Hypoglycemic activity of Onosma hispidum (Ratanjot)

Conclusion: The methanolic root extract of O. hispidum has hypoglycemic action. As diabetes is associated with hyperglycemia and several other pathological changes such as infections, inflammation, foot ulcers and impairment of wound healing, these additional effects besides hypoglycemic effect of O. hispidum may be proved as a breakthrough in the treatment of diabetes. Biochemical and receptor oriented molecular studies are required to find out the exact mechanism of hypoglycemic action of O. hispidum.
Effect of electrical stimulation on blood glucose level and lipid profile of sedentary type 2 diabetic patients

Conclusion: The percentage change in blood glucose level scores of the experimental group (12.71%) was much higher as compared to the control group (4.06%) after a single session of treatment. Following 2 weeks of electrical stimulation, the blood glucose level was significantly (t = 5.026, P = 0.001) reduced in experimental group, but there was an insignificant fall in the same for the control group. Our results suggest that electrical stimulation can be used to assist in the control of blood glucose level in type 2 diabetic patients.
Dhruv K Singh, Ken FarringtonInternational Journal of Diabetes in Developing Countries 2010 30(4):185-190Diabetic nephropathy (DN) is a major microvascular complication of diabetes and a leading cause of end-stage renal disease (ESRD) worldwide. DN is classically understood as a glomerular disease. However, this understanding is currently being contested with recent studies suggesting a pivotal role of the tubulointerstitium in the initiation and progression of DN. We performed a review of the current literature as detailed below. The literature suggests that chronic hyperglycemia may orchestrate several structural and functional abnormalities of the tubulointerstitium before significant glomerular changes. Abnormalities such as increased advanced glycation end products, oxidative stress, ...

Dayananda Lingegowda, Srikanth Moorthy, KP Sreekumar, Rajesh R KannanInternational Journal of Diabetes in Developing Countries 2010 30(4):179-184Diabetic microangiopathy largely affects the infrapopliteal vessels. Extensive infrapopliteal calcification and occlusions, without the presence of collaterals, are common in diabetic patients. When the diabetic patient presents with a nonhealing ulcer, the investigation of first choice should be duplex ultrasound. Ultrasound is not only safe and noninvasive, it also serves to guide further patient management. In a subset of patient in whom intervention is planned, catheter angiogram is the best choice; if the patient has diabetic nephropathy, MR angiogram is an alternative. Multidetector computed tomography angiogram is not a preferred modality i...

Biochemical and receptor oriented molecular studies are required to find out the exact mechanism of hypoglycemic action of O. hispidum.

Conclusions: SLCT activity in RBC is not a predictor of nephropathy in young diabetics of Bangladesh and the development of nephropathy is mainly related to glycemic and insulin status. SLCT may be elevated by marked hypoinsulinemia even without hypertension or family history of hypertension. (Source: International Journal of Diabetes in Developing Countries)

Conclusion: All the three drugs led to significant decrease in HbA1c, fasting and postprandial plasma glucose levels. Glipizide lowered body weight, BMI, total cholesterol (P < 0.05), low density lipoprotein cholesterol (LDL-C) (P < 0.01) and MDA levels (P < 0.05). Metformin reduced body weight (P < 0.01), BMI (P < 0.01), LDL-C, very low density lipoprotein cholesterol (VLDL-C) and improved high density lipoprotein cholesterol (HDL-C); however, it raised Hcy levels (P < 0.01). Rosiglitazone treatment increased body weight (P < 0.01) and LDL-C (P < 0.01); however, it improved HDL-C (P < 0.01) and decreased sVCAM-1 (P < 0.05) and MDA (P < 0.05) levels.
Conclusions: Prevalence of diabetes and IFG among the OA has increased significantly over the last decade. Unless immediate steps are taken to contain the increasing prevalence of obesity, diabetes, IFG and MetS, the health care costs for OA with chronic diseases will pose an enormous financial burden to the country. Despite significant attempts to understand the contributing disease mechanisms for diabetes, there is still no solution for a cure. Investigators are continuously working on strategies for pancreatic transplantation and islet cell replacement. Adult stem cells that appear to be precursors to islet cells and embryonic stem cells that produce insulin seem to have caught the attention of researchers worldwide. However, the universal consensus is that until a therapeutically useful source of human islet cells is developed, a persistent searc...

With globalization, there is a possibility that western environmental models may be replicated in developing countries such as India, where the underlying genetic predisposition makes them particularly susceptible to diabetes. Here we review published information on the relationship bet...

Hierarchical clustering is a multivariate tool often used in phylogenetics, comparative genomics to relate the evolution of species. The patterns seen in microarray expression data can be interpreted as indications of the status of the genes responsible for nephropathy in peripheral blow cells of type 2 diabetes (T2DN). Out of 415 genes totally expressed in the 3 DNA chips it was concluded that only 116 gen...

