic samples. Ceftriaxone and doxiciclin were administered for a total of 12 months with complete resolution of the clinical condition. (Source: International Journal of Dermatology)

We report three cases of lupus vulgaris with atypical presentations. The cases mimicked other chronic skin lesions like actinomycosis, mycetoma, and so on. Strong clinical suspicion, histopathology, and response to antituberculous treatment led to the diagnosis and all three had excellent response to treatment. Today, when tuberculosis threatens to burst into pandemics again, early diagnosis and treatment are more important than ever for control and prevention of morbidity.
Conclusion Increased photosensitivity and sunburn after minimal sun exposure should lead physicians to examine the skin for poikiloderma and the mouth for fragility of the gums, periodontitis and other associated findings. Also, patients should be questioned about any history of blister formation as well as family history. (Source: International Journal of Dermatology)

His face, neck, back, abdomen, upper extremities, flexural areas, palms and soles as well as hair and nails were not involved. Family history was negative for similar lesions. Otherwise, the patient had a normal development. Onset of symptoms occurred during childhood with scales on lower extremities with no more additional features. Treatment included emollients exclusively with partial and temporary remission of cutaneous lesions. Recently, the patient had not received topical or systemic medical treatment. Laboratory investigations were within normal limits. The patient had undetectable levels of STS activity when compared with normal control (0.00 pmol mg-1 pr...

Conclusions Patients who were receiving systemic medications seemed to be more susceptible to the development of BP. Should we consider drug exposure as one of the reasons for the increased prevalence of BP in the elderly? It is important to be more vigilant with this category of patients in order to provide an early diagnosis of the disease. (Source: International Journal of Dermatology)

Background Human papillomavirus (HPV) has been implicated in the development of digital squamous cell carcinoma (SCC). Case reports in the literature mostly identify HPV type 16 present in tumors, but HPV types 2, 31, 34, 35, and 73 have also been isolated. Methods Two cases of digital SCC associated with HPV 16 in young African-American men are presented. Results and conclusions Digital SCC associated with HPV may be difficult to evaluate and treat, particularly in African-Americans and patients with human immunodeficiency virus (HIV). We discuss the need for careful evaluation, treatment, and follow-up of these individuals. (Source: International Journal of Dermatology)

Conclusion No evidence for tachyphylaxis in SD/D treatment by PTZ-based shampoos was found. Compliance could explain the decreasing response rate seen over time; the solution is to choose an affordable therapeutic product that is effective long term without cosmetic trade-offs.
A 25-year-old woman living in Hamedan, Iran, presented originally at 7 years of age with erythematous, hyperkeratotic lesions on the palms and soles with extension to the dorsal side of the hands and feet. Involvement of the elbows and knees was also seen. From 12 years of age, she started to lose her teeth. At the same age, she experienced fever, chills, malaise, myalgia, and right upper quadrant abdominal pain. With a diagnosis of pyogenic liver abscesses, the patient underwent successful surgical treatment. Examination revealed erythematous, hyperkeratotic, scaling plaques on the palms and soles, dorsal side of the hands and feet (Fig. 1), elbows and knees. All the teeth were missing from the mouth (Fig. 2), and she used a dental prosthesis. A surgical scar was observed on the right upp...

An otherwise healthy 72-year-old man presented with a painful eruption composed of grouped hemorrhagic purulent blisters on erythematous plaques, on both palms of his hands, which appeared 12 h after he had been vaccinated against influenza. Based on the patient's history, physical examination, histopathological and laboratory findings, the diagnosis of neutrophilic dermatosis of the hands (NDH) was made. NDH is currently considered a distributional variant of Sweet's syndrome (SS). It is identical to atypical pyoderma gangrenosum (PG) and pustular vasculitis of the hands. There are only seven reported cases of SS after vaccination, none of them with lesions confined solely to the palms or soles. Our current case is the third one of SS following influenza vaccination.
A previously healthy 44-year-old Chinese man presented with a nonitching, painless, skin lesion on the right lateral abdomen of 1-year duration. This lesion presented initially as a nodule of approximately 0.5 cm in diameter, which eventually grew to the size of an egg by August 2006. The patient had experienced no systemic symptoms, and his past medical and family history were noncontributory. Physical examination was normal except for the skin lesion. Peripheral lymph nodes were not palpable. Skin examination showed a lobulated, contusiform, brownish, infiltrated nodule with a wide base on the right lateral abdomen (Fig. 1). Histologic examination of a biopsy specimen taken from the lesion revealed the presence of a dense dermal lymphomatous infiltrate with a diffuse growth pattern, whic...

Conclusion The results of this study indicate that hair density and thickness vary over the entire scalp area in patients suffering from hair loss. The measurement of multiple scalp sites and the observation of changing patterns of hair parameters are very informative in terms of differentiating less apparent hair loss disorders. Through this study, we wish to introduce a doctor- and patient-friendly hair evaluation tool for use in clinic offices. (Source: International Journal of Dermatology)
