    Human immunodeficiency virus (HIV)-1 infection is initiated by the binding of gp120 envelope glyco-protein to its cell receptor (CD4) and a coreceptor (CXCR4 or CCR5), followed by a series of conformational changes in the gp41 transmembrane subunit. These changes include insertion of fusion peptide into the target cell membrane and association of C-heptad repeat (CHR) peptide with the N-heptad repeat (NHR) trimer, a pre-hairpin fusion intermediate. A stable six-helix bundle core is then formed, bringing the viral envelope and target cell membrane into close proximity for fusion. Laboratory tests, including serum 17-hydroxyprogesterone, androstenedione, and testosterone levels are more sensitive than serum cortisol or dehydroepiandrosterone sulfate levels for diagnosing 21-OHD in prepubertal children.

            <description>This study investigated the effects of surgery on the phonation threshold pressure (PTP) in patients with vocal fold polyps. Methods: Prospectively, 32 consecutive patients with vocal fold polyps were enrolled. PTP was measured 1 day before and 6 weeks after laryngomicrosurgery, by means of an airflow interruption method. An accelerometer was used to detect the vocal fold vibration instead of using an acoustic signal. Results: In all 32 patients, the average preoperative PTP was 0.51 +/- 0.23 kPa, and 6 weeks after surgery, it was 0.28 +/- 0.17 kPa; this difference was statistically significant (p &amp;lt; 0.001). Individually, PTP decreased in 31 patients after surgery. PTP increased after surgery in only one patient, from 0.74 to 0.75 kPa.

Most oral neoplastic lesions in pediatric patients are benign, and malignant oral tumors rarely occur in pediatric patients.

Patients with limited lymph node involvement (1-3 nodes) can be cured by surgery alone, whereas a more extensive lymph node metastasis (&amp;gt;/= 4 nodes) eventually results in recurrence. Antituberculous treatment was started and antiviral agents were given continually at adjusted doses. Her symptoms of cough with blood-tinged sputum improved 1 month after antituberculous therapy. Treatment for hepatitis C and pulmonary tuberculosis were completed after 6 months. At 6 months after antiviral therapy, a sustained virological response was achieved and follow-up chest radiography showed a marked regressive change.             <description>We describe a 69-year-old woman with RA who presented initially with fever and an indurated skin lesion on the right thigh. Lobular panniculitis was diagnosed after biopsy and was then treated with prednisolone. After this therapy, pulmonary infiltration developed and was later shown by transbronchial biopsy to be caused by Mycobacterium tuberculosis. The panniculitis skin lesion became smaller after prednisolone therapy and was further improved after antituberculosis drugs were added. Reexamination of the previously biopsied skin tissue disclosed acid-fast bacilli.             <description>We present a 49-year-old man who received abdominal ultrasound and magnetic resonance imaging for a health checkup, during which, a necrotic hepatic tumor was found incidentally. Fluorodeoxyglucose (FDG) positron emission tomography was performed to search for the primary site of malignancy, and lobulated FDG hypermetabolic lesions in the anterior mediastinum were found. The diagnosis of thymic carcinoma with liver metastasis was then confirmed after morphological and immunohistochemical studies of hepatic and mediastinal biopsy specimens.

These findings may be useful to doctors and nurses, especially in counseling women who have had, or plan to have, a hysterectomy.

Among the eight patients, seven were identical HLA-DR matches with the donors. The stem cells were not manipulated in vitro. Graft-versus-host disease (GVHD) prophylaxis consisted of antithymocyte globulin, cyclosporine, and methotrexate. All patients had successful trilineage engraftment. Only one patient had grade 2-4 acute GVHD. Two patients died before day +100 due to complications. Before surgery, acute appendicitis with intra-abdominal abscess was suspected, but during the operation, an infected cyst of the omentum, adjacent and adherent to the redundant transverse colon, was found to have been causing these symptoms. Despite the fact that cysts of the omentum have been reported rarely, the operator should be aware that the cyst is a benign entity and the surgical strategy should be different from that for malignancy. We should keep the possibility of omental cyst in mind to avoid unnecessary bowel resection and potentially harmful inappropriate treatment.

            <description>Conclusion: Binding of OPN to osteoclast cell membrane receptor integrin alphav can activate the osteoclasts and increase their osteolytic activity. In addition, binding of OPN to OKC lining epithelial cell membrane receptor CD44v6 can enhance the motility, migration, invasion and spread of lining epithelial cells into the surrounding cancellous bone. Therefore, we suggest that the local aggressive behavior and high osteolytic ability of OKCs in the jawbone can be explained at least partially by high expression of OPN and CD44v6 in lining epithelial cells of OKCs and high expression of integrin alphav in osteoclasts.

            <description>Conclusion: The SNP rs165599, which has been mapped to the 3'-UTR region of the COMT gene, was significantly associated with schizophrenia in our family study, and possibly associated with the age of onset, delusion/hallucination symptom dimension, and CPT performance. Therefore, COMT may contribute to the genetic risk for schizophrenia not through the Val108/158Met polymorphism, but through other variants that are situated 3' to this region, in the Taiwanese population. Nevertheless, the true associated functional variants in our subjects remain to be elucidated.

The main complications included chronic hypotony (1 case), rubeosis iridis (4 cases), intraocular hemorrhage (1 case), neovascular glaucoma (1 case), and cataract (1 case). Retinectomy with peripheral traction release and silicone oil infusion resulted in improvement or stabilization of visual function in all four cases.

    Retrosternal goiters are commonly situated in the anterior mediastinum, but according to the literature, 10-15% are located in the posterior mediastinum. The authors report two cases of enormous goiter in the posterior mediastinum. Case 1 was a 60-year-old man. His mass measured 12 x 9 x 8 cm and was combined with trachea compression and superior vena cava syndrome. Case 2 was a 59-year-old woman. Her mass measured 9 x 6 x 6 cm and she was admitted with the complaint of dysphagia. Both patients were discharged from hospital after successful operations. They reported normal activities in the follow-up.

            <description>We report a newborn infant with antenatal sonographic diagnosis of bowel dilatation and giant meconium pseudocyst. Postnatal presentation included a palpable abdominal mass and abdominal distention. Abdominal X-ray revealed a huge egg-shell calcified mass containing air-fluid level. Postnatal sonography revealed a cystic mass with air-fluid level and acoustic shadows. Computed tomography showed a giant communicating cyst with egg-shell calcification, which compressed the other intestinal loops to the posterior peritoneal cavity.             <description>We report an immunocompetent 25-year-old male hepatitis B carrier with severe dental disease that led to the development of liver abscess. A periapical abscess in the upper left molar area was seen on his dental X-ray. Two sets of blood cultures grew Prevotella species, bacteria that are commonly found inside the oral cavity. Bacterial culture of the liver abscess drainage sample grew both Prevotella and Fusobacterium necrophorum. This led to our diagnosis of pyogenic liver abscess of dental origin, since we found no other source of infection in our patient except for his dental disease.             <description>We present a 35-year-old woman who suffered from headache and dizziness initially, and then lower back pain, lower limb weakness and paraparesis gradually developed within 1-2 weeks. Magnetic resonance imaging revealed intracranial and spinal SDH. No vascular abnormality was seen by brain and spinal angiography. Platelet count, prothrombin time, activated partial thromboplastin time, and inflammatory markers, including C-reactive protein, were normal. A diagnosis of spontaneous spinal and intracranial SDH was then confirmed surgically. Postoperative recovery was uneventful.
