<description>Abstract&amp;nbsp;&amp;nbsp;A 70-year-old woman presented with clinical features of right heart failure. Cardiopulmonary investigations included an echocardiogram,
which showed a hepatic cyst compromising venous return and affecting right atrial filling and a CT abdomen showed a 15.5&amp;nbsp;×&amp;nbsp;11.5&amp;nbsp;cm-cystic
mass involving the right hepatic lobe and compressing the right atrium. Percutaneous drainage of the cyst was performed. This
led to complete resolution of symptoms but these recurred as the fluid re-accumulated. Subsequent definitive treatment with
excision of the cyst was undertaken with symptomatic cure. This case is the first report of a hepatic cyst presenting as right
heart due to compression of the right atrium.

<description>Abstract&amp;nbsp;&amp;nbsp;Douglas Argyll Robertson’s (1837–1909) experimental work with physostigmine in 1863 sharpened his knowledge of the innervation
of the internal muscles of the eye. So he was ideally prepared in 1869 to analyse the conundrum when he saw patients with
spinal cord disease who had lost the response to light even though accommodation to near objects was normal. By translating
his knowledge of basic science to a clinical problem he drew attention to this phenomenon, known subsequently as the Argyll
Robertson pupil that came to be considered pathognomonic of tabes dorsalis, general paresis and neurovascular syphilis.

<description>Conclusions&amp;nbsp;&amp;nbsp;Administration of metformin has effective inhibitory action on circulating DPP IV. Although this was not associated with significant
metabolic effects in the fasting state, metformin-induced inhibition of DPP IV may contribute to glycaemic control by promoting
insulin-releasing activity of the enteroinsular hormones actively secreted following feeding.

<description>We report a case of this rare disorder treated successfully with a combination
of rituximab and cyclophosphamide, adriamycin, vincristine and prednisolone. The patient developed recurrent unstable ventricular
tachycardia (VT) post-chemotherapy secondary to extensive scarring at the tumour site. The tumour as well as the post-treatment
scarring is well illustrated by cardiac magnetic resonance imaging highlighting its usefulness in this setting. An implantable
cardioverter defibrillator (ICD) was placed. This is only the second case in the literature of PCL to have an ICD placed for
recurrent VT. A brief literature review is included.

<description>Conclusion&amp;nbsp;&amp;nbsp; It is, perhaps, a fair conclusion to state that Irish medical journals flourished in the first half of the nineteenth century
when Irish medicine was itself at the centre of the world stage. There were a number of remarkably able men who were actively
involved in medical editing as well as notable medical discoveries. Dublin was then a very large city with well-organised
hospitals. The population of the country was at a level never seen before or since. Ireland’s population was then more than
half that of England and Wales. Poverty lead to an enormous demand for hospital care with unrivalled opportunities for clinical
observation. Post-mortem examinations were frequent and thorough.

<description>Conclusion&amp;nbsp;&amp;nbsp; The prevalence of recurrent IgA disease following transplantation in an Irish population is less than that reported at other
centres (24% versus 60%). The severity of the original disease and transplant factors may predict recurrence post-transplantation.

<description>Conclusion&amp;nbsp;&amp;nbsp;Medical risk factors for stroke were common among stroke patients and not optimally treated, particularly with regard to atrial
fibrillation and previous stroke. Smoking was a major behavioural risk factor among younger patients and much health gain
could be achieved in this group through primary prevention strategies.

<description>Conclusions&amp;nbsp;&amp;nbsp; The limited consultant supervision and small numbers of NCHDs in training for A&amp;E medicine raise concerns about staffing.
Most hospitals want GPs to work in their A&amp;E departments. This has implications for training and for the interface between
general practice and the A &amp; E department.

<description>Conclusion&amp;nbsp;&amp;nbsp;Work-related and DIY injuries are the most common causes of eye perforation. Many of these injuries are easily preventable
with appropriate eye protection. The proportion of injuries resulting from road traffic accidents was low. Injuries due to
assault are becoming more prevalent and carry a poor visual prognosis.

<description>Conclusion&amp;nbsp;&amp;nbsp; These findings suggest that healthy Irish adults with the Apo E4 genotype have higher serum total and LDL-cholesterol levels
than those with E2 or E3 Apo E genotypes and therefore may have a higher risk of atherosclerotic coronary artery disease and
coronary heart disease in later life.

<description>Conclusion&amp;nbsp;&amp;nbsp; The proportion of clients screened for HCV, HBV and HIV infection has increased since the introduction of a screening protocol
in 1998. Targeted vaccination for opiate users against hepatitis B is more successful than previously shown in Ireland. The
prevalence and incidence of bloodborne viruses remains high among opiate users attending addiction treatment services, despite
an increase in availability of harm reduction interventions.

of Alzheimer’s Disease in Down Syndrome is not associated with a further fall, and may be associated with a rise, in serum
 albumin concentrations.

