            <title>Profiling harmful medication errors in an acute Irish teaching hospital</title>

            <description>Conclusions&amp;nbsp;&amp;nbsp;A cross-tabulation strategy for prioritising medication-associated risks was successfully applied to a hospital database comprising
 medication errors. The profile developed for harmful medication errors in this acute tertiary healthcare setting was broadly

 blood levels. It is reversible on stopping the medication. Early recognition is important to prevent unnecessary investigations

            <description>Conclusions&amp;nbsp;&amp;nbsp;Selecting opiate-dependent methadone-maintained cocaine abusers on the basis of their urine toxicology and offering them counselling
 as a way of reducing their harmful drug use did not prove efficacious. Attempting to address cocaine misuse within this cohort
 may need a more stepped approach including brief interventions, such as motivational interviewing, or other enhancers of motivation
 before we can test the effectiveness of CBCS in this population.

 urine cannabinoids are positive. A positive urine cannabinoids immunoassay alone is insufficient evidence in the linkage of

            <title>Fixation of tibial plateau fractures with synthetic bone graft versus natural bone graft: a comparison study</title>

            <description>Conclusions&amp;nbsp;&amp;nbsp;Use of autologous or allogenic bone graft allows better recovery of long-term flexion, possibly due to reduced inflammatory
 response compared with synthetic bone composites. However, all other parameters, such as maintenance of joint reduction and
 subjective outcome measures were comparable with the use of hydroxyapatite calcium carbonate bone graft. This study shows
 that synthetic bone graft may be a suitable alternative in fixation of unstable tibia plateau fractures, avoiding risk of
 disease transmission with allograft and donor site morbidity associated with autograft.

            <title>Paracetamol overdose: the liver unit perspective</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Liver failure resulting from deliberate or accidental paracetamol overdose continues to be an important reason for referral
 to liver transplant centres. Severe hepatic dysfunction often appears 72–96&amp;nbsp;h after overdose. Liver injury can be prevented
 by timely administration of the specific antidote, N-acetylcysteine. Unfortunately, administration of N-acetylcysteine is frequently delayed due to late presentation or late administration. While N-acetylcysteine works best if given within 8&amp;nbsp;h of overdose, it is beneficial at any time period and should always be given
 if there is concern about significant overdose, irrespective of interval from time of ingestion. Early discussion with liver

            <description>Conclusions&amp;nbsp;&amp;nbsp;The design of clinical trials on the septic patient remains a challenge. Methodology must be rigorous if seemingly positive
 clinical trials which are widely implemented are later discredited as a result of poor study design. Many lessons can be learnt
 from the study design of the PROWESS trial, however there remains room for improvement. This review serves as a stimulus and

            <description>Conclusion&amp;nbsp;&amp;nbsp;A paradigm shift has occurred towards endovascular management of aortic, iliac and femoral arterial disease. Whilst patient
 age &amp;gt;75 significantly impacts on complication rates from index open vascular procedures, age does not impact upon complication
 rates of corresponding endovascular procedures. We, therefore, recommend an endovascular approach be adopted as first line

 patient comfort. Patient safety was not compromised. Further modifications of our protocols are necessary to meet the international

            <description>Conclusions&amp;nbsp;&amp;nbsp;Stage pT1a disease and preoperative low PSA were associated with favourable survival. However, for pT1b and/or high Gleason
 score (≥7), mortality was comparatively higher. Hence, patients with high Gleason score and/or pT1b disease should be considered
 for curative therapy. Additionally, active surveillance may have a role in selected men with incidental prostate cancer.

 for national audit implementation would relate to software and administrative staff recruitment. Implementation of the European

 A cardiac examination, ECG and ECHO are advised at diagnosis, and also on the development of any cardiac symptoms. Treatment
 is determined by the presence of symptoms, the presence of left ventricular outflow gradient and the sudden death risk. Institution

 interventions may reduce outpatient referrals. GPs in group practices may collectively have greater experience and expertise

            <title>Enhanced recovery after elective colorectal surgery: now the standard of care</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Enhanced recovery programmes have been studied in randomised trials with evidence of quicker recovery of gut function, reduced
 morbidity, mortality and hospital stay and improved physiological and nutritional outcomes. They aim to reduce the physiological
 and psychological stress of surgery and consequently the uncontrolled stress response. The key elements, reduced pre-operative
 fasting, intravenous fluid restriction and early feeding after surgery, are in conflict with traditional management plans
 but are supported by strong clinical evidence. Given the strength of the current data enhanced recovery should now be the
 standard of care.

            <title>The trauma of a recession</title>

            <description>Abstract&amp;nbsp;&amp;nbsp;Employment in construction in Ireland fell by 10% from nearly 282,000 in the second quarter of 2007 to 255,000 in the same
 period of 2008. Our study looks at the differences in soft tissue upper limb trauma dynamics of a pre- and post-recession
 Ireland. Construction accounted for 330 patients (27%) of all hand injuries in 2006, but only 18 (3%) in 2009. Our data shows
 a significant drop in hand injuries related to the construction industry, and more home/DIY cases and deliberate self-harm
 presenting in their stead.

            <description>Abstract&amp;nbsp;&amp;nbsp;Skin puckering is a sign that is well associated with certain fractures such as supracondylar humeral fractures in children.
 To our knowledge, there has been only one clinical report of skin puckering associated with fractured neck of humerus of an
 adult. This is a second such case of fractured proximal humerus in an adult presenting with skin puckering and the first from
 the Republic of Ireland. Skin puckering is suggestive of soft tissue interposition and may be an important clinical sign indicating

 in three out of ten patients, the prevalence of low HDL-C and high TGs in high-risk patients is greater than one in three.
 A more integrated approach to the treatment of patients with dyslipidaemia is warranted. Clinical trials are needed to assess

 Introduction&amp;nbsp;&amp;nbsp;In our unit sentinel lymph node biopsy (SLNB) is performed without intra-operative pathological nodal assessment. If node
 biopsies are positive the patients have to return at a later date for a complete axillary node clearance (ANC).
 
 
 
 
 Methods&amp;nbsp;&amp;nbsp;We conducted a retrospective study to ascertain if the use of pre-operative ultrasound assessment of the axilla with fine
 needle aspiration (FNA) sampling could identify patients with nodal metastases and therefore identify patients who should
 proceed primarily to ANC.
 
 
 
 
 Results&amp;nbsp;&amp;nbsp;Our study showed that 40 patients out of 119 had nodal metastases, and ultrasound correctly identified 19 of those patients.

            <description>Conclusions&amp;nbsp;&amp;nbsp;Cordocentesis causes more injury to the extrafetal compartment, which results in a higher level of fetomaternal transfusion.
 However, though a nearly ten times higher fetomaternal transfusion was observed after cordocentesis, there was no essential

 is worryingly poor. Training in sharps handling and dealing with a NI needs to be addressed. HIV is the blood-borne infection

 study of mechanisms linking diet and cancer. A greater understanding of the interplay between dietary constituents and cancer