Conclusions:</b> Ten per cent of the 311 adults screened had hyperglycemia. Half of the interviewed population had some awareness about diabetes and its symptoms. But more than half (75%) of them were not aware of the long term effects of diabetes and diabetic care. The common perception about diet in diabetes was to avoid sweets, rice and fruits and to consume more ragi, millet and wheat chapattis. Diabetes in young adults is common. Relevant knowledge about diabetes is poor in rural population. Hence community level awareness programs have to be organized. Healthcare providers must be aware of community perceptions and practices. (Source: International Journal of Diabetes in Developing Countries)

Cerebral extract from alloxan diabetic rats significantly inhibited the brain AChE activity of normal animals, indicating the presence of an inhibiting factor in the cerebrum of diabetic rats. Cichorium intybus when fed for 10 days offered neuroprotection by stimulating AChE activity. (Source: International Journal of Diabetes in Developing Countries)

Diabetic macrovasculopathy is associated with structural and functional changes in large arteries, which causes endothelial dysfunction, increased arterial stiffness, or decreased arterial distensability. Diabetic complications can be controlled and avoided by strict glycemic control, maintaining normal lipid profiles, regular physical exercise, adopting a healthy lifestyle and pharmacological interventions. Treatment goals for patients with type 2 diabetes specify targets for glycemia and other cardiometabolic risk factors, for example, hypertension and d...

It is the second most common chronic disease in children and account for 5% to 10% of all diagnosed cases of diabetes. India is having an incidence of 10.6 cases/year/100,000, and recent studies indicate that the prevalence of type 1 diabetes in India is increasing.
Conclusions:</b> The results of the present study suggest that Lp(a) levels are increased in type 2 diabetic patients. The elevated Lp(a) levels do not reflect the glycemic status and are also independent of increase in LDL:HDL ratio suggesting different metabolic pathways and the genetic connection for LDL and Lp(a).
Conclusions :</b> Fasting during the month of Ramadan causes weight loss and decrease in calorie intake, which is correlated with a decrease in meal frequency. However, more foods rich in fat and dietary cholesterol were consumed during this period. The latter could constitute a high risk for diabetics who are fasting, in particular when medication advice and/or health care control are insufficient or ignored.
Conclusions</b> : Thus, it was concluded that chronic complications of glucose metabolism disorders might be associated with alterations in the levels of some trace elements. Nevertheless, some more timely and extensive studies are required to clarify the exact mechanisms of each of these changes.
Conclusions</b> : This study concluded that a novel 43 kDa protein obtained from the livers of alloxan-induced diabetic rats shows protease activity as well as antiproliferative activity. Also, this protein may act as a diabetogenic factor as it elicited a significantly gross elevation in the oxidant status level as well as in the levels of lysosomal enzymes and a decrease in the levels of antioxidative enzymes and ionic transporters of PMNs.
Tiwari Sangeeta, Chauhan Ashutosh, Sethi N TInternational Journal of Diabetes in Developing Countries 2008 28(4):130-131Tropical diabetic hand syndrome (TDHS) is a terminology used to describe a specific complication affecting patients with diabetes mellitus in the tropics. The syndrome encompasses a localized cellulitis with variable swelling and ulceration of the hands to progressive, fulminant hand sepsis, potentially fatal. Since this syndrome is less recognized it is often under-reported. Authors present two cases of TDHS and emphasize on aggressive glycemic control and surgical therapy to prevent potential crippling or fatal complications.
This study is, therefore, a meta-summary of the incidence and prevalence of today's emerging silent killer or diabetes in Sub-Saharan Africa. The theme is that time is running out for Africa and that, as was for HIV/AIDS, by the time the governments wake up and stop denying the catastrophic potential of the epidemic, diabetes will simply overwhelm the continent's resources, and the world will witness the death of millions of Africans. The last section is a call for action against diabetes in terms of advocacy, promotion of awareness, and public health policies that empower people to diabetes self-management.
Periyasamy R, Manivannan M, Narayanamurthy V BInternational Journal of Diabetes in Developing Countries 2008 28(3):71-78Diabetic neuropathy is one of the most important factors for foot ulceration in diabetes mellitus (DM) patients. Among different sensibility measures of neuropathy, two-point discrimination (TPD) has been suggested as a reliable method; however, the correlation of TPD with other well-known measures is not known. We measured the loss of protective sensation using Semmes-Weinstein Monofilaments (SWMF), hardness of the foot sole using shore meter (sh), power ratio (PR) using pedopowergraph and TPD using esthesiometer in foot areas of both left and right legs in 14 DM subjects. We have found no correlation either between TPD and shore values (sh) or between TPD and PR. The SW...

Conclusion :</b>The insulin regimen with fewest barriers consists of one SC injection of basal insulin lantus in combination with oral agents. However, to be effective, oral agents must include a secretogogue i.e. glimeperide in addition to a sensitizer i.e. metformin and not multiple sensitizers without a secretogogue. Moreover, this regimen apparently is also the most preferred by the patients, and is cost effective. (Source: International Journal of Diabetes in Developing Countries)

Conclusions:</b> American Diabetes Association recommendations for aspirin use for secondary prevention of cardiovascular diseases were strictly adhered to, in contrast to that for primary prevention. Under-prescription of aspirin could be attributed to the physicians' concern about the burden of poly-pharmacy and toxic effects of aspirin on long-term use. Extensive efforts are necessary to enhance aspirin use in this regard. (Source: International Journal of Diabetes in Developing Countries)