 Knowledge comes, but wisdom lingers

Experimental medicine, therapeutics and surgery throve after the. American and British visitations, which were not allowed
to pass without dissent.11 Progress continues with quickening pace, spurred by increasingly generous state subvention and support from outside bodies
 like the Wellcome Trust.

A century afterAn Introduction to the Study of Experimental Medicine was published by Claude Bernard [1813–1878] in 1855, Ireland was introduced to, and is now zealously immersed in, the practice
as well as the principles of experimental medicine as the foundation of sound clinical practice. Wisdom’s slow progression

<description>Conclusion&amp;nbsp;&amp;nbsp;Severe abdominal pain in conjunction with appropriate clinical factors should lead to a high level of suspicion for mesenteric
ischaemia. These patients must be rapidly and aggressively investigated and treated to maximise chances of survival. The encouraging
results of recent studies using new biochemical markers may facilitate earlier diagnosis in the near future.

<description>Conclusions&amp;nbsp;&amp;nbsp; If the patient is stable and requires emergency laparotomy for other abdominal injuries, he should have immediate realignment
of the urethra. Early realignment of the urethra at taparotomy at 1–2 weeks can be combined with orthopaedic fixation of pelvic
fractures. Patients who remain unstable due to associated injuries should have delayed urethroplasty at three months.

<description>Conclusions&amp;nbsp;&amp;nbsp;HCV is increasingly detected in antenatal clinics. Heterosexual contact is a mode of spread. Maternal HCV viraemia can be
variable in pregnancy. Further study of HCV in pregnancy is needed to define the impact of pregnancy on HCV, accurately predict
infant outcome and selectively target interventions to women at greatest risk of transmission.

<description>Conclusions&amp;nbsp;&amp;nbsp; Both tests performed satisfactorily. However, indeterminate results impaired the usefulness of the laboratory ELISA particularly
when using a new cut-off. The ‘office’ ELISA performed particularly well in young patients, A simpler test using antigens
from locally prevalent strains to optimise accuracy is awaited.

<title>Has there been a turning point in the numbers of AIDS and HIV antibody positive cases in Ireland?</title>

<description>Conclusions&amp;nbsp;&amp;nbsp;Declines in HIV and AIDS incidence in the homosexual and IDU risk groups are indirectly attributable to health safety and
treatment programmes. The increase in HIV antibody positive cases among heterosexuals may reflect a relapse in safe sex behaviour.

            <description>Abstract
Background&amp;nbsp;&amp;nbsp;Programmed cell death, or apoptesis, is a distinct, managed form of cell death. It is fundamentally different from necrosis.
It is a genetically controlled, energy-dependent method of cellular deletion without inflammation. In the cardiovascular system,
apoptosis occurs as a primary and secondary event in disease pathogenesis. This review addresses our current understanding
of the initiation, propagation and significance of apoptosis in the cardiovascular system, as well as assessing therapeutic
 potentials arising therefrom.

Methods&amp;nbsp;&amp;nbsp;A Medline search was performed and relevant publications reviewed. Further articles were obtained from the references of these

activity more accurately. This study does not support the routine use of soluble adhesion molecules as disease activity markers
 in IBD.

<description>Conclusions&amp;nbsp;&amp;nbsp;94% of infants bom to HCV antibody positive women are not HIV infected. Liver transaminase elevation in exposed infants is
not always indicative of infection. A minimum monitoring schedule of testing (PCR and antibody) at six to eight weeks, six
and 18 months allows early diagnosis while detecting late seroconversions.

            <link>http://www.medworm.com/index.php?rid=2921905&amp;cid=s_35978_22_f&amp;fid=35978&amp;url=http%3A%2F%2Fwww.springerlink.com%2Fcontent%2Fx205701r61gv19r1%2F</link>
<description>Conclusions&amp;nbsp;&amp;nbsp; The results show a reduction in fall occurrence from previously reported rates in this at-risk population. We find the falls
clinic a convenient and effective method of managing elderly people who are at increased risk of falling.

<description>Conclusion&amp;nbsp;&amp;nbsp;GPs and receptionists in this sample had mixed views about methadone maintenance which were unchanged by six months experience

<description>Conclusions&amp;nbsp;&amp;nbsp;Thorough screening of injecting drug users for HCV within one treatment service is difficult. There is a need for explicit
policies on this issue involving co-operation between primary care providers and addiction services and hepatoiogy services.
